    Components of the mediterranean diet on cardiovascular disease and mortality in a population at high cardiovascular risk.

    Les malalties cardiovasculars (CV) són una de les primeres causes de morbimortalitat a tot el món. Aquestes malalties, en gran mesura, es podrien prevenir. La Dieta Mediterrània ha estat reconeguda com un dels patrons alimentaris més saludables. Fins el moment, existeix una forta evidència científica que demostra els beneficis de la dieta Mediterrània en la prevenció i el tractament de la malaltia cardiovascular. Aquesta tesi ha estat realitzada en el context de l’estudi PREDIMED, un estudi clínic paral•lel, multi cèntric i aleatoritzat que avaluava l’efecte de la dieta Mediterrània, en comparació a una dieta baixa en greix, en la prevenció primària de la malaltia cardiovascular. L’objectiu va ser determinar l’efecte dels fruits secs, l'oli d’oliva i les seves varietats, i el magnesi en el risc cardiovascular, mortalitat per causa específica i mortalitat per totes les causes en una població Mediterrània amb alt risc cardiovascular. Tots els aliments avaluats són components claus del patró de dieta Mediterrània i són consumits en altes quantitats en la nostra població. Els resultats del nostre treball demostren que consumir fruits secs amb més freqüència estava inversament relacionat amb la mortalitat cardiovascular, mortalitat per càncer i mortalitat total després de seguir als participants durant una mitja de 4.8 anys. L’oli d’oliva, concretament la varietat extra verge, s’associava a un risc reduït de malaltia cardiovascular i mortalitat cardiovascular després de 4.8 anys de mitja de seguiment. També vam observar que el magnesi dietètic s’associava inversament a la mort cardiovascular, per càncer i mortalitat total. En conclusió, els resultats d’aquesta tesi corroboren els efectes beneficiosos dels components de la dieta Mediterrània en la prevenció de malaltia cardiovascular i mort.Las enfermedades cardiovasculares (CV) son una de las primeras causas de morbi-mortalidad en todo el mundo. El objetivo fue determinar el efecto de los frutos secos, aceite de oliva y magnesio en el riesgo cardiovascular, mortalidad por causa específica y mortalidad por todas las causas en una población Mediterráneo con alto riesgo cardiovascular. Todos los alimentos evaluados son componentes claves del patrón de dieta Mediterránea y son consumidos en altas cantidades en nuestra población. Los resultados del presente trabajo demostraron que consumir frutos secos con más frecuencia estaba inversamente relacionado con la mortalidad cardiovascular, mortalidad por cáncer y mortalidad total tras seguir a los participantes durante una media de 4.8 años. Observamos también que el aceite de oliva, concretamente la variedad extra virgen, se asociaba a un riesgo reducido de enfermedad cardiovascular y mortalidad cardiovascular después de 4.8 años de media de seguimiento. También observamos que el magnesio dietético se asociaba inversamente a la muerte cardiovascular, por cáncer y mortalidad total. En conclusión, los resultados corroboran los efectos beneficiosos de los componentes de la dieta Mediterránea en la prevención de enfermedad cardiovascular y muerte.Cardiovascular disease (CVD) is one of the main causes of disability and death worldwide. Importantly, in a large extent, CVD are preventable. The Mediterranean Diet (MedDiet) is recognized as one of the healthier dietary patterns. To date, strong evidence exists supporting the benefits of the MedDiet for the prevention and management of CVD. This thesis has been conducted in the framework of the PREDIMED Study, a parallel-group, multicenter randomized nutrition trial evaluating the efficacy of a MedDiet compared to a low-fat control diet on the primary prevention of CVD. We aimed to asses the associations between nuts, olive oil and its varieties, and magnesium on the risk of CVD, cause-specific and all-cause mortality on an elderly Mediterranean population at high cardiovascular risk. All of these foods are key components of the MedDiet pattern and are highly consumed in our population. The results of the present work demonstrate that the frequency of nut consumption was inversely related to cardiovascular, cancer and total mortality after 4.8 years of follow-up. We found that olive oil consumption, specifically the extra-virgin variety, was associated with reduced risk of cardiovascular disease and cardiovascular mortality after 4.8 years of follow-up. We have also observed that dietary magnesium intake was inversely associated with cardiovascular, cancer and total mortality risk after 4.8 years of follow-up. In conclusion, the findings of this thesis support the healthy benefits of the components of a MedDiet on the primary prevention of cardiovascular disease and mortality

    Effect of Peanut Consumption on Cardiovascular Risk Factors: A Randomized Clinical Trial and Meta-Analysis

    Although numerous studies have reported the protective effect of nut consumption on cardiovascular risk, evidence for the role of peanuts in maintaining cardiometabolic health is inconclusive. Presented here are the results from the ARISTOTLE study, a parallel randomized controlled trial evaluating the impact of regular peanut intake on anthropometric, biochemical, and clinical measurements. The 63 healthy subjects that completed the study consumed their habitual diet plus either: a) 25 g/day of skin roasted peanuts (SRP, n = 21), b) two tablespoons (32 g)/day of peanut butter (PB, n = 23) or c) two tablespoons (32 g)/day of a control butter based on peanut oil (CB, n = 19) for 6 months. In addition, a meta-analysis of clinical trials, including data from the ARISTOTLE study, was carried out to update the evidence for the effects of consuming peanuts, including high-oleic peanuts, and peanut butter on healthy subjects and those at high cardiometabolic risk. After a systematic search on PubMed, Web of Science, Cochrane Library and Scopus databases up to July 2021, 11 studies were found to meet the eligibility criteria. In the ARISTOTLE study, lower total cholesterol/HDL-cholesterol and LDL-cholesterol/HDL-cholesterol ratios were found in the SRP group compared to the CB group (p = 0.019 and p = 0.008). The meta-analysis of clinical trials revealed that peanut consumption is associated with a decrease in triglycerides (MD: −0.13; 95% CI, −0.20 to −0.07; p < 0.0001) and that healthy consumers had lower total cholesterol and LDL-cholesterol/HDL-cholesterol ratios compared to the control groups (MD: −0.40; 95% CI, −0.71 to −0.09; p = 0.01 and MD: −0.19; 95% CI, −0.36 to −0.01; p = 0.03, respectively). However, individuals at high cardiometabolic risk experienced an increase in body weight after the peanut interventions (MD: 0.97; 95% CI, 0.54 to 1.41; p < 0.0001), although not in body fat or body mass index. According to the dose-response analyses, body weight increased slightly with higher doses of peanuts. In conclusion, a regular consumption of peanuts seems to modulate lipid metabolism, reducing triglyceride blood levels.     A risk score to predict type 2 diabetes mellitus in an elderly spanish mediterranean population at high cardiovascular risk.

    Introduction: To develop and test a diabetes risk score to predict incident diabetes in an elderly Spanish Mediterranean population at high cardiovascular risk. Materials and Methods: A diabetes risk score was derived from a subset of 1381 nondiabetic individuals from three centres of the PREDIMED study (derivation sample). Multivariate Cox regression model ß-coefficients were used to weigh each risk factor. PREDIMED-personal Score included body-mass-index, smoking status, family history of type 2 diabetes, alcohol consumption and hypertension as categorical variables; PREDIMED-clinical Score included also high blood glucose. We tested the predictive capability of these scores in the DE-PLAN-CAT cohort (validation sample). The discrimination of Finnish Diabetes Risk Score (FINDRISC), German Diabetes Risk Score (GDRS) and our scores was assessed with the area under curve (AUC). Results: The PREDIMED-clinical Score varied from 0 to 14 points. In the subset of the PREDIMED study, 155 individuals developed diabetes during the 4.75-years follow-up. The PREDIMED-clinical score at a cutoff of $6 had sensitivity of 72.2%, and specificity of 72.5%, whereas AUC was 0.78. The AUC of the PREDIMED-clinical Score was 0.66 in the validation sample (sensitivity = 85.4%; specificity = 26.6%), and was significantly higher than the FINDRISC and the GDRS in both the derivation and validation samples. Discussion: We identified classical risk factors for diabetes and developed the PREDIMED-clinical Score to determine those individuals at high risk of developing diabetes in elderly individuals at high cardiovascular risk. The predictive capability of the PREDIMED-clinical Score was significantly higher than the FINDRISC and GDRS, and also used fewer items in the questionnaire

    Waist-to-Height Ratio and Cardiovascular Risk Factors in Elderly Individuals at High Cardiovascular Risk

    IntroductionSeveral anthropometric measurements have been associated with cardiovascular disease, type-2 diabetes mellitus and other cardiovascular risk conditions, such as hypertension or metabolic syndrome. Waist-to-height-ratio has been proposed as a useful tool for assessing abdominal obesity, correcting other measurements for the height of the individual. We compared the ability of several anthropometric measurements to predict the presence of type-2 diabetes, hyperglycemia, hypertension, atherogenic dyslipidemia or metabolic syndrome.Materials and MethodsIn our cross-sectional analyses we included 7447 Spanish individuals at high cardiovascular risk, men aged 55–80 years and women aged 60–80 years, from the PREDIMED study. Logistic regression models were fitted to evaluate the odds ratio of presenting each cardiovascular risk factor according to various anthropometric measures. The areas under the receiver-operating characteristic curve (AUC) were used to compare the predictive ability of these measurements.ResultsIn this relatively homogeneous cohort with 48.6% of type-2 diabetic individuals, the great majority of the studied anthropometric parameters were significantly and positively associated with the cardiovascular risk factors. No association was found between BMI and body weight and diabetes mellitus. The AUCs for the waist-to-height ratio and waist circumference were significantly higher than the AUCs for BMI or weight for type-2 diabetes, hyperglycemia, atherogenic dyslipidemia and metabolic syndrome. Conversely, BMI was the strongest predictor of hypertension.ConclusionsWe concluded that measures of abdominal obesity showed higher discriminative ability for diabetes mellitus, high fasting plasma glucose, atherogenic dyslipidemia and metabolic syndrome than BMI or weight in a large cohort of elderly Mediterranean individuals at high cardiovascular risk. No significant differences were found between the predictive abilities of waist-to-height ratio and waist circumference on the metabolic disease

    Cross-sectional assessment of nut consumption and obesity, metabolic syndrome and other cardiometabolic risk factors: the PREDIMED study

    INTRODUCTION: Prospective studies have consistently suggested that nut consumption is inversely related to fatal and non-fatal coronary heart disease. Limited data are available on the epidemiological associations between nut intake and cardiometabolic risk factors. OBJECTIVE: To evaluate associations between frequency of nut consumption and prevalence of cardiometabolic risk factors [obesity, metabolic syndrome (MetS), type-2 diabetes, hypertension, and dyslipidemia] in a Mediterranean population at high cardiovascular risk. MATERIALS AND METHODS: Cross-sectional study of 7,210 men and women (mean age, 67 y) recruited into the PREDIMED study. MetS was defined by the harmonized ATPIII and IDF criteria. Diabetes and hypertension were assessed by clinical diagnosis and dyslipidemia (high triglycerides, low HDL-cholesterol, and hypercholesterolemia) by lipid analyses. Nut consumption was assessed using a validated food frequency questionnaire and categorized as 3 servings/wk. Control of confounding was done with multivariate logistic regression. RESULTS: Compared to participants consuming 3 servings/wk had lower adjusted odds ratios (OR) for obesity (0.61, 95% confidence interval 0.54 to 0.68; P-trend <0.001), MetS (0.74, 0.65 to 0.85; P-trend<0.001), and diabetes (0.87, 0.78 to 0.99; P-trend = 0.043). Higher nut consumption was also associated with lower risk of the abdominal obesity MetS criterion (OR 0.68, 0.60 to 0.79; P-trend<0.001). No significant associations were observed for the MetS components high blood pressure, dyslipidemia, or elevated fasting glucose. CONCLUSIONS: Nut consumption was inversely associated with the prevalence of general obesity, central obesity, MetS, and diabetes in subjects at high cardiovascular risk

    High Plasma Glutamate and a Low Glutamine-to-Glutamate Ratio Are Associated with Increased Risk of Heart Failure but Not Atrial Fibrillation in the Prevención con Dieta Mediterránea (PREDIMED) Study

    [Background] Although the association between glutamate and glutamine in relation to cardiometabolic disorders has been evaluated, the role of these metabolites in the development of atrial fibrillation (AF) and heart failure (HF) remains unknown.[Objectives] We examined associations of glutamate, glutamine, and the glutamine-to-glutamate ratio with AF and HF incidence in a Mediterranean population at high cardiovascular disease (CVD) risk.[Methods] The present study used 2 nested case-control studies within the PREDIMED (Prevención con Dieta Mediterránea) study. During ∼10 y of follow‐up, there were 509 AF incident cases matched to 618 controls and 326 HF incident cases matched to 426 controls. Plasma concentrations of glutamate and glutamine were semiquantitatively profiled with LC–tandem MS. ORs were estimated with multivariable conditional logistic regression models. [Results] In fully adjusted models, per 1-SD increment, glutamate was associated with a 29% (95% CI: 1.08, 1.54) increased risk of HF and glutamine‐to‐glutamate ratio with a 20% (95% CI: 0.67, 0.94) decreased risk. Glutamine-to-glutamate ratio was also inversely associated with HF risk (OR per 1-SD increment: 0.80; 95% CI: 0.67, 0.94) when comparing extreme quartiles. Higher glutamate concentrations were associated with a worse cardiometabolic risk profile, whereas a higher glutamine-to-glutamate ratio was associated with a better cardiometabolic risk profile.     Metabolites related to purine catabolism and risk of type 2 diabetes incidence; modifying effects of the TCF7L2-rs7903146 polymorphism.

    Studies examining associations between purine metabolites and type 2 diabetes (T2D) are limited. We prospectively examined associations between plasma levels of purine metabolites with T2D risk and the modifying effects of transcription factor-7-like-2 (TCF7L2) rs7903146 polymorphism on these associations. This is a case-cohort design study within the PREDIMED study, with 251 incident T2D cases and a random sample of 694 participants (641 non-cases and 53 overlapping cases) without T2D at baseline (median follow-up: 3.8 years). Metabolites were semi-quantitatively profiled with LC-MS/MS. Cox regression analysis revealed that high plasma allantoin levels, including allantoin-to-uric acid ratio and high xanthine-to-hypoxanthine ratio were inversely and positively associated with T2D risk, respectively, independently of classical risk factors. Elevated plasma xanthine and inosine levels were associated with a higher T2D risk in homozygous carriers of the TCF7L2-rs7903146 T-allele. The potential mechanisms linking the aforementioned purine metabolites and T2D risk must be also further investigated
