    The zooplankton community in a small, hypertrophic mediterranean reservoir (Foix reservoir, NE Spain)

    A small hypertrophic reservoir (Foix Reservoir, Spain) was sampled from March 1997 to January 2000 to document the composition and spatial and temporal variability of the zooplankton assemblage. Twenty five species of Rotifera, 10 of Cladocera, and one species of copepod (Acanthocyclops robustus (Sars)) were collected. The most abundant rotifer species were Brachionus angularis (Gosse) and Polyarthra spp. Daphnia galeata Sars, Ceriodaphnia quadrangula (Müller), and Chydorus sphaericus (Müller) were the main cladocerans. For each species, brief information is given on ecology, distribution, and occurrence in Spain and in the Foix Reservoir. The zooplankton community of the Foix Reservoir was characterized by the abundance of heleoplanktonic species, which accounted for 50 % of the taxa. Remarkable changes over time in species' composition and occurrence were found for Rotifera and Cladocera during the study period. A. robustus showed high density and biomass values throughout the sampling period. In the summer of 1999, copepods avoided deep layers, whereas some Rotifera (Polyarthra spp,, Filinia spp,, Brachionus angularis) and Chydorus sphaericus were found near the anoxic bottom. A rare rotifer in the Iberian Peninsula, Itura aurita aurita (Ehrenberg), was also found in lotic sections of the reservoir.Un embalse pequeño e hipereutrófico (Embalse de Foix, España) fue muestreado desde Marzo de 1997 a Enero de 2000, con el fin de documentar la composición y variabilidad espacial y temporal de la comunidad zooplantónica. Veinticinco especies de rotíferos, 10 de cladóceros y una especie de copépodo (Acanthocyclops robustus (Sars)) fueron recogidos. Las especies de rotíferos más abundantes fueron Brachionus angularis (Gosse) y Polyarthra spp. Daphnia galeata Sars, Ceriodaphnia quadrangula (Müller) y Chydorus sphaericus (Müller) fueron los principales cladóceros. Para cada especie se da breve información sobre su ecología, distribución y ocurrencia en España y en el Embalse de Foix. La comunidad zooplantónica del embalse se caracterizó por una fuerte presencia de especies heleoplantónicas, que supusieron un 50 % de los taxones. Se constataron cambios remarcables en la composición específica y densidades de rotíferos y cladóceros a lo largo del periodo estudiado. Por el contrario, A. robustus mostró densidades y biomasas altas durante todo el periodo. En el verano de 1999 los copépodos evitaron las capas profundas, mientras que algunos rotíferos (Polyarthra spp, Filinia spp, Brachionus angularis) y Chydorus sphaericus fueron encontrados cerca del fondo anóxico.     Sequential repeated tibial tubercle osteotomy in a two-stage exchange strategy: a superior approach to treating a chronically infected knee arthroplasty?

The most adequate knee surgical approach in cases of PJI is a matter of debate. The purpose of this study was to determine the influence of performing a tibial tubercle osteotomy (TTO) in a two-stage exchange protocol for knee PJI treatment. Methods Retrospective cohort study examining patients managed with two-stage arthroplasty due to chronic knee PJI (2010–2019). Performance and timing of the TTO were collected. Primary end-point was infection control with a minimum FU of 12 months and according to internationally accepted criteria. Correlation between TTO timing and reinfection rate was reviewed. Results Fifty-two cases were finally included. Overall success (average follow-up: 46.2 months) was 90.4%. Treatment success was significantly higher among cases addressed using TTO during the second stage (97.1% vs. 76.5%, p value 0.03). Only 4.8% of the patients relapsed after performing a sequential repeated TTO, that is, during both first and second stages, compared to 23.1% cases in which TTO was not done (p value 0.28). No complications were observed among patients in the TTO group with a significant decrease in soft tissue necrosis (p: 0.052). Conclusion Sequential repeated tibial tubercle osteotomy during a two-stage strategy is a reasonable option and offers high rates of infection control in complex cases of knee PJI with a low rate of complications.Open Access Funding provided by Universitat Autònoma de Barcelona

    Psicosis y diferencias sociales: Comparando la prevalencia de las psicosis en dos medios urbanos diferenciados.

    Objetivos: Contribuir a la reflexión sobre la etiología y/o los factores de riesgo para las psicosis comparando la prevalencia en población general y población de riesgo de la esquizofrenia y otras psicosis en dos barrios de Barcelona (España). Método: Nuestras aportaciones en este trabajo se apoyan sobre todo en un estudio descriptivo transversal de todos los pacientes con psicopatología detectados en la USM de Sant Martí-La Mina: un territorio geodemográfica y asistencialmente delimitado formado por 5 Áreas Básicas de Salud (103.615 habitantes. Resultados: Sobre un total de 21.536 pacientes con registro de casos abierto desde el año 1982 hasta el año 2000, se halló que 838 cumplían los criterios restrictivos para ser diagnosticados como “esquizofrénicos” (N=476) o “afectados por otras psicosis” (N=362). Sin embargo, las prevalencias de esquizofrenia y otras psicosis en el barrio sujeto a más factores de riesgo psicosociales eran alrededor de 2 veces mayores que las encontradas en el barrio colindante por el mismo equipo y en el mismo período temporal. Conclusión: Es necesario tener en cuenta el gran peso de los factores de sociales y psicosociales para poderse explicar las diferencias de prevalencia de la esquizofrenia y otras psicosis entre diferentes poblaciones

    Objective and subjective cognition in survivors of COVID-19 one year after ICU discharge : the role of demographic, clinical, and emotional factors

This study aims to characterize the neuropsychological dysfunction of COVID-19 survivors 12 months after ICU discharge, and to study whether the use of a measure of perceived cognitive deficit allows the detection of objective cognitive impairment. We also explore the relationship between demographic, clinical and emotional factors, and both objective and subjective cognitive deficits. Critically ill COVID-19 survivors from two medical ICUs underwent cognitive and emotional assessment one year after discharge. The perception of cognitive deficit and emotional state was screened through self-rated questionnaires (Perceived Deficits Questionnaire, Hospital Anxiety and Depression Scale and Davidson Trauma Scale), and a comprehensive neuropsychological evaluation was carried out. Demographic and clinical data from ICU admission were collected retrospectively. Out of eighty participants included in the final analysis, 31.3% were women, 61.3% received mechanical ventilation and the median age of patients was 60.73 years. Objective cognitive impairment was observed in 30% of COVID-19 survivors. The worst performance was detected in executive functions, processing speed and recognition memory. Almost one in three patients manifested cognitive complaints, and 22.5%, 26.3% and 27.5% reported anxiety, depression and post-traumatic stress disorder (PTSD) symptoms, respectively. No significant differences were found in the perception of cognitive deficit between patients with and without objective cognitive impairment. Gender and PTSD symptomatology were significantly associated with perceived cognitive deficit, and cognitive reserve with objective cognitive impairment. One-third of COVID-19 survivors suffered objective cognitive impairment with a frontal-subcortical dysfunction 12 months after ICU discharge. Emotional disturbances and perceived cognitive deficits were common. Female gender and PTSD symptoms emerged as predictive factors for perceiving worse cognitive performance. Cognitive reserve emerged as a protective factor for objective cognitive functioning. Trial registration : ClinicalTrials.gov Identifier: NCT04422444; June 9, 2021.     Successful renal transplantation in a patient with atypical hemolytic uremic syndrome carrying mutations in both Factor I and MCP (CD46)

    7 páginas, 4 figuras, 2 tablas -- PAGS nros. 1477-1483Kidney transplantation in patients with atypical hemolytic uremic syndrome (aHUS) carrying mutations in the soluble complement regulators factor H (CFH) or factor I (CFI) is associated with elevated risk of disease recurrence and almost certain graft loss. In contrast, recurrence is unusual in patients with mutations in the membrane-associated complement regulator membrane cofactor protein (MCP) (CD46). Therefore, a panel of experts recently recommended the combined liver–kidney transplantation to minimize aHUS recurrence in patients with mutations in CFH or CFI. There was, however, very limited information regarding transplantation in patients carrying mutations in both soluble and membrane-associated complement regulators to support a recommendation. Here, we report the case of an aHUS patient with a heterozygous mutation in both CFI and MCP who received an isolated kidney transplant expressing normal MCP levels. Critically, the patient suffered from a severe antibody-mediated rejection that was successfully treated with plasmapheresis and IvIgG. Most important, despite the complement activation in the allograft, there was no evidence of thrombotic microangiopathy, suggesting that the normal MCP levels in the grafted kidney were sufficient to prevent the aHUS recurrence.
