    Runoff simulation of ungauged catchments : importance in the Nepalese context

    Nepal is a landlocked country in the foothills of the Himalayan region in South Asia and a country endowed with rich water resources. However, the country is unable to utilize and manage the full potential of available water resources. One of the reasons for this is the lack of an adequate network of river gauging stations necessary to collect hydrologic data. Installation of hydrological stations is an expensive proposition and not financially viable for small water resources projects (water supply, irrigation, mini and micro-hydro projects). This research aims to address the challenges via an alternative strategy - i.e. the use of a hydrological model which can reliably simulate runoff in ungauged catchments even in the absence of adequate hydrologic data. SWAT (Soil & Water Assessment Tool), a popular simulation model with ArcGIS and QGIS interface, was chosen to simulate flow in an ungauged catchment in the mid-western region of Nepal. The model was applied to the West Rapti River basin using five years (1981-1985) of data from the Global Runoff Data Centre (GRDC). The GRDC was the only source, and the dataset was incomplete, limiting the model calibration and validation process. This limitation was addressed by using another simulation model, HEC-HMS (Hydrologic Engineering Centre’s Hydrologic Modeling System), for comparison. The results of the SWAT model were compared with those from HEC-HMS, one of the most widely used rainfall-runoff simulation models. Comparative analysis showed that both models generated comparable results. Historical rainfall data (1979-2009) were extracted from the Global Weather Data for SWAT to predict the rainfall trend in the West Rapti Watershed. This trend in rainfall pattern was used to extract rainfall and simulate runoff for 2023 to 2026, considering rainfall data of 2013 as a baseline. The simulated results showed a minor shift in time to peak and increased peak discharge. Similarly, the simulated runoff trends matched perfectly with the observed rainfall trend in SWAT. Thus, the results proved the reliability of SWAT to simulate runoff in the West Rapti Basin. The conclusion was drawn that the SWAT model can be used reliably to predict runoff in ungauged catchments that assist with managing water resources and contribute to the development of Nepal’s economy

    Prevalence of Substance Use and Associated Factors Among High School Adolescents in Rithepani, Lekhnath-2, kaski, Nepal

    Background: Substance use is a major public health concern in global settings, and is very common during adolescence period leading to physical and/or mental health complications. This study assessed the prevalence of substance use and associated factors among high school adolescents in Rithepani 2, Lekhnath, Kaski, 2073. Objectives: The study was designed to provide estimates of substance use by school-going adolescents in Lekhnath and to identify risk factors associated with. Methods: A school based cross-sectional study was conducted from 17th October to 21st October, 2016 among eighth to 12th grade high school students in the Rithepani-2, Lekhnath. Participants were select­ed by purposive sampling techniques, and data were collected using questionnaire. Frequency, percentage, means, SD and chi-square test was performed to identify factors associated with substance use. Results: Majority of the respondents 93 (56.4%) belonged to the age group 15 - 18 years. Majority of the respondents 88 (53.3%) were males and 50 (30.3%) were studying in grade 11. Majority of the respondents 140 (84.8%) were Hindus and 104 (63%) of the respondents belonged to upper caste group. In terms of education of the respondents’ parents, majority of the respondents’ mothers 68 (41.2%) had completed their secondary education and similarly, majority of the respondents’ fathers 78 (47.3%) had completed their secondary education. Majority of the respondents’ mothers 129 (78.2%) were housewives and majority of the respondents' fathers 48 (29.1%) were businessmen. Majority of the respondents 136 (82.5%) belonged to nuclear family and 93 (56.4%) had per month family income more than Rs 15,000. Among 165 respon­dents prevalence of substance use was found to be 10 (6%). Regarding the associated factors majority of the respondents 162 (98.2%) had good relation with their parents, 101 (61.2%) respon­dents’ family members do not use substance, 128 (77.6%) respondents reported that substance use was not accepted in their culture. the prev­alence of substance use by the respondents is 6.1% in which five (50%) respondents consume alcohol, eight (80%) take cigarette, one (10%) use tobacco and two (20%) take ganja. Among the substance users, four (40%) reported imitating parents and four (40%) reported curiosity as the cause for them to initiate substance use. Regarding the accessibility of the substances among the respondents who use substances, three (30%) respondents said that it’s very difficult whereas one (10%) said that it’s very easy for them to have access to the substances. Majority of the respondents who use substances 6 (60%) avail the substance/s from their friends. Regarding the Association, there is a significant association between prevalence of substance and substance use by family members with the χ2 value of 7.61 and p-value 0.006 which is less than 0.05 significant level. There is also a significant association between the prevalence of substance use and its cultural acceptance with the χ2 value of 4.65 and p-value of 0.031 which is less than 0.05 significant level. There is a significant association between the prevalence of substance abuse and ethnicity with the χ2 value of 11.81and with the p-value 0.037 which is less than 0.05 significant level. Conclusions: The prevalence of substance use among high school ado­lescent students in selected higher secondary school was found to be 6.1%. There was significant association between prevalence of sub­stances use and cultural acceptance of participants, ethnicity and use of substances by the family members.     Elective surgery patients walk to operating room instead of wheeled in on trolley: patient centered care

    Introductions: Habits and beliefs of earlier practitioners influence medicine. We continue to wheel in the elective surgery patients to operating room (OR) which makes them feel not in self-control and increases anxiety. With few exceptions of heavily sedated, in severe pain or frail patients, most prefer to walk to the OR accompanied by nurse and family, if given the opportunity, in line with patient centered care. We aim to assess feasibility to allow elective surgery patients walk to OR accompanied by nurse and family member. Methods:This was a cross-sectional observation of 100 consecutive elective surgery patients admitted at surgery department, Patan Hospital, Nepal. Adult patients of 15 years or above were allowed to walk to OR accompanied by a nurse and family members. Demographic profile, patient satisfaction, anxiety on arriving to OR, patient feeling of autonomy were recorded to assess patient centered care. Microsoft Excel was used to descriptively analyze the data. Ethical approval was obtained. Results:Of 100 consecutive patients, 62 were female and 38 male, average age 52 years (range 15 to 72), and abdomen surgery 62 (62%). Two patients did not want to walk. The 98 patients who walked to the OR, all were satisfied and 89 were not anxious. Conclusions: Elective surgery patients (98 out of 100) accompanied by nurse and family members walked to operating room satisfied and were not anxious.  Keywords: elective surgery, patient autonomy, patient-centered care, patient satisfaction, walking to operating roo

    Multi-serotype pneumococcal nasopharyngeal carriage prevalence in vaccine naĂŻve Nepalese children, assessed using molecular serotyping.

    Invasive pneumococcal disease is one of the major causes of death in young children in resource poor countries. Nasopharyngeal carriage studies provide insight into the local prevalence of circulating pneumococcal serotypes. There are very few data on the concurrent carriage of multiple pneumococcal serotypes. This study aimed to identify the prevalence and serotype distribution of pneumococci carried in the nasopharynx of young healthy Nepalese children prior to the introduction of a pneumococcal conjugate vaccine using a microarray-based molecular serotyping method capable of detecting multi-serotype carriage. We conducted a cross-sectional study of healthy children aged 6 weeks to 24 months from the Kathmandu Valley, Nepal between May and October 2012. Nasopharyngeal swabs were frozen and subsequently plated on selective culture media. DNA extracts of plate sweeps of pneumococcal colonies from these cultures were analysed using a molecular serotyping microarray capable of detecting relative abundance of multiple pneumococcal serotypes. 600 children were enrolled into the study: 199 aged 6 weeks to <6 months, 202 aged 6 months to < 12 months, and 199 aged 12 month to 24 months. Typeable pneumococci were identified in 297/600 (49.5%) of samples with more than one serotype being found in 67/297 (20.2%) of these samples. The serotypes covered by the thirteen-valent pneumococcal conjugate vaccine were identified in 44.4% of samples containing typeable pneumococci. Application of a molecular serotyping approach to identification of multiple pneumococcal carriage demonstrates a substantial prevalence of co-colonisation. Continued surveillance utilising this approach following the introduction of routine use of pneumococcal conjugate vaccinates in infants will provide a more accurate understanding of vaccine efficacy against carriage and a better understanding of the dynamics of subsequent serotype and genotype replacement

    Functional analysis of an unusual type IV pilus in the Gram-positive Streptococcus sanguinis

    Type IV pili (Tfp), which have been studied extensively in a few Gram-negative species, are the paradigm of a group of widespread and functionally versatile nano-machines. Here, we performed the most detailed molecular characterisation of Tfp in a Gram-positive bacterium. We demonstrate that the naturally competent Streptococcus sanguinis produces retractable Tfp, which like their Gram-negative counterparts can generate hundreds of piconewton of tensile force and promote intense surface-associated motility. Tfp power 'train-like' directional motion parallel to the long axis of chains of cells, leading to spreading zones around bacteria grown on plates. However, S. sanguinis Tfp are not involved in DNA uptake, which is mediated by a related but distinct nano-machine, and are unusual because they are composed of two pilins in comparable amounts, rather than one as normally seen. Whole genome sequencing identified a locus encoding all the genes involved in Tfp biology in S. sanguinis. A systematic mutational analysis revealed that Tfp biogenesis in S. sanguinis relies on a more basic machinery (only 10 components) than in Gram-negative species and that a small subset of four proteins dispensable for pilus biogenesis are essential for motility. Intriguingly, one of the piliated mutants that does not exhibit spreading retains microscopic motility but moves sideways, which suggests that the corresponding protein controls motion directionality. Besides establishing S. sanguinis as a useful new model for studying Tfp biology, these findings have important implications for our understanding of these widespread filamentous nano-machines

    Wheat yield forecast using detrended yield over a sub-humid climatic environment in five districts of Uttar Pradesh, India

    A study was carried out to forecast the yield of the wheat crop for five districts of Uttar Pradesh namely Lucknow, Kanpur, Banda, Jhansi and Faizabad. The daily weather data on variables such as maximum temperature, rainfall, minimum temperature, and relative humidity were arranged week wise from sowing to harvesting and the relations between the weather variables and yield was worked out using statistical tools like correlation and regression. The yield has been detrended by obtaining the parameter estimates of the model and subsequently the detrended yield was used to forecast the yield of the crop using ARIMA model. The proposed method of obtaining pre-harvest forecasting of yield of crops was compared with the traditional approaches of forecasting and the proposed method was evaluated in terms of criteria’s such as goodness of fit of the model. It was observed that in all the districts the proposed model performed better as compared to the traditional method both in terms of goodness of fit as well as forecasting performance. Thus it can be concluded that the proposed approach is better and more suitable as compared to the traditional approach for forecasting the wheat yield in the five districts of Uttar Pradesh
