    Fried food intake estimated by the multiple source method is associated with gestational weight gain

    AbstractThis present study aimed to test the association between fried food intake estimated by a semiquantitative food frequency questionnaire (FFQ), multiple 24-hour dietary recalls (24hRs), and the application of the multiple source method (MSM) in relation to gestational weight gain at the second and third trimesters and weight gain ratio (observed weight gain/expected weight gain). We hypothesized that distinct relationships with weight gain would be found given the measurement errors of self-reported dietary approaches. A prospective study was conducted with 88 adult pregnant women. Fried food intake during pregnancy was assessed using a validated 85-item FFQ, two to six 24hRs per woman, and the MSM with and without frequency of food intake as covariate. Linear regression models were used to evaluate the relationship between fried food estimated by the methods and weight gain. For every 100-g increment of fried food intake, the β (95% confidence interval) for weight gain was β 1.87 (0.34, 3.40) and β 2.00 0.55, 3.45) for estimates using MSM with and without the frequency of intake as covariate, respectively, after multiple adjustments. We found that fried food intake estimated by the FFQ and 24hRs β 0.40 (−0.68, 1.48) and β 0.49 (−0.53, 1.52), respectively, was unrelated to weight gain. In relation to weight gain ratio, a positive association was found for estimates using the MSM with [β 0.29 (0.03, 0.54)] and without the frequency of intake as covariate [β 0.31 (0.07, 0.55)]; and no associations were found for estimates by the FFQ or 24hRs. The data showed that fried food intake estimated the MSM, but not by the FFQ and 24hRs, is associated with excessive weight gain during pregnancy

    Extended Exponential Geometric Distribution for Medical Data

    We have considered different estimation procedures for the unknown parameters of the extended exponential geometric distribution. We introduce different types of estimators such as the maximum likelihood, method of moments, modified moments, L-moments, ordinary and weighted least squares, percentile, maximum product of spacings, and minimum distance estimators. The different estimators are compared by using extensive numerical simulations. We discovered that the maximum product of spacings estimator has the smallest mean square errors and mean relative estimates, nearest to one, for both parameters, proving to be the most efficient method compared to other methods. Combining these results with the good properties of the method such as consistency, asymptotic efficiency, normality, and invariance we conclude that the maximum product of spacings estimator is the best one for estimating the parameters of the extended exponential geometric distribution in comparison with its competitors. For the sake of illustration, we apply our proposed methodology in two important data sets, demonstrating that the EEG distribution is a simple alternative to be used for lifetime data

    Padrão de atividade física em gestantes usuárias do Sistema Público de Saúde

    Objetivos: Descrever o padrão de atividade física de gestantes usuárias de Unidades Básicas de Saúde, investigar a média do gasto energético relacionados aos domínios de atividade física. Métodos: Trata-se de um estudo transversal conduzido entre 799 gestantes atendidas nas Unidades Básicas de Saúde de Ribeirão Preto, SP. A prática de atividade física foi avaliada por meio do questionário de “frequencia de  atividade física diária” específico para gestantes, no qual foram obtidas a frequência, duração e o equivalentes metabólicos das atividades físicas. Resultados: A maioria das gestantes apresentaram o padrão sedentário (57%) seguido pelo padrão leve (43%). Em relação aos domínios de atividades físicas, o domínio doméstico teve a maior média de gasto energético (733,7), enquanto que, o domínio de lazer apresentou a menor média (20,5). Verificou-se que as variáveis idade acima de 30 anos , escolaridade baixa , solteira, não ser chefe de família, não trabalhar fora, e ter filhos, tiveram associações com o padrão sedentário. Conclusões: Elevada prevalência de sedentarismo foi observada entre as gestantes estudadas, sugerindo a necessidade de ações de promoção de atividade física neste público, principalmente no domínio de lazer

    Seroprevalence of Hepatitis B virus infection in a Brazilian jail

    Esta pesquisa objetivou estimar a prevalência do marcador do HBV e seus fatores de risco na população masculina carcerária da Penitenciária de Ribeirão Preto - SP, Brasil. De 1.030 presidiários, foram sorteados 333 participantes, os quais foram submetidos à aplicação de um questionário e à coleta de sangue, no período de maio a agosto de 2003. Para diagnóstico de exposição ao HBV foi utilizado o ensaio imunoabsorvente de ligação de enzimas (ELISA) para detecção dos marcadores HBsAg, anti-HBsAg e anti-HBc total. A prevalência total de infecção pelo HBV nos presidiários foi de 19,5% (IC95%: 15,2 - 23,8). A média de idade dos participantes foi de 30,1 anos. Na análise univariada, a infecção pelo HBV esteve associada à idade acima de 30 anos (p<0,001), uso prévio de drogas injetáveis (p<0,001) e compartilhamento de agulhas prévio (p<0,001). Em um modelo multivariado, de regressão logística, a infecção pelo HBV foi associada à idade acima de 30 anos (OR = 3,3: IC95%: 1,8 - 6.1) e uso prévio de droga injetável (OR = 2,7; IC95%: 1.9 - 6.4). Infecção pelo HBV nas prisões representa grave problema de saúde pública, principalmente relacionado à idade acima de 30 anos e uso prévio de drogas injetáveis.This survey aimed to estimate the prevalence of HBV serological markers and risk factors for this infection in male inmates at the Penitentiary of Ribeirão Preto, State of São Paulo, Brazil. Out of 1030 inmates, a simple random sample of 333 participants answered a questionnaire and had blood samples collected, from May to August 2003. Enzyme-linked immunosorbent assay (ELISA) was used to diagnose HBV infection (HBsAg, anti-HBsAg, and total anti-HBc). The overall prevalence for HBV markers in inmates was 19.5% (CI95%: 15.2 - 23.8). The participants' mean age was 30.1 years. In univariate analysis, HBV infection was associated with age > 30 years (p<0.001), previous injecting drug use (p<0.001) and previous sharing of needles (p<0.001). In a logistic regression multivariate model, HBV infection was associated with age > 30 years (OR = 3.3; CI95%: 1.8 - 6.1) and previous injecting drug use (OR = 2.7; CI95%: 1.9 - 6.4). HBV infection on prisons represents a major health problem, mainly associated with age over 30 years and previous injecting drug use

    Hepatitis C virus (HCV) infection has quite high prevalence in the prison system, reaching rates of up to 40%. This survey aimed to estimate the prevalence of HCV infection and evaluate risk factors for this exposure among male inmates at the Ribeirão Preto Prison, State of São Paulo, Brazil, between May and August 2003. A total of 333 participants were interviewed using a standardized questionnaire and underwent immunoenzymatic assaying to investigate anti-HCV. The prevalence of HCV infection among the inmates was 8.7% (95% CI: 5.7-11.7). The participants'mean age was 30.1 years, and the prevalence was predominantly among individuals over 30 years of age. Multivariate analysis showed that the variables that were independently associated with HCV infection were age > 30 years, tattooing, history of previous hepatitis, previous injection drug use and previous needle-sharing.Infecção pelo vírus da hepatite C no sistema prisional apresenta elevada prevalência, chegando a atingir cifras superiores a 40%. Esta pesquisa objetivou estimar a prevalência do HCV e avaliar fatores de risco para esta exposição na população masculina carcerária da Penitenciária de Ribeirão Preto - SP, no período de maio a agosto de 2003. Um total de 333 participantes foi submetido à aplicação de um questionário padronizado e a ensaio imunoenzimático para pesquisa de anti-HCV. A prevalência encontrada de infecção pelo HCV nos presidiários foi de 8,7% (IC 95%: 5,7-11,7). A média de idade dos participantes foi de 30,1 anos, com a prevalência predominando nos indivíduos acima de 30 anos. Na análise multivariada, as variáveis que se mostraram associadas de forma independente à infecção pelo HCV foram idade > 30 anos, tatuagem, história prévia de hepatite, passado de uso de droga injetável e passado de compartilhamento de agulhas

    Progression of the first stage of spontaneous labour: A prospective cohort study in two sub-Saharan African countries.

    BACKGROUND: Escalation in the global rates of labour interventions, particularly cesarean section and oxytocin augmentation, has renewed interest in a better understanding of natural labour progression. Methodological advancements in statistical and computational techniques addressing the limitations of pioneer studies have led to novel findings and triggered a re-evaluation of current labour practices. As part of the World Health Organization's Better Outcomes in Labour Difficulty (BOLD) project, which aimed to develop a new labour monitoring-to-action tool, we examined the patterns of labour progression as depicted by cervical dilatation over time in a cohort of women in Nigeria and Uganda who gave birth vaginally following a spontaneous labour onset. METHODS AND FINDINGS: This was a prospective, multicentre, cohort study of 5,606 women with singleton, vertex, term gestation who presented at ≤ 6 cm of cervical dilatation following a spontaneous labour onset that resulted in a vaginal birth with no adverse birth outcomes in 13 hospitals across Nigeria and Uganda. We independently applied survival analysis and multistate Markov models to estimate the duration of labour centimetre by centimetre until 10 cm and the cumulative duration of labour from the cervical dilatation at admission through 10 cm. Multistate Markov and nonlinear mixed models were separately used to construct average labour curves. All analyses were conducted according to three parity groups: parity = 0 (n = 2,166), parity = 1 (n = 1,488), and parity = 2+ (n = 1,952). We performed sensitivity analyses to assess the impact of oxytocin augmentation on labour progression by re-examining the progression patterns after excluding women with augmented labours. Labour was augmented with oxytocin in 40% of nulliparous and 28% of multiparous women. The median time to advance by 1 cm exceeded 1 hour until 5 cm was reached in both nulliparous and multiparous women. Based on a 95th percentile threshold, nulliparous women may take up to 7 hours to progress from 4 to 5 cm and over 3 hours to progress from 5 to 6 cm. Median cumulative duration of labour indicates that nulliparous women admitted at 4 cm, 5 cm, and 6 cm reached 10 cm within an expected time frame if the dilatation rate was ≥ 1 cm/hour, but their corresponding 95th percentiles show that labour could last up to 14, 11, and 9 hours, respectively. Substantial differences exist between actual plots of labour progression of individual women and the 'average labour curves' derived from study population-level data. Exclusion of women with augmented labours from the study population resulted in slightly faster labour progression patterns. CONCLUSIONS: Cervical dilatation during labour in the slowest-yet-normal women can progress more slowly than the widely accepted benchmark of 1 cm/hour, irrespective of parity. Interventions to expedite labour to conform to a cervical dilatation threshold of 1 cm/hour may be inappropriate, especially when applied before 5 cm in nulliparous and multiparous women. Averaged labour curves may not truly reflect the variability associated with labour progression, and their use for decision-making in labour management should be de-emphasized
