    Estimating youth locomotion ground reaction forces using an accelerometer-based activity monitor.

    To address a variety of questions pertaining to the interactions between physical activity, musculoskeletal loading and musculoskeletal health/injury/adaptation, simple methods are needed to quantify, outside a laboratory setting, the forces acting on the human body during daily activities. The purpose of this study was to develop a statistically based model to estimate peak vertical ground reaction force (pVGRF) during youth gait. 20 girls (10.9 ± 0.9 years) and 15 boys (12.5 ± 0.6 years) wore a Biotrainer AM over their right hip. Six walking and six running trials were completed after a standard warm-up. Average AM intensity (g) and pVGRF (N) during stance were determined. Repeated measures mixed effects regression models to estimate pVGRF from Biotrainer activity monitor acceleration in youth (girls 10-12, boys 12-14 years) while walking and running were developed. Log transformed pVGRF had a statistically significant relationship with activity monitor acceleration, centered mass, sex (girl), type of locomotion (run), and locomotion type-acceleration interaction controlling for subject as a random effect. A generalized regression model without subject specific random effects was also developed. The average absolute differences between the actual and predicted pVGRF were 5.2% (1.6% standard deviation) and 9% (4.2% standard deviation) using the mixed and generalized models, respectively. The results of this study support the use of estimating pVGRF from hip acceleration using a mixed model regression equation

    Simple methods to quantify ground reaction forces (GRFs) outside a laboratory setting are needed to understand daily loading sustained by the body. Here, we present methods to estimate peak vertical GRF (pGRFvert) and peak braking GRF (pGRFbrake) in adults using raw hip activity monitor (AM) acceleration data. The purpose of this study was to develop a statistically based model to estimate pGRFvert and pGRFbrake during walking and running from ActiGraph GT3X+ AM acceleration data. 19 males and 20 females (age 21.2 ± 1.3 years, height 1.73 ± 0.12 m, mass 67.6 ± 11.5 kg) wore an ActiGraph GT3X+ AM over their right hip. Six walking and six running trials (0.95-2.19 and 2.20-4.10 m/s, respectively) were completed. Average of the peak vertical and anterior/posterior AM acceleration (ACCvert and ACCbrake, respectively) and pGRFvert and pGRFbrake during the stance phase of gait were determined. Thirty randomly selected subjects served as the training dataset to develop generalized equations to predict pGRFvert and pGRFbrake. Using a holdout approach, the remaining 9 subjects were used to test the accuracy of the models. Generalized equations to predict pGRFvert and pGRFbrake included ACCvert and ACCbrake, respectively, mass, type of locomotion (walk or run), and type of locomotion acceleration interaction. The average absolute percent differences between actual and predicted pGRFvert and pGRFbrake were 8.3% and 17.8%, respectively, when the models were applied to the test dataset. Repeated measures generalized regression equations were developed to predict pGRFvert and pGRFbrake from ActiGraph GT3X+ AM acceleration for young adults walking and running. These equations provide a means to estimate GRFs without a force plate

Recent hot-fire testing has provided promising indications that the incorporation of roughened damping seals at the impeller interstages may further increase the stability margin of this machine. A summary of the analysis which led to the conclusion that roughened seals would enhance the stability margin is presented along with a correlation of the analysis with recent test data

    Evaluation of an exercise field test using heart rate monitors to assess cardiorespiratory fitness and heart rate recovery in an asymptomatic population.

    PurposeMeasures of cardiorespiratory fitness (CRF) and heart rate recovery (HRR) can improve risk stratification for cardiovascular disease, but these measurements are rarely made in asymptomatic individuals due to cost. An exercise field test (EFT) to assess CRF and HRR would be an inexpensive method for cardiovascular disease risk assessment in large populations. This study assessed 1) the predictive accuracy of a 12-minute run/walk EFT for estimating CRF ([Formula: see text]) and 2) the accuracy of HRR measured after an EFT using a heart rate monitor (HRM) in an asymptomatic population.MethodsFifty subjects (48% women) ages 18-45 years completed a symptom-limited exercise tolerance test (ETT) (Bruce protocol) and an EFT on separate days. During the ETT, [Formula: see text] was measured by a metabolic cart, and heart rate was measured continuously by a HRM and a metabolic cart.ResultsEFT distance and sex independently predicted[Formula: see text]. The average absolute difference between observed and predicted [Formula: see text] was 0.26 ± 3.27 ml·kg-1·min-1 for our model compared to 7.55 ± 3.64 ml·kg-1·min-1 for the Cooper model. HRM HRR data were equivalent to respective metabolic cart values during the ETT. HRR at 1 minute post-exercise during ETT compared to the EFT had a moderate correlation (r=0.75, p<0.001).ConclusionA more accurate model to estimate CRF from a 12-minute run/walk EFT was developed, and HRR can be measured using a HRM in an asymptomatic population outside of clinical settings

    ‘Arm-based’ parameterization for network meta-analysis

    We present an alternative to the contrast‐based parameterization used in a number of publications for network meta‐analysis. This alternative “arm‐based” parameterization offers a number of advantages: it allows for a “long” normalized data structure that remains constant regardless of the number of comparators; it can be used to directly incorporate individual patient data into the analysis; the incorporation of multi‐arm trials is straightforward and avoids the need to generate a multivariate distribution describing treatment effects; there is a direct mapping between the parameterization and the analysis script in languages such as WinBUGS and finally, the arm‐based parameterization allows simple extension to treatment‐specific random treatment effect variances. We validated the parameterization using a published smoking cessation dataset. Network meta‐analysis using arm‐ and contrast‐based parameterizations produced comparable results (with means and standard deviations being within +/− 0.01) for both fixed and random effects models. We recommend that analysts consider using arm‐based parameterization when carrying out network meta‐analyses

    Unsolicited written narratives as a methodological genre in terminal illness: challenges and limitations

    Stories about illness have proven invaluable in helping health professionals understand illness experiences. Such narratives have traditionally been solicited by researchers through interviews and the collection of personal writings, including diaries. These approaches are, however, researcher driven; the impetus for the creation of the story comes from the researcher and not the narrator. In recent years there has been exponential growth in illness narratives created by individuals, of their own volition, and made available for others to read in print or as Internet accounts. We sought to determine whether it was possible to identify such material for use as research data to explore the subject of living with the terminal illness amyotrophic lateral sclerosis/motor neuron disease—the contention being that these accounts are narrator driven and therefore focus on issues of greatest importance to the affected person. We encountered and sought to overcome a number of methodological and ethical challenges, which is our focus here

    Network meta-analysis on the log-hazard scale, combining count and hazard ratio statistics accounting for multi-arm trials: a tutorial.

    BACKGROUND: Data on survival endpoints are usually summarised using either hazard ratio, cumulative number of events, or median survival statistics. Network meta-analysis, an extension of traditional pairwise meta-analysis, is typically based on a single statistic. In this case, studies which do not report the chosen statistic are excluded from the analysis which may introduce bias. METHODS: In this paper we present a tutorial illustrating how network meta-analyses of survival endpoints can combine count and hazard ratio statistics in a single analysis on the hazard ratio scale. We also describe methods for accounting for the correlations in relative treatment effects (such as hazard ratios) that arise in trials with more than two arms. Combination of count and hazard ratio data in a single analysis is achieved by estimating the cumulative hazard for each trial arm reporting count data. Correlation in relative treatment effects in multi-arm trials is preserved by converting the relative treatment effect estimates (the hazard ratios) to arm-specific outcomes (hazards). RESULTS: A worked example of an analysis of mortality data in chronic obstructive pulmonary disease (COPD) is used to illustrate the methods. The data set and WinBUGS code for fixed and random effects models are provided. CONCLUSIONS: By incorporating all data presentations in a single analysis, we avoid the potential selection bias associated with conducting an analysis for a single statistic and the potential difficulties of interpretation, misleading results and loss of available treatment comparisons associated with conducting separate analyses for different summary statistics

    Randomised, controlled trial of alternating pressure mattresses compared with alternating pressure overlays for the prevention of pressure ulcers : PRESSURE (pressure relieving support surfaces) trial

    Objective To compare whether differences exist between alternating pressure overlays and alternating pressure mattresses in the development of new pressure ulcers, healing of existing pressure ulcers, and patient acceptability. Design Pragmatic, open, multicentre, randomised controlled trial. Setting 11 hospitals in six NHS trusts. Participants 1972 people admitted to hospital as acute or elective patients. Interventions Participants were randomised to an alternating pressure mattress (n = 982) of- an alternating pressure overlay (n = 990). Main outcome measures The proportion of participants developing a new pressure ulcer of grade 2 or worse; time to development of new pressure ulcers; proportions of participants developing a new ulcer within 30 days; healing of existing pressure ulcers; and patient acceptability Results Intention to treat analysis found no difference in the proportions of participants developing a new pressure ulcer of grade 2 or worse (10.7% overlay patients, 10.3% mattress patients; difference 0.4%, 95% confidence interval - 23% to 3.1%, P = 0.75). More overlay patients requested change owing to dissatisfaction (23.3%) than mattress patients (18.9%, P = 0.02). Conclusion No difference was found between alternating pressure mattresses and alternating pressure overlays in the proportion of people who develop a pressure ulcer
