Magnetic resonance imaging features of craniofacial fibrous dysplasia
Purpose: To assess the value of magnetic resonance imaging (MRI) in detecting craniofacial fibrous dysplasia (CFD) and diagnosing and differentiating it from intraosseous meningioma. Additionally, the MRI appearance of the typical computed tomography (CT) imaging feature, the ground glass phenomenon, was evaluated. Material and methods: MRI datasets of 32 patients with CFD were analysed retrospectively. Detectability in MRI was assessed by analysis of 10 randomly selected patients with CFD and 10 normal controls by two blinded readers. Changes of affected bone, internal lesion structure, T1 and T2 signal intensity, and contrast enhancement of the lesion in general and ground glass areas in particular were assessed. Ten patients with intraosseous meningioma (one in each) served as differential diagnosis for CFD. Results: All 10 CFD lesions were reliably detected in MRI. In 32 patients 36 CFD lesions were evaluated. In 66.7% CFD were iso- to hypointense in T1 and hyperintense in T2; this proportion was similar for ground glass areas (65.7%). Ground glass areas were more homogeneously structured than the whole CFD lesion in both T1 (100% vs. 56%, respectively) and T2 (91% vs. 61%, respectively). Contrast enhancement was found in 97% of complete CFD lesions and 93% of ground glass areas. The accuracy for CFD vs. intraosseous meningioma was 100% for 'no soft-tissue component' and 98% for âbone broadening' in MRI. Conclusions: Distinct morphological changes of CFD are reliably detected in MRI and allow differentiation from intraosseous meningioma. Areas with ground glass phenomenon in CT show a predominantly homogenous internal structure in MRI with contrast enhancement
Increased incidence trend of low-grade and high-grade neuroendocrine neoplasms
Purpose: The incidence of neuroendocrine neoplasms is increasing. This work aimed at: (i) establishing worldwide incidence trend of low-grade neuroendocrine neoplasms; (ii) defining the incidence and temporal trend of high-grade neuroendocrine neoplasms in USA utilizing the Surveillance Epidemiology and End Results database; (iii) comparing trends for low-grade vs. high-grade neuroendocrine neoplasms. Methods: We conducted a literature search on MEDLINE and Scopus databases and incidence trends were plotted for 1973-2012. The Surveillance Epidemiology and End Results database was used to identify incidence rates in USA for 1973-2012. Incidence rates were stratified according to histological grade, gender and ethnicity. Trends were summarized as annual percent change and corresponding 95% confidence interval. Results: 11 studies were identified involving 72,048 cases; neuroendocrine neoplasm incidence rates increased over time in all countries for all sites, except for appendix. In Surveillance Epidemiology and End Results low-grade neuroendocrine neoplasm incidence rate increased from 1.09 in 1973 to 3.51 per 100,000 in 2012. During this interval, high-grade neuroendocrine neoplasm incidence rate increased from 2.54 to 10.52 per 100,000. African Americans had the highest rates of digestive neuroendocrine neoplasms with male prevalence in high-grade. Conclusions: Our data indicate an increase in the incidence of neuroendocrine neoplasms as a worldwide phenomenon, affecting most anatomical sites and involving both low-grade and high-grade neoplasms. © 2017, The Author(s)
Strangers on TV : the representation of ethnic minority groups in entertainmentformats of German television
Im Jahr 2009 ĂŒberschritt die Anzahl von Personen mit Migrationshintergrund in der Bundesrepublik Deutschland erstmals die 16 Millionen Marke, wodurch deutlich wird, dass Deutschland mittlerweile Heimat fĂŒr Menschen unterschiedlichster Herkunft geworden ist, sogenannten allochthonen Gruppen. Der daraus entstehende Pluralismus in der Gesellschaft ist einerseits förderlich, kann andererseits aber auch Konflikte verursachen. Massenmedien können durch die Art und Weise, wie sie allochthonen Gruppen darstellen, auf die Einstellung der Gesellschaft bezĂŒglichen Menschen mit Migrationshintergrund einwirken und sie positiv oder negativ beeinflussen. Das Fernsehen ist dabei weiterhin eines der meistgenutzten Medien, welches zu quotenstarken Zeiten vor allem Unterhaltungsformate sendet, die ein breites Publikum erreichen. Die vorliegende Arbeit beschĂ€ftigt sich nun mit der Frage, in wie weit die Darstellung von Migranten in fiktiven, deutschen Unterhaltungsformaten ihren Integrationsverlauf widerspiegelt. Dazu werden beispielhafte Unterhaltungsformate aus dem Zeitraum der 1970er Jahre bis in die Gegenwart ausgewĂ€hlt und darauf untersucht, ob allochthone Gruppen im Fernsehprogramm als Teil der Gesellschaft und somit integriert dargestellt werden oder ob der Fokus ihrer Darstellung auf den fremden Eigenschaften der Migranten liegt
Charakteristika und Therapie gastroenteropankreatischer neuroendokriner Neoplasien G3
Introduction: The WHO classification of 2010 defines neuroendocrine carcinoma
(NEC) G3 as poorly differentiated neuroendocrine neoplasm (NEN) with a
proliferation rate exceeding 20%. The subgroup of NEC account for 10-20% of
the rare disease of neuroendocrine neoplasms and usually show a poor
prognosis. Based on WHO classification NEC G3 are only classified by its
grading. However, there is evidence of well differentiated neuroendocrine
tumors with a proliferation rate exceeding twenty percent. This subgroup of
well differentiated neuroendocrine tumors with a G3 grading (NET G3) is not
defined by the WHO classification so far. The aim of this retrospective,
multicentric study was the assessment and characterization of NEN G3 and the
differentiation of NET G3 and NEC with regard to biological behavior,
prognosis and response to treatment. Methods: Eight European centers
specialized in NET assessed clinical and pathological features of NET G3 and
NEC. Finally, 204 patients were analyzed (37 NET G3 and 167 NEC). Results:
Median age was 64 (21-89) years. Tumor origin included pancreas (32%) and
colon-rectum (27%). Median Ki-67 index was 70% and was significantly lower in
NET G3 compared to NEC (30% vs. 80%, P<0.001). Median overall survival (OS)
for all patients was 23 (95% CI: 18â28) months and proofed to be significantly
higher in NET G3 (99 vs 17 months; HR=8.3; P<0.001) compared to NEC. Platinum-
etoposide first line chemotherapy was administered in 68% (n=113) of NEC and
32% (n=12) NET G3 patients. In NEC disease control rate and progression free
survival (PFS) after first line therapy was significantly higher compared to
NET G3 (P<0.05). In contrast overall survival was significantly prolonged in
NET G3 (P=0.003). Conclusions: Within G3 neuroendocrine neoplasms a subgroup
of well differentiated neuroendocrine tumors exists, that is not taken into
account by the actual WHO classification. This study observed significant
differences between NET G3 and NEC regarding proliferation rate and prognosis.
NET G3 seem to show limited response to first line treatment with Platinum-
etoposide. At the participating NET centers NET G3 were mainly treated with
alternative treatment modalities that are routinely used in NET G2. The
subgroup of NET G3 should be considered by the WHO classification due to its
divergent clinical behavior. Standard treatment of NET G3 remains unclear and
needs to be elucidated in prospective comparative trials.EinfĂŒhrung: Nach der WHO Klassifikation von 2010 werden
gastroenteropankreatische neuroendokrine Karzinome (NEC) G3 als schlecht
differenzierte neuroendokrine Neoplasien (NENs) mit einer Wachstumsfraktion
von ĂŒber 20% definiert. Die Subgruppe der NEC macht lediglich 10-20% der
ohnehin seltenen neuroendokrinen Neoplasien aus und zeigen eine schlechte
Prognose. Basierend auf der WHO Klassifikation werden NEC G3 lediglich durch
das Grading klassifiziert. Es gibt jedoch Hinweise, dass auch gut
differenzierte neuroendokrine Tumore eine Proliferationsrate von ĂŒber 20%
aufweisen können. Die Subgruppe gut differenzierter neuroendokriner Tumore mit
einer G3 Wachstumsfraktion (NET G3) ist bisher durch die WHO Klassifikation
nicht definiert. Das Ziel dieser retrospektiven, multizentrischen Studie war
die Erfassung und Charakterisierung der NEN G3 sowie der Vergleich von NET G3
und NEC bezĂŒglich des biologischen Verhaltens, der Prognose und des
Therapieansprechens. Methodik: In acht europÀischen spezialisierten NET -
Zentren erfolgte die Erfassung klinischer und pathologischer Charakteristika
von NET G3 und NEC. Es wurden 204 Patienten untersucht (37 NET G3 und 167
NEC). Ergebnisse: Das mediane Alter betrug 64 (21-89) Jahre. Der Primarius war
ĂŒberwiegend im Pankreas (32%) und Kolon bzw. Rektum (27%) lokalisiert. Der
mediane Ki-67 Index betrug 70% und war bei NET G3 signifikant niedriger als
bei NEC (30% vs. 80%, P<0,001). Das mediane GesamtĂŒberleben betrug 23 Monate
(95% CI: 18-28) und war bei Patienten mit NET G3 signifikant verlÀngert (99
vs. 17 Monate; HR=8,3; P<0,001) im Vergleich zu NEC. Achtundsechzig Prozent
(N=113) der Patienten mit NEC und 32% (N=12) der Patienten mit NET G3
erhielten eine Erstlinien-Chemotherapie mit Platin/Etoposid. Bei Patienten mit
NEC war das progressionsfreie Ăberleben und die Rate von Tumorstabilisierung
bzw. Remission nach Erstlinientherapie im Vergleich zu NET G3 deutlich erhöht
(P<0,05). WĂ€hrenddessen war das GesamtĂŒberleben bei NET G3 signifikant
verlÀngert (P=0,003). Schlussfolgerung: Innerhalb der NEN G3 gibt es eine
Subgruppe gut differenzierter neuroendokriner Tumore, die in der aktuellen WHO
Klassifikation nicht berĂŒcksichtigt werden. Diese Studie zeigt signifikante
Unterschiede zwischen NET G3 und NEC bezĂŒglich der Wachstumsfraktion und der
Prognose. Es gibt zudem Hinweise, dass NET G3 ein schlechteres Ansprechen auf
die standardisierte Erstlinientherapie mit Cisplatin/Etoposid zeigen. An den
hier beteiligten zertifizierten NET-Zentren erhielten Patienten mit NET G3
ĂŒberwiegend alternative TherapiemodalitĂ€ten, welche zur Behandlung von NET G2
eingesetzt werden. Die Subgruppe der NET G3 sollte aufgrund des
unterschiedlichen biologischen Verhaltens in der WHO Klassifikation
berĂŒcksichtigt werden. Die Standardtherapie der NET G3 ist unklar und muss in
weiteren prospektiven komparativen Studien untersucht werden

Untersuchungen zur biofunktionellen Kompartimentbildung beim Schlucken mittels peroraler kontrollierter FlĂŒssigkeitszufuhr und intraoraler Manometrie
Hintergrund: Das biofunktionelle Modell nach Engelke erlaubt den Schluckakt als einen Vorgang aufzufassen, bei dem es zur Verschlussbildung intraoraler FunktionsrĂ€ume mit Bildung von negativen intraoralen Druckamplituden kommt. Die Propulsion des Bolus kann als Transport entlang eines DruckgefĂ€lles zwischen mehreren FunktionsrĂ€umen verstanden werden. Dies ergĂ€nzt bisherige Auffassungen einer aktiven myogenen Transportfunktion von Zunge und orofazialer Muskulatur.Material und Methoden: Mit Hilfe eines Vakuumaktivators (SilencosÂź Bredent, Senden, Deutschland) und eines angeschlossenen Infusionssystems wird eine fraktionierte kontrollierte FlĂŒssigkeitszufuhr (2-20 ml/min) in die Mundhöhle eingefĂŒhrt. Ăber ein handgehaltenes Druckmesssystem (GMS 3150, Greisinger, Erlangen, Deutschland) erfolgt eine kontinuierliche intraorale Manometrie des interokklusalen Funktionsraumes. Bestimmt werden die HĂ€ufigkeit von Druckamplituden sowie deren Höhe als Funktion der Zeit in AbhĂ€ngigkeit zur FlĂŒssigkeitszufuhr.Ergebnisse: Bei 20 gesunden Patienten, bei denen zahnĂ€rztlich eine Okklusionsstörung ausgeschlossen wurde und klinisch keine Schluckstörung bestand, wurde die Schluckfrequenz sowie die Amplitude der negativen Druckspitzen gemessen. Hierbei lieĂen sich wĂ€hrend des Schluckaktes negative Druckwerte nachweisen, nicht jedoch im Ruhezustand.Diskussion: Durch Verwendung der hier gezeigten Technik lassen sich objektive Parameter fĂŒr die Beurteilung des normalen Schluckvorganges darstellen und zur Grundlage eines klinischen manometrischen PrĂŒfverfahrens bei Schluckstörungen entwickeln

Potential Analysis Of Flexible Small Series Production Of Spare Parts By Direct Polymer Additive Tooling
In recent decades, there has been an increasing creation of vehicle model variants and product
individualization for customer needs. There is a need for a greater variety of spare parts which is leading to
high requirements on their logistics. As a result of this trend, automotive production companies have to
ensure the provision of many different spare parts as part of their post-fulfillment obligations. However, the
surplus spare parts stock will increase the operation cost of the company. This implies the need for a more
flexible approach to spare parts production and provision to improve profitability compared to existing
process chains. In this research paper, the potential of flexible spare parts production and provision is
discussed. For this purpose, the state of spare part production technologies and existing approaches for spare
part service are analyzed regarding their technological characteristics, flexibility as well as cost structure.
Following this, an approach for flexible production for spare parts with regard to its potential is analyzed
and demonstrated based on three exemplary automotive use cases using additively manufactured production
resources. In the casting use case, a Fused Filament Fabricated (FFF) additively manufactured speedometer
screw model will be used as a sand casting form. The production of deep-drawn car body parts using
polymer-based FFF forming tools is investigated in the second use case. Lastly, the production of ignition
distributor caps by PolyJet Modelling (PJM) manufactured Injection Molding (PUR-RIM) molds is
presented
