Ear care: an update for nurses (part 1)
A healthy ear is vital not just for hearing, but for balance and for full engagement with the community. In the first of two articles, Kat Millward reviews the anatomy of the ear, outlines methods of dealing with cerumen, and discusses methods of assessment and diagnosis
Our ears are essential for both hearing and balance. Up to 4% of the population will have difficulties with their ears relating to impacted cerumen and many of them will present in primary care with ear discomfort or hearing difficulties. In the first of two articles, we review the anatomy and physiology of the ear and cerumen production, followed by a discussion of the key aspects of making an assessment using the SOAP (Subjective, Objective, Assessment, Plan) mnemonic
Quantified Self Analytics Tools for Self-regulated Learning with myPAL
One of the major challenges in higher education is developing self-regulation skills for lifelong learning. We address this challenge within the myPAL project, in medical education context, utilising the vast amount of student assessment and feedback data collected throughout the programme. The underlying principle of myPAL is Quantified Self -- the use of personal data to enable students to become lifelong learners. myPAL is facilitating this with learning analytics combined with interactive nudges. This paper reviews the state of the art in Quantified Self analytics tools to identify what approaches can be adopted in myPAL and what gaps require further research. The paper contributes to awareness and reflection in technology-enhanced learning by: (i) identifying requirements for intelligent personal adaptive learning systems that foster self-regulation (using myPAL as an example); (ii) analysing the state of the art in text analytics and visualisation related to Quantified Self for self-regulated learning; and (iii) identifying open issues and suggesting possible ways to address them
Ultra-short pulses in linear and nonlinear media
We consider the evolution of ultra-short optical pulses in linear and
nonlinear media. For the linear case, we first show that the initial-boundary
value problem for Maxwell's equations in which a pulse is injected into a
quiescent medium at the left endpoint can be approximated by a linear wave
equation which can then be further reduced to the linear short-pulse equation.
A rigorous proof is given that the solution of the short pulse equation stays
close to the solutions of the original wave equation over the time scales
expected from the multiple scales derivation of the short pulse equation. For
the nonlinear case we compare the predictions of the traditional nonlinear
Schr\"odinger equation (NLSE) approximation which those of the short pulse
equation (SPE). We show that both equations can be derived from Maxwell's
equations using the renormalization group method, thus bringing out the
contrasting scales. The numerical comparison of both equations to Maxwell's
equations shows clearly that as the pulse length shortens, the NLSE
approximation becomes steadily less accurate while the short pulse equation
provides a better and better approximation
In-service Initial Teacher Education in the Learning and Skills Sector in England: Integrating Course and Workplace Learning
The aim of the paper is to advance understanding of in-service learning and skills sector trainee teachersâ learning and propose ways of improving their learning. A conceptual framework is developed by extending Billettâs (International Journal of Educational Research 47:232â240, 2008) conceptualisation of workplace learning, as a relationally interdependent process between the opportunities workplaces afford for activities and interactions and how individuals engage with these, to a third base of participation, the affordances of the initial teacher education course. Hager and Hodkinsonâs (British Educational Research Journal 35:619â638, 2009) metaphor of âlearning as becomingâ is used to conceptualise the ways trainees reconstruct learning in a continuous transactional process of boundary crossing between course and workplace. The findings of six longitudinal case studies of traineesâ development, and evidence from other studies, illustrate the complex interrelationships between LSS workplace affordances, course affordances and trainee characteristics and the ways in which trainees reconstruct learning in each setting. The experience of teaching and interacting with learners, interactions with colleagues, and access to workplace resources and training are important workplace affordances for learning. However, some trainees have limited access to these affordances. Teaching observations, course activities and experiences as a learner are significant course affordances. Traineesâ beliefs, prior experiences and dispositions vary and significantly influence their engagement with course and workplace affordances. It is proposed that better integration of course and workplace learning through guided participation in an intentional workplace curriculum and attention to the ways trainees choose to engage with this, together with the use of practical theorising has the potential to improve trainee learning
Cricket, migration and diasporic communities
Ever since different communities began processes of global migration, sport has been an integral feature in how we conceptualise and experience the notion of being part of a diaspora. Sport provides diasporic communities with a powerful means for creating transnational ties, but also shapes ideas of their ethnic and racial identities. In spite of this, theories of diaspora have been applied sparingly to sporting discourses. Due mainly to its central role in spreading dominant white racial narratives within the British Empire, and the various ways different ethnic groups have âplayedâ with the meanings and associations of the sport in the (post-)colonial period, cricket is an interesting focus for academic research. Despite W.G. Graceâs claim that cricket advances civilisation by promoting a common bond, binding together peoples of vastly different backgrounds, to this day cricket operates strict symbolic boundaries; defining those who do, and equally, do not belong. C.L.R. Jamesâ now famous metaphor of looking âbeyond the boundaryâ captures the belief that, to fully understand the significance of cricket, and the sportâs roles in changing and shaping society, one must consider the wider social and political contexts within which the game is played. The effect of melt viscosity on thermal efficiency for single screw extrusion of HDPE
YesIn this work, a highly instrumented single screw extruder has been used to study the effect of polymer rheology on the thermal efficiency of the extrusion process. Three different molecular weight grades of high density polyethylene (HDPE) were extruded at a range of conditions. Three geometries of extruder screws were used at several set temperatures and screw rotation speeds. The extruder was equipped with real-time quantification of energy consumption; thermal dynamics of the process were examined using thermocouple grid sensors at the entrance to the die. Results showed that polymer rheology had a significant effect on process energy consumption and thermal homogeneity of the melt. Highest specific energy consumption and poorest homogeneity was observed for the highest viscosity grade of HDPE. Extruder screw geometry, set extrusion temperature and screw rotation speed were also found to have a direct effect on energy consumption and melt consistency. In particular, specific energy consumption was lower using a barrier flighted screw compared to single flighted screws at the same set conditions. These results highlight the complex nature of extrusion thermal dynamics and provide evidence that rheological properties of the polymer can significantly influence the thermal efficiency of the process. High EMSY expression defines a BRCAâlike subgroup of highâgrade serous ovarian carcinoma with prolonged survival and hypersensitivity to platinum
Background
Approximately half of highâgrade serous ovarian carcinomas (HGSOCs) demonstrate homologous recombination repair (HR) pathway defects, resulting in a distinct clinical phenotype comprising hypersensitivity to platinum, superior clinical outcome, and greater sensitivity to poly(adenosine diphosphateâribose) polymerase (PARP) inhibitors. EMSY, which is known to be amplified in breast and ovarian cancers, encodes a protein reported to bind and inactivate BRCA2. Thus, EMSY overexpression may mimic BRCA2 mutation, resulting in HR deficiency. However, to our knowledge, the phenotypic consequences of EMSY overexpression in HGSOC patients has not been explored.
Methods
Here we investigate the impact of EMSY expression on clinical outcome and sensitivity to platinumâbased chemotherapy using available data from transcriptomically characterized HGSOC cohorts.
Results
High EMSY expression was associated with better clinical outcome in a cohort of 265 patients with HGSOC from Edinburgh (overall survival multivariable hazard ratio, 0.58 [95% CI, 0.38â0.88; P = .011] and progressionâfree survival multivariable hazard ratio, 0.62 [95% CI, 0.40â0.96; P = .030]). Superior outcome also was demonstrated in the Medical Research Council ICON7 clinical trial and multiple publicly available data sets. Patients within the Edinburgh cohort who had high EMSY expression were found to demonstrate greater rates of complete response to multiple platinumâcontaining chemotherapy regimens (radiological complete response rate of 44.4% vs 12.5% at second exposure; P = .035) and corresponding prolonged time to disease progression (median, 151.5 days vs 60.5 days after third platinum exposure; P = .004).
Conclusions
Patients with HGSOCs demonstrating high EMSY expression appear to experience prolonged survival and greater platinum sensitivity, reminiscent of BRCAâmutant cases. These data are consistent with the notion that EMSY overexpression may render HGSOCs HR deficient
Investigation of transition frequencies of two acoustically coupled bubbles using a direct numerical simulation technique
The theoretical results regarding the ``transition frequencies'' of two
acoustically interacting bubbles have been verified numerically. The theory
provided by Ida [Phys. Lett. A 297 (2002) 210] predicted the existence of three
transition frequencies per bubble, each of which has the phase difference of
between a bubble's pulsation and the external sound field, while
previous theories predicted only two natural frequencies which cause such phase
shifts. Namely, two of the three transition frequencies correspond to the
natural frequencies, while the remaining does not. In a subsequent paper [M.
Ida, Phys. Rev. E 67 (2003) 056617], it was shown theoretically that transition
frequencies other than the natural frequencies may cause the sign reversal of
the secondary Bjerknes force acting between pulsating bubbles. In the present
study, we employ a direct numerical simulation technique that uses the
compressible Navier-Stokes equations with a surface-tension term as the
governing equations to investigate the transition frequencies of two coupled
bubbles by observing their pulsation amplitudes and directions of translational
motion, both of which change as the driving frequency changes. The numerical
results reproduce the recent theoretical predictions, validating the existence

Does Monitoring Goal Progress Promote Goal Attainment? A Meta-Analysis of the Experimental Evidence
Control theory and other frameworks for understanding self-regulation suggest that monitoring goal progress is a crucial process that intervenes between setting and attaining a goal, and helps to ensure that goals are translated into action. However, the impact of progress monitoring interventions on rates of behavioral performance and goal attainment has yet to be quantified. A systematic literature search identified 138 studies (N = 19,951) that randomly allocated participants to an intervention designed to promote monitoring of goal progress versus a control condition. All studies reported the effects of the treatment on (a) the frequency of progress monitoring and (b) subsequent goal attainment. A random effects model revealed that, on average, interventions were successful at increasing the frequency of monitoring goal progress (d+ = 1.98, 95% CI [1.71, 2.24]) and promoted goal attainment (d+ = 0.40, 95% CI [0.32, 0.48]). Furthermore, changes in the frequency of progress monitoring mediated the effect of the interventions on goal attainment. Moderation tests revealed that progress monitoring had larger effects on goal attainment when the outcomes were reported or made public, and when the information was physically recorded. Taken together, the findings suggest that monitoring goal progress is an effective self-regulation strategy, and that interventions that increase the frequency of progress monitoring are likely to promote behavior change
