    A Mobile and Web Platform for Crowdsourcing OBD-II Vehicle Data

    On-Board Diagnostics 2 (OBD-II) protocol allows monitoring vehicle status parameters. Analyzing them is highly useful for Intelligent Transportation Systems (ITS) research, applications and services. Unfortunately, large-scale OBD datasets are not publicly available due to the effort of producing them as well as due to competitiveness in the automotive sector. This paper proposes a framework to enable a worldwide crowdsourcing approach to the generation of OBD-II data, similarly to OpenStreetMap (OSM) for cartography. The proposal comprises: (i) an extension of the GPX data format for route logging, augmented with OBD-II parameters; (ii) a fork of an open source Android OBD-II data logger to store and upload route traces, and (iii) a Web platform extending the OSM codebase to support storage, search and editing of traces with embedded OBD data. A full platform prototype has been developed and early scalability tests have been carried out in various workloads to assess the sustainability of the proposal

    Higher 90-Day Mortality after Surgery for Hip Fractures in Patients with COVID-19: A Case-Control Study from a Single Center in Italy

    The mortality of hip fracture (HF) patients is increased by concomitant COVID-19; however, evidence is limited to only short follow-up. A retrospective matched case-control study was designed with the aim to report the 90-day mortality and determine the hazard ratio (HR) of concomitant HF and COVID-19 infection. Cases were patients hospitalized for HF and diagnosed with COVID-19. Controls were patients hospitalized for HF not meeting the criteria for COVID-19 diagnosis and were individually matched with each case through a case-control (1:3) matching algorithm. A total of 89 HF patients were treated during the study period, and 14 of them were diagnosed as COVID-19 positive (overall 15.7%). Patients' demographic, clinical, and surgical characteristics were similar between case and control groups. At 90 days after surgery, 5 deaths were registered among the 14 COVID-19 cases (35.7%) and 4 among the 42 HF controls (9.5%). COVID-19-positive cases had a higher risk of mortality at 30 days (HR = 4.51; p = 0.0490) and 90 days (HR = 4.50; p = 0.025) with respect to controls. Patients with concomitant HF and COVID-19 exhibit high perioperative mortality, which reaches a plateau of nearly 30-35% after 30 to 45 days and is stable up to 90 days. The mortality risk is more than four-fold higher in patients with COVID-19

    Measuring the photon distribution by ON/OFF photodectors

    Reconstruction of photon statistics of optical states provide fundamental information on the nature of any optical field and find various relevant applications. Nevertheless, no detector that can reliably discriminate the number of incident photons is available. On the other hand the alternative of reconstructing density matrix by quantum tomography leads to various technical difficulties that are particular severe in the pulsed regime (where mode matching between signal an local oscillator is very challenging). Even if on/off detectors, as usual avalanche PhotoDiodes operating in Geiger mode, seem useless as photocounters, recently it was shown how reconstruction of photon statistics is possible by considering a variable quantum efficiency. Here we present experimental reconstructions of photon number distributions of both continuous-wave and pulsed light beams in a scheme based on on/off avalanche photodetection assisted by maximum-likelihood estimation. Reconstructions of the distribution for both semiclassical and quantum states of light (as single photon, coherent, pseudothermal and multithermal states) are reported for single-mode as well as for multimode beams. The stability and good accuracy obtained in the reconstruction of these states clearly demonstrate the interesting potentialities of this simple technique.Comment: 6 pages, 7 figures, to appear on Laser Physic

    Management of respiratory tract exacerbations in people with cystic fibrosis: Focus on imaging

    Respiratory tract exacerbations play a crucial role in progressive lung damage of people with cystic fibrosis, representing a major determinant in the loss of functional lung tissue, quality of life and patient survival. Detection and monitoring of respiratory tract exacerbations are challenging for clinicians, since under- and over-treatment convey several risks for the patient. Although various diagnostic and monitoring tools are available, their implementation is hampered by the current definition of respiratory tract exacerbation, which lacks objective “cut-offs” for clinical and lung function parameters. In particular, the latter shows a large variability, making the current 10% change in spirometry outcomes an unreliable threshold to detect exacerbation. Moreover, spirometry cannot be reliably performed in preschool children and new emerging tools, such as the forced oscillation technique, are still complementary and need more validation. Therefore, lung imaging is a key in providing respiratory tract exacerbation-related structural and functional information. However, imaging encompasses several diagnostic options, each with different advantages and limitations; for instance, conventional chest radiography, the most used radiological technique, may lack sensitivity and specificity in respiratory tract exacerbations diagnosis. Other methods, including computed tomography, positron emission tomography and magnetic resonance imaging, are limited by either radiation safety issues or the need for anesthesia in uncooperative patients. Finally, lung ultrasound has been proposed as a safe bedside option but it is highly operator-dependent and there is no strong evidence of its possible use during respiratory tract exacerbation. This review summarizes the clinical challenges of respiratory tract exacerbations in patients with cystic fibrosis with a special focus on imaging. Firstly, the definition of respiratory tract exacerbation is examined, while diagnostic and monitoring tools are briefly described to set the scene. This is followed by advantages and disadvantages of each imaging technique, concluding with a diagnostic imaging algorithm for disease monitoring during respiratory tract exacerbation in the cystic fibrosis patient

    Acute Delta Hepatitis in Italy spanning three decades (1991–2019): Evidence for the effectiveness of the hepatitis B vaccination campaign

    Updated incidence data of acute Delta virus hepatitis (HDV) are lacking worldwide. Our aim was to evaluate incidence of and risk factors for acute HDV in Italy after the introduction of the compulsory vaccination against hepatitis B virus (HBV) in 1991. Data were obtained from the National Surveillance System of acute viral hepatitis (SEIEVA). Independent predictors of HDV were assessed by logistic-regression analysis. The incidence of acute HDV per 1-million population declined from 3.2 cases in 1987 to 0.04 in 2019, parallel to that of acute HBV per 100,000 from 10.0 to 0.39 cases during the same period. The median age of cases increased from 27 years in the decade 1991-1999 to 44 years in the decade 2010-2019 (p < .001). Over the same period, the male/female ratio decreased from 3.8 to 2.1, the proportion of coinfections increased from 55% to 75% (p = .003) and that of HBsAg positive acute hepatitis tested for by IgM anti-HDV linearly decreased from 50.1% to 34.1% (p < .001). People born abroad accounted for 24.6% of cases in 2004-2010 and 32.1% in 2011-2019. In the period 2010-2019, risky sexual behaviour (O.R. 4.2; 95%CI: 1.4-12.8) was the sole independent predictor of acute HDV; conversely intravenous drug use was no longer associated (O.R. 1.25; 95%CI: 0.15-10.22) with this. In conclusion, HBV vaccination was an effective measure to control acute HDV. Intravenous drug use is no longer an efficient mode of HDV spread. Testing for IgM-anti HDV is a grey area requiring alert. Acute HDV in foreigners should be monitored in the years to come
