    Seroprevalence of hepatitis B virus and its co-infection with hepatitis D virus and hepatitis C virus in Iranian adult population

    Context: Hepatitis B virus (HBV) infection is one of the most prevalent public health problems worldwide (especially in developing countries). Aims: This study was carried out to determine the seroprevalence of HBV and its co-infection with hepatitis D (HDV) and C (HCV) viruses in the northeastern part of Iran. Setting and Design: A population-based cross-sectional study in Iran. Materials and Methods: As many as 1,850 subjects were explored for HBsAg. Anti-HDV and anti-HCV antibodies were assessed in HBsAg-positive cases. Statistical Analysis Used: Proportions were compared by Chi-square and Fisher's exact tests. Results: The mean age of subjects was 43.86 ± 11.2 years. The age- and sex-standardized prevalence for HBsAg positivity was 9.7%. It was higher in males than in females (OR: 1.28; 95% CI: 0.9-1.7). The risk of infection in singles was significantly higher than in married cases (OR: 2.13). Eight (5.8%) of HBsAg-positive cases were infected with HDV, and 17 (12.3%) were positive for anti-HCV antibody. Conclusion: This study demonstrates that the prevalence of HBsAg seropositivity in Golestan province of Iran is higher than the levels reported by WHO and previous studies from Iran. It is very important, especially for health providers and policy makers, to recognize the risk factors of HBV infection and its co-infection with HDV and HCV in this area and design effective preventive programs

    Temperature measurement and control system for transtibial prostheses: single subject clinical evaluation.

    The snug fit of a prosthetic socket over the residual limb can disturb thermal balance and put skin integrity in jeopardy by providing an unpleasant and infectious environment. The prototype of a temperature measurement and control (TM&C) system was previously introduced to resolve thermal problems related to prostheses. This study evaluates its clinical application in a setting with reversal, single subject design. The TM&C system was installed on a fabricated prosthetic socket of a man with unilateral transtibial amputation. Skin temperature of the residual limb without prosthesis at baseline and with prosthesis during rest and walking was evaluated. The thermal sense and thermal comfort of the participant were also evaluated. The results showed different skin temperature around the residual limb with a temperature decrease tendency from proximal to distal. The TM&C system decreased skin temperature rise after prosthesis wearing. The same situation occurred during walking, but the thermal power of the TM&C system was insufficient to overcome heat build-up in some regions of the residual limb. The participant reported no significant change of thermal sense and thermal comfort. Further investigations are warranted to examine thermography pattern of the residual limb, thermal sense, and thermal comfort in people with amputation.N/

    Temperature measurement and control system for transtibial prostheses: functional evaluation.

    The accumulation of heat inside the prosthetic socket increases skin temperature and fosters perspiration, which consequently leads to high tissue stress, friction blister, discomfort, unpleasant odor, and decreased prosthesis suspension and use. In the present study, the prototype of a temperature measurement and control (TM&C) system was designed, fabricated, and functionally evaluated in a phantom model of the transtibial prosthetic socket. The TM&C system was comprised of 12 thermistors divided equally into two groups that arranged internal and external to a prosthetic silicone liner. Its control system was programmed to select the required heating or cooling function of a thermal pump to provide thermal equilibrium based on the amount of temperature difference from a defined set temperature, or the amount of difference between the mean temperature recorded by inside and outside thermistors. A thin layer of aluminum was used for thermal conduction between the thermal pump and different sites around the silicone liner. The results showed functionality of the TM&C system for thermoregulation inside the prosthetic socket. However, enhancing the structure of this TM&C system, increasing its thermal power, and decreasing its weight and cost are main priorities before further development.The Hong Kong Polytechnic University supported all expenses of functional evaluation.     Evaluating environmental effects in construction and demolition waste recycling plant with the Iranian Leopold Matrix method

    BACKGROUND AND OBJECTIVES: Recycling and reusing construction and demolition debris is a productive step toward solving this problem. Still, the recycling process also leaves industrial effluents, which is evident in producing recycled sand. The present research has investigated the environmental effects of recycling construction debris at sand recycling plants. Considering the negative impacts of sand washing mud produced at the plant in the Aab'Ali Landfill of Tehran in Iran, the material's physicochemical characteristics and environmental impact have also been investigated to regulate practices.                        METHODS: The Environmental Impact Assessment has been carried out in physicochemical, biological, socio-cultural, and economic-technical areas. Due to the large dispersion of the studied soil and the composition diversity in each sampling, 30 samples of the sand washing mud and the material mixed with the surrounding soil have been collected. The exploitation phase during the factory construction plan's implementation stage was considered the current research's main phase. Hence, 13 micro activities and 23 environmental parameters were identified, and the results were analyzed in the Environmental Impact Assessment Plus Software using the Iranian Leopold Matrix method and discussed based on the results of the experiments.FINDINGS: According to the results of the matrix calculation, the three micro-activities included washing the sand through a sand-washing machine, fine sand washing through the EvoWash machine with a score of -3.6, converting concrete pieces and large boulders into smaller pieces by jackhammers, transferring to the jaw crusher machine with a score of -2.8, and transferring the remaining sand washing mud produced by the EvoWash machine to the storage pond with a score of -2.7 had the most negative effects. The three micro-activities of waste processing for green space irrigation (+2.2), selling products (+0.9), and hiring employees with a score of +0.5 have the most positive effects on the environment. As ranking smaller than -31 forming 50% of the total average of rows and columns, the activity of the plant and the sand extraction process in this landfill is approved by providing modification alternatives.CONCLUSION: Considering the positive impact on the economy, increasing green spaces in the region, job creation, and also reducing the amount of increasing debris accumulated in the landfill is evaluated positively and can be done considering the reforms; including the prevention of releasing remnant sand washing mud freely and recycling it instead. Reusing the sand washing mud requires improving the water purification systems used in the EvoWash machine

    Pilot-aided estimation and equalisation of a Radio-over-Fibre system in Wideband Code Division Multiple Access

    In this study, the impact of a Radio-over-Fibre (RoF) subsystem on the capacity performance of wideband code division multiple access is evaluated. This study investigates the use of pilot-aided channel estimation to compensate for the optical subsystem non-linearities for different channel conditions, estimation intervals and coding schemes. The results show that pilot-aided channel estimation is an effective method for compensating the composite impairments of the optical subsystem and the radio frequency (RF) channel. It is found that there is always a suitable pilot power level which maximises the system capacity performance regardless of coding scheme and channel condition. Also, the peak capacity is only slightly affected by a decrease in the estimation interval

    Molecular pathology of 6 novel GJB2 allelic variants detected in familial and sporadic Iranian non syndromic hearing loss cases

    Background: Mutations of GJB2 gene encoding connexion 26 are the most common cause of hearing loss in many populations. A very wide spectrum of GJB2 gene mutations associated with hearing loss have been detected but pathogenic role has been tested only for a part of them. In this study, we have provided genetic evidence on the pathogenicity of our previously reported novel GJB2 allelic variants. Methods: The pathogenic role of GJB2 allelic variants were assessed using co segregation of each allelic variant with hearing loss in family members, absence of the allelic variants in control populations, coexistence with a second GJB2 mutation, nature of the amino acid substitution and evolutionary conservation of the appropriate amino acid. Results: The GJB2 allelic variants including 363delC, 327delGGinsA, H16R and G200R have been co segregated with autosomal recessive non syndromic hearing loss in five families and are not found in control subjects. The G130V and K102Q were found in heterozygous state in two deaf individuals. G130V results in an exchange a residue highly conserved among all the connexins but was found with a rate of 1% in control subjects and K102Q results in an exchange a residue not conserved among all the connexins and not identified in control subjects. Conclusion: We conclude that, 363delC, 327delGGinsA, H16R and G200R may be pathogenic. However, the pathogenicity and inheritance of K102Q and G130V can not be assessed clearly and remains to be identified
