    En los últimos años, el número de brotes hospitalarios causados por Klebsiella pneumoniae resistente a carbapenems ha aumentado drásticamente. Dichas infecciones están asociadas a una mayor estancia hospitalaria, a un incremento de los costes sociosanitarios y a un aumento de la mortalidad, que en algunos casos es de hasta el 50%. El objetivo principal de nuestro estudio consiste en describir la epidemiología y las características demográficas de los pacientes infectados por Klebsiella pneumoniae resistente a carbapenems en los pacientes intervenidos de cirugía cardíaca con circulación extracorpórea y cirugía abdominal en el Hospital Clínico Universitario de Valladolid. Estudio observacional, retrospectivo de cohortes de pacientes intervenidos de cirugía cardíaca con circulación extracorpórea y cirugía abdominal entre octubre de 2015 y septiembre de 2016, en el Hospital Clínico Universitario de Valladolid, España. Se identificaron 28 casos de Klebsiella pneumoniae resistente a carbapenems. La edad media fue de 68,6 9,73 años, con predominio del sexo masculino (67,9 %). Se pudo observar un claro aumento de la incidencia en pacientes intervenidos de cirugía cardíaca con circulación extracorpórea (82,1%), pacientes con puntuaciones SOFA elevadas (10,32 5,06), ventilación mecánica prolongada (11,7 13,09 días) y reintervenidos (32,1%).     La crisis económica ha diezmado las arcas públicas, provocando que se aumenten los esfuerzos para conseguir ingresos. Para ello, se intenta eliminar la existencia de capital no controlado por la Hacienda pública, a través de una legislación cada vez más expansiva. En materia de blanqueo de capitales también se ha vivido esta expansión, quedando incriminadas conductas con una clara vocación delictiva pero también otras que se realizan dentro de los estándares sociales, las conocidas como actos neutrales. Analizaremos la posible subsunción de tales acciones en el tipo del delito de blanqueo de capitales, mediante el estudio del debate doctrinal y jurisprudencial, para fijar los límites de la intervención del derecho penal respecto de las conductas socialmente adecuadasThe economic crisis has decimated the public coffers, producing an increase in efforts to generate revenue.     Attractive regular stochastic chains: perfect simulation and phase transition

    We prove that uniqueness of the stationary chain, or equivalently, of the gg-measure, compatible with an attractive regular probability kernel is equivalent to either one of the following two assertions for this chain: (1) it is a finitary coding of an i.i.d. process with countable alphabet, (2) the concentration of measure holds at exponential rate. We show in particular that if a stationary chain is uniquely defined by a kernel that is continuous and attractive, then this chain can be sampled using a coupling-from-the-past algorithm. For the original Bramson-Kalikow model we further prove that there exists a unique compatible chain if and only if the chain is a finitary coding of a finite alphabet i.i.d. process.     Patients with altered skin immunity, such as individuals with atopic dermatitis (AD), can have a life-threatening disruption of the epidermis known as eczema vaccinatum after vaccinia virus (VV) infection of the skin. Here, we sought to better understand the mechanism(s) by which VV associates with keratinocytes. The class A scavenger receptor known as MARCO (macrophage receptor with collagenous structure) is expressed on human and mouse keratinocytes and found to be abundantly expressed in the skin of patients with AD. VV bound directly to MARCO, and overexpression of MARCO increased susceptibility to VV infection. Furthermore, ligands with affinity for MARCO, or excess soluble MARCO, competitively inhibited VV infection. These findings indicate that MARCO promotes VV infection and highlights potential new therapeutic strategies for prevention of VV infection in the skin

    [Extract] Telemedicine is a technology that has come of age, now it must be put to use to address inequities in cancer care. The principles and applications of telemedicine have undergone rapid evolution over the last two years globally.1, 2 Prior to the COVID-19 pandemic, the utilisation of telemedicine was largely driven by local champions and lacked systematic uptake despite Government investment and incentives at both state and commonwealth levels. The pandemic and the resulting need for social distancing saw rapid and widespread adoption of telephone and telehealth services aided by Government funding, even for patients in metropolitan areas. Now is the time to leverage this momentum to embed telemedicine in our health systems
