    Family lifestyle dynamics and childhood obesity: evidence from the millennium cohort study.

    Abstract Background The prevalence of childhood obesity has been increasing but the causes are not fully understood. Recent public health interventions and guidance aiming to reduce childhood obesity have focused on the whole family, as opposed to just the child but there remains a lack of empirical evidence examining this relationship. Methods Using data from the longitudinal Millennium Cohort Study (MCS), we investigate the dynamic relationship between underlying family lifestyle and childhood obesity during early childhood. The MCS interviewed parents shortly after the birth of their child and follow up interviews were carried out when the child was 3, 5 and 7 years. We use a dynamic latent factor model, an approach that allows us to identify family lifestyle, its evolution over time (in this case between birth and 7 years) and its influence on childhood obesity and other observable outcomes. Results We find that family lifestyle is persistent, 87.43% of families which were above the 95th percentile on the lifestyle distribution, remained above the 95th percentile when the child was 7 years old. Family lifestyle has a significant influence on all outcomes in the study, including diet, exercise and parental weight status; family lifestyle accounts for 11.3% of the variation in child weight by age 7 years. Conclusion The analysis suggests that interventions should therefore be prolonged and persuasive and target the underlying lifestyle of a family as early as possible during childhood in order to have the greatest cumulative influence. Our results suggest that children from advantaged backgrounds are more likely to be exposed to healthier lifestyles and that this leads to inequalities in the prevalence of obesity. To reduce inequalities in childhood obesity, policy makers should target disadvantaged families and design interventions specifically for these families

    Accelerated resolution therapy: an innovative mental health intervention to treat post traumatic stress disorder

To access the final edited and published work see http://jramc.bmj.com/content/162/2/90.full.Post-traumatic stress disorder (PTSD) is a disabling trauma and stress-related disorder that may occur after a person experiences a traumatic event, and evokes a combination of intrusion and avoidance symptoms, negative alterations in cognitions and mood, and alterations in arousal and reactivity. Accelerated resolution therapy (ART) is an emerging psychotherapy that provides fast and lasting resolution for mental health problems such as PTSD. ART has been shown to achieve a positive result in one to five sessions, typically over a 2-week period, and requires no homework, skills practice or repeated exposure to targeted events. Initial research, including one randomised control trial, has demonstrated that ART interventions can significantly reduce symptoms of psychological trauma in both civilians and US service members and veterans. These results suggest that ART be considered as either a primary treatment option or for refractory PTSD in those with a suboptimal response to endorsed first-line therapies. Conservative estimates indicate substantial potential cost savings in PTSD treatment. Despite the need for more definitive clinical trials, there is increasing interest in ART in the USA, including in the US Army. The growing positive empirical evidence is compelling, and there appears to be sufficient evidence to warrant UK researchers undertaking ART research. The armed forces offer the potential for comparative international trials. However, equally important are veterans, emergency services personnel and those subjected to violence. ART appears to also have application in other conditions, including depression, anxiety disorders, and alcohol or drug misuse. ART can potentially help personnel traumatised by the unique challenges of war and conflict zones by providing brief psychotherapy in a readily accessible and culturally competent manner. ART facilitates the provision of interventions and resolutions in theatre, thus enhancing forces’ fighting capability

    Cancer Screening Practices Among Chinese and Vietnamese in the Greater Houston Area

    National data on Asian Americans indicate that compared to other groups in the US, cancer incidence and prevalence is relatively lower (Miller, Kolonel et al. 1996; American Cancer Society 2006). However, when the data is examined further based on specific Asian subgroups and for specific cancers, Asian Americans bear a disproportionate burden for cancers of infectious origin, such as cervical, liver, and stomach cancer (Chen 2005). Furthermore, Asian Americans are also experiencing increasing rates of cancers associated with “Westernization,” such as breast and prostate cancer (Kolonel, Yoshizawa et al. 1988; Whittemore, Kolonel et al. 1995; Ziegler, Hoover et al. 1996). Early detection and screening are among the frontline strategies in cancer control, yet Asian American and Pacific Islanders have the lowest cancer screening rates of all ethnic groups in the US (American Cancer Society 2006). The Asian American Health Needs Assessment (AsANA) project was designed to collect data on the rapidly growing Asian American community in the Greater Houston area. The AsANA project included a telephone survey to over 800 randomly selected households in the Chinese and Vietnamese communities, two of the largest Asian American subgroups in Texas. Included in the telephone interview were questions regarding cancer screening practices. This article describes the reported screening practices among the surveyed populations and discusses the implications for developing targeted programs that can address cancer screening disparities in this community

    The Stellar Population of h and chi Persei: Cluster Properties, Membership, and the Intrinsic Colors and Temperatures of Stars

    (Abridged) From photometric observations of \sim 47,000 stars and spectroscopy of \sim 11,000 stars, we describe the first extensive study of the stellar population of the famous Double Cluster, h and χ\chi Persei, down to subsolar masses. Both clusters have E(B-V) \sim 0.52--0.55 and dM = 11.8--11.85; the halo population, while more poorly constrained, likely has identical properties. As determined from the main sequence turnoff, the luminosity of M supergiants, and pre-main sequence isochrones, ages for h Persei, χ\chi Persei and the halo population all converge on \approx 14 Myr. From these data, we establish the first spectroscopic and photometric membership lists of cluster stars down to early/mid M dwarfs. At minimum, there are \sim 5,000 members within 10' of the cluster centers, while the entire h and χ\chi Persei region has at least \sim 13,000 and as many as 20,000 members. The Double Cluster contains \approx 8,400 M_{\odot} of stars within 10' of the cluster centers. We estimate a total mass of at least 20,000 M_{\odot}. We conclude our study by outlining outstanding questions regarding the properties of h and χ\chi Persei. From comparing recent work, we compile a list of intrinsic colors and derive a new effective temperature scale for O--M dwarfs, giants, and supergiants.Comment: 88 pages, many figures, Accepted for publication in The Astrophysical Journal Supplements.     Lifestyle factors and ovarian cancer outcomes

    Purpose: Few studies have reported on the lifestyle characteristics of ovarian cancer survivors. The objectives of this study were to characterize the associations between physical activity (PA) and body size (BS) with health-related quality of life (HRQOL) and ovarian cancer recurrence in a sample of regional and distal stage ovarian cancer survivors. Methods: Epithelial ovarian cancer survivors in their first clinical remission, with no evidence of recurrent disease were identified from The University of Texas MD Anderson Cancer Center tumor registry. A total of 51 survivors consented to participate in a battery of self-reported questionnaires. Trained staff collected data on anthropometric and recurrence data were collected from the tumor registry. Generalized linear models were used to assess the relationship between PA, BS, and HRQOL. Cox proportional hazard models were used to assess the associations between PA, BS, and recurrence-free survival. Results: Most (59%) women were overweight or obese (BMI \u3c 25 kg/m2) , 49% met current guidelines for PA (150 minutes of moderate to vigorous PA/week), and 29% displayed characteristics of abdominal obesity (\u3e88 centimeters). Women who were not obese reported significantly higher (better) overall HRQOL (point difference = 10.8, P \u3c 0.05) and mental health (point difference = 12.4, P \u3c 0.05) scores than women who were obese. Elevated waist circumference and physical activity were not significantly associated with HRQOL outcomes and we did not find any associations between lifestyle behaviors and recurrence free survival (all P \u3e 0.05). Conclusions: Ovarian cancer survivors with characteristics of overall and abdominal obesity may be at risk for deficits in HRQOL and could benefit from interventions designed to reduce weight. More research is needed to determine whether meeting guidelines for physical activity is associated with improvements in health outcomes this population

    A Spectral Classification System for Hydrogen-deficient Carbon Stars

    Stellar spectral classification, and especially the Yerkes system, has been highly useful in the study of stars. While there is a currently accepted classification system for carbon stars, the subset of Hydrogen-deficient Carbon (HdC) stars has not been well described by such a system, due in part to their rarity and their variability. Here we present a new system for the classification of HdCs based on their spectra, which is made wholly on their observable appearance. We use a combination of dimensionality reduction and clustering algorithms with human classification to create such a system. We classify over half of the known sample of HdC stars using this, and roughly calibrate the temperatures of each class using their colors. Additionally, we express trends in the occurrence of certain spectral peculiarities such as the presence of Hydrogen and Lithium lines. We also present three previously unpublished spectra, and report the discovery of three new Galactic dustless HdC (dLHdC) stars and additionally discuss one especially unique star that appears to border between the hottest HdCs and the coolest Extreme Helium (EHe) stars.Comment: 30 pages, 18 figures, submitted to MNRA

    Length of Stay in Ambulatory Surgical Oncology Patients at High Risk for Sleep Apnea as Predicted by STOP-BANG Questionnaire

    Background. The STOP-BANG questionnaire has been used to identify surgical patients at risk for undiagnosed obstructive sleep apnea (OSA) by classifying patients as low risk (LR) if STOP-BANG score < 3 or high risk (HR) if STOP-BANG score ≥ 3. Few studies have examined whether postoperative complications are increased in HR patients and none have been described in oncologic patients. Objective. This retrospective study examined if HR patients experience increased complications evidenced by an increased length of stay (LOS) in the postanesthesia care unit (PACU). Methods. We retrospectively measured LOS and the frequency of oxygen desaturation (<93%) in cancer patients who were given the STOP-BANG questionnaire prior to cystoscopy for urologic disease in an ambulatory surgery center. Results. The majority of patients in our study were men (77.7%), over the age of 50 (90.1%), and had BMI < 30 kg/m2 (88.4%). STOP-BANG results were obtained on 404 patients. Cumulative incidence of the time to discharge between HR and the LR groups was plotted. By 8 hours, LR patients showed a higher cumulative probability of being discharged early (80% versus 74%, P=0.008). Conclusions. Urologic oncology patients at HR for OSA based on the STOP-BANG questionnaire were less likely to be discharged early from the PACU compared to LR patients

    De novo valve tissue morphology following bioscaffold mitral valve replacement in a juvenile non-human primate model

    The utility of implanting a bioscaffold mitral valve consisting of porcine small intestinal submucosa (PSIS) in a juvenile baboon model (12 to 14 months old at the time of implant; n = 3) to assess their in vivo tissue remodeling responses was investigated. Our findings demonstrated that the PSIS mitral valve exhibited the robust presence of de novo extracellular matrix (ECM) at all explantation time points (at 3-, 11-, and 20-months). Apart from a significantly lower level of proteoglycans in the implanted valve’s annulus region (p \u3c 0.05) at 3 months compared to the 11-and 20-month explants, there were no other significant differences (p \u3e 0.05) found between any of the other principal valve ECM components (collagen and elastin) at the leaflet, annulus, or chordae tendinea locations, across these time points. In particular, neochordae tissue had formed, which seamlessly integrated with the native papillary muscles. However, additional processing will be required to trigger accelerated, uniform and complete valve ECM formation in the recipient. Regardless of the specific processing done to the bioscaffold valve, in this proof-of-concept study, we estimate that a 3-month window following bioscaffold valve replacement is the timeline in which complete regeneration of the valve and integration with the host needs to occur
