Differential effects of apolipoprotein E isoforms on phosphorylation at specific sites on tau by glycogen synthase kinase-3β identified by nano-electrospray mass spectrometry
AbstractPreviously published data have shown an allele-specific variation in the in vitro binding of apolipoprotein E (apoE) to tau, which prompted the hypothesis that apoE binding may protect tau from phosphorylation, apoE3 being more efficient than apoE4. We have, therefore, investigated the effects of apoE on tau phosphorylation in vitro by the proline-directed kinase, glycogen synthase kinase (GSK)-3β. The phosphopeptide maps of tau alone, of tau with apoE3 and of tau with apoE4 were very similar. When apoE2 was present a further four spots were evident. Additionally, of the 15 peptides phosphorylated in the presence or absence of apoE, subtle differences, some isoform-specific, in the relative amounts of phosphorylation were observed

Marked improvements in glycaemic outcomes following insulin pump therapy initiation in people with type 1 diabetes:a nationwide observational study in Scotland

ETM/47).Aims/hypothesis Our aim was to assess the use of continuous subcutaneous insulin infusion (CSII) in people with type 1 diabetes in Scotland and its association with glycaemic control, as measured by HbA1c levels, frequency of diabetic ketoacidosis (DKA) and severe hospitalised hypoglycaemia (SHH), overall and stratified by baseline HbA1c. Methods We included 4684 individuals with type 1 diabetes from the national Scottish register, who commenced CSII between 2004 and 2019. We presented crude within-person differences from baseline HbA1c over time since initiation, crude DKA and SHH event-rates pre-/post-CSII exposure. We then used mixed models to assess the significance of CSII exposure, taking into account: (1) the diffuse nature of the intervention (i.e. structured education often precedes initiation); (2) repeated within-person measurements; and (3) background time-trends occurring pre-intervention. Results HbA1c decreased after CSII initiation, with a median within-person change of â5.5 mmol/mol (IQR â12.0, 0.0) (â0.5% [IQR â1.1, 0.0]). Within-person changes were most substantial in those with the highest baseline HbA1c, with median â21.0 mmol/mol (â30.0, â11.0) (â1.9% [â2.7, â1.0]) change in those with a baseline >84 mmol/mol (9.8%) within a year of exposure, that was sustained: â19.0 mmol/mol (â27.6, â6.5) (â1.7% [â2.5, â0.6]) at âĽ5 years. Statistical significance and magnitude of change were supported by the mixed models results. The crude DKA event-rate was significantly lower in post-CSII person-time compared with pre-CSII person-time: 49.6 events (95% CI 46.3, 53.1) per 1000 person-years vs 67.9 (64.1, 71.9); rate ratio from Bayesian mixed models adjusting for pre-exposure trend: 0.61 (95% credible interval [CrI] 0.47, 0.77; posterior probability of reduction ppâ=â1.00). The crude overall SHH event-rate in post-CSII vs pre-CSII person-time was also lower: 17.8 events (95% CI 15.8, 19.9) per 1000 person-years post-exposure vs 25.8 (23.5, 28.3) pre-exposure; rate ratio from Bayesian mixed models adjusting for pre-exposure trend: 0.67 (95% CrI 0.45, 1.01; ppâ=â0.97). Conclusions/interpretation CSII therapy was associated with marked falls in HbA1c especially in those with high baseline HbA1c. CSII was independently associated with reduced DKA and SHH rates. CSII appears to be an effective option for intensive insulin therapy in people with diabetes for improving suboptimal glycaemic control.Publisher PDFPeer reviewe
Rising Rates And Widening Socio-economic Disparities In Diabetic Ketoacidosis In Type 1 Diabetes In Scotland:A Nationwide Retrospective Cohort Observational Study
OBJECTIVE:
Whether advances in the management of type 1 diabetes are reducing rates of
diabetic ketoacidosis (DKA) is unclear. We investigated time trends in DKA rates
in a national cohort of individuals with type 1 diabetes monitored for 14 years,
overall and by socioeconomic characteristics.
RESEARCH DESIGN AND METHODS:
All individuals in Scotland with type 1 diabetes who were alive and at least 1 year
old between 1 January 2004 and 31 December 2018 were identified using the
national register (N 5 37,939). DKA deaths and hospital admissions were
obtained through linkage to Scottish national death and morbidity records.
Bayesian regression was used to test for DKA time trends and association with
risk markers, including socioeconomic deprivation.
RESULTS:
There were 30,427 DKA admissions and 472 DKA deaths observed over 393,223
person-years at risk. DKA event rates increased over the study period (incidence
rate ratio [IRR] per year 1.058 [95% credibility interval 1.054â1.061]). Males had
lower rates than females (IRR male-to-female 0.814 [0.776â0.855]). DKA incidence rose in all age-groups other than 10- to 19-year-olds, in whom rates were
the highest, but fell over the study. There was a large socioeconomic differential
(IRR least-to-most deprived quintile 0.446 [0.406â0.490]), which increased during
follow-up. Insulin pump use or completion of structured education were associated with lower DKA rates, and antidepressant and methadone prescription were
associated with higher DKA rates.
CONCLUSIONS:
DKA incidence has risen since 2004, except in 10- to 19-year-olds. Of particular
concern are the strong and widening socioeconomic disparities in DKA outcomes.

Understanding the potential impact of different drug properties on SARS-CoV-2 transmission and disease burden : a modelling analysis

The unprecedented public health impact of the COVID-19 pandemic has motivated a rapid
search for potential therapeutics, with some key successes. However, the potential impact of
different treatments, and consequently research and procurement priorities, have not been clear.
Methods and Findings
develop a mathematical model of SARS-CoV-2 transmission, COVID-19 disease and
clinical care to explore the potential public-health impact of a range of different potential
therapeutics, under a range of different scenarios varying: i) healthcare capacity, ii) epidemic
trajectories; and iii) drug efficacy in the absence of supportive care. In each case, the outcome
of interest was the number of COVID-19 deaths averted in scenarios with the therapeutic
compared to scenarios without. We find the impact of drugs like dexamethasone (which are
delivered to the most critically-ill in hospital and whose therapeutic benefit is expected to
depend on the availability of supportive care such as oxygen and mechanical ventilation) is
likely to be limited in settings where healthcare capacity is lowest or where uncontrolled
epidemics result in hospitals being overwhelmed. As such, it may avert 22% of deaths in highincome countries but only 8% in low-income countries (assuming R=1.35). Therapeutics for
different patient populations (those not in hospital, early in the course of infection) and types
of benefit (reducing disease severity or infectiousness, preventing hospitalisation) could have
much greater benefits, particularly in resource-poor settings facing large epidemics.
Conclusions
There is a global asymmetry in who is likely to benefit from advances in the treatment of
COVID-19 to date, which have been focussed on hospitalised-patients and predicated on an
assumption of adequate access to supportive care. Therapeutics that can feasibly be delivered
to those earlier in the course of infection that reduce the need for healthcare or reduce
infectiousness could have significant impact, and research into their efficacy and means of

An inclusive Research and Education Community (iREC) model to facilitate undergraduate science education reform
Funding: This work was supported by Howard Hughes Medical Institute grants to DIH is GT12052 and MJG is GT15338.Over the last two decades, there have been numerous initiatives to improve undergraduate student outcomes in STEM. One model for scalable reform is the inclusive Research Education Community (iREC). In an iREC, STEM faculty from colleges and universities across the nation are supported to adopt and sustainably implement course-based research â a form of science pedagogy that enhances student learning and persistence in science. In this study, we used pathway modeling to develop a qualitative description that explicates the HHMI Science Education Alliance (SEA) iREC as a model for facilitating the successful adoption and continued advancement of new curricular content and pedagogy. In particular, outcomes that faculty realize through their participation in the SEA iREC were identified, organized by time, and functionally linked. The resulting pathway model was then revised and refined based on several rounds of feedback from over 100 faculty members in the SEA iREC who participated in the study. Our results show that in an iREC, STEM faculty organized as a long-standing community of practice leverage one another, outside expertise, and data to adopt, implement, and iteratively advance their pedagogy. The opportunity to collaborate in this manner and, additionally, to be recognized for pedagogical contributions sustainably engages STEM faculty in the advancement of their pedagogy. Here, we present a detailed pathway model of SEA that, together with underpinning features of an iREC identified in this study, offers a framework to facilitate transformations in undergraduate science education.Peer reviewe
Safety of bendamustine for the treatment of indolent non-Hodgkin lymphoma: a UK real-world experience
Introduction: Bendamustine is among the most effective chemotherapeutics for indolent B-cell non-Hodgkin lymphomas (iNHL), but trial reports of significant toxicity, including opportunistic infections and excess deaths, led to prescriber warnings. We conducted a multicentre observational study evaluating bendamustine toxicity in real-world practice. Methods: Patients receiving at least one dose of bendamustine (B) +/- rituximab (R) for iNHL were included. Demographics, lymphoma and treatment details and grade 3-5 adverse events (AEs) were analysed. Results: 323 patients were enrolled from 9 NHS hospitals. Most patients (96%) received BR and 46% R maintenance. 21.7% experienced serious AEs (SAE) related to treatment, including infections in 12%, with absolute risk highest during induction (63%), maintenance (20%), and follow-up (17%), and the relative risk highest during maintenance (54%), induction (34%) and follow-up (28%). Toxicity led to permanent treatment discontinuation in 13% of patients, and 2.8% died of bendamustine-related infections (n=5), myelodysplastic syndrome (n=3), and cardiac disease (n=1). More SAEs per patient were reported in patients with mantle cell lymphoma, poor pre-induction PS, poor pre-maintenance PS, abnormal pre-induction total globulins and in those receiving growth factors. Use of antimicrobial prophylaxis was variable, and 3/10 opportunistic infections occurred despite prophylaxis. Conclusion: In this real-world analysis, bendamustine-related deaths and treatment discontinuation were similar to trial populations of younger, fitter patients. Poor PS, mantle cell histology and maintenance rituximab were potential risk factors. Infections, including late onset events, were the most common treatment-related SAE and cause of death warranting extended antimicrobial prophylaxis and infectious surveillance, especially in maintenance-treated patients
