    Ce mémoire étudie un corpus de compositions musico-théâtrales ayant abouti à la création de l’oeuvre Naissances. Cet opéra contemporain pour ensemble, électroniques et conception d’éclairage a été nourri de nombreuses oeuvres ayant servi d’études préalables aux thématiques, techniques et concepts abordés dans Naissances. Les différentes pièces comportent une importante part d’interdisciplinarité par la théâtralité qui leur est intrinsèque. Chacune des compositions a mis en lumière des aspects de la composition musicale faisant appel au théâtre (et inversement), traçant les contours encore flous des paradigmes de la création sonore interdisciplinaire. S’inspirant des diverses figures de proue du théâtre moderne et contemporain, le présent mémoire questionne la filiation de mes compositions avec les pratiques actuelles, et les potentielles avenues rendues possibles par mon approche de la création musico-théâtrale.This thesis studies a corpus of music-theatrical compositions that led to the creation of the work Naissances. This contemporary opera for ensemble, electronics and lighting design was nourished by numerous works that served as preliminary studies for the themes, techniques and concepts addressed in Naissances. The different pieces have an important interdisciplinary aspect due to the theatricality that is intrinsic to them. Each of the compositions has highlighted aspects of musical composition that call upon the theater (and vice versa), tracing the still blurred contours of the paradigms of interdisciplinary sound creation. Inspired by the various key figures of modern and contemporary theater, the present thesis questions the filiation of my compositions with current practices, and the potential avenues made possible by my approach to music-theatre creation

    Umbilical artery velocity and adverse neonatal outcome in sudanese pregnant women with severe preeclampsia

    Background: Doppler umbilical artery waveform is an important imaging predictor of adverse neonatal outcome in patients with severe preeclampsia. The present study aims to evaluate the adverse neonatal outcome associated with abnormal umbilical artery velocity in patients with severe preeclampsia.Methods: In a prospective descriptive analytical hospital-based study, umbilical artery velocity and adverse fetal outcome of (104) patients with severe preeclampsia were evaluated by Doppler ultrasound. The study was conducted through the period (February to August 2015) in obstetric Sudanese population.Results: The umbilical artery end diastolic flow was reduced in 20.2 %, absent in 5.8%, reversed in 1.9% and normal in 72.1% of the study population. The end diastolic velocity was significantly abnormal and associated with low birth weight (P-value=0.001), early neonatal death (P-value= 0.00), fetal respiratory distress (P-value = 0.00) and Apgar score (P-value = 0.00). The abnormal end diastolic flow was not statistically associated with emergency caesarean delivery (P value=0.275).Conclusions: The end diastolic flow velocity of the umbilical artery was significantly associated with adverse neonatal and perinatal outcomes in Sudanese pregnant cases with severe preeclampsia.

    Flexible Labor Contracts, Firm-specific Pay, and Wages

    In this paper, we use comprehensive employer-employee data for the Netherlands to investigate the labor income effects of flexible labor contracts in two different settings: wage determination as in the AKM model, and an analysis of earnings losses after job displacement. In both settings, we find that flexible contracts lead to lower wages, but that workers with flexible contracts primarily earn less because they work at or join lower paying firms. This implies that the negative effects of flexible contracts on wage income are overstated, if firm-specific pay differentials are not taken into account

    A randomised control trial protocol of MuST for vascular access cannulation in hemodialysis patients (MuST Study): contributions for a safe nursing intervention

    Background: The vascular access preservation and the maintenance of a complication-free fistula remains an Achilles’heel of hemodialysis in chronic kidney patients due to its substantial contribution to the morbidity and mortality. Systematic studies in the area of examining cannulation practices, achieving complication-free cannulation, and strategies to improve fistula survival are needed. For this reason, we consider it essential to create and investigate new methodologies for approaching fistula in patients on regular HD. The Multiple Single Cannulation Technique (MuST) is based on the association between the rope-ladder (RL) using the arteriovenous vessel through progressive rotation, and the buttonhole (BH) since there are three specific cannulation sites for each cannulation day during the week. The MuST is simple to implement and seems to be a very promising technique in terms of patient safety. Previous studies already showed an arteriovenous fistula survival similar to RL but significantly higher than BH. Methods: This MuST study is a multicenter, prospective, non-blind, parallel-group, randomized controlled trial with the intervention group submitted to MuST and a control group undergoing the rope-ladder, up to 100 subjects for each group. Patients will be randomized 1:1 to one of two cannulation technique (CT), and the follow-up period of this study will be 12 months. Primary outcome is to evaluate the arteriovenous fistula survival rate at 12 months determined by the percentage of fistulas in use from the beginning of the study to the date of the first clinical intervention by angioplasty or vascular surgery, to maintain or restore patency (unassisted patency). Secondary outcome is to evaluate arteriovenous fistula survival rate at 12 month determined by the percentage of fistulas in use from the study start to the date of access abandonment due to dysfunction, patient abandonment, or death, treatment change modality or study end. We will also evaluate the assisted primary patency and include the following secondary outcomes associated with the cannulation technique: Infection, Hematoma, Aneurysm development, and pain. Discussion: The study will investigate whether fistula survival can be improved when using cannulation by MuST compared to the RL. MuST study will provide important information on fistula survival when cannulated by MuST but also information related to its use in fistulas previously cannulated by other CTs.info:eu-repo/semantics/publishedVersio
