Clinical investigation of an outbreak of alveolitis and asthma in a car engine manufacturing plant
Background Exposure to metal working fluid (MWF) has been associated with outbreaks of EAA in the US, with bacterial contamination of MWF being a possible cause, but was uncommon in the UK. Twelve workers developed extrinsic allergic alveolitis (EAA) in a car engine manufacturing plant in the UK, presenting clinically between December 2003 and May 2004. This paper reports the subsequent epidemiological investigation of the whole workforce. This had three aims:-
âą To measure the extent of the outbreak by identifying other workers who may have developed EAA or other work-related respiratory diseases.
âą To provide case-detection so that those affected can be treated.
âą To provide epidemiological data to identify the cause of the outbreak.
Methods The outbreak was investigated in a three-phase cross-sectional survey of the workforce.
Phase I A respiratory screening questionnaire was completed by 808/836 workers (96.7%) in May 2004.
Phase II 481 employees with at least one respiratory symptom on screening and 50 asymptomatic controls were invited for investigation at the factory in June 2004. This included a questionnaire, spirometry and clinical opinion. 454/481(94.4%) responded along with 48/50(96%) controls. Workers were identified who needed further investigation and serial measurements of peak expiratory flow (PEF).
Phase III 162 employees were seen at the Birmingham Occupational Lung Disease clinic. 198 employees returned PEF records, including 141 of the 162 who attended for clinical investigation. Case definitions for diagnoses were agreed.
Results 87 workers (10.4% of workforce) met case definitions for occupational lung disease, comprising EAA(19), occupational asthma(74) and humidifier fever(7). 12 workers had more than one diagnosis. The peak onset of work-related breathlessness was Spring 2003. The proportion of workers affected was higher for those using metal working fluid (MWF) from a large sump(27.3%) compared with working all over the manufacturing area (7.9%) (OR=4.39,p<0.001). Two workers had positive specific provocation tests to the used but not the unused MWF solution.
Conclusions Extensive investigation of the outbreak of EAA detected a large number of affected workers, not only with EAA but also occupational asthma. This is the largest reported outbreak in Europe. Mist from used MWF is the likely cause. In workplaces using MWF, there is a need to carry out risk assessments, to monitor and maintain fluid quality, to control mist and to carry out respiratory health surveillance

Abstract
BACKGROUND: Lamivudine therapy is effective for chronic hepatitis B infection in adults. We evaluated the efficacy and tolerability of lamivudine as a treatment for chronic infection with hepatitis B virus (HBV) in children.
METHODS: Children with chronic hepatitis B were randomly assigned in a 2:1 ratio to receive either oral lamivudine (3 mg per kilogram of body weight; maximum, 100 mg) or placebo once daily for 52 weeks. The primary end point was virologic response (defined by the absence of serum hepatitis B e antigen and serum HBV DNA) at week 52 of treatment.
RESULTS: Of the 403 children screened, 191 were randomly assigned to receive lamivudine and 97 to receive placebo. The rate of virologic response at week 52 was higher among children who received lamivudine than among those who received placebo (23 percent vs. 13 percent, P=0.04). Lamivudine therapy was well tolerated and was also associated with higher rates of seroconversion from hepatitis B e antigen to hepatitis B e antibody, normalization of alanine aminotransferase levels, and suppression of HBV DNA.
CONCLUSIONS: In children with chronic hepatitis B, 52 weeks of treatment with lamivudine was associated with a significantly higher rate of virologic response than was placebo.

Equation of State of Oscillating Brans-Dicke Scalar and Extra Dimensions
We consider a Brans-Dicke scalar field stabilized by a general power law
potential with power index at a finite equilibrium value. Redshifting
matter induces oscillations of the scalar field around its equilibrium due to
the scalar field coupling to the trace of the energy momentum tensor. If the
stabilizing potential is sufficiently steep these high frequency oscillations
are consistent with observational and experimental constraints for arbitrary
value of the Brans-Dicke parameter . We study analytically and
numerically the equation of state of these high frequency oscillations in terms
of the parameters and and find the corresponding evolution of the
universe scale factor. We find that the equation of state parameter can be
negative and less than -1 but it is not related to the evolution of the scale
factor in the usual way. Nevertheless, accelerating expansion is found for a
certain parameter range. Our analysis applies also to oscillations of the size
of extra dimensions (the radion field) around an equilibrium value. This
duality between self-coupled Brans-Dicke and radion dynamics is applicable for

Search for CP violation in D0 and D+ decays
A high statistics sample of photoproduced charm particles from the FOCUS
(E831) experiment at Fermilab has been used to search for CP violation in the
Cabibbo suppressed decay modes D+ to K-K+pi+, D0 to K-K+ and D0 to pi-pi+. We
have measured the following CP asymmetry parameters: A_CP(K-K+pi+) = +0.006 +/-
0.011 +/- 0.005, A_CP(K-K+) = -0.001 +/- 0.022 +/- 0.015 and A_CP(pi-pi+) =
+0.048 +/- 0.039 +/- 0.025 where the first error is statistical and the second
error is systematic. These asymmetries are consistent with zero with smaller
errors than previous measurements.Comment: 12 pages, 4 figure
A measurement of lifetime differences in the neutral D-meson system
Using a high statistics sample of photoproduced charm particles from the
FOCUS experiment at Fermilab, we compare the lifetimes of neutral D mesons
decaying via D0 to K- pi+ and K- K+ to measure the lifetime differences between
CP even and CP odd final states. These measurements bear on the phenomenology
of D0 - D0bar mixing. If the D0 to K-pi+ is an equal mixture of CP even and CP
odd eigenstates, we measure yCP = 0.0342 \pm 0.0139 \pm 0.0074.Comment: 15 pages, 5 figure
Measurements of the Sigma_c^0 and Sigma_c^{++} Mass Splittings
Using a high statistics sample of photoproduced charmed particles from the
FOCUS experiment at Fermilab (FNAL-E831), we measure the mass splittings of the
charmed baryons Sigma_c^0 and Sigma_c^{++}. We find M(Sigma_c^0 - Lambda_c^+) =
167.38 +/- 0.21 +/- 0.13 MeV/c^2 and M(Sigma_c^++ - Lambda_c^+) = 167.35 +/-
0.19 +/- 0.12 MeV/c^2 with samples of 362 +/- 36 and 461 +/- 39 events,
respectively. We measure the isospin mass splitting M(Sigma_c^++ - Sigma_c^0)
to be -0.03 +/- 0.28 +/- 0.11 Mev/c^2. The first errors are statistical and the

The role and importance of gene polymorphisms in the development of atherosclerosis
The development of atherosclerosis is a multifactorial process. The purpose of the study was to examine three genetic polymorphisms playing a role in the metabolic processes underlying the disease. We compared the data of 348 atherosclerotic non-diabetic patients with 260 atherosclerotic diabetic patients and 384 healthy controls. We analyzed the prevalence of myocardial infarction and stroke in three different groups of patients carrying different polymorphisms. It was proved that if the mutant TT eNOS Glu298ASP variant is present, a significantly higher number of myocardial infarctions can be observed than in patients carrying heterozygote GT or normal GG genotype. We proved that in the case of MTHFR 677CT heterozygote variants, the occurrence of myocardial infarction is significantly higher and the difference is also significant in case of the 677TT homozygote variant. It was verified that among patients with the mutant TNF-α AA genotype the occurrence of cardiovascular events was significantly higher. Screening the genetically high risk groups on the long run should be considered as an early detection opportunity that may give better chances for prevention and treatment. Understanding the inflammatory mechanisms of the atherosclerosis may give new therapeutical targets to pharmacologists
