    CRISPR Content Correlates with the Pathogenic Potential of Escherichia coli

    Guide RNA molecules (crRNA) produced from clustered regularly interspaced short palindromic repeat (CRISPR) arrays, altogether with effector proteins (Cas) encoded by cognate cas (CRISPR associated) genes, mount an interference mechanism (CRISPR-Cas) that limits acquisition of foreign DNA in Bacteria and Archaea. The specificity of this action is provided by the repeat intervening spacer carried in the crRNA, which upon hybridization with complementary sequences enables their degradation by a Cas endonuclease. Moreover, CRISPR arrays are dynamic landscapes that may gain new spacers from infecting elements or lose them for example during genome replication. Thus, the spacer content of a strain determines the diversity of sequences that can be targeted by the corresponding CRISPR-Cas system reflecting its functionality. Most Escherichia coli strains possess either type I-E or I-F CRISPR-Cas systems. To evaluate their impact on the pathogenicity of the species, we inferred the pathotype and pathogenic potential of 126 strains of this and other closely related species and analyzed their repeat content. Our results revealed a negative correlation between the number of I-E CRISPR units in this system and the presence of pathogenicity traits: the median number of repeats was 2.5-fold higher for commensal isolates (with 29.5 units, range 0–53) than for pathogenic ones (12.0, range 0–42). Moreover, the higher the number of virulence factors within a strain, the lower the repeat content. Additionally, pathogenic strains of distinct ecological niches (i.e., intestinal or extraintestinal) differ in repeat counts. Presentamos el caso de un paciente con trombopenia que sufrió un traumatismo banal. Necesitó fasciotomía del compartimiento anteroexterno de la pierna y, más tarde, esplenectomía. El diagnóstico y tratamiento tardíos de esta complicación, así como una descomprensión inadecuada, pueden conducir a la pérdida de función en una extremidad.The compartment syndrome is a well described cli- nical entity that results from increased pressure within a myo- fascial compartment.     Drag reduction on a blunt body by self-adaption of rear flexibly hinged flaps

    We study the aerodynamics of a blunt-based body with rear flexibly-hinged rigid flaps, subject to a turbulent flow of Reynolds number Re = 12000, under aligned and cross flow conditions with yaw angle β = 0◦ and β = 4◦. To that aim, different values of the equivalent torsional stiffness are considered, to cover the range of reduced velocity U∗ = (0, 3.48] in water tank experiments. The effect of the angular deflection of plates on the drag and near wake flow is analyzed, experimentally and numerically. The results show that, in the range of U∗ herein considered, the plates undergo an inwards quasi-static, self-adaptive deflection, which is symmetric for yaw angles β = 0◦ and asymmetric for β = 4◦. In particular, the plates feature small mean deformation angles for values of U∗ < 1, whereas a sharp and monotonic increase of such deflection occurs for U∗ > 1, i.e. for lower values of the hinge’s stiffness, with an asymptotic trend towards the larger values of U∗. A critical value of reduced velocity of U∗ ≃ 0.96 is obtained as the instability threshold above which plates depart from their initial equilibrium position. The progressive streamlining of the trailing edge translates into significant reductions of the associated mean drag coefficients. Thus, reductions close to 19% with respect to reference static plates configurations are obtained for the most flexible case of U∗ = 3.48 for both β = 0◦ and β = 4◦. A close inspection of the near wake reveals that the inwards progressive mean displacement of the plates yields a reduction in the recirculation bubble size. A symmetric evolution of the recirculating bubble is observed for β = 0◦, whereas the bubble becomes asymmetric for β = 4◦, with a larger leeward clockwise vortex. In both cases, the drag coefficient is shown to vary linearly with the global aspect ratio of the recirculating bubble. The analysis of the numerical results shows that the reduced extension of the recirculating bubble significantly alters the formation length and intensity of the eddies size and associated pressure. It is observed that despite the local pressure decrease in the vortices shed from the trailing edges, the plates self adaption reduces their size and prevents the eddies from entering the cavity, thus, creating a dead flow region with a consequent pressure increase at the body base.Junta de Andalucia FEDER-UJA 1262764Universidad de JaenEuropean CommissionSpanish MCIN/AEI PDC2021-121288-I00European Union Next Generation EU/PRT

    Clinical prediction rules for adverse evolution in patients with COVID-19 by the Omicron variant

    Objective: We identify factors related to SARS-CoV-2 infection linked to hospitalization, ICU admission, and mortality and develop clinical prediction rules. Methods: Retrospective cohort study of 380,081 patients with SARS-CoV-2 infection from March 1, 2020 to January 9, 2022, including a subsample of 46,402 patients who attended Emergency Departments (EDs) having data on vital signs. For derivation and external validation of the prediction rule, two different periods were considered: before and after emergence of the Omicron variant, respectively. Data collected included sociodemographic data, COVID-19 vaccination status, baseline comorbidities and treatments, other background data and vital signs at triage at EDs. The predictive models for the EDs and the whole samples were developed using multivariate logistic regression models using Lasso penalization. Results: In the multivariable models, common predictive factors of death among EDs patients were greater age; being male; having no vaccination, dementia; heart failure; liver and kidney disease; hemiplegia or paraplegia; coagulopathy; interstitial pulmonary disease; malignant tumors; use chronic systemic use of steroids, higher temperature, low O2 saturation and altered blood pressure-heart rate. The predictors of an adverse evolution were the same, with the exception of liver disease and the inclusion of cystic fibrosis. Similar predictors were found to be related to hospital admission, including liver disease, arterial hypertension, and basal prescription of immunosuppressants. Similarly, models for the whole sample, without vital signs, are presented. Conclusions: We propose risk scales, based on basic information, easily-calculable, high-predictive that also function with the current Omicron variant and may help manage such patients in primary, emergency, and hospital care.     Care dependency in patients with heart failure: A cross-sectional study in Spain

    Background: Heart failure (HF) is a progressive and debilitating condition that represents an ever-growing problem for health systems worldwide. HF patients feel that they are a burden on their families, they feel socially isolated and have a low perception of their health. Accordingly, the objectives of this study were to: (1) to explore the profile of care dependency in a representative sample of Spanish HF patients through the Care Dependency Scale (CDS), and (2) to identify correlates of care dependency in this population. Material and Methods: We performed a cross-sectional study of 187 patients admitted for HF decompensation to the Hospital Clínico of Zaragoza (Spain). Results: Only 15% of our sample was highly or completely dependent on care from others. More specifically, our results indicate that HF patients felt a greater level of dependency on care from others when it comes to moving, getting dressed and undressed, maintaining good personal hygiene, participating in daily and recreational activities and being continent. Conclusions: We find association between the CDS categories that present a low score for care dependency in HF patients and the patients’ physical deterioration
