Épidémiologie de la maladie d’Alzheimer et des syndromes apparentés
La maladie d’Alzheimer et les syndromes apparentés (c’est-à -dire les autres causes de démences, essentiellement) sont devenus un problème majeur de santé publique en France et sont en passe de devenir, enfin, une priorité, en raison du vieillissement de la population. L’épidémiologie de ces affections est un des éléments essentiels pour la décision en santé publique et pour la connaissance de ces maladies et de leurs déterminants. Selon les estimations les plus raisonnables, il y aurait actuellement en France environ 850 000 cas de maladie d’Alzheimer et syndromes apparentés, et environ 220 000 nouveaux cas par an. Les progrès de la prise en charge font que la durée de la maladie augmente, ainsi que le nombre des malades. En dehors du développement d’un traitement curatif bien peu probable à court terme, seule une politique de prévention devrait permettre de contenir l’accroissement de ce nombre dans un proche avenir. Quatre pistes de prévention sont crédibles : les facteurs de risques cardiovasculaire, notamment l’hypertension artérielle ; la nutrition ; les activités stimulantes et physiques ; l’équilibre affectif et social.Alzheimer’s disease and related disorders (dementia) are a major public health problem due to the number of cases in the general population, the projections for the future, and the consequences of these diseases. We can estimate that about 850 000 cases of dementia were present in France in 2005 and this number will increase to 1 200 000 in 2020 and 2 100 000 in 2040 if the incidence and the duration of the disease did not change. Dairy Product Intake and Long-Term Risk for Frailty among French Elderly Community Dwellers
Dairy products (DP) are part of a food group that may contribute to the prevention of physical frailty. We aimed to investigate DP exposure, including total DP, milk, fresh DP and cheese, and their cross-sectional and prospective associations with physical frailty in community-dwelling older adults. The cross-sectional analysis was carried out on 1490 participants from the Three-City Bordeaux cohort. The 10-year frailty risk was examined in 823 initially non-frail participants. A food frequency questionnaire was used to assess DP exposure. Physical frailty was defined as the presence of at least 3 out of 5 criteria of the frailty phenotype: weight loss, exhaustion, slowness, weakness, and low physical activity. Among others, diet quality and protein intake were considered as confounders. The baseline mean age of participants was 74.1 y and 61% were females. Frailty prevalence and incidence were 4.2% and 18.2%, respectively. No significant associations were observed between consumption of total DP or DP sub-types and frailty prevalence or incidence (OR = 1.40, 95%CI 0.65–3.01 and OR = 1.75, 95%CI 0.42–1.32, for a total DP consumption >4 times/d, respectively). Despite the absence of beneficial associations of higher DP consumption on frailty, older adults are encouraged to follow the national recommendations regarding DP

Evolution of dementia diagnosis over time (1988-2013): Evidence from French and English cohorts. Implication for secular trends analyses.
INTRODUCTION: The aims of this study are to examine the evolution of clinical dementia diagnosis over 3 decades and to investigate secular trends of dementia. METHODS: Four cohorts covering a period from 1988 to 2013 were used: the Personnes Agées Quid and Three-City-Bordeaux studies, and the Cognitive Function and Aging Study (CFAS) I and II. Mini-Mental State Examination scores at clinical diagnosis were evaluated over a 24-year follow-up period in French studies. An algorithmic approach was applied to CFAS I and II to provide dementia prevalence and incidence estimates. RESULTS: A significant increase of the Mini-Mental State Examination score at diagnosis was observed until 2000 and a significant decrease after. We reported a prevalence of 8.8% for CFAS I (1990-1993) compared with a prevalence of 6.5% in CFAS II (2008-2011). The 2-year incidence rate was estimated at 31.2/1000 (95% confidence interval = 28.0-34.8) for CFAS I and 15.0/1000 (95% confidence interval = 13.5-16.7) for CFAS II. DISCUSSION: Applying a stable algorithm to different cohorts across time can provide a robust method for time trends estimation.Includes MRC grants

Purpose: We investigated the cross-sectional associations between macular pigment optical density (MPOD), plasma lutein (L), and zeaxanthin (Z) concentrations and cognitive function in 184 older adults of the 3-City-Bordeaux cohort. Methods: MPOD was measured using the two-wavelength autofluorescence method with a modified scanning laser ophthalmoscope. Plasma L and Z (L+Z) concentrations were determined by high-performance liquid chromatography and were considered either crude or expressed as a ratio of the concentration of plasma lipids (total cholesterol [TC] + triglycerides [TG]). Cognitive performances were assessed using the following four separate neuropsychological tests: the Mini-Mental State Examination (MMSE), the Isaacs Set Test (IST), the Benton Visual Retention Test (BVRT), and the sum of the three free recalls of the Free and Cued Selective Reminding Test (FCSRT). These test results were summarized by a composite global cognitive z-score. Results: Higher MPOD at 0.5 degrees was significantly associated with a higher composite z-score (beta = 0.15, 95% confidence interval [CI] 0.04-0.26), higher BVRT (beta = 0.39, 95%CI 0.08-0.70), and higher IST (beta = 1.16, 95%CI 0.11-2.22) performances. Higher plasma L+Z concentrations were significantly associated with higher IST scores (beta = 0.97, 95%CI 0.01-1.94). Furthermore, a higher L+Z/TC+TG ratio was associated with a higher composite z-score (beta = 0.12, 95%CI 0.01-0.23), along with higher IST (beta = 1.02, 95%CI 0.002-2.04) and FCSRT (beta = 1.55, 95%CI 0.41-2.69) performances. Conclusions: This analysis suggested that both higher MPOD and L+Z concentrations were significantly associated with higher cognitive performances. However, MPOD measurements have the advantage of being a fast and representative measure of long-term carotenoid intake
Patient-perceived progression in multiple system atrophy: natural history of quality of life
Health-related quality of life (Hr-QoL) scales provide crucial information on
neurodegenerative disease progression, help improving patient care, and
constitute a meaningful endpoint for therapeutic research. However, Hr-QoL
progression is usually poorly documented, as for multiple system atrophy (MSA),
a rare and rapidly progressing alpha-synucleinopathy. This work aimed to
describe Hr-QoL progression during the natural course of MSA, explore
disparities between patients, and identify informative items using a four-step
statistical strategy.We leveraged the data of the French MSA cohort comprising
annual assessments with the MSA-QoL questionnaire for more than 500 patients
over up to 11 years. The four-step strategy (1) determined the subdimensions of
Hr-QoL in MSA; (2) modelled the subdimension trajectories over time, accounting
for the risk of death; (3) mapped the sequence of item impairments with disease
stages; and (4) identified the most informative items specific to each disease
stage.Among the 536 patients included, 50% were women and they were aged on
average 65.1 years old at entry. Among them, 63.1% died during the follow-up.
Four dimensions were identified. In addition to the original motor, nonmotor,
and emotional domains, an oropharyngeal component was highlighted. While the
motor and oropharyngeal domains deteriorated rapidly, the nonmotor and
emotional aspects were already slightly to moderately impaired at cohort entry
and deteriorated slowly over the course of the disease. Impairments were
associated with sex, diagnosis subtype, and delay since symptom onset. Except
for the emotional domain, each dimension was driven by key identified
items.Hr-QoL is a multidimensional concept that deteriorates progressively over
the course of MSA and brings essential knowledge for improving patient care. As
exemplified with MSA, the thorough description of Hr-QoL using the 4-step
original analysis can provide new perspectives on neurodegenerative diseases'
management to ultimately deliver better support focused on the patient's
perspective
Mediterranean Diet Adherence and Risk of Depressive Symptomatology in a French Population-Based Cohort of Older Adults
Several foods from the Mediterranean Diet (MeDi) have already been characterized as beneficial for depression risk, while studies focusing on adherence to the overall MeDi are lacking among older adults at higher risk of depression. The aim of this study was to assess the association between MeDi adherence and the risk of depressive symptomatology (DS) in an older French cohort followed for 15 years. Participants from the Three-City Bordeaux cohort answered a food frequency questionnaire used to assess their MeDi adherence. The Center for Epidemiologic Studies Depression (CES-D) scale score of 16 or greater and/or use of antidepressant treatment ascertained at each visit defined incident DS. Random-effect logistic regression models were adjusted for potential confounders. Among 1018 participants, aged 75.6 years (SD 4.8 years) on average at baseline, 400 incident cases of DS were identified during the follow-up. Only when restricting the definition of DS to a CES-D score ≥ 16 was a borderline-significant trend towards a benefit of greater adherence to the MeDi with reduced odds of DS found (p-value = 0.053). In this large sample of older French adults, a potential benefit of greater adherence to the MeDi regarding the risk of DS would depend on the definition of DS

Studies using health administrative databases (HAD) may lead to biased results since information on potential confounders is often missing. Methods that integrate confounder data from cohort studies, such as multivariate imputation by chained equations (MICE) and two-stage calibration (TSC), aim to reduce confounding bias. We provide new insights into their behavior under different deviations from representativeness of the cohort. We conducted an extensive simulation study to assess the performance of these two methods under different deviations from representativeness of the cohort. We illustrate these approaches by studying the association between benzodiazepine use and fractures in the elderly using the general sample of French health insurance beneficiaries (EGB) as main database and two French cohorts (Paquid and 3C) as validation samples. When the cohort was representative from the same population as the HAD, the two methods are unbiased. TSC was more efficient and faster but its variance could be slightly underestimated when confounders were non-Gaussian. If the cohort was a subsample of the HAD (internal validation) with the probability of the subject being included in the cohort depending on both exposure and outcome, MICE was unbiased while TSC was biased. The two methods appeared biased when the inclusion probability in the cohort depended on unobserved confounders. When choosing the most appropriate method, epidemiologists should consider the origin of the cohort (internal or external validation) as well as the (anticipated or observed) selection biases of the validation sample
