    When Patient Activation Levels Change, Health Outcomes and Costs Change, Too

    Patient engagement has become a major focus of health reform. However, there is limited evidence showing that increases in patient engagement are associated with improved health outcomes or lower costs. This report examined the extent to which a single assessment of engagement, the Patient Activation Measure, was associated with health outcomes and costs over time, and whether changes in assessed activation were related to expected changes in outcomes and costs. The report uses data on adult primary care patients from a single large health care system where the Patient Activation Measure is routinely used. Results indicating higher activation in 2010 were associated with nine out of thirteen better health outcomes -- including better clinical indicators, more healthy behaviors, and greater use of women's preventive screening tests -- as well as with lower costs two years later. Changes in activation level were associated with changes in over half of the health outcomes examined, as well as costs, in the expected directions. These findings suggest that efforts to increase patient activation may help achieve key goals of health reform and that further research is warranted to examine whether the observed associations are causal

    Community development projects continue despite the COVID-19 pandemic. Participatory evaluation of these projects is crucial. Ripple effect mapping (REM) is a participatory approach to evaluation that captures coalition and community member perspectives on program outcomes and impacts. In response to COVID-19, the Louisiana State University AgCenter Healthy Communities Initiative adapted REM for online delivery. The REM evaluation was found to be an effective way for community coalitions to reflect on outcomes and impacts and to motivate continued engagement

    Diversifying the Academy: Librarians Coaching Mellon Mays Undergraduate Fellows through the Scholarly Research Cycle

    Increasing diversity within the scholarly community is a priority for academic libraries. Often the focus is on diversifying our staff, programs, collections, and services. But the library can also have an important role to play in campus diversity initiatives that focus on equity and inclusion among the student body and professorate. This poster will detail a library coaching program developed as part of a local instance of the Mellon Mays Undergraduate Fellowship program, a nationwide initiative to increase faculty diversity by supporting students of color to pursue PhDs and careers as professors. The coaching program pairs librarians with undergraduates in their junior and senior years. Library coaches build a research-focused relationship with students geared toward ongoing empowerment and the development of the research and information literacy skills that students need to be successful. The poster will present the key elements of this successful program including background, setting expectations for coaches, librarian training and development, program assessment, and strategies for engagement with campus stakeholders. The poster will also elaborate on how the program can serve as a model for coaching other student cohorts that would benefit from additional librarian support, such as first-generation students, affinity groups, student athletes, library student workers, or honors students. This poster is relevant to those who perform outreach to student groups and programs and/or are involved in diversity initiatives, as well as all who are interested in alternative teaching and learning programming. Attendees will leave with concrete ideas they can apply at their own institutions

    Distance to the RR Lyrae Star V716 Monocerotis

    We present high quality BVRI CCD photometry of the variable star V716 Monocerotis (= NSV 03775). We confirm it to be an RR Lyrae star of variability type ab (i.e. a fundamental mode pulsator), and determine its metallicity ([Fe/H] = -1.33 +/- 0.25), luminosity (Mv = 0.80 +/- 0.06), and foreground reddening (E(B-V) = 0.05-0.17) from the Fourier components of its light curve. These parameters indicate a distance of 4.1 +/- 0.3 kpc, placing V716 Mon near the plane of the Galaxy well outside the solar circle.     Temporal and spatial variation in springtime ichthyoplankton assemblages in Puget Sound: the search for an ecological baseline

    Our knowledge of historical baselines for many marine fisheries is scant, making it difficult to determine the extent of change in commercial and non-commercial stocks alike. Providing a unique glimpse at entire communities and relatively easy to sample, ichthyoplankton surveys are a valuable tool for assessing change in populations. Our study evaluates the degree of spatial and temporal variation in larval fish assemblages across the sub-basins of Puget Sound by comparing historical and current surveys. Historical data for comparison was drawn from a study in 1967 conducted throughout the sub-basins (Waldron 1972). Larval fish were also collected in April of 2011, using a combination of horizontal and vertical tows, at 77 sample sites across a similar spatial extent. Although densities were fairly constant in both studies, we found a region-wide decrease in the density of previously dominant families, most notably in Merluccidae (hake) and Gadidae (cod) which experienced a 98% and 85% decrease, respectively, as well as the total disappearance of smaller, poorly understood families like Cyclopteridae (lumpsuckers). Within sub-basins, a substantial change at a compositional level was observed, shifting away from few, dominant families to more diverse assemblages. These findings reflect significant temporal and spatial changes in ichthyoplankton assemblages over the past 44 years that parallel changes in commercial harvest practices. When examined alongside coastal larval data over the same time frame, we found coastal stocks do not follow trends observed in Puget Sound in terms of changes in species composition or disappearances. In demonstrating the value of larval studies as a tool to assess long-term composition and density changes, we call for additional efforts to describe and monitor larval fish densities in Puget Sound to better our understanding of adult population dynamics

    Relationship Between Hispanic Nativity, Residential Environment, and Productive Activity Among Individuals With Traumatic Brain Injury: A TBI Model Systems Study

    Objective: To examine the influence of nativity and residential characteristics on productive activity among Hispanics at 1 year after traumatic brain injury (TBI). Setting: Acute rehabilitation facilities and community follow-up. Participants: A total of 706 Hispanic individuals in the TBI Model Systems National Database. Design: Secondary data analysis from a multicenter longitudinal cohort study. Main Measures: Nativity (foreign born or US native), productive activity derived from interview questions regarding employment status, and other demographic information. Census data were extracted by zip code to represent residential characteristics of aggregate household income and proportion of foreign language speakers (FLS). Results: Among foreign-born individuals with TBI, those living in an area with a higher proportion of FLS were 2.8 times more likely to be productive than those living in areas with a lower proportion of FLS. Among individuals living in an area with a lower proportion of FLS, US-born Hispanics were 2.7 times more likely to be productive compared with Hispanic immigrants. Conclusion: The relationship between nativity and productive activity at 1 year post-TBI was moderated by the residential proportion of FLS. Findings underscore the importance of considering environmental factors when designing vocational rehabilitation interventions for Hispanics after TBI

    Enhancement and suppression effects resulting from information structuring in sentences

    Information structuring through the use of cleft sentences increases the processing efficiency of references to elements within the scope of focus. Furthermore, there is evidence that putting certain types of emphasis on individual words not only enhances their subsequent processing, but also protects these words from becoming suppressed in the wake of subsequent information, suggesting mechanisms of enhancement and suppression. In Experiment 1, we showed that clefted constructions facilitate the integration of subsequent sentences that make reference to elements within the scope of focus, and that they decrease the efficiency with reference to elements outside of the scope of focus. In Experiment 2, using an auditory text-change-detection paradigm, we showed that focus has similar effects on the strength of memory representations. These results add to the evidence for enhancement and suppression as mechanisms of sentence processing and clarify that the effects occur within sentences having a marked focus structure

    Physical activity and academic achievement across the curriculum (A + PAAC): rationale and design of a 3-year, cluster-randomized trial

    Abstract Background Improving academic achievement and reducing the rates of obesity in elementary school students are both of considerable interest. Increased physical activity during academic instruction time during school offers a potential intervention to address both issues. A program titled “Physical Activity Across the Curriculum” (PAAC) was developed in which classroom teachers in 22 elementary schools were trained to deliver academic instruction using physical activity with a primary aim of preventing increased BMI. A secondary analysis of data assessed the impact of PAAC on academic achievement using the Weschler Individual Achievement Test-II and significant improvements were shown for reading, math and spelling in students who participated in PAAC. Based on the results from PAAC, an adequately powered trial will be conducted to assess differences in academic achievement between intervention and control schools called, “Academic Achievement and Physical Activity Across the Curriculum (A + PAAC).” Methods/design Seventeen elementary schools were cluster randomized to A + PAAC or control for a 3-year trial. Classroom teachers were trained to deliver academic instruction through moderate-to-vigorous physical activity with a target of 100+ minutes of A + PAAC activities per week. The primary outcome measure is academic achievement measured by the Weschler Individual Achievement Test-III, which was administered at baseline (Fall 2011) and will be repeated in the spring of each year by assessors blinded to condition. Potential mediators of any association between A + PAAC and academic achievement will be examined on the same schedule and include changes in cognitive function, cardiovascular fitness, daily physical activity, BMI, and attention-to-task. An extensive process analysis will be conducted to document the fidelity of the intervention. School and student recruitment/randomization, teacher training, and baseline testing for A + PAAC have been completed. Nine schools were randomized to the intervention and 8 to control. A random sample of students in each school, stratified by gender and grade (A + PAAC = 370, Control = 317), was selected for outcome assessments from those who provided parental consent/child assent. Baseline data by intervention group are presented. Discussion If successful, the A + PAAC approach could be easily and inexpensively scaled and disseminated across elementary schools to improve both educational quality and health.     Family Planning Decisions, Perceptions and Gender Dynamics among Couples in Mwanza, Tanzania: A Qualitative Study.

    Contraceptive use is low in developing countries which are still largely driven by male dominated culture and patriarchal values. This study explored family planning (FP) decisions, perceptions and gender dynamics among couples in Mwanza region of Tanzania. Twelve focus group discussions and six in-depth interviews were used to collect information from married or cohabiting males and females aged 18-49. The participants were purposively selected. Qualitative methods were used to explore family planning decisions, perceptions and gender dynamics among couples. A guide with questions related to family planning perceptions, decisions and gender dynamics was used. The discussions and interviews were tape-recorded, transcribed verbatim and analyzed manually and subjected to content analysis. Four themes emerged during the study. First, "risks and costs" which refer to the side effects of FP methods and the treatment of side -effects as well as the costs inherit in being labeled as an unfaithful spouse. Second, "male involvement" as men showed little interest in participating in family planning issues. However, the same men were mentioned as key decision-makers even on the number of children a couple should have and the child spacing of these children. Third, "gender relations and communication" as participants indicated that few women participated in decision-making on family planning and the number of children to have. Fourth, "urban-rural differences", life in rural favoring having more children than urban areas therefore, the value of children depended on the place of residence. Family Planning programs should adapt the promotion of communication as well as joint decision-making on FP among couples as a strategy aimed at enhancing FP use
