    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    La thĂšse explore un projet de conception d’une grande bibliothĂšque dans une mĂ©tropole française rĂ©cemment crĂ©Ă©e, appelĂ©e Ă  ĂȘtre un Ă©quipement culturel phare Ă  l’échelle de l’agglomĂ©ration. Les enjeux de cette grande bibliothĂšque censĂ©e ĂȘtre « porteuse de sens et d’avenir pour les habitants » sont donc trĂšs importants pour les Ă©lus Ă  divers titres (dynamisme de la politique culturelle, image de la ville, rayonnement de la mĂ©tropole, etc.). La recherche se penche plus prĂ©cisĂ©ment sur l’activitĂ© des bibliothĂ©caires occupĂ©s Ă  travailler sur cette bibliothĂšque, « en mode projet ». Ma problĂ©matique dĂ©rive d'un Ă©tonnement :on pourrait s’attendre Ă  ce que la conception d’un tel Ă©quipement soit pour les professionnels un moment d’effervescence, d’ébullition, de confrontation de visions, d’expĂ©rimentation et d’inventivitĂ©. Or, trois ans aprĂšs la prise de poste de la cheffe de projet et le dĂ©but de mon enquĂȘte, il semble bien que la bibliothĂšque qui se dessine ressemble de prĂšs Ă  nombre d'autres grands Ă©tablissements crĂ©Ă©s en France ces derniĂšres annĂ©es (par l'architecture, les services, etc.). « En fait, je crois que la commande des politiques c’était d’avoir une bibliothĂšque », constate Ă  regret une bibliothĂ©caire responsable d’un groupe de travail [dĂ©but 2020]. Cette surprenante remarque dit bien l'Ă©cart entre l'ouverture que semblait promettre la perspective du projet et le caractĂšre ordinaire de que l'on va faire :finalement, il ne s'agissait que de concevoir une bibliothĂšque. Cette thĂšse cherche Ă  comprendre comment le processus d’imagination de cette grande bibliothĂšque, menĂ© en « mode projet » suivant un dispositif managĂ©rial qui imprĂšgne aujourd’hui fortement le secteur culturel, est une dĂ©marche qui tend Ă  altĂ©rer l’inventivitĂ©, l’imagination et la recherche concernant le futur lieu. Il faut souligner qu’ « altĂ©rer » ne signifie pas interdire ou dĂ©truire :je cherche plutĂŽt Ă  montrer que le mode projet fait du projet autre chose qu'une dĂ©marche centrĂ©e sur la conception d'un lieu « porteur de sens et d’avenir pour les habitants ». En m’inspirant de l’approche de la sociologie pragmatique, je m’efforcerai de saisir le projet Ă  travers la circulation des modĂšles qui concourent Ă  son cadrage et l’emprise de la managĂ©rialisation, tout en restant attentive aux interstices et dĂ©bordements par oĂč se donnent Ă  voir d’autres dĂ©sirs et d’autres prises sur cette grande bibliothĂšque.The thesis explores a project for the design of a large library in a recently created French metropolis, which is intended to be a flagship cultural facility for the agglomeration. The stakes of this large library, which is supposed to be 'meaningful and forward-looking for the inhabitants', are therefore very important for the elected officials in various ways (dynamism of the cultural policy, image of the city, influence of the metropolis, etc.). The research focuses more specifically on the activity of librarians working on this library, "in project mode". My problematic stems from a surprise: one might expect the design of such a facility to be a moment of effervescence, ebullience, confrontation of visions, experimentation and inventiveness for the professionals. Now, three years after the project manager took up her post and my investigation began, it seems that the library that is taking shape closely resembles many other large establishments created in France in recent years (in terms of architecture, services, etc.). "In fact, I think that the politicians' order was to have a library," says a librarian in charge of a working group [in early 2020]. This surprising remark highlights the gap between the openness that the project's perspective seemed to promise and the ordinary nature of what is going to be done: in the end, it was just a matter of designing a library. This thesis seeks to understand how the process of imagining this great library, carried out in 'project mode' following a managerial device that today strongly permeates the cultural sector, is a process that tends to alter inventiveness, imagination and research concerning the future place. It should be said that 'altering' does not mean prohibiting or destroying: rather, I seek to show that the project mode makes the project something other than a process centered on the design of a place that 'carries meaning and a future for the inhabitants'. Inspired by the approach of pragmatic sociology, I will try to grasp the project through the circulation of models that contribute to its framing and the hold of managerialization, while remaining attentive to the interstices and overflows through which other desires and other holds on this great library are revealed.Doctorat en Sciences politiques et socialesinfo:eu-repo/semantics/nonPublishe
