HEPATITE C EM CASAIS INFĂRTEIS DO SETOR DE REPRODUĂĂO HUMANA DO HOSPITAL DE CLĂNICAS DE PORTO ALEGRE
Objective: Hepatitis C virus transmission in assisted reproduction may pose a risk for the baby, technicians, and gametes or embryos from non-contaminated parents. This study aimed at determining the prevalence and risk factors for hepatitis C virus infection in a group of infertile couples.Methods: Four hundred and nine patients attending the infertility clinic at Hospital de ClĂnicas de Porto Alegre, Brazil, between 1997 and 1998, were screened for anti-HCV (ELISA) and HBsAg (ELFA). Hepatitis C virus (HCV) infection and semen viremia was also investigated using HCV RNA detection Results: All subjects were negative for HBV and HIV. The overall prevalence of anti-HCV was 3.2% (8/ 248) among women and 3.7% (6/161) among men. From the 14 HCV-positive patients, two were lost, and serum was collected from the remaining 12 patients for assessment of HCV RNA, resulting in five HCVpositive cases (one woman and four men). Only one of these positive cases had viremia levels > 500,000 RNA copies/ml. There was a significant risk association for being HCV-positive in women with HCV-positive male partners (P < 0.001). In male patients, the correlation between use of intravenous drugs and HCVpositivity was also significant (P < 0.001).Conclusions: Infertile patients should be screened before assisted reproductive technology (ART) procedures, since the risk for vertical and laboratory HCV infection is not well determined, and HCV prevalence is not negligible in this group.Objetivo: A possibilidade transmissĂŁo do vĂrus da hepatite C atravĂ©s dos gametas pode acarretar riscos para o pessoal tĂ©cnico, bem como para os envolvidos no processo e para o prĂłprio feto. Este estudo teve como objetivo determinar a prevalĂȘncia e os fatores de risco da infecção pelo vĂrus da hepatite C em um grupo de casais infĂ©rteis.MĂ©todos: Em 409 pacientes atendidas no ambulatĂłrio de infertilidade do Hospital de ClĂnicas de Porto Alegre (HCPA), entre 1997 e 1998, realizou-se triagem sorolĂłgica para antiHCV (ELISA) e HBsAg (ELFA). A infecção pelo vĂrus da hepatite C (HCV) e a viremia seminal tambĂ©m foram investigadas com detecção de HCV-RNA.Resultados: A prevalĂȘncia geral de anti-HCV foi de 3,2% (8/248) entre as mulheres e 3,7% (6/161) entre os homens. Todos os indivĂduos eram negativos para o vĂrus da hepatite B (HBV) e HIV. Das 14 pacientes HCV-positivas, duas foram perdidas, e foi coletado soro das 12 pacientes remanescentes para detecção de HCV-RNA, resultando em cinco casos HCVpositivos (uma mulher e quatro homens). Apenas um dos casos positivos tinha nĂvel de viremia > 500.000 cĂłpias de RNA/ml. Houve uma associação de risco significativa da positividade para HCV nas mulheres com parceiros HCV-positivos (P < 0,001). Oxidative stress in type 2 diabetes: the role of fasting and postprandial glycaemia
Oxidative stress, through the production of reactive oxygen species (ROS), has been proposed as the root cause underlying the development of insulin resistance, ÎČ-cell dysfunction, impaired glucose tolerance and type 2 diabetes mellitus (T2DM). It has also been implicated in the progression of long-term diabetes complications, including microvascular and macrovascular dysfunction. Excess nourishment and a sedentary lifestyle leads to glucose and fatty acid overload, resulting in production of ROS. Additionally, reaction of glucose with plasma proteins forms advanced glycation end products, triggering production of ROS. These ROS initiate a chain reaction leading to reduced nitric oxide availability, increased markers of inflammation and chemical modification of lipoproteins, all of which may increase the risk of atherogenesis. With the postulation that hyperglycaemia and fluctuations in blood glucose lead to generation of ROS, it follows that aggressive treatment of fasting and postprandial hyperglycaemia is important for prevention of micro and macrovascular complications in T2DM
InfluĂȘncia da tĂ©cnica de amamentação nas freqĂŒĂȘncias de aleitamento materno exclusivo e lesĂ”es mamilares no primeiro mĂȘs de lactação The influence of breastfeeding technique on the frequencies of exclusive breastfeeding and nipple trauma in the first month of lactation
OBJETIVO: Investigar a influĂȘncia da tĂ©cnica de amamentação nas freqĂŒĂȘncias de aleitamento materno exclusivo e de lesĂ”es mamilares no primeiro mĂȘs de lactação. MĂTODOS: Foram pesquisados parĂąmetros desfavorĂĄveis Ă amamentação (cinco relacionados ao posicionamento mĂŁe/bebĂȘ e trĂȘs Ă pega do bebĂȘ) em 211 mĂŁes/bebĂȘs na maternidade e, aos 30 dias, no domicĂlio. Foram comparadas as freqĂŒĂȘncias desses parĂąmetros entre as duplas com e sem amamentação exclusiva aos 7 e 30 dias, e entre as mĂŁes com e sem lesĂ”es mamilares na maternidade. RESULTADOS: O nĂșmero de parĂąmetros desfavorĂĄveis na maternidade foi semelhante nas duplas com e sem amamentação exclusiva aos 7 e 30 dias. PorĂ©m, aos 30 dias, foi, em mĂ©dia, menor nas duplas em amamentação exclusiva, tanto no posicionamento (1,7±1,2 versus 2,2±1,1; p = 0,009) quanto na pega (1,0±0,6 versus 1,4±0,6; p < 0,001). O nĂșmero de parĂąmetros desfavorĂĄveis de pega na maternidade foi semelhante entre as mulheres com e sem lesĂŁo mamilar, porĂ©m as mulheres sem essa complicação apresentaram um nĂșmero maior de parĂąmetros desfavorĂĄveis de posicionamento (2,0±1,4 versus 1,4±1,2; p = 0,04). CONCLUSĂES: As freqĂŒĂȘncias de amamentação exclusiva no primeiro mĂȘs e de lesĂ”es mamilares nĂŁo foram influenciadas pela tĂ©cnica de amamentação na maternidade, mas houve associação entre melhor tĂ©cnica aos 30 dias e prĂĄtica da amamentação exclusiva. Novos estudos poderĂŁo elucidar se uma melhora da tĂ©cnica ao longo do tempo seria responsĂĄvel pela manutenção da amamentação exclusiva ou se a introdução de mamadeira exerceria efeito negativo na tĂ©cnica.<br>OBJECTIVE: To investigate the influence of breastfeeding technique on the frequencies of exclusive breastfeeding and nipple trauma in the first month of lactation. METHODS: We searched for unfavorable parameters of breastfeeding (five related to mother/baby positioning and three related to baby' latch on) in 211 mother-baby pairs in the maternity ward and at day 30, at home. We compared the frequencies of these parameters between mothers practicing or not exclusive breastfeeding at days 7 and 30, and between mothers with or without nipple trauma at the hospital. RESULTS: The number of unfavorable parameters in the maternity ward was similar for mother-baby pairs practicing or not exclusive breastfeeding at day 7 and 30. However, at day 30, it was, on average, lower among those under exclusive breastfeeding, regarding positioning (1.7±1.2 vs 2.2±1.1; p = 0.009) as well as latch on (1.0±0.6 vs 1.4±0.6; p < 0.001). The number of unfavorable parameters related to latch on in the maternity ward was similar for women with or without nipple trauma, but women without trauma presented a higher number of unfavorable parameters related to positioning. (2.0±1.4 vs 1.4±1.2; p = 0.04). CONCLUSIONS: The frequencies of exclusive breastfeeding in the first month and of nipple trauma were not influenced by the breastfeeding technique in the maternity ward, but there was an association between a better technique at day 30 and the practice of exclusive breastfeeding. New studies may help to elucidate whether an improvement in breastfeeding practices over time helps the maintenance of exclusive breastfeeding or whether the introduction of bottle-feeding determines a negative effect on breastfeeding

PrevalĂȘncia e determinantes de anemia em crianças de Porto Alegre, RS, Brasil Prevalence and risk factors for anemia among children in Brazil
OBJETIVO: O estudo foi realizado com o objetivo de verificar a prevalĂȘncia de anemia e seus possĂveis determinantes em crianças de 0 a 36 meses de idade que freqĂŒentam escolas municipais infantis. MĂTODOS: Realizou-se um estudo transversal pelo qual foram estudadas 557 crianças de 0 a 36 meses de idade de todas as escolas municipais infantis de Porto Alegre, RS. Foi feita antropometria e dosagem de hemoglobina pelo fotĂŽmetro portĂĄtil HemoCue, considerando-se anemia nĂveis inferiores a 11 g/dl. As informaçÔes sobre as crianças foram obtidas por questionĂĄrio aplicado Ă s mĂŁes. A associação das variĂĄveis estudadas com a anemia foi analisada pela tĂ©cnica de regressĂŁo log-binomial aplicada ao modelo hierĂĄrquico. RESULTADOS: Encontrou-se uma prevalĂȘncia de anemia de 47,8% entre toda a população estudada, cujos determinantes foram: famĂlias com renda per capita igual ou inferior a um salĂĄrio-mĂnimo (razĂŁo de prevalĂȘncia [RP] = 1,6), faixa etĂĄria de 12 a 23 meses (RP=1,4) e presença de dois ou mais irmĂŁos com menos de cinco anos (RP=1,4). CONCLUSĂES: A prevalĂȘncia de anemia na população estudada Ă© bastante elevada, especialmente nas crianças de nĂvel socioeconĂŽmico mais baixo, na faixa etĂĄria de 12 a 23 meses, e nas crianças com dois ou mais irmĂŁos com menos de cinco anos, indicando a necessidade urgente de medidas efetivas visando o seu combate e a sua prevenção.<br>OBJECTIVE: To verify the prevalence of anemia among children aged 0 to 36 months, who attend public day care centers in Porto Alegre, Brazil, and assess its possible risk factors. METHODS: A cross-sectional study was carried out in 557 children aged 0 to 36 months of all public day care centers in Porto Alegre. Anthropometric measurements and hemoglobin levels were performed. The portable HemoCue photometer was employed to measure hemoglobin levels, and anemia was considered when the hemoglobin level was below 11 g/dl. Information regarding each child was obtained by means of a questionnaire applied to the mother. The association of the variables studied to anemia was analyzed using the log-binomial regression technique applied to the hierarchical model. RESULTS: A 47.8% prevalence of anemia was found in this population. The risk factors for anemia in the studied group were: families with per capita income equal or less than one monthly minimal wage (prevalence ratio -- RP =1.6), age between 12 and 23 months (RP=1.4), and having 2 or more siblings younger than 5 years old (RP=1.4). CONCLUSIONS: There is a high prevalence of anemia among children aged 0 to 36 months in public day care centers, especially among children with the lowest socioeconomic level, in the 12 to 23 months age group, and who have 2 or more siblings under 5 years of age, indicating that there is an urgent need for effective measures to fight and prevent this condition
