    Progettazione e prototipazione di un dispositivo per ustionati pediatrici in contesti poveri di risorse

    Ogni anno, circa il 95% delle morti per ustioni si verifica in contesti poveri di risorse e di queste più di un terzo sono rappresentate da bambini con meno di 4 anni. Per determinare le caratteristiche del dispositivo è stata necessaria una lunga fase preliminare alla progettazione, dove si è analizzata l’entità del problema delle ustioni e i trattamenti attualmente eseguiti. La principale causa di morte per ustioni è dovuta alle infezioni per cui la priorità è creare un ambiente pulito e disinfettato, dove il paziente può essere trattato o trasportato ad un ospedale specializzato. Successivamente a questa fase di analisi dei bisogni, si sono identificate le funzionalità del dispositivo: il controllo della temperatura e dell’umidità per contrastare l’ipotermia e l’ipermetabolismo, la ventilazione d’aria filtrata per prevenire le infezioni e il sistema di lavaggio per disinfettare le lesioni. Il dispositivo è stato progettato per bambini con statura inferiore ad un metro. Da qui si sono derivate le specifiche tecniche di progetto, su cui è stata svolta l’analisi dei rischi e progettato un sistema di mitigazione più o meno efficace per ottenere un rischio residuo accettabile. Terminata la progettazione generale, si è disegnato lo schema circuitale e si sono scelti i componenti elettronici per l’implementazione del prototipo. Il sistema deve esser fornito di sensori di temperatura, umidità, anidride carbonica, ed attuatori per regolare questi parametri, come l’elemento riscaldante, la ventola e la pompa idraulica. Per la realizzazione del prototipo si è utilizzata una schema Wemos e un sistema di comunicazione I2C. Successivamente alla costruzione della struttura e al cablaggio, si è implementato un modello in Simulink per testare i coefficienti del controllore PID per il sistema di termoregolazione. In fine, si sono eseguiti dei test per valutare il controllo della temperatura all’interno dello scompartimento del bambino, e si sono ottenuti buoni risultati

    Phase-contrast microtomography: are the tracers necessary for stem cell tracking in infarcted hearts?

    Recent literature has identified innovative approaches of cellular therapy to generate new myocardium involving transcoronary and intramyocardial injection of cardiac progenitor cells (CPCs). One of the limiting factors in the overall interpretation of these preclinical results is the lack of reliable methods for 3D imaging and quantification of the injected cells and for the assessment of their fate within the myocardium. Here, for the first time to the authors' knowledge, we support by demonstrative experiments the hypothesis that phase-contrast microtomography (PhC-microCT) could offer an efficient 3D imaging approach to track the injected cells within the myocardium, without the need of any cell tracer. This deduction has been validated by several observations: i) a strong phase-contrast signal was observed in infarcted hearts injected with unlabeled cells; ii) the PhC-microCT 3D reconstructions of hearts injected with only vehicle saline solution and rhodamine particles, i.e. without CPCs, did not show any contrast; (iii) in the 3D PhC-microCT reconstructions of non infarcted hearts, injected with unlabeled CPCs, the contrast signal of the cells was present but differently distributed; and iv) the contrast signal of injected cells diminished over time apparently following the same timing of cell engraftment and differentiation, as confirmed in literature by histology and fluorescence analysis. The chance to avoid cell tracers is of paramount interest in determining the fate of transplanted stem cells because the quantification of the signal will not be any more dependent on injected dose, concentration of the tracer, cell proliferation and tracer uptake kinetics

    Functional Connectivity Changes After Initial Treatment With Fingolimod in Multiple Sclerosis

    On the basis of recent functional MRI studies, Multiple Sclerosis (MS) has been interpreted as a multisystem disconnection syndrome. Compared to normal subjects, MS patients show alterations in functional connectivity (FC). However, the mechanisms underlying these alterations are still debated. The aim of the study is to investigate resting state (RS) FC changes after initial treatment with fingolimod, a proven anti-inflammatory and immunomodulating agent for MS. We studied 32 right-handed relapsing-remitting MS patients (median Expanded Disability Status Scale: 2.0, mean disease duration: 8.8 years) who underwent both functional and conventional MRI with a 3 Tesla magnet. All assessments were performed 3 weeks before starting fingolimod, then, at therapy-initiation stage and at month 6. Each imaging session included scans at baseline (run1) and after (run2) a 25-min, within-session, motor-practice task, consisting of a paced right-thumb flexion. FC was assessed using a seed on the left primary motor cortex to obtain parametric maps at run1 and task-induced FC change (run2-run1). Comparison between 3-week before- and fingolimod start sessions accounted for a test-retest effect. The main outcome was the changes in both baseline and task-induced changes in FC, between initiation and 6 months. MRI contrast enhancement was detected in 14 patients at initiation and only in 3 at month 6. There was a significant improvement (p < 0.05) in cognitive function, as measured by the Paced Auditory Serial Addition Task, at month 6 compared to initiation. After accounting for test-retest effect, baseline FC significantly decreased at month 6, with respect to initiation (p < 0.05, family-wise error corrected) in bilateral occipito-parietal areas and cerebellum. A task-induced change in FC at month 6 showed a significant increment in all examined sessions, involving not only areas of the sensorimotor network, but also posterior cortical areas (cuneus and precuneus) and areas of the prefrontal and temporal cortices (p < 0.05, family-wise error corrected). Cognitive improvement at month 6 was significantly (p < 0.05) related to baseline FC reduction in posterior cortical areas. This study shows significant changes in functional connectivity, both at baseline and after the execution of a simple motor task following 6 months of fingolimod therapy

    Human mesenchymal stem cells (hMSCs) are pluripotent adult stem cells capable of being differentiated into osteoblasts, adipocytes, and chondrocytes. The osteogenic differentiation of hMSCs is regulated either by systemic hormones or by local growth factors able to induce specific intracellular signal pathways that modify the expression and activity of several transcription factors.     Vaccination coverage in healthcare workers: a multicenter cross-sectional study in Italy

    IntroductionIn recent years, a phenomenon known as "vaccine hesitancy" has spread throughout the world, even among health workers, determining a reduction in vaccination coverage (VC). A study aimed at evaluating VC among healthcare workers (HCWs) in 10 Italian cities (L'Aquila, Genoa, Milan, Palermo, Sassari, Catanzaro, Ferrara, Catania, Naples, Messina) was performed.Materials and methodsAnnex 3 of the Presidential Decree n. 445 of 28 December 2000 was used to collect information on the vaccination status of HCWs. The mean and standard deviation (SD) were calculated with regard to the quantitative variable (age), while absolute and relative frequencies were obtained for categorical data (sex, professional profile, working sector, vaccination status). The connection between VC and the categorical variables was evaluated by chi-square method (statistical significance at p<0.05). The statistical analyses were performed by SPSS and Stata software.ResultsA total of 3,454 HCWs participated in the project: 1,236 males and 2,218 females. The sample comprised: physicians (26.9%), trainee physicians (16.1%), nurses (17.2%) and other professional categories (9.8%). Low VC was generally recorded. Higher VC was found with regard to polio, hepatitis B, tetanus and diphtheria, while coverage was very low for measles, mumps, rubella, pertussis, chickenpox and influenza (20-30%). ConclusionsThis study revealed low VC rates among HCWs for all the vaccinations. Measures to increase VC are therefore necessary in order to prevent HCWs from becoming a source of transmission of infections with high morbidity and/or mortality both within hospitals and outside

    Characterization of the patterns of drug-resistance mutations in newly diagnosed HIV-1 infected patients naïve to the antiretroviral drugs

    <p>Abstract</p> <p>Background</p> <p>The transmission of HIV-1 drug-resistant strains in drug naive patients may seriously compromise the efficacy of a first-line antiretroviral treatment. To better define this problem, a study in a cohort of newly diagnosed HIV-1 infected individuals has been conducted. This study is aimed to assess the prevalence and the patterns of the mutations recently associated with transmitted drug resistance in the reverse transcriptase (RT) and in protease (PR) of HIV-1.</p> <p>Methods</p> <p>Prevalence of transmitted drug resistant strains is determined in 255 newly diagnosed HIV-1 infected patients enrolled in different counselling and testing (CT) centres in Central Italy; the Avidity Index (AI) on the first available serum sample is also used to estimate time since infection. Logistic regression models are used to determine factors associated with infection by drug resistant HIV-1 strains.</p> <p>Results</p> <p>The prevalence of HIV-1 strains with at least one major drug resistance mutation is 5.9% (15/255); moreover, 3.9% (10/255) of patients is infected with HIV nucleoside reverse transcriptase inhibitor (NRTI)-resistant viruses, 3.5% (9/255) with HIV non-NRTI-resistant viruses and 0.4% (1/255) with HIV protease inhibitor (PI)-resistant viruses. Most importantly, almost half (60.0%) of patients carries HIV-1 resistant strains with more than one major drug resistance mutation. In addition, patients who had acquired HIV through homosexual intercourses are more likely to harbour a virus with at least one primary resistance mutation (OR 7.7; 95% CI: 1.7–35.0, P = 0.008).</p> <p>Conclusion</p> <p>The prevalence of drug resistant HIV-1 strains among newly diagnosed individuals in Central Italy is consistent with the data from other European countries. Nevertheless, the presence of drug-resistance HIV-1 mutations in complex patterns highlights an additional potential risk for public health and strongly supports the extension of wide genotyping to newly diagnosed HIV-1 infected patients.</p

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p &lt; 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p &lt; 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p &lt; 0.0001) or urgent (20.4% vs. 38.5%; p &lt; 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p &lt; 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice
