    Resistance and the paradox of legal entitlement – A theoretical analysis of migrant women’s responses to domestic abuse in the host country

    This article provides a theoretically informed examination of migrant women’s responses to domestic abuse in the host country. It departs from an analysis of research on South Asian women in England, on Portuguese women in England and on Portuguese women in Canada to suggest that women’s apparent lack of mobilisation of law (primarily by eschewing contact with the justice system of the host country and preferring informality), both perpetuates hegemonic discourses and presents a possibility for change. The theoretical approach undertaken combines literature on legal consciousness, power and resistance, and on socio-cultural structures and barriers that affect migrant women. The article ultimately suggests that, rather than an acceptance of hegemonic discourses, women’s behaviour is best understood as a form of resistance to, and from within, socio-cultural pressures encountered in everyday life; as a form of “entrenched” resistance

    Effect of TENS on pain in relation to central sensitization in patients with osteoarthritis of the knee: study protocol of a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Central sensitization has recently been documented in patients with knee osteoarthritis (OAk). So far, the presence of central sensitization has not been considered as a confounding factor in studies assessing the pain inhibitory effect of tens on osteoarthritis of the knee. The purpose of this study is to explore the pain inhibitory effect of burst tens in OAk patients and to explore the prognostic value of central sensitization on the pain inhibitory effect of tens in OAk patients.</p> <p>Methods</p> <p>Patients with knee pain due to OAk will be recruited through advertisements in local media. Temporal summation, before and after a heterotopic noxious conditioning stimulation, will be measured. In addition, pain on a numeric rating score, WOMAC subscores for pain and function and global perceived effect will be assessed. Patients will be randomly allocated to one of two treatment groups (tens, sham tens). Follow-up measurements will be scheduled after a period of 6 and 12 weeks.</p> <p>Discussion</p> <p>Tens influences pain through the electrical stimulation of low-threshold A-beta cutaneous fibers. The responsiveness of central pain-signaling neurons of centrally sensitized OAk patients may be augmented to the input of these electrical stimuli. This would encompass an adverse therapy effect of tens. To increase treatment effectiveness it might be interesting to identify a subgroup of symptomatic OAk patients, i.e., non-sensitized patients, who are likely to benefit from burst tens.</p> <p>Trial Registration</p> <p>ClinicalTrials.gov: <a href="http://www.clinicaltrials.gov/ct2/show/NCT01390285">NCT01390285</a></p

    Paneth Cell Alterations During Ischemia-reperfusion, Follow-up, and Graft Rejection After Intestinal Transplantation

    BACKGROUND Ischemia-reperfusion (IR) injury is inevitable during intestinal transplantation (ITx) and executes a key role in the evolution towards rejection. Paneth cells (PC) are crucial for epithelial immune defense and highly vulnerable to IR injury. We investigated the effect of ITx on PC after reperfusion (T0), during follow-up, and rejection. Moreover, we investigated whether PC loss was associated with impaired graft homeostasis. METHODS Endoscopic biopsies, collected according to center-protocol and at rejection episodes, were retrospectively included (n=28 ITx, n=119 biopsies) Biopsies were immunohistochemically co-stained for PC (lysozyme) and apoptosis, and PC/crypt and lysozyme intensity were scored. RESULTS We observed a decrease in PC/crypt and lysozyme intensity in the first week after ITx (W1) compared to T0. There was a tendency towards a larger decline in PC/crypt (p=0.08) and lysozyme intensity (p=0.08) in W1 in patients who later developed rejection compared to patients without rejection. Follow-up biopsies showed that the PC number recovered, whereas lysozyme intensity remained reduced. This persisting innate immune defect may contribute to the well-known vulnerability of the intestine to infection. There was no clear evidence that PC were affected throughout rejection. CONCLUSION This study revealed a transient fall in PC numbers in the early post-ITx period, but a permanent reduction in lysozyme intensity following ITx. Further research is needed to determine the potential clinical impact of PC impairment after ITx

    Bioethical implications of end-of-life decision-making in patients with dementia:a tale of two societies

    End-of-life decision-making in patients with dementia is a complex topic. Belgium and the Netherlands have been at the forefront of legislative advancement and progressive societal changes concerning the perspectives toward physician-assisted death (PAD). Careful consideration of clinical and social aspects is essential during the end-of-life decision-making process in patients with dementia. Geriatric assent provides the physician, the patient and his family the opportunity to end life with dignity. Unbearable suffering, decisional competence, and awareness of memory deficits are among the clinical considerations that physicians should incorporate during the end-of-life decision-making process. However, as other societies introduce legislature granting the right of PAD, new social determinants should be considered; Mexico City is an example. Current perspectives regarding advance euthanasia directives (AED) and PAD in patients with dementia are evolving. A new perspective that hinges on the role of the family and geriatric assent should help culturally heterogeneous societies in the transition of their public health care policies regarding end-of-life choices.</p

    Neuromuscular training improves movement competency and physical performance measures in 11-13 year old female netball athletes

    The purpose of this study was to examine the effects of a neuromuscular training program on movement competency and measures of physical performance in youth female netball players. It was hypothesized that significant improvements would be found in movement competency and physical performance measures following the intervention. Twenty-three junior female netball players (age, 12.17 ± 0.94 yrs; height, 1.63 ± 0.08 m; weight, 51.81 ± 8.45 kg) completed a test battery before and after a six-week training intervention. 13 of these athletes underwent six weeks of neuromuscular training, which incorporated plyometrics and resistance training. Trained athletes showed significant improvements in 20 m sprint time, 505 agility time, countermovement jump height and peak power (p ≀ 0.05, g \u3e 0.8). Additionally, trained athletes significantly improved their score in the Netball Movement Screening Tool (NMST) (p \u3c 0.05, g \u3e -1.30); while the athletes also demonstrated increased reach in the anterior and posteromedial directions for the right and left leg, and in the posterolateral direction for the left leg only in the Star Excursion Balance Test (SEBT) (p \u3c 0.05, g \u3e -0.03). Control subjects did not exhibit any significant changes during the 6-week period. Significant negative correlations were found between improved score on the NMST and decreased 5 m, 10 m and 20 m sprint time, and 505 change of direction time (r \u3e 0.4, p ≀ 0.05). Results of the study affirm the hypothesis that a six-week neuromuscular training intervention can improve performance and movement competency in youth netball player
