    Learn it, Live it, Teach it, Embed it: Implementing a whole school approach to foster positive mental health and wellbeing through Positive Education

    Schools provide unique environments for the implementation of interventions to support the mental health and wellbeing of young people. While promising as an intervention setting, the school system is inherently complex, and any successful intervention or programme must account for, and adapt to, such complexity. Whole-school approaches that comprise multi-components and promote collaborative and collective action across the school system appear promising for accounting for these complexities. This paper reports the updated implementation processes of one whole-school approach, the Geelong Grammar School Applied Model for Positive Education, for fostering positive mental health and wellbeing among the school community. Drawing upon existing frameworks and from successes observed in the fields of Social and Emotional Learning, mental health prevention, and health promotion, adapted to meet the goals of Positive Education, we propose four interconnecting, cyclical processes; Learn it, Live it, Teach it, Embed it. In combination, these processes assist schools in designing and reviewing ongoing implementation. This study extends the literature to date by synthesising organisational, systems change, education, and anecdotal evidence to identify barriers to implementation and subsequent outcomes of missed, or poorly executed processes. While schools will be unique in factors such as context, specific needs, and available resources, it is envisaged that reporting these processes and potential barriers to success may assist schools with their own future implementation endeavours

    Sustaining the use of Positive Education coping skills to meet the challenges of the emerging adulthood period

    The Positive Education approach is a whole-school program implemented in primary and secondary school settings that aims to equip students with effective coping skills. During the transition to adulthood (often termed “emerging adulthood”), young people encounter a variety of challenges and are expected to draw on their own resources to cope. The current qualitative study aimed to provide initial insights into whether young people exposed to Positive Education training at school continue to use these coping strategies during emerging adulthood, when they have left the school setting. Twenty-one graduates (9 male, 12 female, 18 – 19 years old) of a Victorian independent school who had received four years of explicit Positive Education training completed semi-structured interviews via telephone. The interviews focused on the challenges the participants encountered after leaving school, skills they used to cope with those challenges, and factors that affected their use of Positive Education skills. Qualitative analysis showed that all participants were using skills that are taught within Positive Education to some extent, once they had left the secondary school setting. Young people identified a range of barriers and enablers to their continued use of these skills beyond the school setting. The findings suggest that Positive Education training may be helpful in preparing young people for the challenges of the emerging adulthood period. However, further program development is needed to promote the sustained use of these skills.

    Associations between mentally-passive and mentally-active sedentary behaviours during adolescence and psychological distress during adulthood.

    It is unclear if different types of sedentary behaviour during the adolescence are differentially associated with psychological distress during adolescence and adulthood. It is also unknown what may mediate this potential proposed association. The current study aimed to analyse the association of mentally-active and mentally-passive sedentary behaviours during adolescence (16y) with subsequent psychological distress during adulthood (42y), and to examine the role of potential mediators (42y). Data from the 1970 British Cohort Study was used (N = 1787). At age 16y participants reported time and frequency in mentally-passive (TV-viewing and watching movies) and mentally-active (reading books, doing homework and playing computer games) sedentary behaviours, psychological distress and organized sports participation. At 42y, participants reported cognition (vocabulary test), TV-viewing, psychological distress, self-rated health, body mass index and employment status. Education was collected throughout the follow-up years. Logistic regression and mediation models assessed associations. Multiple imputation using chained equations was used to assess the impact of missing data. Mentally-passive sedentary behaviour in adolescence was a risk factor for psychological distress during adulthood in complete-cases analysis [OR:1.44(95%CI:1.09-1.90)], which was confirmed by the model with multiple imputation. Mentally-active sedentary behaviour at 16y was not associated with psychological distress at 42y. Adult TV-viewing during weekends (24.7%), and self-rated health (19.0%) mediated the association between mentally-passive sedentary behaviour during adolescence and psychological distress during adulthood. However, the mediation was not clear in the models with multiple imputation. Mentally-passive sedentary behaviour during adolescence was associated with elevated psychological distress during adulthood and this association was mediated TV-viewing and self-rated health in adulthood

    Sugar-and intense-sweetened drinks in Australia: a systematic review on cardiometabolic risk

    Sugar-sweetened beverages (SSBs) are consumed globally, and have been associated with adverse health outcomes, including weight gain, high blood pressure, type 2 diabetes (T2D), and cardiovascular disease (CVD). There is global variation in beverage formulation in terms of glucose and fructose concentration, which may pose unique health risks linked to glycemic control for Australian consumers. However, previous systematic reviews have overlooked Australian-based literature. A systematic review was performed to synthesise evidence for the associations between consumption of SSBs and intense-sweetened beverages with clinical cardiometabolic risk factors in the Australian population. Articles were sourced from Global Health, Health Source: Nursing/Academic Edition, Medline, and Culmative Index to Nursing and Allied Health Literature. To be eligible for review, studies had to report on the consumption of sugar-sweetened (including fruit juice and fruit drinks) and/or intense-sweetened beverages, and at least one clinical cardiometabolic risk factor. Eighteen studies were included in this review. Research has mostly focused on the relationship between SSB consumption and adiposity-related outcomes. No studies have examined indices of glycaemic control (glucose/insulin), and the evidence for the health impact of intense-sweetened drinks is limited. In addition, studies have primarily been of cross-sectional design, and have examined children and adolescents, as opposed to adult populations. In the Australian population, there is modest but consistent evidence that SSB consumption has adverse associations with weight, but there is insufficient data to assess relationships with cardiometabolic outcomes.     Pathways to Increasing Adolescent Physical Activity and Wellbeing: A Mediation Analysis of Intervention Components Designed Using a Participatory Approach.

    We assessed which intervention components were associated with change in moderate-to-vigorous physical activity (MVPA) and wellbeing through proposed psychosocial mediators. Eight schools (n = 1319; 13-14 years) ran GoActive, where older mentors and in-class-peer-leaders encouraged classes to conduct two new activities/week; students gained points and rewards for activity. We assessed exposures: participant-perceived engagement with components (post-intervention): older mentorship, peer leadership, class sessions, competition, rewards, points entered online; potential mediators (change from baseline): social support, self-efficacy, group cohesion, friendship quality, self-esteem; and outcomes (change from baseline): accelerometer-assessed MVPA (min/day), wellbeing (Warwick-Edinburgh). Mediation was assessed using linear regression models stratified by gender (adjusted for age, ethnicity, language, school, BMI z-score, baseline values), assessing associations between (1) exposures and mediators, (2) exposures and outcomes (without mediators) and (3) exposure and mediator with outcome using bootstrap resampling. No evidence was found to support the use of these components to increase physical activity. Among boys, higher perceived teacher and mentor support were associated with improved wellbeing via various mediators. Among girls, higher perceived mentor support and perception of competition and rewards were positively associated with wellbeing via self-efficacy, self-esteem and social support. If implemented well, mentorship could increase wellbeing among adolescents. Teacher support and class-based activity sessions may be important for boys' wellbeing, whereas rewards and competition warrant consideration among girls

    Does Fruit and Vegetable Consumption During Adolescence Predict Adult Depression? A Longitudinal Study of US Adolescents

    The relationship between better diet quality and decreased depression across the life span is consistent and compelling. Fruit and vegetable consumption has been of particular interest. The nutritional benefits from the consumption of fruits and vegetables may mitigate non-communicable diseases and promote brain and mental health. This study aimed to determine whether fruit and vegetable consumption during adolescence was associated with a reduced risk of developing depression in adulthood in a large, representative sample of US individuals. Data from the Add Health Study were analyzed, which included 3,696 participants who were aged approximately 17 years at baseline (1994–1995), and 29 years at follow-up (2007–2008). The Center for Epidemiologic Studies Depression Scale was used to assess depression and a self-report item asked how many times the participant consumed fruit/vegetables on the previous day. Individuals who were depressed at both times points had the highest proportion who failed to consume any fruit (31%) or vegetables (42%) on the previous day. Fruit and vegetable consumption did not predict of adult depression in fully adjusted models. Cross sectional associations existed for diet and adolescent depression only. Our initial findings supported fruit and vegetable consumption as being protective against adult depression, but this association was subsequently attenuated on adjustment for other relevant factors. Future research will benefit from more precise measures of dietary intakes

    The COVID-19 pandemic has afforded the opportunity for some to improve lifestyle behaviours, while for others it has presented key challenges. Adverse changes in global lifestyle behaviours, including physical activity, sleep, and screen time can affect proximal mental health and in turn distal cardiovascular outcomes. We investigated differences in physical activity, sleep, and screen time in parents and children during early stages of the COVID-19 pandemic in Australia compared to pre-COVID-19 national data; and estimated associations between these movement behaviours with parent and child mental health. Cross-sectional baseline data from the COVID-19 Pandemic Adjustment Study (CPAS; N = 2,365) were compared to nationally representative pre-pandemic data from the Longitudinal Study of Australian Children (LSAC; N = 9,438). Participants were parents of children aged ≀ 18 years, residing in Australia. Parents provided self-report measures of mental health, physical activity and sleep quality, and reported on child mental health, physical activity and screen time. Children in CPAS had significantly more sleep problems and more weekend screen time. Their parents had significantly poorer sleep quality, despite increased weekly physical activity. Children's sleep problems were significantly associated with increased mental health problems, after accounting for socioeconomic status, physical activity, and screen time. Poorer parent sleep quality and lower levels of physical activity were significantly associated with poorer mental health. Monitoring this cohort over time will be important to examine whether changes in movement behaviour are enduring or naturally improve with the easing of restrictions; and whether these changes have lasting effects on either parent or child mental health, and in turn, future risk for CVD

    School-based systems change for obesity prevention in adolescents: Outcomes of the Australian Capital Territory 'It's Your Move!'

    Objective: The Australian Capital Territory ‘It’s Your Move!’ (ACT-IYM) was a three-year (2012- 2014) systems intervention to prevent obesity among adolescents. Methods: The ACT-IYM project involved three intervention schools and three comparison schools and targeted secondary students aged 12-16 years. The intervention consisted of multiple initiatives at individual, community, and school policy level to support healthier nutrition and physical activity. Intervention school-specific objectives related to increasing active transport, increasing time spent physically active at school, and supporting mental wellbeing. Data were collected in 2012 and 2014 from 656 students. Anthropometric data were objectively measured and behavioural data self-reported. Results: Proportions of overweight or obesity were similar over time within the intervention (24.5% baseline and 22.8% follow-up) and comparison groups (31.8% baseline and 30.6% follow-up). Within schools, two of three the intervention schools showed a significant decrease in the prevalence of overweight and obesity (p<0.05). Conclusions: There was some evidence of effectiveness of the systems approach to preventing obesity among adolescents. Implications for public health: The incorporation of systems thinking has been touted as the next stage in obesity prevention and public health more broadly. These findings demonstrate that the use of systems methods can be effective on a small scale.     The associations between sedentary behaviour and mental health among adolescents:A systematic review

    Background: With technological developments and modernised sedentary lifestyles has come an increase in diseases associated with inactivity such as obesity and other non-communicable diseases. Emerging evidence suggests that time spent sedentary may also interact with mental health. This systematic review examined the associations between sedentary behaviour and mental health problems among adolescents. Methods: This systematic review followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses, and applied a quality assessment tool for quantitative studies to identity best available evidence. Following stringent search strategy of the databases; Cumulative Index to Nursing and Allied Health Literature, Global Health, Health Source: Nursing and Academic Edition, MEDLINE, PsychARTICLES and PsycINFO, we identified 32 articles eligible for review. Results: All studies reported leisure screen time among adolescents, and two thirds of identified studies examined depressive symptomatology. Other mental health measures were; anxiety symptoms, self-esteem, suicide ideation, loneliness, stress, and psychological distress. Strong consistent evidence was found for the relationship between both depressive symptomatology and psychological distress, and time spent using screens for leisure. Moderate evidence supported the relationship between low self-esteem and screen use. Poorer mental health status was found among adolescents using screen time more than 2-3 h per day, and gender differences exist. Essential information was missing for quality of evidence including heterogeneity in mental health and screen time-based measures, and self-report data collection methods. Conclusions: The findings are of particular significance given the global public health concern of lifestyle-attributed diseases and the possibility for novel approaches to mental health. Future research should examine the psychological impact of reducing time spent using screens for leisure among adolescents, whilst accounting for possible confounding factors such as physical activity and dietary behaviours. It is critical that the reciprocal relationship between lifestyle behaviours and mental health is represented in both the psychiatric and public health forum
