    Formação de professores, vozes, experiências e construções a partir da prática

    This reflection article, prepared by a graduate, two students, and their practice advisor, exposes constructions related to teacher training, within the framework of the practice developed from the deepening cycle of the bachelor’s degree in Early Childhood Education at the Social Crecer Foundation (Hereinafte fsc).It starts from the conceptual construction around the concept of Training, after which, three axes are exposed. First: Practice at the fsc and the relevance of reflection concerning the context in teacher training. Second: Teacher training, located in the implementation of actions related to reflection. Third: Articulations between practice and research training. A possible ideal in fsc bachelor’s practice. The constructions are associated with the process developed between 2019 and 2020, for which it is linked with presence and virtuality.El presente artículo de reflexión, elaborado por una egresada, dos estudiantes y su asesora de práctica, expone construcciones relacionadas con la formación docente, en el marco de la práctica desarrollada desde el ciclo de profundización de la Licenciatura en Educación Infantil1 en la Fundación Social Crecer (en adelante fsc). Así pues, se parte de la construcción conceptual en torno al concepto Formación, posterior a ello, se exponen tres ejes. Primero: Práctica en la fsc y la relevancia de la reflexión en relación con el contexto en la formación del maestro. Segundo: Formación del maestro, ubicada en la puesta en marcha de acciones relacionadas con la reflexión. Tercero: Articulaciones entre la práctica y la formación en investigación. Un ideal posible en la práctica de la licenciatura en la fsc.     Identification of Myocardial Insulin Resistance by Using Liver Tests: A Simple Approach for Clinical Practice

These two phenotypes (mIR vs. mIS) can only be assessed using time-consuming and expensive methods. The aim of the present study is to search a simple and reliable surrogate to identify both phenotypes. Methods: Forty-seven patients with T2D underwent myocardial [18F]FDG PET/CT at baseline and after a hyperinsulinemic–euglycemic clamp (HEC) to determine mIR were prospectively recruited. Biochemical assessments were performed before and after the HEC. Baseline hepatic steatosis index and index of hepatic fibrosis (FIB-4) were calculated. Furthermore, liver stiffness measurement was performed using transient elastography. Results: The best model to predict the presence of mIR was the combination of transaminases, protein levels, FIB-4 score and HOMA (AUC = 0.95; sensibility: 0.81; specificity: 0.95). We observed significantly higher levels of fibrosis in patients with mIR than in those with mIS (p = 0.034). In addition, we found that patients with mIR presented a reduced glucose uptake by the liver in comparison with patients with mIS. Conclusions: The combination of HOMA, protein, transaminases and FIB-4 is a simple and reliable tool for identifying mIR in patients with T2D.     Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection

    La salud es el estado de bienestar integral, y es un derecho fundamental, por tanto la atención en salud es una estrategia que se dinamiza con el proceso de educación ya que esta es la mejor herramienta demostrada universalmente para la reducción de la incidencia y mortalidad de enfermedades, la evidencia sugiere el folleto con asesoría verbal, ya que su claridad, permite una lectura rápida las veces que sea necesario, en el presente estudio se propone aplicar un proceso de validación a las herramientas educativas. El diseño corresponde a una validación aplicada de herramientas educativas con enfoque cuantitativo, para la preparación de pacientes que requieren de procedimientos ambulatorios en las 32 especialidades de consulta externa del Hospital San José; Para esto se tuvo en cuenta la metodología que propone la Unicef, este esquema, permitió evaluar de forma sencilla y precisa la información de 21 folletos de 9 especialidades de consulta externa del Hospital San José. La metodología tiene cinco dominios, que fueron evaluados por 8 expertos y 210 pacientes, en los resultados de los expertos y la opinión de los pacientes, se coincidió en la necesidad de ajustar o buscar un equilibrio entre lo que se quiere mostrar y lo que el paciente pueda comprender. Al finalizar se concluye que los contenidos encontrados en cada uno de los folletos son adecuados y de gran relevancia para la orientación del usuario, de igual manera con la validación de esta herramienta se presenta la oportunidad de realizar una sensibilización e importancia de tener instructivos que disminuyan la cancelación de citas y procedimientos en el hospital San JoséPregradoEnfermero(a

    Risk Factors Associated with Mortality among Patients with COVID-19: Analysis of a Cohort of 1213 Patients in a Tertiary Healthcare Center

    The presence of cardio-metabolic and respiratory comorbidities, immunosuppression, and chronic kidney disease have been associated with an increase in mortality from COVID-19. The objective of this study is to establish the risk factors associated with 30-day mortality in a cohort of hospitalized patients with COVID-19. This paper conducts a retrospective and analytical study of patients hospitalized for COVID-19 in a tertiary care center. A Cox proportional hazard analysis was performed to estimate the association of comorbidities with 30-day mortality. A total of 1215 patients with a median age of 59 years were included. In the adjusted Cox proportional hazards regression model, hypothyroidism, D-dimer ≥ 0.8 μg/mL, LHD ≥ 430 IU/L, CRP ≥ 4.83 ng/mL, and triglycerides ≥ 214 mg/dL were associated with an increased risk of death. The presence of a history of hypothyroidism and biomarkers (D-dimer, lactic dehydrogenase, CRP, and triglycerides) were associated with an increase in mortality in the studied cohort
