Successful novel treatment of Schamberg’s purpura with colchicine
Schamberg’s disease is a rare condition that predominantlyinvolves the lower extremities. A caucasian female patient withrecurrent episodes of diffuse and painless purpuric skin eruptionsin the lower extremities is presented. A skin biopsy along with theclinical appearance made the diagnosis of Schamberg’s disease.She was treated with colchicine (0.5 mg twice a day) and within aweek showed complete disappearance of all purpuric eruptions.Colchicine was taken for the next 6 months and then discontinued.Three months later she was well without any recurrences
Time to revisit the definition and clinical criteria for anaphylaxis?
Anaphylaxis represents the severe end of the spectrum of allergic reactions. A number of different definitions for anaphylaxis are currently foundin the literature (Table 1).[1-6]Manydefine anaphylaxis as a life-threatening reaction. However, data from large case series and patient registries have demonstrated that despite the fact thatthe vast majority of anaphylaxisreactionsare not treated appropriately with prompt administration of epinephrine/adrenaline, ingeneral this does not result in increased mortality or morbidity(such as hospitalization);[7-9]this observation is also consistent with national epidemiological datafor food anaphylaxis, which indicate that fatal anaphylaxis is a rare (but unpredictable) event.[10-12]Therefore, the majority of anaphylaxis reactions cannot be described as life-threatening in themselves,althoughdue to our inability to predict severity of reaction, [12]we emphasise that all anaphylaxis must be appropriately treated with intramuscular epinephrine/adrenaline. Both the descriptions used by the Australasian Society of Clinical Immunology and Allergy (ASCIA)[4] and National Institute of Allergy and Infectious Disease (NIAID)[5] refer to anaphylaxis as a serious allergic reaction, and acknowledge the spectrum of severity in terms of identifying the potential for anaphylaxis to be life-threatening
CLASH: Precise New Constraints on the Mass Profile of Abell 2261
We precisely constrain the inner mass profile of Abell 2261 (z=0.225) for the
first time and determine this cluster is not "over-concentrated" as found
previously, implying a formation time in agreement with {\Lambda}CDM
expectations. These results are based on strong lensing analyses of new 16-band
HST imaging obtained as part of the Cluster Lensing and Supernova survey with
Hubble (CLASH). Combining this with revised weak lensing analyses of Subaru
wide field imaging with 5-band Subaru + KPNO photometry, we place tight new
constraints on the halo virial mass M_vir = 2.2\pm0.2\times10^15 M\odot/h70
(within r \approx 3 Mpc/h70) and concentration c = 6.2 \pm 0.3 when assuming a
spherical halo. This agrees broadly with average c(M,z) predictions from recent
{\Lambda}CDM simulations which span 5 <~ 8. Our most significant
systematic uncertainty is halo elongation along the line of sight. To estimate
this, we also derive a mass profile based on archival Chandra X-ray
observations and find it to be ~35% lower than our lensing-derived profile at
r2500 ~ 600 kpc. Agreement can be achieved by a halo elongated with a ~2:1 axis
ratio along our line of sight. For this elongated halo model, we find M_vir =
1.7\pm0.2\times10^15 M\odot/h70 and c_vir = 4.6\pm0.2, placing rough lower
limits on these values. The need for halo elongation can be partially obviated
by non-thermal pressure support and, perhaps entirely, by systematic errors in
the X-ray mass measurements. We estimate the effect of background structures
based on MMT/Hectospec spectroscopic redshifts and find these tend to lower
Mvir further by ~7% and increase cvir by ~5%.Comment: Submitted to the Astrophysical Journal. 19 pages, 14 figure
In-flight allergic emergencies
Allergic and hypersensitivity reactions such as anaphylaxis and asthma exacerbations may occur during air travel. Although the exact incidence of in-flight asthma and allergic emergencies is not known, we have concerns that this subject has not received the attention it warrants. There is a need to provide passengers at risk and airlines with the necessary measures to prevent and manage these emergencies. A review of the epidemiology, management and approaches to prevention of allergic and asthma emergencies during air travel is presented with the goal of increasing awareness about these important, potentially preventable medical events.
