    The acquisition system which operates in conjunction with a balloon-borne TV system, boresighted to a telescope is described. It has two main functions, a star field monitor and an offset star tracker. The design of the system was strongly influenced by the TV camera, which uses the same interlaced scanning system as is employed in commercial television broadcasting. To reduce power and bandwidth requirements, the star field information transmitted in our system consists only of the horizontal and vertical coordinates of each star and its brightness. As a star field monitor the system provides video thresholding, camera blemish suppression, coordinate digitization in 3 axes, circuity to recognize as single star the dispersed video signals resulting from one star overlapping adjacent scanning lines and storage of all signals for readout by the telemetry at appropriate times

    Propofol-Based Procedural Sedation with or without Low-Dose Ketamine in Children

    Objective Examine comparative dosing, efficacy, and safety of propofol alone or with an initial, subdissociative dose of ketamine approach for deep sedation. Background Propofol is a sedative-hypnotic agent used increasingly in children for deep sedation. As a nonanalgesic agent, use in procedures (e.g., bone marrow biopsies/aspirations, renal biopsies) is debated. Our intensivist procedural sedation team sedates using one of two protocols: propofol-only (P-O) approach or age-adjusted dose of 0.25 or 0.5 mg/kg intravenous ketamine (K + P) prior to propofol. With either approach, an initial induction dose of 1 mg/kg propofol is recommended and then intermittent dosing throughout the procedure to achieve adequate sedation to safely and effectively perform the procedure. Approach: Retrospective evaluation of 754 patients receiving either the P-O or K + P approach to sedation. Results A total of 372 P-O group patients and 382 K + P group. Mean age (7.3 ± 5.5 years for P-O; 7.3 ± 5.4 years for K + P) and weight (30.09 ± 23.18 kg for P-O; 30.14 ± 24.45 kg for K + P) were similar in both groups (p = NS). All patients successfully completed procedures with a 16% combined incidence of hypoxia (SPO2 < 90%). Procedure time was 3 minutes longer for K + P group than P-O group (18.68 ± 15.13 minutes for K + P; 15.11 ± 12.77 minutes for P-O; p < 0.01), yet recovery times were 5 minutes shorter (17.04 ± 9.36 minutes for K + P; 22.17 ± 12.84 minutes for P-O; p < 0.01). Mean total dose of propofol was significantly greater in P-O than in K + P group (0.28 ± 0.20 mg/kg/min for K + P; 0.40 ± 0.26 mg/kg/min for P-O; p < 0.0001), and might explain the shorter recovery time. Conclusion Both sedation approaches proved to be well tolerated and equally effective. Addition of ketamine was associated with reduction in the recovery time, probably explained by the statistically significant decrease in the propofol dose

    Characterisation of spatial network-like patterns from junctions' geometry

    We propose a new method for quantitative characterization of spatial network-like patterns with loops, such as surface fracture patterns, leaf vein networks and patterns of urban streets. Such patterns are not well characterized by purely topological estimators: also patterns that both look different and result from different morphogenetic processes can have similar topology. A local geometric cue -the angles formed by the different branches at junctions- can complement topological information and allow to quantify the large scale spatial coherence of the pattern. For patterns that grow over time, such as fracture lines on the surface of ceramics, the rank assigned by our method to each individual segment of the pattern approximates the order of appearance of that segment. We apply the method to various network-like patterns and we find a continuous but sharp dichotomy between two classes of spatial networks: hierarchical and homogeneous. The first class results from a sequential growth process and presents large scale organization, the latter presents local, but not global organization.Comment: version 2, 14 page

    The organisational and human resource challenges facing primary care trusts : protocol of a multiple case study

    BACKGROUND: The study is designed to assess the organisational and human resource challenges faced by Primary Care Trusts (PCTs). Its objectives are to: specify the organisational and human resources challenges faced by PCTs in fulfilling the roles envisaged in government and local policy; examine how PCTs are addressing these challenges, in particular, to describe the organisational forms they have adopted, and the OD/HR strategies and initiatives they have planned or in place; assess how effective these structures, strategies and initiatives have been in enabling the PCTs to meet the organisational and human resources challenges they face; identify the factors, both internal to the PCT and in the wider health community, which have contributed to the success or failure of different structures, strategies and initiatives. METHODS: The study will be undertaken in three stages. In Stage 1 the key literature on public sector and NHS organisational development and human resources management will be reviewed, and discussions will be held with key researchers and policy makers working in this area. Stage 2 will focus on detailed case studies in six PCTs designed to examine the organisational and human resources challenges they face. Data will be collected using semi-structured interviews, group discussion, site visits, observation of key meetings and examination of local documentation. The findings from the case study PCTs will be cross checked with a Reference Group of up to 20 other PCG/Ts, and key officers working in organisational development or primary care at local, regional and national level. In Stage 3 analysis of findings from the preparatory work, the case studies and the feedback from the Reference Group will be used to identify practical lessons for PCTs, key messages for policy makers, and contributions to further theoretical development

    Effect of the Synthetic Bile Salt Analog CamSA on the Hamster Model of Clostridium difficile Infection

    Clostridium difficile infection (CDI) is the leading cause of antibiotic-associated diarrhea and has gained worldwide notoriety due to emerging hypervirulent strains and the high incidence of recurrence. We previously reported protection of mice from CDI using the antigerminant bile salt analog CamSA. Here we describe the effects of CamSA in the hamster model of CDI. CamSA treatment of hamsters showed no toxicity and did not affect the richness or diversity of gut microbiota; however, minor changes in community composition were observed. Treatment of C. difficile-challenged hamsters with CamSA doubled the mean time to death, compared to control hamsters. However, CamSA alone was insufficient to prevent CDI in hamsters. CamSA in conjunction with suboptimal concentrations of vancomycin led to complete protection from CDI in 70% of animals. Protected animals remained disease-free at least 30 days postchallenge and showed no signs of colonic tissue damage. In a delayed-treatment model of hamster CDI, CamSA was unable to prevent infection signs and death. These data support a putative model in which CamSA reduces the number of germinating C. difficile spores but does not keep all of the spores from germinating. Vancomycin halts division of any vegetative cells that are able to grow from spores that escape CamSA

    Long-Lasting Science Returns from the Apollo Heat Flow Experiments

    The Apollo astronauts deployed geothermal heat flow instruments at landing sites 15 and 17 as part of the Apollo Lunar Surface Experiments Packages (ALSEP) in July 1971 and December 1972, respectively. These instruments continuously transmitted data to the Earth until September 1977. Four decades later, the data from the two Apollo sites remain the only set of in-situ heat flow measurements obtained on an extra-terrestrial body. Researchers continue to extract additional knowledge from this dataset by utilizing new analytical techniques and by synthesizing it with data from more recent lunar orbital missions such as the Lunar Reconnaissance Orbiter. In addition, lessons learned from the Apollo experiments help contemporary researchers in designing heat flow instruments for future missions to the Moon and other planetary bodies. For example, the data from both Apollo sites showed gradual warming trends in the subsurface from 1971 to 1977. The cause of this warming has been debated in recent years. It may have resulted from fluctuation in insolation associated with the 18.6-year-cycle precession of the Moon, or sudden changes in surface thermal environment/properties resulting from the installation of the instruments and the astronauts' activities. These types of reanalyses of the Apollo data have lead a panel of scientists to recommend that a heat flow probe carried on a future lunar mission reach 3 m into the subsurface, approx 0.6 m deeper than the depths reached by the Apollo 17 experiment. This presentation describes the authors current efforts for (1) restoring a part of the Apollo heat flow data that were left unprocessed by the original investigators and (2) designing a compact heat flow instrument for future robotic missions to the Moon. First, at the conclusion of the ALSEP program in 1977, heat flow data obtained at the two Apollo sites after December 1974 were left unprocessed and not properly archived through NASA. In the following decades, heat flow data from January 1975 through February 1976, as well as the metadata necessary for processing the data (the data reduction algorithm, instrument calibration data, etc.), were somehow lost. In 2010, we located 450 original master archival tapes of unprocessed data from all the ALSEP instruments for a period of April through June 1975 at the Washington National Records Center. We are currently extracting the heat flow data packets from these tapes and processing them. Second, on future lunar missions, heat flow probes will likely be deployed by a network of small robotic landers, as recommended by the latest Decadal Survey of the National Academy of Science. In such a scenario, the heat flow probe must be a compact system, and that precludes use of heavy excavation equipment such as a rotary drill for reaching the 3-m target depth. The new heat flow system under development uses a pneumatically driven penetrator. It utilizes a stem that winds out of a reel and pushes its conical tip into the regolith. Simultaneously, gas jets, emitted from the cone tip, loosen and blow away the soil. Lab experiments have demonstrated its effectiveness in lunar vacuum
