    The management of bipolar mania: a national survey of baseline data from the EMBLEM study in Italy

    <p>Abstract</p> <p>Background</p> <p>Although a number of studies have assessed the management of mania in routine clinical practice, no studies have so far evaluated the short- and long-term management and outcome of patients affected by bipolar mania in different European countries.</p> <p>The objective of the study is to present, in the context of a large multicenter survey (EMBLEM study), an overview of the baseline data on the acute management of a representative sample of manic bipolar patients treated in the Italian psychiatric hospital and community settings. EMBLEM is a 2-year observational longitudinal study that evaluates across 14 European countries the patterns of the drug prescribed in patients with bipolar mania, their socio-demographic and clinical features and the outcomes of the treatment.</p> <p>Methods</p> <p>The study consists of a 12-week acute phase and a ≀ 24-month maintenance phase. Bipolar patients were included into the study as in- or out-patients, if they initiated or changed, according to the decision of their psychiatrist, oral antipsychotics, anticonvulsants and/or lithium for the treatment of an episode of mania.</p> <p>Data concerning socio-demographic characteristics, psychiatric and medical history, severity of mania, prescribed medications, functional status and quality of life were collected at baseline and during the follow-up period.</p> <p>Results</p> <p>In Italy, 563 patients were recruited in 56 sites: 376 were outpatients and 187 inpatients. The mean age was 45.8 years. The mean CGI-BP was 4.4 (± 0.9) for overall score and mania, 1.9 (± 1.2) for depression and 2.6 (± 1.6) for hallucinations/delusions. The YMRS showed that 14.4% had a total score < 12, 25.1% ≄ 12 and < 20, and 60.5% ≄ 20. At entry, 75 patients (13.7%) were treatment-naĂŻve, 186 (34.1%) were receiving a monotherapy (of which haloperidol [24.2%], valproate [16.7%] and lithium [14.5%] were the most frequently prescribed) while 285 (52.2%) a combined therapy (of which 8.0% were represented by haloperidol/lithium combinations). After a switch to an oral medication, 137 patients (24.8%) were prescribed a monotherapy while the rest (415, 75.2%) received a combination of drugs.</p> <p>Conclusion</p> <p>Data collected at baseline in the Italian cohort of the EMBLEM study represent a relevant source of information to start addressing the short and long-term therapeutic strategies for improving the clinical as well as the socio-economic outcomes of patients affected by bipolar mania. Although it's not an epidemiological investigation and has some limitations, the results show several interesting findings as a relatively late age of onset of bipolar disorder, a low rate of past suicide attempts, a low lifetime rate of alcohol abuse and drug addiction.</p

    Switchbacks in the Near-Sun Magnetic Field: Long Memory and Impact on the Turbulence Cascade

    International audienceOne of the most striking observations made by Parker Solar Probe during its first solar encounter is the omnipresence of rapid polarity reversals in a magnetic field that is otherwise mostly radial. These so-called switchbacks strongly affect the dynamics of the magnetic field. We concentrate here on their macroscopic properties. First, we find that these structures are self-similar, and have neither a characteristic magnitude, nor a characteristic duration. Their waiting time statistics show evidence of aggregation. The associated long memory resides in their occurrence rate, and is not inherent to the background fluctuations. Interestingly, the spectral properties of inertial range turbulence differ inside and outside of switchback structures; in the latter the 1/f range extends to higher frequencies. These results suggest that outside of these structures we are in the presence of lower-amplitude fluctuations with a shorter turbulent inertial range. We conjecture that these correspond to a pristine solar wind

    Kinetic-scale Spectral Features of Cross Helicity and Residual Energy in the Inner Heliosphere

    In this work, we present the first results from the flux angle (FA) operation mode of the Faraday Cup instrument on board the Parker Solar Probe (PSP). The FA mode allows rapid measurements of phase space density fluctuations close to the peak of the proton velocity distribution function with a cadence of 293 Hz. This approach provides an invaluable tool for understanding kinetic-scale turbulence in the solar wind and solar corona. We describe a technique to convert the phase space density fluctuations into vector velocity components and compute several turbulence parameters, such as spectral index, residual energy, and cross helicity during two intervals when the FA mode was used in PSP's first encounter at 0.174 au distance from the Sun

    Magnetic connectivity of the ecliptic plane within 0.5 AU : PFSS modeling of the first PSP encounter

    We compare magnetic field measurements taken by the FIELDS instrument on Parker Solar Probe (PSP) during its first solar encounter to predictions obtained by Potential Field Source Surface (PFSS) modeling. Ballistic propagation is used to connect the spacecraft to the source surface. Despite the simplicity of the model, our results show striking agreement with PSPs first observations of the heliospheric magnetic field from 0.5 AU (107.5 Rs) down to 0.16 AU (35.7 Rs). Further, we show the robustness of the agreement is improved both by allowing the photospheric input to the model to vary in time, and by advecting the field from PSP down to the PFSS model domain using in situ PSP/SWEAP measurements of the solar wind speed instead of assuming it to be constant with longitude and latitude. We also explore the source surface height parameter (RSS) to the PFSS model finding that an extraordinarily low source surface height (1.3-1.5Rs) predicts observed small scale polarity inversions which are otherwise washed out with regular modeling parameters. Finally, we extract field line traces from these models. By overlaying these on EUV images we observe magnetic connectivity to various equatorial and mid-latitude coronal holes indicating plausible magnetic footpoints and offering context for future discussions of sources of the solar wind measured by PSP

    Prognostic utility of HOXB13 : IL17BR and molecular grade index in early-stage breast cancer patients from the Stockholm trial

    Background: A dichotomous index combining two gene expression assays, HOXB13:IL17BR (H:I) and molecular grade index (MGI), was developed to assess risk of recurrence in breast cancer patients. The study objective was to demonstrate the prognostic utility of the combined index in early-stage breast cancer. Methods: In a blinded retrospective analysis of 588 ER-positive tamoxifen-treated and untreated breast cancer patients from the randomized prospective Stockholm trial, H:I and MGI were measured using real-time RT-PCR. Association with patient outcome was evaluated by Kaplan-Meier analysis and Cox proportional hazard regression. A continuous risk index was developed using Cox modeling. Results: The dichotomous H:I+MGI was significantly associated with distant recurrence and breast cancer death. The &gt;50% of tamoxifen-treated patients categorized as low-risk had &lt;3% 10-year distant recurrence risk. A continuous risk model (Breast Cancer Index (BCI)) was developed with the tamoxifen-treated group and the prognostic performance tested in the untreated group was 53% of patients categorized as low-risk with an 8.3% 10-year distant recurrence risk. Conclusion: Retrospective analysis of this randomized, prospective trial cohort validated the prognostic utility of H:I+MGI and was used to develop and test a continuous risk model that enables prediction of distant recurrence risk at the patient level.Original Publication:Piiha-Lotta Jerevall, Xiai-Jun Ma, Hongying Li, Ranelle Salunga, Nicole C.     Search for the Θ+\Theta^+ pentaquark in the reaction γd→pK−K+n\gamma d \to p K^- K^+ n

    A search for the \thp in the reaction Îłd→pK−K+n\gamma d \to pK^-K^+n was completed using the CLAS detector at Jefferson Lab. A study of the same reaction, published earlier, reported the observation of a narrow \thp resonance. The present experiment, with more than 30 times the integrated luminosity of our earlier measurement, does not show any evidence for a narrow pentaquark resonance. The angle-integrated upper limit on \thp production in the mass range of 1.52 to 1.56 GeV/c2^2 for the Îłd→pK−Θ+\gamma d \to pK^-\Theta^+ reaction is 0.3 nb (95% CL). This upper limit depends on assumptions made for the mass and angular distribution of \thp production. Using \lamstar production as an empirical measure of rescattering in the deuteron, the cross section upper limit for the elementary Îłn→K−Θ+\gamma n \to K^-\Theta^+ reaction is estimated to be a factor of 10 higher, {\it i.e.}, ∌3\sim 3 nb (95% CL).Comment: 5 figures, submitted to PRL, revised for referee comment

    Single pi+ Electroproduction on the Proton in the First and Second Resonance Regions at 0.25GeV^2 < Q^2 < 0.65GeV^2 Using CLAS

    The ep -> e'pi^+n reaction was studied in the first and second nucleon resonance regions in the 0.25 GeV^2 < Q^2 < 0.65 GeV^2 range using the CLAS detector at Thomas Jefferson National Accelerator Facility. For the first time the absolute cross sections were measured covering nearly the full angular range in the hadronic center-of-mass frame. The structure functions sigma_TL, sigma_TT and the linear combination sigma_T+epsilon*sigma_L were extracted by fitting the phi-dependence of the measured cross sections, and were compared to the MAID and Sato-Lee models.Comment: Accepted for publication in PR

    Photodisintegration of 4^4He into p+t

    The two-body photodisintegration of 4^4He into a proton and a triton has been studied using the CEBAF Large-Acceptance Spectrometer (CLAS) at Jefferson Laboratory. Real photons produced with the Hall-B bremsstrahlung-tagging system in the energy range from 0.35 to 1.55 GeV were incident on a liquid 4^4He target. This is the first measurement of the photodisintegration of 4^4He above 0.4 GeV. The differential cross sections for the γ\gamma4^4He→pt\to pt reaction have been measured as a function of photon-beam energy and proton-scattering angle, and are compared with the latest model calculations by J.-M. Laget. At 0.6-1.2 GeV, our data are in good agreement only with the calculations that include three-body mechanisms, thus confirming their importance. These results reinforce the conclusion of our previous study of the three-body breakup of 3^3He that demonstrated the great importance of three-body mechanisms in the energy region 0.5-0.8 GeV .Comment: 13 pages submitted in one tgz file containing 2 tex file and 22 postscrip figure

    π0\pi^0 photoproduction on the proton for photon energies from 0.675 to 2.875 GeV

    Differential cross sections for the reaction Îłp→pπ0\gamma p \to p \pi^0 have been measured with the CEBAF Large Acceptance Spectrometer (CLAS) and a tagged photon beam with energies from 0.675 to 2.875 GeV. The results reported here possess greater accuracy in the absolute normalization than previous measurements. They disagree with recent CB-ELSA measurements for the process at forward scattering angles. Agreement with the SAID and MAID fits is found below 1 GeV. The present set of cross sections has been incorporated into the SAID database, and exploratory fits have been extended to 3 GeV. Resonance couplings have been extracted and compared to previous determinations.Comment: 18 pages, 48 figure

    Exclusive ρ0\rho^0 electroproduction on the proton at CLAS

    The ep→eâ€Čpρ0e p\to e^\prime p \rho^0 reaction has been measured, using the 5.754 GeV electron beam of Jefferson Lab and the CLAS detector. This represents the largest ever set of data for this reaction in the valence region. Integrated and differential cross sections are presented. The WW, Q2Q^2 and tt dependences of the cross section are compared to theoretical calculations based on tt-channel meson-exchange Regge theory on the one hand and on quark handbag diagrams related to Generalized Parton Distributions (GPDs) on the other hand. The Regge approach can describe at the ≈\approx 30% level most of the features of the present data while the two GPD calculations that are presented in this article which succesfully reproduce the high energy data strongly underestimate the present data. The question is then raised whether this discrepancy originates from an incomplete or inexact way of modelling the GPDs or the associated hard scattering amplitude or whether the GPD formalism is simply inapplicable in this region due to higher-twists contributions, incalculable at present.Comment: 29 pages, 29 figure
