Alignment in total knee arthroplasty : whatâs in a name?
Dissatisfaction following total knee arthroplasty is a well-documented phenomenon. Although many factors have been implicated, including modifiable and nonmodifiable patient factors, emphasis over the past decade has been on implant alignment and stability as both a cause of, and a solution to, this problem. Several alignment targets have evolved with a proliferation of techniques following the introduction of computer and robotic-assisted surgery. Mechanical alignment targets may achieve mechanically-sound alignment while ignoring the soft tissue envelope; kinematic alignment respects the soft tissue envelope while ignoring the mechanical environment. Functional alignment is proposed as a hybrid technique to allow mechanically-sound, soft tissue-friendly alignment targets to be identified and achieved
Ph-Metrie oesophagienne a double canal dans le diagnostic du reflux gastro-oesophagien Avec manifestations ORL
Objectif : DĂ©terminer la frĂ©quence du reflux proximal pathologique chez des patients prĂ©sentant un reflux gastro-oesophagien (RgO) avec manifestations oto-rhino-laryngologiques (ORL) et explorĂ©s par une ph-mĂ©trie oesophagienne des 24 heures, utilisant une sonde Ă double canal, dĂ©terminer le taux de concordance entre les symptĂŽmes ORL et les Ă©pisodes de RgO proximal et enfin, rechercher des facteurs épidĂ©miologiques, cliniques et ph-mĂ©triques distaux associĂ©s Ă un plus grand risque de survenue de RgO proximal. : Nous avons revu tous les tracĂ©s et comptes rendus des phmĂ©tries oesophagiennes des 24 heures Ă double canal pratiquĂ©es entre 2008 et 2010, pour exploration de manifestations ORL de RgO.MatĂ©riel et mĂ©thodes : Nous avons revu tous les tracĂ©s et comptes rendus des ph-mĂ©tries oesophagiennes des 24 heures Ă double canal pratiquĂ©es entre 2008 et 2010, pour exploration de manifestations ORL de RgO.RĂ©sultats : il sâagissait de 33 patients, dâĂąge moyen de 43,2 ans (10-73 ans). Un reflux proximal pathologique a Ă©tĂ© retrouvĂ© chez 27 patients. Il Ă©tait associĂ© Ă un reflux distal chez 16 patients et isolĂ© chez 11 patients. Un reflux proximal nocturne pathologique a Ă©tĂ© retrouvĂ© chez 13 patients. Une bonne concordance symptomatique a Ă©tĂ© retrouvĂ©e chez 9 malades. Concernant les facteurs prĂ©dictifs du reflux proximal pathologique, nous avons retrouvĂ© une corrĂ©lation statistiquement significative avec lâexistence dâun reflux distal associĂ© et lâexistence dâun reflux distal nocturne.Conclusion : en attendant le dĂ©veloppement et la diffusion de  lâimpĂ©dancemĂ©trie oesophagienne, la ph-mĂ©trie oesophagienne utilisant une sonde Ă double canal doit ĂȘtre proposĂ©e Ă tous les patients souffrant dâun RgO avec manifestations ORL.Mots-clĂ©s : reflux gastro-oesophagien, ph-mĂ©trie oesophagienne des 24 heures, oto-rhino-laryngologie
Detariffing and the Death of the Filed Tariff Doctrine: Deregulating in the âSelfâ Interest
This Article reviews the history of the FCC\u27s detariffing efforts, addressing the major issue raised not so much by detariffing itself, but by the FCC\u27s view of detariffing orders impact on the Filed Tariff Doctrine. Notwithstanding the existence of the Doctrine for nearly a century, the FCC, through detariffing, has declared the Doctrine dead. This Article formally opposes the FCC\u27s declaration and suggests that the FCC\u27s motivations behind detariffing have failed to consider, much less attempted to properly balance, the conflicting public interests involved. Comparing and contrasting the legal rights enjoyed by long-distance carriers under the Filed Tariff Doctrine to the rights and potential liabilities of carriers in its absence, this Article discusses the actions several states have taken immediately following the July 31, 2001 effective date of the FCC\u27s mass-market detariffing order. The Article concludes that the FCC\u27s action is a manifest injustice to both carriers and consumers alike, and is a prime example of irresponsible agency regulation
Training deep neural density estimators to identify mechanistic models of neural dynamics
Mechanistic modeling in neuroscience aims to explain observed phenomena in terms of underlying causes. However, determining which model parameters agree with complex and stochastic neural data presents a significant challenge. We address this challenge with a machine learning tool which uses deep neural density estimators-- trained using model simulations-- to carry out Bayesian inference and retrieve the full space of parameters compatible with raw data or selected data features. Our method is scalable in parameters and data features, and can rapidly analyze new data after initial training. We demonstrate the power and flexibility of our approach on receptive fields, ion channels, and Hodgkin-Huxley models. We also characterize the space of circuit configurations giving rise to rhythmic activity in the crustacean stomatogastric ganglion, and use these results to derive hypotheses for underlying compensation mechanisms. Our approach will help close the gap between data-driven and theory-driven models of neural dynamics
Applicability of Modified Effective-Range Theory to positron-atom and positron-molecule scattering
We analyze low-energy scattering of positrons on Ar atoms and N2 molecules
using Modified Effective-Range Theory (MERT) developped by O'Malley, Spruch and
Rosenberg [Journal of Math. Phys. 2, 491 (1961)]. We use formulation of MERT
based on exact solutions of Schroedinger equation with polarization potential
rather than low-energy expansions of phase shifts into momentum series. We show
that MERT describes well experimental data, provided that effective-range
expansion is performed both for s- and p-wave scattering, which dominate in the
considered regime of positron energies (0.4 - 2 eV). We estimate the values of
the s-wave scattering lenght and the effective range for e+ - Ar and e+ - N2

Disorder effects on infrared reflection spectra of InN films
The effect of plasmon and LO-phonon damping on the optical measurements of InN films is discussed. Phonon and plasmon damping dramatically modifies the spectral features of the optical spectra and destabilizes the coupled modes of the system. Phonon damping affects the optical properties in a qualitatively different way from plasmon damping. Increased phonon damping leads to a merging of the coupled modes for a certain range of carrier density. Plasmon damping broadens the spectral linewidths of both of the coupled modes when plasmon energies are of the order of LO-phonon energies. However, when plasmon energies are larger than LO-phonon energies, increasing plasmon damping makes the higher-energy mode completely degenerate with plasmon, and may even have lower energies than plasmon. In weakly damped situations, we also discuss the low-energy optical transmission region that forms in between the coupled modes. Renormalization Group calculations with k|| dependent couplings in a ladder
We calculate the phase diagram of a ladder system, with a Hubbard interaction
and an interchain coupling . We use a Renormalization Group method, in
a one loop expansion, introducing an original method to include
dependence of couplings. We also classify the order parameters corresponding to
ladder instabilities. We obtain different results, depending on whether we
include dependence or not. When we do so, we observe a region with
large antiferromagnetic fluctuations, in the vicinity of small ,
followed by a superconducting region with a simultaneous divergence of the Spin
Density Waves channel. We also investigate the effect of a non local backward
interchain scattering : we observe, on one hand, the suppression of singlet
superconductivity and of Spin Density Waves, and, on the other hand, the
increase of Charge Density Waves and, for some values of , of triplet
superconductivity. Our results eventually show that is an influential
variable in the Renormalization Group flow, for this kind of systems.Comment: 20 pages, 19 figures, accepted in Phys. Rev. B 71 v. 2
