    Myasthenia Gravis (MG) is a long-term neuromuscular disease that leads to varying degrees of skeletal muscle weakness. The underlying defect is a decrease in the number of available acetylcholine receptors (AChRs) at neuromuscular junctions due to an antibody mediated autoimmune attack. The most commonly affected muscles are those of the eyes, face and swallowing. The cause of this disease can be understood in view of Upahata Dhatu Ushma, Srotas and Marutha respectively. The present observation was conducted with an objective to find out the efficacy of Ayurvedic management in Myasthenia gravis. Here is a case of 50years old Hindu female who was diagnosed as myasthenia gravis (MG) reported with complaining of slurred speech, low pitch of voice, difficulty to open the mouth and to swallow food and weakness in the both upper limbs since 4months was registered in OPD of SKAMCH&RC, Bengaluru. Considering the signs and symptoms patient was treated on the line of Ardita Vata chikitsa, Sarvanga Abhyanga with Moorchita tila taila, Nasya with Yashtimadhu taila, Gandusha with Erimedadi taila, Jihwa nirlekhana with Vacha churna were done. Treatment shows significant improvement in the symptoms without any side effects

    Background: Allergic rhinitis also called hay fever; it is a very common in India. Harmless exposing substances cause an allergic reaction. Allergic rhinitis is of the two types, one is the seasonal occurred with the change of seasons, second is the perennial means any time during the year.Methods: This was a cross-sectional study to determine the cost analysis allergic rhinitis drugs used in treatment of ENT OPD in a tertiary care teaching hospital, Kamothe, Navi Mumbai. The patients were interviewed; prescriptions were analyzed number of medicines prescribed.Results: Incidence of polypharmacy two drugs per prescriptions were prescribed 140 and three drugs per prescriptions 60. In the total number of 200 prescriptions, the cost of 43 prescriptions were in between 251-300 Indian rupees and 18 prescriptions were 301-350 Indian rupees. Patient knowledge about the drug use in this study, out of 200 pts 37% of allergic rhinitis patients knew the correct dosage of the prescribed drugs while 63% were not aware.Conclusions: Majority of the participants drugs were prescribed two to three drugs, per prescription cost was high because all the drugs prescribed by brand name and no drugs were prescribed by generic name. Improvement in knowledge about generic medicines is important factor to prefer generic medicine. Reduction in cost of drug will improve compliance

    Background: This study aimed to assess pain and quality of life in a large cohort of patients with multiple hereditary exostoses. Methods: All 322 known patients with multiple hereditary exostoses in the Netherlands were asked to participate. An age-specific questionnaire was sent to children (less than eighteen years old) and adults. The questionnaire focused on pain, daily activities, and school and/or professional situation. Adults also filled out the RAND-36 questionnaire.Results were statistically analyzed with use of the SPSS 15.0 software and with the chi-square test and multiple logistic regression. A p value of <0.05 was regarded as significant. Results: Two hundred and eighty-three patients (88%), including 184 adults (65%) and ninety-nine children (35%), completed the questionnaire. Multiple hereditary exostoses resulted in various physical and social consequences. The majority of adults (119) were employed; however, thirty-three (28%) had changed jobs because of the symptoms of multiple hereditary exostoses and twenty-five (21%) required adjustments in their working environment. Of the sixty-five adults who were not employed, thirteen were medically unfit to work. Of eighty-five children attending school, forty-five (53%) experienced problems at school. The symptoms of multiple hereditary exostoses caused twenty-seven children (27%) and eighty-five adults (46%) to stop participating in sporting activities. Pain was the greatest problem, with sixty-two children (63%) and 152 adults (83%) who reported recent pain. On multivariate analysis, pain in adults was correlated most significantly with age and problems at work, and pain in children was correlated with the perception of the disease and problems at school. Adult patients with multiple hereditary exostoses had a lower quality of life than the Dutch reference groups, with lower scores on six of eight RAND-36 subscales. Conclusions: Our study confirms that multiple hereditary exostoses is a chronic disease causing a profound impact on quality of life. The results suggest that pain is not the only problem associated with multiple hereditary exostoses, as it has an extensive influence on daily activities, as well as on social and psychological well-being, causing significant disability

    Wide-field choroidal thickness profile in healthy eyes

    The introduction of wide field optical coherence tomography (WF-OCT) has provided newer insights in the imaging of peripheral choroid. We evaluated choroidal thickness (CT) and large choroidal vessel thickness (LCVT) of 20 eyes in horizontal and vertical meridians using WF-OCT. A high-definition line scan through the fovea in both horizontal and vertical meridian was captured in primary and extremes of gaze to obtain scans up to mid-equator. CT and LCVT measurements were done across predefined points in macular area and all quadrants. LCVT was calculated after identifying a large choroidal vessel near choroidoscleral interface. The main outcome measures were differences in CT and LCVT in macular and four quadrants. Mean CT (331.23\u2009\ub1\u200976.34\u2009\ub5) and LCVT (201.46\u2009\ub1\u200954.31\u2009\ub5) in vertical macular segment were significantly more than CT (245.79\u2009\ub1\u200955.38\u2009\ub5; p\u2009=\u20090.0002) and LCVT (150.48\u2009\ub1\u200952.58\u2009\ub5; p\u2009=\u20090.004) in horizontal macular segment. CT at peripheral points in all quadrants was significantly reduced as compared to subfoveal CT (all p values\u2009&lt;\u20090.05) with maximum reduction in inferior quadrant (64.5%). Using linear regression, only quadrant had a significant effect on CT and LCVT (both p\u2009&lt;\u20090.001). CT and LCVT are highest at the macular area with reduction towards the periphery. The contribution of LCVT to CT is less at the fovea compared to other peripheral points

    A prospective, parallel group, open label, observational study to compare efficacy and feto-maternal outcomes in treatment of pregnancy induced hypertensive patients

    Background: Pregnancy induced  hypertension (PIH) is defined as  elevation in blood pressure 140/90 mmHg or greater after 20 weeks of gestation  on two occasions more than 4 hours apart in a newly diagnosed patients. Complications of pregnancy are the major health problems leading to maternal and perinatal mortality and morbidity. Some anti-hypertensive drugs are commonly used to control hypertension.Methods: A prospective, parallel group, open label observation study was carried out at SVS Medical Hospital, Mahabubnagar. Patients were divided into three groups based on severity of hypertension and the drug used for treatment.Results: Of 120 patients diagnosed with PIH majority (50%) of women were in the age group of 21-24 years with 33-37 gestational age. In the present study there was a significant reduction in BP after the treatment with Labetalol when compare to Nifedepine and Methyldopa. Although, all the three groups have shown significant reduction in BP during 24 hrs treatments and the mean time to achieve target BP was shown less in Labetalol group when compare to Methyldopa and Nifedepine and the maximum doses required to achieve target BP was in between 4-6 doses/day. 40% of complication observed was HELLP syndrome.Conclusions: Our study coincides with the previous findings that labetolol is an efficacious and safer drug for use in control of PIH and mean time required to achieve target BP is low when compared to nifedipne and methyldopa.

    Plant host range of Verticillium longisporum and microsclerotia density in Swedisch soils

    Verticillium longisporum is a soil-borne fungal pathogen causing vascular wilt of Brassica crops. This study was conducted to enhance our knowledge on the host range of V. longisporum. Seven crop species (barley, oat, oilseed rape, pea, red clover, sugar beet and wheat) and five weed species (barren brome, black-grass, charlock, cleavers and scentless mayweed) all common in southern Sweden were evaluated for infection by response to V. longisporum. Oat, spring wheat, oilseed rape, scentless mayweed and charlock inoculated with V. longisporum in a greenhouse showed stunting to various degrees close to the fully ripe stage. Based on the extent of microsclerotia formation, explants were separated into four groups: for pea and wheat, 80%. The results suggest that plant species outside the Brassicaceae can act as reservoirs of V. longisporum inoculum. Soil inoculum densities in nine fields were monitored over a period of 12 months, which ranged from 1 to 48 cfu g¿1 soil. Density of microsclerotia was lowest just after harvest, reaching its maximum six months later. No significant correlation between inoculum density in soil and disease incidence on oilseed rape plants was found. However, the data suggest that a threshold of 1 cfu g¿1 soil is needed to cause disease on oilseed rape. Species identification based on microsclerotia morphology and PCR analysis showed that V. longisporum dominated in soil of seven, and V. dahliae in two of the nine fields studied

    <p>Abstract</p> <p>Background</p> <p>Human hookworm infection is a major cause of anemia and malnutrition of adults and children in the developing world. As part of on-going efforts to control hookworm infection, The Human Hookworm Vaccine Initiative has identified candidate vaccine antigens from the infective L3 larval stages and adult stages of the parasite. Adult stage antigens include the cytosolic glutathione-S-transferases (GSTs). Nematode GSTs facilitate the inactivation and degradation of a variety of electrophilic substrates (drugs) via the nucleophilic addition of reduced glutathione. Parasite GSTs also play significant roles in multi-drug resistance and the modulation of host-immune defense mechanisms.</p> <p>Results</p> <p>The crystal structures of <it>Na</it>-GST-1 and <it>Na</it>-GST-2, two major GSTs from <it>Necator americanus </it>the main human hookworm parasite, have been solved at the resolution limits of 2.4 Å and 1.9 Å respectively. The structure of <it>Na</it>-GST-1 was refined to R-factor 18.9% (R-free 28.3%) while that of <it>Na</it>-GST-2 was refined to R-factor 17.1% (R-free 21.7%). Glutathione usurped during the fermentation process in bound in the glutathione binding site (G-site) of each monomer of <it>Na</it>-GST-2. <it>Na</it>-GST-1 is uncomplexed and its G-site is abrogated by Gln 50. These first structures of human hookworm parasite GSTs could aid the design of novel hookworm drugs.</p> <p>Conclusion</p> <p>The 3-dimensional structures of <it>Na</it>-GST-1 and <it>Na</it>-GST-2 show two views of human hookworm GSTs. While the GST-complex structure of <it>Na</it>-GST-2 reveals a typical GST G-site that of <it>Na</it>-GST-1 suggests that there is some conformational flexibility required in order to bind the substrate GST. In addition, the overall binding cavities for both are larger, more open, as well as more accessible to diverse ligands than those of GSTs from organisms that have other major detoxifying mechanisms. The results from this study could aid in the design of novel drugs and vaccine antigens.</p
