    Defense-Oriented Psychoanalytic Psychotherapy as a Tailored Treatment for Boys: Neurobiological Underpinnings to Male-Specific Response Tested in Regulation-Focused Psychotherapy for Children

    This paper presents defense-oriented psychoanalytic psychotherapy as a tailored treatment for boys through a neurophysiological hypothesis. Male central nervous system development is reviewed, with a focus on the development of the emotion regulation system. The organizational effects of pre- and post-natal androgens delay central nervous system development in males relative to females, following a caudal to rostral phylogenetic framework. Ventromedial prefrontal structures mature at an earlier developmental age than dorsolateral prefrontal structures, creating less of a gender gap in the available underlying neural architecture for responsivity to targeted therapeutic intervention. The hypothesized operation of defense analysis upon ventromedial prefrontal cortical structures and corticolimbic connectivity therefore positions boys to benefit from psychotherapy equally as girls. In this study, we explored gender differences in presentation and response to a short-term, manualized defense-oriented psychoanalytic psychotherapy named regulation-focused psychotherapy for children. In a sample size of 43 school-aged children, consisting of 32 boys and 11 girls, with oppositional defiant disorder, we found no statistically significant differences in participant characteristics upon entry nor in treatment response, as measured by changes in scores on the Oppositional Defiant Disorder Rating Scale, the oppositional defiant problems subscale of the Child Behavior Checklist, the suppression and reappraisal subscales of the Emotion Regulation Questionnaire for Children and Adolescents, and the lability and negativity subscale of the Emotion Regulation Checklist. The findings were comparable with the gendered findings of preexisting studies of play therapy, where boys and girls improve equally, but not of behaviorally predominant psychotherapy, where girls appear to have superior responses. Our findings suggest that the treatment as a general play therapy, but with a focus on the implicit emotion regulation system, was successful in meeting boys’ gendered treatment needs. Conclusions are drawn with implications for further study

    Clinical outcomes after implantation of a new hydrophobic acrylic toric IOL during routine cataract surgery

    PURPOSE: To assess the clinical outcomes after implantation of a new hydrophobic acrylic toric intraocular lens (IOL) to correct preexisting corneal astigmatism in patients having routine cataract surgery. SETTING: Four hospital eye clinics throughout Europe. DESIGN: Cohort study. METHODS: This study included eyes with at least 0.75 diopter (D) of preexisting corneal astigmatism having routine cataract surgery. Phacoemulsification was performed followed by insertion and alignment of a Tecnis toric IOL. Patients were examined 4 to 8 weeks postoperatively; uncorrected distance visual acuity (UDVA), corrected distance visual acuity, manifest refraction, and keratometry were measured. Individual patient satisfaction with uncorrected vision and the surgeon’s assessment of ease of handling and performance of the IOL were also documented. The cylinder axis of the toric IOL was determined by dilated slitlamp examination. RESULTS: The study enrolled 67 eyes of 60 patients. Four to 8 weeks postoperatively, the mean UDVA was 0.15 logMAR G 0.17 (SD) and the UDVA was 20/40 or better in 88% of eyes. The mean refractive cylinder decreased significantly postoperatively, from -1.91 +/- 1.07 D to -0.67 +/- 0.54 D. No significant change in keratometric cylinder was observed. The mean absolute IOL misalignment from the intended axis was 3.4 degrees (range 0 to 12 degrees). The good UDVA resulted in high levels of patient satisfaction. CONCLUSION: Implantation of the new toric IOL was an effective, safe, and predictable method to manage corneal astigmatism in patients having routine cataract surgery

    Early tolerance in pediatric liver allograft recipients

    The authors report on six pediatric liver transplant recipients for whom allograft tolerance occurred shortly after transplantation (ie, less than 1.5 years). All the patients had associated life-threatening viral complications. They are currently immmunocompetent. The tolerant state may be related to the development of a TH2 cytokine pattern. © 1994

    Spacecraft/Rover Hybrids for the Exploration of Small Solar System Bodies

    This study investigated a mission architecture that allows the systematic and affordable in-situ exploration of small solar system bodies, such as asteroids, comets, and Martian moons (Figure 1). The architecture relies on the novel concept of spacecraft/rover hybrids,which are surface mobility platforms capable of achieving large surface coverage (by attitude controlled hops, akin to spacecraft flight), fine mobility (by tumbling), and coarse instrument pointing (by changing orientation relative to the ground) in the low-gravity environments(micro-g to milli-g) of small bodies. The actuation of the hybrids relies on spinning three internal flywheels. Using a combination of torques, the three flywheel motors can produce a reaction torque in any orientation without additional moving parts. This mobility concept allows all subsystems to be packaged in one sealed enclosure and enables the platforms to be minimalistic. The hybrids would be deployed from a mother spacecraft, which would act as a communication relay to Earth and would aid the in-situ assets with tasks such as localization and navigation (Figure 1). The hybrids are expected to be more capable and affordable than wheeled or legged rovers, due to their multiple modes of mobility (both hopping and tumbling), and have simpler environmental sealing and thermal management (since all components are sealed in one enclosure, assuming non-deployable science instruments). In summary, this NIAC Phase II study has significantly increased the TRL (Technology Readiness Level) of the mobility and autonomy subsystems of spacecraft/rover hybrids, and characterized system engineering aspects in the context of a reference mission to Phobos. Future studies should focus on improving the robustness of the autonomy module and further refine system engineering aspects, in view of opportunities for technology infusion

    Resampling methods for parameter-free and robust feature selection with mutual information

    Combining the mutual information criterion with a forward feature selection strategy offers a good trade-off between optimality of the selected feature subset and computation time. However, it requires to set the parameter(s) of the mutual information estimator and to determine when to halt the forward procedure. These two choices are difficult to make because, as the dimensionality of the subset increases, the estimation of the mutual information becomes less and less reliable. This paper proposes to use resampling methods, a K-fold cross-validation and the permutation test, to address both issues. The resampling methods bring information about the variance of the estimator, information which can then be used to automatically set the parameter and to calculate a threshold to stop the forward procedure. The procedure is illustrated on a synthetic dataset as well as on real-world examples

    Homoarginine, heart failure, and sudden cardiac death in haemodialysis patients

    Aims Sudden cardiac death (SCD) is a major contributor to the excess mortality of patients on maintenance dialysis. Homoarginine deficiency may lead to decreased nitric oxide availability and endothelial dysfunction. Based on this rationale we assessed whether homoarginine deficiency is a risk factor for SCD in dialysis patients. Methods and results This study examined the association of homoarginine with cardiovascular outcomes in 1255 diabetic haemodialysis patients from the German diabetes and dialysis study. During a median of 4 years of follow-up, hazard ratios (HR) (95% CI) for reaching the following pre-specified, adjudicated endpoints were determined: SCD, myocardial infarction, stroke, death due to heart failure, and combined cardiovascular events. There was a strong association of low homoarginine concentrations with the presence of congestive heart failure and left ventricular hypertrophy as well as increased levels of brain natriuretic peptide. Per unit decrease in homoarginine, the risk of SCD increased three-fold (HR 3.1, 95% CI 2.0–4.9), attenuating slightly in multivariate models (HR 2.4; 95% CI 1.5–3.9). Patients in the lowest homoarginine quintile experienced a more than two-fold increased risk of SCD, and more than three-fold increased risk of heart failure death than patients in the highest quintile, which accounted for the high incidence of combined cardiovascular events. Low homoarginine showed a trend towards increased risk of stroke, however, myocardial infarction was not meaningfully affected. Conclusion Low homoarginine is a strong risk factor for SCD and death due to heart failure in haemodialysis patients. Further studies are needed to elucidate the underlying mechanisms, offering the potential to develop new interventional strategies

    Constrained Cosmological Simulations of Dark Matter Halos

    The formation and structure of dark matter (DM) halos is studied by means of constrained realizations of Gaussian fields using N-body simulations. A series of experiments of the formation of a 10^{12} Msun halo is designed to study the dependence of the density profile on its merging history. We confirm that the halo growth consists of violent and quiescent phases, with the density well approximated by the Navarro-Frenk-White (NFW) profile during the latter phases. We find that (1) the NFW scale radius R_s stays constant during the quiescent phase and grows abruptly during the violent one. In contrast, the virial radius grows linearly during the quiescent and abruptly during the violent phases. (2) The central density stays unchanged during the quiescent phase while dropping abruptly during the violent phase. (3) The value of \rs reflects the violent merging history of the halo, and depends on the number of violent events and their fractional magnitudes, independent of the time and order of these events. It does not reflect the formation time of the halo. (4) The fractional change in R_s is a nonlinear function of the fractional absorbed kinetic energy within R_s in a violent event.Comment: 5 pages, 2 postscript figures, ApJ emulator, submitted to ApJ Letter

We therefore developed a novel approach for isolating polymorphic microsatellites from the club-legged grasshopper (Gomphocerus sibiricus), an emerging quantitative genetic and behavioral model system. Whole genome shotgun Illumina MiSeq sequencing was used to generate over three million 300 bp paired-end reads, of which 67.75% were grouped into 40,548 clusters within RepeatExplorer. Annotations of the top 468 clusters, which represent 60.5% of the reads, revealed homology to satellite DNA and a variety of transposable elements. Evaluating 96 primer pairs in eight wild-caught individuals, we found that primers mined from singleton reads were six times more likely to amplify a single polymorphic microsatellite locus than primers mined from clusters. Our study provides experimental evidence in support of the notion that microsatellites associated with repetitive elements are less likely to successfully amplify. It also reveals how advances in high-throughput sequencing and graph-based repetitive DNA analysis can be leveraged to isolate polymorphic microsatellites from complex genomes
