    Credibilidad y confianza del comité de convivencia laboral de la Administración Municipal de Itagüí (2014 – 2015)

    El acoso laboral es una problemática que aqueja a la población laboral mundial y es uno de los principales riesgos psicosociales en el trabajo, pues no sólo afecta la productividad de los trabajadores, sino que también tiene un fuerte impacto en su salud mental. En Colombia se promulgó la Ley 1010 de 2006 con el fin de combatir este flagelo, sin embargo, se cuestiona la eficacia de esta ley y las resoluciones emanadas del Ministerio del Trabajo que crean los Comités de Convivencia Laboral. El acoso laboral, continúa siendo una de las principales problemáticas que viven los empleados en Colombia, situación que no es ajena en la Administración Municipal de Itagüí. La pregunta que orientó la presente investigación fue: ¿Genera credibilidad y confianza el Comité de Convivencia Laboral entre los funcionarios de la Administración Municipal de Itagüí frente a la solución de las problemáticas relacionadas con el acoso laboral? Los resultados obtenidos a partir de una encuesta realizada a 254 funcionarios y una entrevista con los miembros del Comité de Convivencia Laboral (CCL), permite concluir que el CCL de la Administración Municipal de Itagüí, presenta fallas en la gestión que viene realizando y que existe un clima de desconfianza y falta de credibilidad hacia el comité, lo que se traduce en que la Ley 1010 de 2006 pierda eficacia instrumental en la prevención del acoso laboral en la administración municipal de Itagüí.Bullying is a problem that afflicts the global workforce and is one of the main psychosocial risks at work, it not only affects the productivity of workers, but also has a strong impact on their mental health. In Colombia the 1010 Law 2006 was enacted in order to combat this scourge, however, the effectiveness of this law and the resolutions issued by the Ministry of Labour to create Coexistence Labor Committees questioned; bullying, remains one of the main problems that employees living in Colombia, a situation that is not employed in the municipal administration of Itagui. Given this problem, the question that guided this research was: Does it generate credibility and trust Labor Committee Coexistence between officials of the Municipal Administration of Itagui against solving the problems related to bullying? The results from a survey of 254 officials and an interview with members of the Committee on Labor Coexistence (CCL), to conclude that the CCL of the Municipal Administration of Itagui, has flaws in management being done and that there a climate of distrust and lack of credibility to the committee, all of which ultimately translates into Law 1010 2006 instrumental lose effectiveness in preventing bullying in Itagui municipal administration

    Coffee consumption increases the antioxidant capacity of plasma and has no effect on the lipid profile or vascular function in healthy adults in a randomized controlled trial

    ABSTRACT: Coffee, a source of antioxidants, has controversial effects on cardiovascular health. Objective: We evaluated the bioavailability of chlorogenic acids (CGAs) in 2 coffees and the effects of their consumption on the plasma antioxidant capacity (AC), the serum lipid profile, and the vascular function in healthy adults. Methods: Thirty-eight men and 37 women with a mean 6 SD age of 38.5 6 9 y and body mass index of 24.1 6 2.6 kg/m2 were randomly assigned to 3 groups: a control group that did not consume coffee or a placebo and 2 groups that consumed 400 mL coffee/d for 8 wk containing a medium (MCCGA; 420 mg) or high (HCCGA; 780 mg) CGA content. Both were low in diterpenes (0.83 mg/d) and caffeine (193 mg/d). Plasma caffeic and ferulic acid concentrations were measured by GC, and the plasma AC was evaluated with use of the ferric-reducing antioxidant power method. The serum lipid profile, nitric oxide (NO) plasma metabolites, vascular endothelial function (flow-mediated dilation; FMD), and blood pressure (BP) were evaluated.Results: After coffee consumption (1 h and 8 wk), caffeic and ferulic acid concentrations increased in the coffee-drinking groups, although the values of the 2 groups were significantly different (P < 0.001); caffeic and ferulic acid concentrations were undetectable in the control group. At 1 h after consumption, the plasma AC in the control group was significantly lower than the baseline value (22%) and significantly increased in the MCCGA (6%) and HCCGA (5%) groups (P < 0.05). After 8 wk, no significant differences in the lipid, FMD, BP, or NO plasma metabolite values were observed between the groups. Conclusions: Both coffees, which contained CGAs and were low in diterpenes and caffeine, provided bioavailable CGAs and had a positive acute effect on the plasma AC in healthy adults and no effect on blood lipids or vascular function. The group that did not drink coffee showed no improvement in serum lipid profile, FMD, BP, or NO plasma metabolites. This trial was registered at registroclinico.sld.cu as RPCEC00000168

    This article is a product of the development of the First International Entrepreneurship Fair "A meeting of ideas for sustainable development", promoted through inter-institutional agreements between the institutions IU Digital de Antioquia, Incubadora de Empresas Genesis for Colombia, the Chamber of Commerce of Medellin for Antioquia and Universidad Veracruzana Mexico, with the objective of promoting the development of entrepreneurship, also known as the fundamental pillar of the entrepreneurial culture established by the four institutions.     Alternativas para el desarrollo sostenible del turismo pospandemia en los municipios de Apartadó, Arboletes y Necoclí del Urabá antioqueño colombiano

    The article is the result of a research work whose objective was to design alternatives for the sustainable development of the post-pandemic tourism sector in the municipalities of Apartadó, Arboletes and Necoclí of the Urabá subregion of Colombian Antioquia. In methodological terms, it was proposed as mixed research with an analytical-descriptive character, which combines documentary sources of diverse nature, among them a survey to 203 tourist operators of the subregion to which a model under factorial analysis of principal coefficients (PCA) was applied. Among the most significant results is that the municipalities have an important opportunity to consolidate themselves as sustainable tourist destinations due to the natural wealth of the territory and its privileged geographic position. However, the sustainable use of resources must be ensured in order to convert them into a source of material wealth while at the same time recovering and conserving them for the enjoyment of future generations.El artículo es resultado de un trabajo de investigación que se planteó como objetivo diseñar alternativas para el desarrollo sostenible del sector turístico pospandemia en los municipios de Apartadó, Arboletes y Necoclí, ubicados en la subregión del Urabá antioqueño colombiano.     The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Clustering COVID-19 ARDS patients through the first days of ICU admission. An analysis of the CIBERESUCICOVID Cohort

    Background Acute respiratory distress syndrome (ARDS) can be classified into sub-phenotypes according to different inflammatory/clinical status. Prognostic enrichment was achieved by grouping patients into hypoinflammatory or hyperinflammatory sub-phenotypes, even though the time of analysis may change the classification according to treatment response or disease evolution. We aimed to evaluate when patients can be clustered in more than 1 group, and how they may change the clustering of patients using data of baseline or day 3, and the prognosis of patients according to their evolution by changing or not the cluster.Methods Multicenter, observational prospective, and retrospective study of patients admitted due to ARDS related to COVID-19 infection in Spain. Patients were grouped according to a clustering mixed-type data algorithm (k-prototypes) using continuous and categorical readily available variables at baseline and day 3.Results Of 6205 patients, 3743 (60%) were included in the study. According to silhouette analysis, patients were grouped in two clusters. At baseline, 1402 (37%) patients were included in cluster 1 and 2341(63%) in cluster 2. On day 3, 1557(42%) patients were included in cluster 1 and 2086 (57%) in cluster 2. The patients included in cluster 2 were older and more frequently hypertensive and had a higher prevalence of shock, organ dysfunction, inflammatory biomarkers, and worst respiratory indexes at both time points. The 90-day mortality was higher in cluster 2 at both clustering processes (43.8% [n = 1025] versus 27.3% [n = 383] at baseline, and 49% [n = 1023] versus 20.6% [n = 321] on day 3). Four hundred and fifty-eight (33%) patients clustered in the first group were clustered in the second group on day 3. In contrast, 638 (27%) patients clustered in the second group were clustered in the first group on day 3.Conclusions During the first days, patients can be clustered into two groups and the process of clustering patients may change as they continue to evolve. This means that despite a vast majority of patients remaining in the same cluster, a minority reaching 33% of patients analyzed may be re-categorized into different clusters based on their progress. Such changes can significantly impact their prognosis
