Linfoma angioinmunoblástico de células T como imitador de lupus eritematoso sistémico
Angioimmunoblastic T-cell lymphoma (LAIT) is a rare non-Hodgkin lymphoma, can mimic autoimmune diseases, and has a poor prognosis. We present the case of a 36-year-old woman with a 3-year illness characterized by fever, arthralgia and weight loss. She was initially diagnosed as systemic lupus erythematosus, but finding no improvement with treatment, her diagnosis was reconsidered. In a new hospitalization, multiple lymph nodes were identified. They performed a biopsy of one of the adenopathies, the pathology was compatible with LAIT. Three chemotherapy sessions were indicated, however, she developed multiple organ failure with a fatal outcome. LAIT is a diagnostic challenge because it can mimic several autoimmune pathologies. Its suspicion and ruling out is very important to initiate early treatment that improves patient survival.El linfoma angioinmunoblástico de células T (LAIT) es un linfoma no Hodgkin poco frecuente, puede imitar a enfermedades autoinmunes y es de pobre pronóstico. Se presenta el caso de una mujer de 36 años con 3 años de enfermedad caracterizada por fiebre, artralgias y baja de peso. La paciente fue diagnosticada inicialmente como lupus eritematoso sistémico, pero al no encontrar mejoría con el tratamiento su diagnóstico fue replanteado. En una nueva hospitalización se le identificaron múltiples adenomegalias. Le realizaron la biopsia de una de las adenopatías, la patología fue compatible con LAIT. Se indicó 3 sesiones de quimioterapia, sin embargo, desarrolló falla multiorgánica con desenlace fatal. El LAIT es un reto diagnóstico debido a que puede imitar varias patologías autoinmunes. Es muy importante su sospecha y descarte para iniciar un tratamiento precoz que mejore la sobrevida de los pacientes
Neurofibromatosis type 1 associated to cognition deficit, hydrocephaly, bone dysplasia and elephantiasis
Se presenta el caso de un paciente varón de 31 años, procedente de Cajamarca, con problemas cognitivos desde su escolaridad temprana, que desde los 12 años desarrolló incremento progresivo de volumen y deformación de su miembro inferior derecho y en menor cuantía del izquierdo, asociado a plegamiento de piel, crecimiento de nódulos subcutáneos, con formación de cordones nerviosos en muslo y pierna que le daban un aspecto elefantiásico. Dolor y limitación funcional para la marcha. Concomitantemente, presencia en piel de manchas café con leche generalizadas y nódulos de Lisch en ambos iris. En la resonancia magnética y TAC con contraste, se evidenciaron abundantes nódulos plexiformes en cavidad abdominopélvica, que continuaban por todo el muslo, de tamaño uniforme, formando tumoraciones y cordones. Asimismo, presentó displasia ósea (adelgazamiento de la cortical del fémur y tibia, formación de neocótilo izquierdo, pseudoartrosis). La resonancia cerebral con contraste descubrió una hidrocefalia triventricular, normotensa.We report the case of a 31 year-old male from Cajamarca, Peru, with cognitive problems since childhood, who at 12 year-old developed increased volume and deformation of his right lower limb and to a lesser extent of his left, associated to massive skin folding, growth of subcutaneous nodules, elephantine look due to nerve cords formation in thigh and leg, pain and functional walking limitation. Concomitantly generalized skin café au lait spots, subcutaneous nodules, and Lisch nodules in both irides. Magnetic resonance and contrast TAC revealed abundant uniform plexiform nodules in abdomino-pelvic cavity forming masses and cords that continued throughout his thigh as lumps and cords. In addition, the patient presented bone dysplasia (thinning of femur and tibia cortex, pseudoarthrosis, new left cotyle formation). Contrast magnetic resonance showed triventricular normotensive hydrocephalus
Características del metabolismo óseo en mujeres con osteoporosis posmenopáusica de un hospital de Lima
Introduction: In our context, there are no studies which had evaluated the markers of bone turnover in postmenopausal women with osteoporosis, considering the high rate of morbidity and mortality associated with fragility fractures. Objective: To know the biochemical features of bone metabolism in women with postmenopausal osteoporosis. Methods: A cross-sectional study. 49 women with postmenopausal osteoporosis were included. Bone mineral density in spine, hip and forearm was measured by dual x-ray absorptiometry. Calcium, phosphorus, osteocalcin, alkaline phosphatase, vitamin D and paratohormone were measured in blood. N-telopeptide of collagen type I, calcium and phosphorus were measured in urine by conventional methods. Results: The average age of the participants was 59,1± 7,5 years. The average time since menopause was 12,0 ± 7,9 years. All had osteoporosis in forearm; 46,9% had insufficient levels of vitamin D and 8,2% had deficient values. The osteocalcin was normal in 73,5%, decreased in 20,1% and increased in 6,4% participants. Age showed statistically significant correlation with serum parathyroid hormone and was negatively correlated with vitamin D. The time of menopause had a positive and statistically significant correlation with alkaline phosphatase. Conclusion: The extension of osteoporosis had a positive correlation with older age and longer menopause. The bone turnover markers provide complementary information to bone densitometry.Introducción: En nuestro medio no existen estudios que evaluen los diversos marcadores de recambio óseo en mujeres con osteoporosis posmenopáusica, considerando la alta tasa de morbimortalidad asociada a las fracturas por fragilidad. Objetivo: Conocer las características bioquímicas del metabolismo óseo en mujeres posmenopáusicas con osteoporosis. Métodos: Estudio transversal. Se incluyeron a 49 mujeresposmenopáusicas con osteoporosis, a quienes se midió la densidad mineral ósea por absorciometría dual de rayos X en columna lumbar, cadera y antebrazo. Se determinó calcio, fósforo, osteocalcina, fosfatasa alcalina, vitamina D y paratohormona en sangre y en orina, N-telopéptido de enlaces de colágeno tipo I, calcio y fósforo por métodos convencionales. Resultados: La edad promedio fue 59,1 ± 7,5 años. El tiempo medio de menopausia fue 12,0±7,9 años. Todas tuvieron osteoporosis en antebrazo, 55,1%, en columna lumbar y 18,4%, en cadera total. Valores insuficientes de vitamina D tuvo el 46,9% y 8,2, valores deficientes. La osteocalcina fue normal en 73,5%, disminuida en 20,1% y alta en 6,4%. Se encontró correlación positiva entre la edad y la concentración de paratohormona, y negativa con la vitamina D. La fosfatasa alcalina tuvo correlación positiva con la duración de la menopausia. Conclusión: La mayor extensión de osteoporosis tuvo correlación con la edad y tiempo de menopausia. Más de la mitad de mujeres tuvieron niveles bajos de vitamina D
Histological Examination in Obtaining a Diagnosis in Patients with Lymphadenopathy in Lima, Peru.
The differential diagnosis for lymphadenopathy is wide and clinical presentations overlap, making obtaining an accurate diagnosis challenging. We sought to characterize the clinical and radiological characteristics, histological findings, and diagnoses for a cohort of patients with lymphadenopathy of unknown etiology. 121 Peruvian adults with lymphadenopathy underwent lymph node biopsy for microbiological and histopathological evaluation. Mean patient age was 41 years (Interquartile Range 26-52), 56% were males, and 39% were HIV positive. Patients reported fever (31%), weight loss (23%), and headache (22%); HIV infection was associated with fever (P < 0.05) and gastrointestinal symptoms (P < 0.05). Abnormalities were reported in 40% of chest X-rays (N = 101). Physicians suspected TB in 92 patients (76%), lymphoma in 19 patients (16%), and other malignancy in seven patients (5.8%). Histological diagnoses (N = 117) included tuberculosis (34%), hyperplasia (27%), lymphoma (13%), and nonlymphoma malignancy (14%). Hyperplasia was more common (P < 0.001) and lymphoma less common (P = 0.005) among HIV-positive than HIV-negative patients. There was a trend toward reduced frequency of caseous necrosis in samples from HIV-positive than HIV-negative TB patients (67 versus 93%, P = 0.055). The spectrum of diagnoses was broad, and clinical and radiological features correlated poorly with diagnosis. On the basis of clinical features, physicians over-diagnosed TB, and under-diagnosed malignancy. Although this may not be inappropriate in resource-limited settings where TB is the most frequent easily treatable cause of lymphadenopathy, diagnostic delays can be detrimental to patients with malignancy. It is important that patients with lymphadenopathy undergo a full diagnostic work-up including sampling for histological evaluation to obtain an accurate diagnosis
Nutritional value of Acacia amentacea and parkinsonia texana grown in semiarid conditions
In order to evaluate the nutritional value of Parkinsonia texana and Acacia amentacea, two leguminosae species of the Tamaulipan scrubland, Northeastern Mexico, two experiments were carried out: the first tested the effects of season and browse species on chemical composition as nutritional variable to small ruminants;
the second tested the effect of the addition of polyethylene glycol (PEG) on fermentation parameters. Foliage samples were collected from three sites. Data of chemical composition were analysed using analysis of variance for a bi-factorial arrangement, whereas the
effect of PEG was analysed by a strip plot design. Results of chemical composition were affected by interacting factors season*species as individually they were significantly different (P<0.001). Addition of PEG affected (P<0.001) fermentation parameters.
Significantly higher values of neutral detergent fibre (42%), condensed tannins (19%), purines (9 μmol), partitioning factor (PF) (6.1) and gross energy losses (GEL=6.7%) were found in A. amentacea, while P. texana
gave higher crude protein (18%), in vitro true organic matter digestibility (82%), metabolisable energy (ME) [2.1 Mcal/kg dry matter (DM)], A (183 mL), c (0.07/h) and L (0.86 h). Addition of PEG increased ME, and affected
(P<0.001) fermentation parameters A and c, while purines and PF decreased. Results indicate that chemical composition and fermentation parameters vary according to seasons and species. PEG addition increases the fermentation parameters, which indicates that PEG
counteracts the detrimental effects of secondary components of samples. Data suggest that using both species combined could supply necessary nutritional requirements to small ruminants in the Tamaulipan scrubland
WSES Guidelines for the management of acute left sided colonic diverticulitis in the emergency setting
Acute left sided colonic diverticulitis is one of the most common clinical conditions encountered by surgeons in acute setting. A World Society of Emergency Surgery (WSES) Consensus Conference on acute diverticulitis was held during the 3rd World Congress of the WSES in Jerusalem, Israel, on July 7th, 2015. During this consensus conference the guidelines for the management of acute left sided colonic diverticulitis in the emergency setting were presented and discussed. This document represents the executive summary of the final guidelines approved by the consensus conference.Peer reviewe
