Information content of spatially distributed ground-based measurements for hydrologic-parameter calibration in mixed rain-snow mountain headwaters
Parameters in hydrologic models used in mixed rain-snow regions are often uncertain to calibrate and overfitted on streamflow. To contribute addressing these challenges, we used an algorithm that assesses modeling performances through time (Dynamic Identifiability Analysis) to quantify the information content of spatially distributed ground-based measurements for identifying optimal parameter values in the Precipitation Runoff Modeling System (PRMS) model. Including spatially distributed ground-based measurements in Identifiability Analysis allowed us to unambiguously estimate more parameter values than only using streamflow (seven parameters instead of two out of a pool of thirty-three). Peaks in information gain were obtained when using dew-point temperature to identify precipitation phase-partitioning parameters. Multi-attribute identifiability analysis also yielded optimal parameter values that were temporally less variable than those estimated using streamflow alone. Overall, identifying parameter values using ground-based measurements improved the simulation of key drivers of the surface-water budget, such as air temperature and precipitation-phase partitioning. However, parameters simulating surface-to-subsurface mass fluxes like snow accumulation and melt or evapotranspiration were poorly identified by any attribute and so emerged as key sources of predictive uncertainty for this distributed-parameter hydrologic model. This work demonstrates the value of expanded ground-based measurements for identifying parameters in distributed-parameter hydrologic models and so diagnosing their conceptual uncertainty across the water budget
sscMap: An extensible Java application for connecting small-molecule drugs using gene-expression signatures
Background: Connectivity mapping is a process to recognize novel
pharmacological and toxicological properties in small molecules by comparing
their gene expression signatures with others in a database. A simple and robust
method for connectivity mapping with increased specificity and sensitivity was
recently developed, and its utility demonstrated using experimentally derived
gene signatures.
Results: This paper introduces sscMap (statistically significant connections'
map), a Java application designed to undertake connectivity mapping tasks using
the recently published method. The software is bundled with a default
collection of reference gene-expression profiles based on the publicly
available dataset from the Broad Institute Connectivity Map 02, which includes
data from over 7000 Affymetrix microarrays, for over 1000 small-molecule
compounds, and 6100 treatment instances in 5 human cell lines. In addition, the
application allows users to add their custom collections of reference profiles
and is applicable to a wide range of other 'omics technologies.
Conclusions: The utility of sscMap is two fold. First, it serves to make
statistically significant connections between a user-supplied gene signature
and the 6100 core reference profiles based on the Broad Institute expanded
dataset. Second, it allows users to apply the same improved method to
custom-built reference profiles which can be added to the database for future

A simple and robust method for connecting small-molecule drugs using gene-expression signatures
Interaction of a drug or chemical with a biological system can result in a
gene-expression profile or signature characteristic of the event. Using a
suitably robust algorithm these signatures can potentially be used to connect
molecules with similar pharmacological or toxicological properties. The
Connectivity Map was a novel concept and innovative tool first introduced by
Lamb et al to connect small molecules, genes, and diseases using genomic
signatures [Lamb et al (2006), Science 313, 1929-1935]. However, the
Connectivity Map had some limitations, particularly there was no effective
safeguard against false connections if the observed connections were considered
on an individual-by-individual basis. Further when several connections to the
same small-molecule compound were viewed as a set, the implicit null hypothesis
tested was not the most relevant one for the discovery of real connections.
Here we propose a simple and robust method for constructing the reference
gene-expression profiles and a new connection scoring scheme, which importantly
allows the valuation of statistical significance of all the connections
observed. We tested the new method with the two example gene-signatures (HDAC
inhibitors and Estrogens) used by Lamb et al and also a new gene signature of
immunosuppressive drugs. Our testing with this new method shows that it
achieves a higher level of specificity and sensitivity than the original
method. For example, our method successfully identified raloxifene and
tamoxifen as having significant anti-estrogen effects, while Lamb et al's
Connectivity Map failed to identify these. With these properties our new method
has potential use in drug development for the recognition of pharmacological

The failure of suicide prevention in primary care: family and GP perspectives - a qualitative study
Background Although Primary care is crucial for suicide prevention, clinicians tend to report completed suicides in their care as non-preventable. We aimed to examine systemic inadequacies in suicide prevention from the perspectives of bereaved family members and GPs.Methods Qualitative study of 72 relatives or close friends bereaved by suicide and 19 General Practitioners who have experienced the suicide of patients.Results Relatives highlight failures in detecting symptoms and behavioral changes and the inability of GPs to understand the needs of patients and their social contexts. A perceived overreliance on anti-depressant treatment is a major source of criticism by family members. GPs tend to lack confidence in the recognition and management of suicidal patients, and report structural inadequacies in service provision.Conclusions Mental health and primary care services must find innovative and ethical ways to involve families in the decision-making process for patients at risk of suicide
Family physicians' effort to stay in charge of the medical treatment when patients have home care by district nurses. A grounded theory study
<p>Abstract</p> <p>Background</p> <p>District nurses (DNs) provide home care for old persons with a mixture of chronic diseases, symptoms and reduced functional ability. Family physicians (FPs) have been criticised for their lack of involvement in this care. The aim of this study was to obtain increased knowledge concerning the FP's experience of providing medical treatment for patients with home care provided by DNs by developing a theoretical model that elucidates how FPs handle the problems they encounter regarding the individual patients and their conditions.</p> <p>Methods</p> <p>Semi-structured interviews were conducted with 13 Swedish FPs concerning one of their registered patients with home care by a DN, and the treatment of this patient. Grounded theory methodology (GTM) was used in the analyses.</p> <p>Results</p> <p>The core category was the effort to stay in charge of the medical treatment. This involved three types of problems: gaining sufficient insight, making adequate decisions, and maintaining appropriate medical treatment. For three categories of patients, the FPs had problems staying in charge. Patients with reduced functional ability had problems providing information and maintaining treatment. Patients who were "fixed in their ways" did not provide information and did not comply with recommendations, and for patients with complex conditions, making adequate decisions could be problematic. To overcome the problems, four different strategies were used: relying on information from others, supporting close observation and follow-up by others, being constantly ready to change the goal of the treatment, and relying on others to provide treatment.</p> <p>Conclusion</p> <p>The patients in this study differed from most other patients seen at the healthcare centre as the consultation with the patient could not provide the usual foundation for decisions concerning medical treatment. Information from and collaboration with the DN and other home care providers was essential for the FP's effort to stay in charge of the medical treatment. The complexity of the situation made it problematic for the FP to make adequate decisions about the goal of the medical treatment. The goal of the treatment had to be constantly evaluated based on information from the DN and other care providers, and thus this information was absolutely crucial.</p
The frequency of genes encoding three putative group B streptococcal virulence factors among invasive and colonizing isolates
BACKGROUND: Group B Streptococcus (GBS) causes severe infections in very young infants and invasive disease in pregnant women and adults with underlying medical conditions. GBS pathogenicity varies between and within serotypes, with considerable variation in genetic content between strains. Three proteins, Rib encoded by rib, and alpha and beta C proteins encoded by bca and bac, respectively, have been suggested as potential vaccine candidates for GBS. It is not known, however, whether these genes occur more frequently in invasive versus colonizing GBS strains. METHODS: We screened 162 invasive and 338 colonizing GBS strains from different collections using dot blot hybridization to assess the frequency of bca, bac and rib. All strains were defined by serotyping for capsular type, and frequency differences were tested using the Chi square test. RESULTS: Genes encoding the beta C protein (bac) and Rib (rib) occurred at similar frequencies among invasive and colonizing isolates, bac (20% vs. 23%), and rib (28% vs. 20%), while the alpha (bca) C protein was more frequently found in colonizing strains (46%) vs, invasive (29%). Invasive strains were associated with specific serotype/gene combinations. CONCLUSION: Novel virulence factors must be identified to better understand GBS disease

NASA's Solar Probe Plus (SPP) mission will make the first in situ measurements of the solar corona and the birthplace of the solar wind. The FIELDS instrument suite on SPP will make direct measurements of electric and magnetic fields, the properties of in situ plasma waves, electron density and temperature profiles, and interplanetary radio emissions, amongst other things. Food access and diet quality are associated with quality of life outcomes among HIV-infected individuals in Uganda.
BACKGROUND: Food insecurity is associated with poor nutritional and clinical outcomes among people living with HIV/AIDS. Few studies investigate the link between food insecurity, dietary diversity and health-related quality of life among people living with HIV/AIDS. OBJECTIVE: We investigated whether household food access and individual dietary diversity are associated with health-related quality of life among people living with HIV/AIDS in Uganda. METHODS: We surveyed 902 people living with HIV/AIDS and their households from two clinics in Northern Uganda. Health-related quality of life outcomes were assessed using the Medical Outcomes Study (MOS)-HIV Survey. We performed multivariate regressions to investigate the relationship between health-related quality of life, household food insecurity and individual dietary diversity. RESULTS: People living with HIV/AIDS from severe food insecurity households have mean mental health status scores that are 1.7 points lower (p<.001) and physical health status scores that are 1.5 points lower (p<.01). Individuals with high dietary diversity have mean mental health status scores that were 3.6 points higher (p<.001) and physical health status scores that were 2.8 points higher (p<.05). CONCLUSIONS: Food access and diet quality are associated with health-related quality of life and may be considered as part of comprehensive interventions designed to mitigate psychosocial consequences of HIV
Stress, ageing and their influence on functional, cellular and molecular aspects of the immune system
The immune response is essential for keeping an organism healthy and for defending it from different types of pathogens. It is a complex system that consists of a large number of components performing different functions. The adequate and controlled interaction between these components is necessary for a robust and strong immune response. There are, however, many factors that interfere with the way the immune response functions. Stress and ageing now consistently appear in the literature as factors that act upon the immune system in the way that is often damaging. This review focuses on the role of stress and ageing in altering the robustness of the immune response first separately, and then simultaneously, discussing the effects that emerge from their interplay. The special focus is on the psychological stress and the impact that it has at different levels, from the whole system to the individual molecules, resulting in consequences for physical health
