    The impact of working memory load on task execution and online plan adjustment during multitasking in a virtual environment

    Three experiments investigated the impact of working memory load on online plan adjustment during a test of multitasking in young, nonexpert, adult participants. Multitasking was assessed using the Edinburgh Virtual Errands Test (EVET). Participants were asked to memorize either good or poor plans for performing multiple errands and were assessed both on task completion and on the extent to which they modified their plans during EVET performance. EVET was performed twice, with and without a secondary task loading a component of working memory. In Experiment 1, articulatory suppression was used to load the phonological loop. In Experiment 2, oral random generation was used to load executive functions. In Experiment 3, spatial working memory was loaded with an auditory spatial localization task. EVET performance for both good- and poor-planning groups was disrupted by random generation and sound localization, but not by articulatory suppression. Additionally, people given a poor plan were able to overcome this initial disadvantage by modifying their plans online. It was concluded that, in addition to executive functions, multiple errands performance draws heavily on spatial, but not verbal, working memory resources but can be successfully completed on the basis of modifying plans online, despite a secondary task load

    Estimating the contribution of subclinical tuberculosis disease to transmission: an individual patient data analysis from prevalence surveys

    Background: Individuals with bacteriologically confirmed pulmonary tuberculosis (TB) disease who do not report symptoms (subclinical TB) represent around half of all prevalent cases of TB, yet their contribution to Mycobacterium tuberculosis (Mtb) transmission is unknown, especially compared to individuals who report symptoms at the time of diagnosis (clinical TB). Relative infectiousness can be approximated by cumulative infections in household contacts, but such data are rare. Methods: We reviewed the literature to identify studies where surveys of Mtb infection were linked to population surveys of TB disease. We collated individual-level data on representative populations for analysis and used literature on the relative durations of subclinical and clinical TB to estimate relative infectiousness through a cumulative hazard model, accounting for sputum-smear status. Relative prevalence of subclinical and clinical disease in high-burden settings was used to estimate the contribution of subclinical TB to global Mtb transmission. Results: We collated data on 414 index cases and 789 household contacts from three prevalence surveys (Bangladesh, the Philippines, and Viet Nam) and one case-finding trial in Viet Nam. The odds ratio for infection in a household with a clinical versus subclinical index case (irrespective of sputum smear status) was 1.2 (0.6–2.3, 95% confidence interval). Adjusting for duration of disease, we found a per-unit-time infectiousness of subclinical TB relative to clinical TB of 1.93 (0.62–6.18, 95% prediction interval [PrI]). Fourteen countries across Asia and Africa provided data on relative prevalence of subclinical and clinical TB, suggesting an estimated 68% (27–92%, 95% PrI) of global transmission is from subclinical TB. Conclusions: Our results suggest that subclinical TB contributes substantially to transmission and needs to be diagnosed and treated for effective progress towards TB elimination

The conformation about the C=N double bond [1.268 (3) angstrom] is E. An intramolecular N-H center dot center dot center dot N hydrogen bond occurs. Linear supramolecular chains along the b axis mediated by O-H center dot center dot center dot O hydrogen-bonding interactions feature in the crystal structure. These chains are also stabilized by weak C-H center dot center dot center dot N contacts

    Salivary flow rates measured during radiation therapy in head and neck cancer patients: A pilot study assessing salivary sediment formation

    Statement of problem: Xerostomia often occurs in patients being managed for head and neck cancer who receive radiation therapy. Although accurate salivary sampling can be therapeutically important to measure during radiation, sampling errors can occur because of salivary sediments. Determining the impact that salivary sediments have on measured salivary flow rates during radiation is important for management of patients. Purpose: The purpose of this study was to assess the magnitude of error associated with the inclusion of nonsalivary components (sediment) in the calculation of whole stimulated saliva flow rates prior to and during radiation therapy (SS and SSR) in patients with head and neck cancer. Material and methods: Whole paraffin-stimulated saliva was collected in large-mouth centrifuge tubes from 20 patients with head and neck cancer prior to and during the third week of radiation therapy. Gravimetric methods were used to calculate the flow rates at g/5 min. After centrifugation, supernatant saliva was removed and the sediment was oven-dried to remove residual moisture. Sediment weight was subtracted from the original weight of saliva specimens and flow rates were recalculated. Means and standard deviations were determined and flow rate differences before (BC) and after (AC) sediment correction were evaluated statistically with the paired t test (α=.05). A nonparametric analysis of the flow rate data with the Wilcoxon matched-pairs signed-ranks test was also used to examine the magnitude and direction of the intrapair (BC-AC) differences (α=.05). Results: On average, salivary sediment contributed less than 1% of the total uncorrected weight of saliva prior to radiation therapy. In specimens collected during radiation therapy, sediment contributed an average of 14% of the total uncorrected weight and as high as 95.4% in 1 patient. Sediment percentages were 20% and higher in 4 patients. In the Wilcoxon analysis, 19 out of 20 paired BC and AC flow rates were higher in the BC group in the SS and SSR samples. Conclusions: The error associated with the inclusion of salivary sediment in the calculation of saliva flow rates prior to radiation treatment was small, but statistically significant. The magnitude of the sediment effect was more pronounced in specimens taken during radiotherapy and was significant, as determined by the Wilcoxon test, but the mean paired differences were not significantly different according to the t test.
