    Transplacental Transmission of Leishmania infantum as a Means for Continued Disease Incidence in North America

    Dogs are a favored feeding source for sand flies that transmit human L. infantum infection. Zoonotic visceral leishmaniasis (ZVL) is an emerging problem in some U.S. dog breeds, with over 20% of at-risk Foxhounds infected. Although classically Leishmania is transmitted by infected sand flies which exist in the United States, no role has yet been determined for vector-borne transmission. Means of ongoing L. infantum transmission in U.S. dogs is unknown. Possibilities include transplacental and horizontal/venereal transmission. Aims for this study were to establish whether transplacental transmission occurred in Leishmania-infected U.S. dogs and determine the effect of this transmission on immune recognition of Leishmania. This novel report describes wide-spread infection as identified by kqPCR in 8 day-old pups born to a naturally-infected, seropositive U.S. dog with no travel history. This is the first report of transplacental transmission of L. infantum in naturally-infected dogs in North America. Evidence that mom-to-pup transmission of ZVL may continue disease in an otherwise non-endemic region has significant implications on current control strategies for ZVL. Determining frequency of vertical transmission and incorporating canine sterilization with vector control may have a more significant impact on ZVL transmission to people in endemic areas than current control efforts

    LeishVet guidelines for the practical management of canine leishmaniosis

    The LeishVet group has formed recommendations designed primarily to help the veterinary clinician in the management of canine leishmaniosis. The complexity of this zoonotic infection and the wide range of its clinical manifestations, from inapparent infection to severe disease, make the management of canine leishmaniosis challenging. The recommendations were constructed by combining a comprehensive review of evidence-based studies, extensive clinical experience and critical consensus opinion discussions. The guidelines presented here in a short version with graphical topic displays suggest standardized and rational approaches to the diagnosis, treatment, follow-up, control and prevention of canine leishmaniosis. A staging system that divides the disease into four stages is aimed at assisting the clinician in determining the appropriate therapy, forecasting prognosis, and implementing follow-up steps required for the management of the leishmaniosis patient

    One Health: The global challenge of epidemic and endemic leishmaniasis

    'One Health' proposes the unification of medical and veterinary sciences with the establishment of collaborative ventures in clinical care, surveillance and control of cross-species disease, education, and research into disease pathogenesis, diagnosis, therapy and vaccination. The concept encompasses the human population, domestic animals and wildlife, and the impact that environmental changes ('environmental health') such as global warming will have on these populations. Visceral leishmaniasis is a perfect example of a small companion animal disease for which prevention and control might abolish or decrease the suffering of canine and human patients, and which aligns well with the One Health approach. In this review we discuss how surveillance for leishmaniases is undertaken globally through the control of anthroponootic visceral leishmaniasis (AVL) and zoonotic visceral leishmaniasis (ZVL). The ZVL epidemic has been managed to date by the culling of infected dogs, treatment of human cases and control of the sandfly vector by insecticidal treatment of human homes and the canine reservoir. Recently, preventive vaccination of dogs in Brazil has led to reduction in the incidence of the canine and human disease. Vaccination permits greater dog owner compliance with control measures than a culling programme. Another advance in disease control in Africa is provided by a surveillance programme that combines remote satellite sensing, ecological modelling, vector surveillance and geo-spatial mapping of the distribution of vectors and of the animal-to-animal or animal-to-human pathogen transmission. This coordinated programme generates advisory notices and alerts on emerging infectious disease outbreaks that may impede or avoid the spreading of visceral leishmaniasis to new areas of the planet as a consequence of global warming

    Control of Visceral Leishmaniasis in Latin America—A Systematic Review

    Visceral leishmaniasis is a vector-borne disease characterized by fever, spleen and liver enlargement, and low blood cell counts. In the Americas VL is zoonotic, with domestic dogs as main animal reservoirs, and is caused by the intracellular parasite Leishmania infantum (syn. Leishmania chagasi). Humans acquire the infection through the bite of an infected sand fly. The disease is potentially lethal if untreated. VL is reported from Mexico to Argentina, with recent trends showing a rapid spread in Brazil. Control measures directed against the canine reservoir and insect vectors have been unsuccessful, and early detection and treatment of human cases remains as the most important strategy to reduce case fatality. Well-designed studies evaluating diagnosis, treatment, and prevention/control interventions are scarce. The available scientific evidence reasonably supports the use of rapid diagnostic tests for the diagnosis of human disease. Properly designed randomized controlled trials following good clinical practices are needed to inform drug policy. Routine control strategies against the canine reservoirs and insect vectors are based on weak and conflicting evidence, and vector control strategies and vaccine development should constitute research priorities

    Scoping review of indicators and methods of measurement used to evaluate the impact of dog population management interventions

    Background: Dogs are ubiquitous in human society and attempts to manage their populations are common to most countries. Managing dog populations is achieved through a range of interventions to suit the dog population dynamics and dog ownership characteristics of the location, with a number of potential impacts or goals in mind. Impact assessment provides the opportunity for interventions to identify areas of inefficiencies for improvement and build evidence of positive change. Methods: This scoping review collates 26 studies that have assessed the impacts of dog population management interventions. Results: It reports the use of 29 indicators of change under 8 categories of impact and describes variation in the methods used to measure these indicators. Conclusion: The relatively few published examples of impact assessment in dog population management suggest this field is in its infancy; however this review highlights those notable exceptions. By describing those indicators and methods of measurement that have been reported thus far, and apparent barriers to efficient assessment, this review aims to support and direct future impact assessment

    Effect of insecticide-impregnated dog collars on incidence of zoonotic visceral leishmaniasis in Iranian children: a matched-cluster randomised trial.

    BACKGROUND: Deltamethrin-impregnated dog collars reduce sandfly bite rates on dogs, and are effective in killing sandflies that attempt to feed. Because domestic dogs are the principal reservoir hosts of zoonotic visceral leishmaniasis, we tested whether community-wide application of dog collars could protect children against infection with Leishmania infantum, the parasite that causes the disease. METHODS: 18 villages were paired, matched by preintervention child prevalence of L infantum infection. Within pairs, villages were randomly assigned to either control or intervention. All domestic dogs in intervention villages were provided with collars for the transmission season. The main outcome measure was incidence of L infantum infection after 1 year measured by seroconversion. Secondary outcomes were leishmanin skin test (LST) conversion and seroconversion in dogs. FINDINGS: The seroconversion rate in children was 1.49% (17/1141) in the intervention villages and 2.41% (26/1078) in control villages (odds ratio 0.57, 95% CI 0.36-0.90, p=0.017). LST conversion was also lowered, but not significantly (odds ratio 0.66, 0.41-1.08, p=0.096). The seroconversion rate in dogs in intervention villages was also significantly reduced (0.46, 0.30-0.70, p=0.0003). INTERPRETATION: Community-wide application of deltamethrin-impregnated dog collars not only protects domestic dogs from L infantum infections, but might also reduce the risk of L infantum infection in children. These dog collars could have a role in control of visceral leishmaniasis and replace controversial dog culling programmes in some countries. However, the effectiveness of dog collars will depend on the importance of wild versus domestic canids as reservoir hosts of L infantum

    Comparison of Nomadic and non-Nomadic Lifestyles in Transmission of Visceral Leishaniasis

    Background and Objective: Epidemiology of Visceral Leishmaniasis is affected by combination quality and interaction of Parasite-Vector-Host and environmental conditions. So, disease cycle related and eco-social factors and environmental risk factors co-analyzing, help to understanding these interactions, prognosis and orientation in disease control and treatment. This study was done to determine the role of nomadic and non-nomadic lifestyle in transmission of Kala-Azar in the Northwest of Iran. Materials and Methods: In this case-control study, firstly the prevalence of Kala-Azar among people living in selected villages were determined by both serological test (Direct Agglutination Test: DAT) and immunological test (Montenegro Skin Test: MST) in the Northwest of Iran, on the way of Shahsavan tribe travelling in summer/winter quarters villages. Then DAT was conducted on the dogs presenting in those villages during 2006. One year later Seroconversion rate was calculated through collection of the individual’s negative sera and re-analyzing them via DAT. Finally, occurrence of Visceral Leishmaniasis in relation with various involving factors like dog density/abundance and nomadic and non-nomadic lifestyle using Chi-Square test were determined. Results: Both MST and DAT were significantly higher in the nomadic lifestyle than in the non-nomadic lifestyle (P<0.05). Three values of prevalence (5.5%), seropositivity (2.7%) and seroconversion (2.5%) were higher in nomads than non-nomads. The GIS studies and electronically prepared maps showed that the endemicity and the infection rate are higher in nomads than non-nomads. There were a negative correlation between general distribution of Visceral Leishmaniasis in relation with environmental conditions altitude, mean temperature and rainfall. Conclusion: This study indicated that Nomadic lifestyle can play as a risk factor in transmission of Visceral Leishmaniasis due to nomads/dog contacting, their entering in the wild cycle of disease and travelling
