Antibacterial treatment was investigated, taking into consideration its major importance for patients having IE with indications “quo ad vitam”. The patients were divided into two groups – group No. 1, the ones with a positive blood culture, and group No. 2 and the ones with a negative blood culture. We analyzed separately the sensitivity of the detected microorganisms from different studied groups to antibacterial preparations. We studied the frequency of use and effectiveness of antimicrobial preparations in both cases: when the etiology was known, and when the blood cultures were negative. Исследование основано на обследовании 94 пациентов с инфекционным эндокардитом (ИЭ). Антибиотикотерапия была изучена в виду её важности в борьбе с этим тяжелым инфекционным заболеванием. Пациенты были разделены на две группы: группа № 1 включала больных с положительным результатом гемокультуры, и группа № 2 с отрицательной гемокультурой. Отдельно мы проанализировали чувствительность обнаруженных микроорганизмов к различным группам антибактериальных препаратов. Влияние эмболических осложнений при инфекционном эндокардите

The incidence of cerebral embolism is 17-20% of all patients with IE, being probably underestimated because of its silent clinical evolution. Our survey data have revealed the presence of the embolic complications in 17% of all cases of IE, of which cerebral embolism prevailed 6.4%, followed by lung embolism in 4.3% of cases. The embolic syndrome developed more frequently in patients with valvular vegetations in 72.2%, being influenced by vegetation mobility, aortic location and by the pathogenic agent – Stph. Aureus. In IE, the embolic complications are widely undiagnosed and require imaging investigations (CT, MRI, Doppler investigation) for early diagnosis, initiation of appropriate treatment and improved prognosis in these patients.Инфекционный эндокардит (ИЭ) – это тяжёлое заболевание, с госпитальной летальностью до 20%, в основном из-за эмболических осложнений (ЭО), которые увеличивают риск смерти в 3 раза. Частота эмболий в мозговых артериях достигает 17-20% из всех пациентов с ИЭ. У обследованных нами больных эмболические осложнения развились в 17% случаях и преобладали церебральные эмболии в 6,4% случаях и легочные эмболии в 4,3% случаях. ЭО более часто, в 72,2% случаях, развились у пациентов с вегетациями на клапанах сердца, особое влияние имело мобильность вегетаций, аортальная локализация и возбудитель – золотистый стафилококк. Эмболические осложнения часто несвоевременно диагносцируются, из-за «скрытой» клиники, поэтому необходима ранняя диагностика (КТ, МРТ, Допплер сосудов) для уточнения диагноза, назначение своевременного лечения, критерии, которые повлияют положительно на прогноз пациентов с ИЭ

Electrocardiographic and echocardiographic changes in patients with thyreotoxic cardiopathy

Thyreotoxic cardiopathy represents a cardiac pathology, caused by the toxic influence of the thyroid hormones on the myocardium. The aim of this study is to determine the frequency of cardiovascular changes in patients with thyreotoxic cardiopathy according to the ECG and EchoCG results. The study included 72 patients with Graves’ disease and thyreotoxic cardiopathy, whose medical records have been analyzed. Such criteria, as heart rate, heart rhythm disorders, left ventricle hypertrophy, repolarization abnormalities (in ECG), ejection fraction, valve disorders, heart cavities dilation, left ventricle hypertrophy, impaired cardiac relaxation and pulmonary hypertension (in EchoCG) have been taken into consideration. The results showed a high percentage of patients with ECG disturbances (75%) and a high percentage of patients with EchoCG disturbances (100%). According to the ECG results, the disturbances included heart rhythm disorders (51.4%), left ventricle hypertrophy (36.1%) and repolarization abnormalities (53.3%). The most frequent heart rhythm disorders determined by ECG have been atrial fibrillation (19.5%), sinus tachycardia (13.9%), Guis’ fascicular blocks (13.9%) and extrasystoles (9.7%). The EchoCG examination of the patients identified valve disorders (94.4%), the left ventricle hypertrophy (52.8%), the left ventricle impaired relaxation (11.1%), the simultaneous dilatation of three cavities of heart (left atrium, right ventricle and right atrium) (26.4%) and pulmonary hypertension (52.8%). However, the influence of concomitant cardiovascular diseases (25%) on the obtained results cannot be excluded, since the moment of their appearance is unknown

Prognostic value of renal function estimating formulas in type 2 cardiorenal syndrome

Renal function worsening is often observed in chronic heart failure (CHF). Glomerular filtration rate (GFR) is widely accepted as a marker for renal function evaluation, and usually, is estimated with the use of creatinine-based formulas.Purpose.To compare the prognostic value of estimating glomerular filtration rate formulas in type 2 cardiorenal syndrome.Material and methods.A total of 170 consecutive hospitalized CHF patients with intermediate or reduced ejection fraction and renal changes (51.8 % men, age 68.33 ± 1.06 years) were studied. Renal function and GFR) was assessed using the most popular formulas for GFR estimation: the Cockcroft-Gault (CG), the four-variable Simplified Modification of Diet in Renal Disease (sMDRD), CKD-EpidemiologyDiet in Renal Disease (sMDRD), CKD-Epidemiology Collaboration (CKD-EPI) based on serum cystatin-C, creatinine and their combination, and the simple cystatin-C formula.Results.During 6 months follow-up, 29 (16.2%) deaths were recorded. Renal biomarkers and estimated GFR showed different prognostic value. AUC was 0.58 (95%CI:0.47-0.69, P = 0.05) for creatinine, 0.67 (95% CI: 0.56-0.79, P< 0.05) for cystatin-C. Conclusions.GFR is an independent predictor for type 2 cardiorenal syndrome short term mortality. Cystatin-C based formulas for offer improved prognostication in this population, while CG formula, serum cystatin-C and serum creatinine fail to predict short term mortality

Socio-Economic Variations Determine the Clinical Presentation, Aetiology and Outcome of Infective Endocarditis: a Prospective Cohort Study from the ESC-EORP EURO-ENDO (European Infective Endocarditis) Registry

Background: Infective endocarditis (IE) is a life-threatening disease associated with high mortality and morbidity worldwide. We sought to determine how socio-economic factors might influence its epidemiology, clinical presentation, investigation and management, and outcome, in a large international multi-centre registry. Methods: The EurObservationalProgramme (EORP) of the European Society of Cardiology (ESC) EURO-ENDO registry comprises a prospective cohort of 3113 adult patients admitted for IE in 156 hospitals in 40 countries between January 2016 and March 2018. Patients were separated in 3 groups, according to World Bank economic stratification (Group 1 - high income [75.6%]; Group 2 - upper-middle income [15.4%]; Group 3 - lower-middle income [9.1%]). Results: Group 3 patients were younger (median age [IQR]: Group 1 - 66 [53-75] years; Group 2 - 57 [41-68] years; Group 3 - 33 [26-43] years; p&lt;0.001) with a higher frequency of smokers, intravenous drug use and human immunodeficiency virus (HIV) infection (all p&lt;0.001) and presented later (median [IQR) days since symptom onset: Group 1 - 12 [3-35]; Group 2 - 19 [6-54]; Group 3 - 31 [12-62]; p&lt;0.001) with a higher likelihood of developing congestive heart failure (13.6%; 11.1%; and 22.6%, respectively; p&lt;0.001) and persistent fever (9.8%; 14.2%; 27.9%; p&lt;0.001). Among 2157 (69.3%) patients with theoretical indication for cardiac surgery, surgery was performed less frequently in Group 3 patients (75.5%, 76.8% and 51.3%, respectively p&lt;0.001) who also demonstrated the highest mortality (15.0%, 23.0% and 23.7%, respectively; p&lt;0.001). Conclusions: Socio-economic factors influence the clinical profile of patients presenting with IE across the world. Despite younger age, patients from the poorest countries presented with more frequent complications and higher mortality associated with delayed diagnosis and lower use of surgery

Surgery and outcome of infective endocarditis in octogenarians: prospective data from the ESC EORP EURO-ENDO registry

Purpose: High mortality and a limited performance of valvular surgery are typical features of infective endocarditis (IE) in octogenarians, even though surgical treatment is a major determinant of a successful outcome in IE. Methods: Data from the prospective multicentre ESC EORP EURO-ENDO registry were used to assess the prognostic role of valvular surgery depending on age. Results: As compared to &lt; 80&nbsp;yo patients, ≥ 80&nbsp;yo had lower rates of theoretical indication for valvular surgery (49.1% vs. 60.3%, p &lt; 0.001), of surgery performed (37.0% vs. 75.5%, p &lt; 0.001), and a higher in-hospital (25.9% vs. 15.8%, p &lt; 0.001) and 1-year mortality (41.3% vs. 22.2%, p &lt; 0.001). By multivariable analysis, age per se was not predictive of 1-year mortality, but lack of surgical procedures when indicated was strongly predictive (HR 2.98 [2.43–3.66]). By propensity analysis, 304 ≥ 80&nbsp;yo were matched to 608 &lt; 80&nbsp;yo patients. Propensity analysis confirmed the lower rate of indication for valvular surgery (51.3% vs. 57.2%, p = 0.031) and of surgery performed (35.3% vs. 68.4%, p &lt; 0.0001) in ≥ 80&nbsp;yo. Overall mortality remained higher in ≥ 80&nbsp;yo (in-hospital: HR 1.50[1.06–2.13], p = 0.0210; 1-yr: HR 1.58[1.21–2.05], p = 0.0006), but was not different from that of &lt; 80&nbsp;yo among those who had surgery (in-hospital: 19.7% vs. 20.0%, p = 0.4236; 1-year: 27.3% vs. 25.5%, p = 0.7176). Conclusion: Although mortality rates are consistently higher in ≥ 80&nbsp;yo patients than in &lt; 80&nbsp;yo patients in the general population, mortality of surgery in ≥ 80&nbsp;yo is similar to &lt; 80&nbsp;yo after matching patients. These results confirm the importance of a better recognition of surgical indication and of an increased performance of surgery in ≥ 80&nbsp;yo patients

The ESC-EORP EURO-ENDO (European Infective Endocarditis) registry

Aims: The European Society of Cardiology (ESC) EURObservational Research Programme (EORP) European Endocarditis (EURO-ENDO) registry aims to study the care and outcomes of patients diagnosed with infective endocarditis (IE) and compare findings with recommendations from the 2015 ESC Clinical Practice Guidelines for the management of IE and data from the 2001 Euro Heart Survey. Methods and results: Patients (n = 3116) aged over 18 years with a diagnosis of IE based on the ESC 2015 IE diagnostic criteria were prospectively identified between 1 January 2016 and 31 March 2018. Individual patient data were collected across 156 centres and 40 countries. The primary endpoint is all-cause mortality in hospital and at 1 year. Secondary endpoints are 1-year morbidity (all-cause hospitalization, any cardiac surgery, and IE relapse), the clinical, epidemiological, microbiological, and therapeutic characteristics of patients, the number and timing of non-invasive imaging techniques, and adherence to recommendations as stated in the 2015 ESC Clinical Practice Guidelines for the management of IE. Conclusion: EURO-ENDO is an international registry of care and outcomes of patients hospitalized with IE which will provide insights into the contemporary profile and management of patients with this challenging disease

Surgery and outcome of infective endocarditis in octogenarians: prospective data from the ESC EORP EURO-ENDO registry

Purpose: High mortality and a limited performance of valvular surgery are typical features of infective endocarditis (IE) in octogenarians, even though surgical treatment is a major determinant of a successful outcome in IE. Methods: Data from the prospective multicentre ESC EORP EURO-ENDO registry were used to assess the prognostic role of valvular surgery depending on age. Results: As compared to < 80 yo patients, ≥ 80 yo had lower rates of theoretical indication for valvular surgery (49.1% vs. 60.3%, p < 0.001), of surgery performed (37.0% vs. 75.5%, p < 0.001), and a higher in-hospital (25.9% vs. 15.8%, p < 0.001) and 1-year mortality (41.3% vs. 22.2%, p < 0.001). By multivariable analysis, age per se was not predictive of 1-year mortality, but lack of surgical procedures when indicated was strongly predictive (HR 2.98 [2.43–3.66]). By propensity analysis, 304 ≥ 80 yo were matched to 608 < 80 yo patients. Propensity analysis confirmed the lower rate of indication for valvular surgery (51.3% vs. 57.2%, p = 0.031) and of surgery performed (35.3% vs. 68.4%, p < 0.0001) in ≥ 80 yo. Overall mortality remained higher in ≥ 80 yo (in-hospital: HR 1.50[1.06–2.13], p = 0.0210; 1-yr: HR 1.58[1.21–2.05], p = 0.0006), but was not different from that of < 80 yo among those who had surgery (in-hospital: 19.7% vs. 20.0%, p = 0.4236; 1-year: 27.3% vs. 25.5%, p = 0.7176). Conclusion: Although mortality rates are consistently higher in ≥ 80 yo patients than in < 80 yo patients in the general population, mortality of surgery in ≥ 80 yo is similar to < 80 yo after matching patients. These results confirm the importance of a better recognition of surgical indication and of an increased performance of surgery in ≥ 80 yo patients
