Total Pelvic Exenteration for Gynecologic Malignancies
Total pelvic exenteration (PE) is a radical operation, involving en bloc resection of pelvic organs, including reproductive structures, bladder, and rectosigmoid. In gynecologic oncology, it is most commonly indicated for the treatment of advanced primary or locally recurrent cancer. Careful patient selection and counseling are of paramount importance when considering someone for PE. Part of the evaluation process includes comprehensive assessment to exclude unresectable or metastatic disease. PE can be curative for carefully selected patients with gynecologic cancers. Major complications can be seen in as many as 50% of patients undergoing PE, underscoring the need to carefully discuss risks and benefits of this procedure with patients considering exenterative surgery
Superpotentials for M-theory on a G_2 holonomy manifold and Triality symmetry
For -theory on the holonomy manifold given by the cone on {\bf
S^3}\x {\bf S^3} we consider the superpotential generated by membrane
instantons and study its transformations properties, especially under monodromy
transformations and triality symmetry. We find that the latter symmetry is,
essentially, even a symmetry of the superpotential. As in Seiberg/Witten
theory, where a flat bundle given by the periods of an universal elliptic curve
over the -plane occurs, here a flat bundle related to the Heisenberg group
appears and the relevant universal object over the moduli space is related to

Retroperitoneal Hematoma as an Atypical Presentation of Choriocarcinoma: A Case Report
A 38-year-old female with an etonogestrel implant in place and history of previous ectopic pregnancy presented with acute abdominal pain and vaginal bleeding. She was found to have a beta-hCG of \u3e12,000 mIU/mL and free fluid noted on a focused assessment with sonography in trauma exam. She underwent an emergent diagnostic laparoscopy due to the suspicion of a ruptured ectopic pregnancy. Findings at the time of surgery included a normal-appearing uterus and left fallopian tube, a surgically absent right fallopian tube and large volume hemoperitoneum with a rapidly expanding left retroperitoneal hematoma. A postoperative computerized tomography (CT) angiogram suggested active bleeding from a pseudoaneurysm of the left renal artery which was successfully embolized by interventional radiology. Biopsy confirmed gestational trophoblastic neoplasia (GTN) after metastases to the brain. In this report, we describe the details of this case of GTN with an atypical presentation

MetricGAN+/- : increasing robustness of noise reduction on unseen data
Training of speech enhancement systems often does not incorporate knowledge of human perception and thus can lead to unnatural sounding results. Incorporating psychoacoustically motivated speech perception metrics as part of model training via a predictor network has recently gained interest. However, the performance of such predictors is limited by the distribution of metric scores that appear in the training data. In this work, we propose MetricGAN+/- (an extension of MetricGAN+, one such metric-motivated system) which introduces an additional network - a "de-generator" which attempts to improve the robustness of the prediction network (and by extension of the generator) by ensuring observation of a wider range of metric scores in training. Experimental results on the VoiceBank-DEMAND dataset show relative improvement in PESQ score of 3.8% (3.05 vs 3.22 PESQ score), as well as better generalisation to unseen noise and speech
