The Influence of Energetic Factors on Biomarkers of Postmenopausal Breast Cancer Risk
Strong and consistent evidence exists that physical activity reduces breast cancer risk by 10-25Â %, and several proposed biologic mechanisms have now been investigated in randomized, controlled, exercise intervention trials. Leading hypothesized mechanisms relating to postmenopausal breast cancer include adiposity, endogenous sex hormones, insulin resistance, and chronic low-grade inflammation. In addition, other pathways are emerging as potentially important, including those involving oxidative stress and telomere length, global DNA hypomethylation, immune function, and vitamin D exposure. Recent exercise trials in overweight/obese postmenopausal women implicate weight loss as a mechanism whereby exercise induces favorable changes in circulating estradiol levels and other biomarkers as well. Still it is plausible that some exercise-induced biomarker changes do not require loss of body fat, whereas others depend on abdominal fat loss. We highlight the latest findings from randomized, controlled trials of healthy postmenopausal women, relating exercise to proposed biomarkers for postmenopausal breast cancer risk
Star Clusters in M 31. IV. A Comparative Analysis of Absorption Line Indices in Old M 31 and Milky Way Clusters
We present absorption line indices measured in the integrated spectra of
globular clusters both from the Galaxy and from M 31. Our samples include 41
Galactic globular clusters, and more than 300 clusters in M 31. The conversion
of instrumental equivalent widths into the Lick system is described, and
zero-point uncertainties are provided. Comparison of line indices of old M 31
clusters and Galactic globular clusters suggests an absence of important
differences in chemical composition between the two cluster systems. In
particular, CN indices in the spectra of M 31 and Galactic clusters are
essentially consistent with each other, in disagreement with several previous
works. We reanalyze some of the previous data, and conclude that reported CN
differences between M 31 and Galactic clusters were mostly due to data
calibration uncertainties. Our data support the conclusion that the chemical
compositions of Milky Way and M 31 globular clusters are not substantially
different, and that there is no need to resort to enhanced nitrogen abundances
to account for the optical spectra of M 31 globular clusters.Comment: 72 pages, including 15 figures and 14 tables. Published by the

NIMBUS: The Near-Infrared Multi-Band Ultraprecise Spectroimager for SOFIA
We present a new and innovative near-infrared multi-band ultraprecise
spectroimager (NIMBUS) for SOFIA. This design is capable of characterizing a
large sample of extrasolar planet atmospheres by measuring elemental and
molecular abundances during primary transit and occultation. This wide-field
spectroimager would also provide new insights into Trans-Neptunian Objects
(TNO), Solar System occultations, brown dwarf atmospheres, carbon chemistry in
globular clusters, chemical gradients in nearby galaxies, and galaxy
photometric redshifts. NIMBUS would be the premier ultraprecise spectroimager
by taking advantage of the SOFIA observatory and state of the art infrared
technologies.
This optical design splits the beam into eight separate spectral bandpasses,
centered around key molecular bands from 1 to 4 microns. Each spectral channel
has a wide field of view for simultaneous observations of a reference star that
can decorrelate time-variable atmospheric and optical assembly effects,
allowing the instrument to achieve ultraprecise calibration for imaging and
photometry for a wide variety of astrophysical sources. NIMBUS produces the
same data products as a low-resolution integral field spectrograph over a large
spectral bandpass, but this design obviates many of the problems that preclude
high-precision measurements with traditional slit and integral field

Counts-in-Cylinders in the Sloan Digital Sky Survey with Comparisons to N-body Simulations
Environmental statistics provide a necessary means of comparing the
properties of galaxies in different environments and a vital test of models of
galaxy formation within the prevailing, hierarchical cosmological model. We
explore counts-in-cylinders, a common statistic defined as the number of
companions of a particular galaxy found within a given projected radius and
redshift interval. Galaxy distributions with the same two-point correlation
functions do not necessarily have the same companion count distributions. We
use this statistic to examine the environments of galaxies in the Sloan Digital
Sky Survey, Data Release 4. We also make preliminary comparisons to four models
for the spatial distributions of galaxies, based on N-body simulations, and
data from SDSS DR4 to study the utility of the counts-in-cylinders statistic.
There is a very large scatter between the number of companions a galaxy has and
the mass of its parent dark matter halo and the halo occupation, limiting the
utility of this statistic for certain kinds of environmental studies. We also
show that prevalent, empirical models of galaxy clustering that match observed
two- and three-point clustering statistics well fail to reproduce some aspects
of the observed distribution of counts-in-cylinders on 1, 3 and 6-Mpc/h scales.
All models that we explore underpredict the fraction of galaxies with few or no
companions in 3 and 6-Mpc/h cylinders. Roughly 7% of galaxies in the real
universe are significantly more isolated within a 6 Mpc/h cylinder than the
galaxies in any of the models we use. Simple, phenomenological models that map
galaxies to dark matter halos fail to reproduce high-order clustering

Optical Colors of Intracluster Light in the Virgo Cluster Core
We continue our deep optical imaging survey of the Virgo cluster using the
CWRU Burrell Schmidt telescope by presenting B-band surface photometry of the
core of the Virgo cluster in order to study the cluster's intracluster light
(ICL). We find ICL features down to mu_b ~ 29 mag sq. arcsec, confirming the
results of Mihos et al. (2005), who saw a vast web of low-surface brightness
streams, arcs, plumes, and diffuse light in the Virgo cluster core using V-band
imaging. By combining these two data sets, we are able to measure the optical
colors of many of the cluster's low-surface brightness features. While much of
our imaging area is contaminated by galactic cirrus, the cluster core near the
cD galaxy, M87, is unobscured. We trace the color profile of M87 out to over
2000 arcsec, and find a blueing trend with radius, continuing out to the
largest radii. Moreover, we have measured the colors of several ICL features
which extend beyond M87's outermost reaches and find that they have similar
colors to the M87's halo itself, B-V ~ 0.8. The common colors of these features
suggests that the extended outer envelopes of cD galaxies, such as M87, may be
formed from similar streams, created by tidal interactions within the cluster,
that have since dissolved into a smooth background in the cluster potential.Comment: 14 pages. Published in ApJ, September 201
Identifying High Metallicity M Giants at Intragroup Distances with SDSS
Tidal stripping and three-body interactions with the central supermassive
black hole may eject stars from the Milky Way. These stars would comprise a set
of `intragroup' stars that trace the past history of interactions in our
galactic neighborhood. Using the Sloan Digital Sky Survey DR7, we identify
candidate solar metallicity red giant intragroup stars using color cuts that
are designed to exclude nearby M and L dwarfs. We present 677 intragroup
candidates that are selected between 300 kpc and 2 Mpc, and are either the
reddest intragroup candidates (M7-M10) or are L dwarfs at larger distances than

Does delayed measurement affect patient reports of provider performance? Implications for performance measurement of medical assistance with tobacco cessation: A Dental PBRN study
<p>Abstract</p> <p>Background:</p> <p>We compared two methods of measuring provider performance of tobacco control activities: immediate "exit cards" versus delayed telephone follow-up surveys. Current standards, e.g. HEDIS, use delayed patient measures that may over or under-estimate overall performance.</p> <p>Methods:</p> <p>Patients completed exit cards in 60 dental practices immediately after a visit to measure whether the provider "asked" about tobacco use, and "advised" the patient to quit. One to six months later patients were asked the same questions by telephone survey. Using the exit cards as the standard, we quantified performance and calculated sensitivity (agreement of those responding yes on telephone surveys compared with exit cards) and specificity (agreement of those responding no) of the delayed measurement.</p> <p>Results:</p> <p>Among 150 patients, 21% reporting being asked about tobacco use on the exit cards and 30% reporting being asked in the delayed surveys. The sensitivity and specificity were 50% and 75%, respectively. Similarly, among 182 tobacco users, 38% reported being advised to quit on the exit cards and this increased to 51% on the delayed surveys. The sensitivity and specificity were 75% and 64%, respectively. Increasing the delay from the visit to the telephone survey resulted in increasing disagreement.</p> <p>Conclusion:</p> <p>Patient reports differed considerably in immediate versus delayed measures. These results have important implications because they suggest that our delayed measures may over-estimate performance. The immediate exit cards should be included in the armamentarium of tools for measuring providers' performance of tobacco control, and perhaps other service delivery.</p
Malaria parasitemia among blood donors in Uganda
Background: Malaria remains a leading transfusion associated infectious risk in endemic areas. However, the prevalence of malaria parasitemia has not been well characterized in blood donor populations. This study sought to determine the prevalence of Plasmodium in red blood cell (RBC) and whole blood (WB) units after the rainy season in Uganda.
Methods and materials: Between May and July 2018, blood was collected from the sample diversion pouch of 1000 WB donors in Kampala and Jinja, Uganda. The RBC pellet from ethylenediamine tetraacetic acid (EDTA) anticoagulated blood was stored at -80°C until testing. DNA was extracted and nested PCR was used to screen samples at the genus level for Plasmodium, with positive samples further tested for species identification.
Results: Malaria parasitemia among asymptomatic, eligible blood donors in two regions of Uganda was 15.4%; 87.7% (135/154) of infections were with P. falciparum, while P. malariae and P. ovale were also detected. There were 4.3% of blood donors who had mixed infection with multiple species. Older donors (>30 years vs. 17-19 years; aPR = 0.31 [95% CI = 0.17-0.58]), females (aPR = 0.60 [95% CI = 0.42-0.87]), repeat donors (aPR = 0.44 [95% CI = 0.27-0.72]) and those donating near the capital city of Kampala versus rural Jinja region (aPR = 0.49 [95% CI = 0.34-0.69]) had a lower prevalence of malaria parasitemia.
Conclusions: A high proportion of asymptomatic blood donors residing in a malaria endemic region demonstrate evidence of parasitemia at time of donation. Further research is needed to quantify the risk and associated burden of transfusion-transmitted malaria (TTM) in order to inform strategies to prevent TTM
Short and medium-term effects of an education self-management program for individuals with osteoarthritis of the knee, designed and delivered by health professionals: A quality assurance study
<p>Abstract</p> <p>Background</p> <p>Self-management (SM) programs are effective for some chronic conditions, however the evidence for arthritis SM is inconclusive. The aim of this case series project was to determine whether a newly developed specific self-management program for people with osteoarthritis of the knee (OAK), implemented by health professionals could achieve and maintain clinically meaningful improvements.</p> <p>Methods</p> <p><it>Participants: </it>79 participants enrolled; mean age 66, with established osteoarthritis of the knee. People with coexisting inflammatory joint disease or serious co-morbidities were excluded.</p> <p><it>Intervention: </it>6-week disease (OA) and site (knee) specific self-management education program that included disease education, exercise advice, information on healthy lifestyle and relevant information within the constructs of self-management. This program was conducted in a community health care setting and was delivered by health professionals thereby utilising their knowledge and expertise.</p> <p><it>Measurements: </it>Pain, physical function and mental health scales were assessed at baseline, 8 weeks, 6 and 12 months using WOMAC and SF-36 questionnaires. Changes in pain during the 8-week intervention phase were monitored with VAS.</p> <p>Results</p> <p>Pain improved during the intervention phase: mean (95% CI) change 15 (8 to 22) mm. Improvements (0.3 to 0.5 standard deviation units) in indices of pain, mental health and physical functioning, assessed by SF-36 and WOMAC questionnaires were demonstrated from baseline to 12 months.</p> <p>Conclusion</p> <p>This disease and site-specific self-management education program improved health status of people with osteoarthritis of the knee in the short and medium term.</p
