    Simulations of ionospheric turbulence produced by HF heating near the upper hybrid layer

    Heating of the ionosphere by high-frequency (HF), ordinary (O) mode electromagnetic waves can excite magnetic field aligned density striations (FAS), associated with upper and lower hybrid turbulence and electron heating. We have used Vlasov simulations in one spatial and two velocity dimensions to study the induced turbulence in the presence of striations when the O-mode pump is mode converted to large amplitude upper hybrid oscillations trapped in a striation. Parametric processes give rise to upper and lower hybrid turbulence, as well as to large amplitude, short wavelength electron Bernstein waves. The latter excite stochastic electron heating when their amplitudes exceed a threshold for stochasticity, leading to a rapid increase of the electron temperature by several thousands of Kelvin. The results have relevance for high latitude heating experiments

    Randomized controlled trial of laparoscopic anterior 180° partial versus posterior 270° partial fundoplication.

This item is under embargo for a period of 12 months from the date of publication, in accordance with the publisher's policy.BACKGROUND: Previous trials show good outcomes following anterior and posterior partial versus Nissen fundoplication for gastro-oesophageal reflux. However, it is unclear which partial fundoplication performs best. This study compared anterior 180° versus posterior 270° fundoplication. METHODS: At three hospitals, patients were randomized to anterior 180° versus posterior 270° partial fundoplication, and clinical outcomes were determined using a structured questionnaire at 3, 6 and 12 months. Heartburn, dysphagia and satisfaction were assessed using 0-10 analoue scales, and adverse outcomes and side effects were determined. Endoscopy, manometry and pH monitoring were performed 6 months after surgery. RESULTS: Forty-seven patients were randomized to anterior (n = 23) versus posterior (n = 24) fundoplication. Clinical outcomes for 93-98% of patients were available at each follow-up point. At 12 months, the mean heartburn score was higher following anterior fundoplication (2.7 versus 0.8, P = 0.045), although differences were not significant at earlier follow-up. Conversely, following posterior fundoplication, patients were less able to belch at 3 (56% versus 16%, P = 0.013) and 6 months (43% versus 9%, P = 0.017). No significant differences were demonstrated for dysphagia. Both groups had high rates of satisfaction with the outcome - 85% versus 86% satisfied at 12 months follow-up. CONCLUSION: Both partial fundoplications are effective treatments for gastro-oesophageal reflux. Posterior partial fundoplication is associated with less reflux symptoms offset by more side effects.Australian National Health & Medical Research Council

    Optical performance of neodymium nanoparticles doped tellurite glasses

    A series of neodymium NPs doped tellurite glass with composition of (TeO2)0.7(B2O3)0.3]0.7 (ZnO)0.3}1- x (Nd2O3 NPs) x (x = 0.005, 0.01, 0.02, 0.03, 0.04 and 0.05 mol%) was fabricated via melt-quenching method. Density and molar volume were measured and analyzed for the glass series. The optical properties of neodymium NPs doped tellurite glass were measured by UV–Vis spectrometer, photoluminescence and Z-scan technique. The optical band gap energy of the glass network was in the range of 3.178–3.209 eV. The upconversion emission of the laser glass excited at 800 nm was found in the ultraviolet region. Electronic polarizability, oxide ion polarizability, optical basicity and metallization criterion were calculated. Moreover, linear absorption coefficient, α, nonlinear refractive index ƞ (cm2/W), nonlinear absorption, β (x 10−3) and third order susceptibilities, χ (x 10−6) were measured.     Association of a Low-Frequency Variant in HNF1A With Type 2 Diabetes in a Latino Population

    Importance: Latino populations have one of the highest prevalences of type 2 diabetes worldwide. Objectives: To investigate the association between rare protein-coding genetic variants and prevalence of type 2 diabetes in a large Latino population and to explore potential molecular and physiological mechanisms for the observed relationships. Design, Setting, and Participants: Whole-exome sequencing was performed on DNA samples from 3756 Mexican and US Latino individuals (1794 with type 2 diabetes and 1962 without diabetes) recruited from 1993 to 2013. One variant was further tested for allele frequency and association with type 2 diabetes in large multiethnic data sets of 14 276 participants and characterized in experimental assays. Main Outcome and Measures: Prevalence of type 2 diabetes. Secondary outcomes included age of onset, body mass index, and effect on protein function. Results: A single rare missense variant (c.1522G>A [p.E508K]) was associated with type 2 diabetes prevalence (odds ratio [OR], 5.48; 95% CI, 2.83-10.61; P = 4.4 × 10−7) in hepatocyte nuclear factor 1-α (HNF1A), the gene responsible for maturity onset diabetes of the young type 3 (MODY3). This variant was observed in 0.36% of participants without type 2 diabetes and 2.1% of participants with it. In multiethnic replication data sets, the p.E508K variant was seen only in Latino patients (n = 1443 with type 2 diabetes and 1673 without it) and was associated with type 2 diabetes (OR, 4.16; 95% CI, 1.75-9.92; P = .0013). In experimental assays, HNF-1A protein encoding the p.E508K mutant demonstrated reduced transactivation activity of its target promoter compared with a wild-type protein. In our data, carriers and noncarriers of the p.E508K mutation with type 2 diabetes had no significant differences in compared clinical characteristics, including age at onset. The mean (SD) age for carriers was 45.3 years (11.2) vs 47.5 years (11.5) for noncarriers (P = .49) and the mean (SD) BMI for carriers was 28.2 (5.5) vs 29.3 (5.3) for noncarriers (P = .19). Conclusions and Relevance: Using whole-exome sequencing, we identified a single low-frequency variant in the MODY3-causing gene HNF1A that is associated with type 2 diabetes in Latino populations and may affect protein function. This finding may have implications for screening and therapeutic modification in this population, but additional studies are required.publishedVersio

    Investigating the psychometric properties of the Suicide Stroop task

    Behavioral measures are increasingly used to assess suicidal thoughts and behaviors. Some measures, such as the Suicide Stroop Task, have yielded mixed findings in the literature. An understudied feature of these behavioral measures has been their psychometric properties, which may affect the probability of detecting significant effects and reproducibility. In the largest investigation of its kind, we tested the internal consistency and concurrent validity of the Suicide Stroop Task in its current form, drawing from seven separate studies (N = 875 participants, 64% female, aged 12 to 81 years). Results indicated that the most common Suicide Stroop scoring approach, interference scores, yielded unacceptably low internal consistency (rs = -.09-.13) and failed to demonstrate concurrent validity. Internal consistency coefficients for mean reaction times (RTs) to each stimulus type ranged from rs = .93-.94. All scoring approaches for suicide-related interference demonstrated poor classification accuracy (AUCs = .52-.56) indicating that scores performed near chance in their ability to classify suicide attempters from nonattempters. In the case of mean RTs, we did not find evidence for concurrent validity despite our excellent reliability findings, highlighting that reliability does not guarantee a measure is clinically useful. These results are discussed in the context of the wider implications for testing and reporting psychometric properties of behavioral measures in mental health research

    Vlasov simulations of electron acceleration by radio frequency heating near the upper hybrid layer

    It is shown by using a combination of Vlasov and test particles simulations that the electron distribution function resulting from energization due to Upper Hybrid (UH) plasma turbulence depends critically on the closeness of the pump wave to the double resonance, defined as omega≈omega_UH≈n omega_ce where n is an integer. For pump frequencies, away from the double resonance the electron distribution function is very close to Maxwellian, while as the pump frequency approaches the double resonance it develops a high energy tail. The simulations show turbulence involving coupling between Lower Hybrid (LH) and UH waves, followed by excitation of Electron Bernstein (EB) modes. For the particular case of a pump with frequency between n=3 and n=4 the EB modes cover the range from the first to the 5th mode. The simulations show that when the injected wave frequency is between the 3rd and 4th electron cyclotron frequency, bulk electron heating occurs due to the interaction between the electrons and large amplitude EB waves, primarily on the first EB branch leading to an essentially thermal distribution. On the other hand, when the frequency is slightly above the 4th electron cyclotron harmonic, the resonant interaction is predominantly due to the UH branch and leads to a further acceleration of high-velocity electrons and a distribution function with a suprathermal tail of energetic electrons. The results are consistent with ionospheric experiments and relevant to the production of Artificial Ionospheric Plasma Layers

    Evaluation of compliance and outcomes of a management protocol for massive postpartum hemorrhage at a tertiary care hospital in Pakistan

    <p>Abstract</p> <p>Background</p> <p>Massive postpartum hemorrhage is a life threatening obstetric emergency. In order to prevent the complications associated with this condition, an organized and step-wise management protocol should be immediately initiated.</p> <p>Methods</p> <p>An evidence based management protocol for massive postpartum hemorrhage was implemented at Aga Khan University Hospital, Karachi, Pakistan after an audit in 2005. We sought to evaluate the compliance and outcomes associated with this management protocol 3 years after its implementation. A review of all deliveries with massive primary postpartum hemorrhage (blood loss ≥ 1500 ml) between January, 2008 to December, 2008 was carried out. Information regarding mortality, mode of delivery, possible cause of postpartum hemorrhage and medical or surgical intervention was collected. The estimation of blood loss was made via subjective and objective assessment.</p> <p>Results</p> <p>During 2008, massive postpartum hemorrhage occurred in 0.64% cases (26/4,052). No deaths were reported. The mean blood loss was 2431 ± 1817 ml (range: 1500 - 9000 ml). Emergency cesarean section was the most common mode of delivery (13/26; 50%) while uterine atony was the most common cause of massive postpartum hemorrhage (14/26; 54%). B-lynch suture (24%) and balloon tamponade (60%) were used more commonly as compared to our previously reported experience. Cesarean hysterectomy was performed in 3 cases (12%) for control of massive postpartum hemorrhage. More than 80% compliance was observed in 8 out of 10 steps of the management protocol. Initiation of blood transfusion at 1500 ml blood loss (89%) and overall documentation of management (92%) were favorably observed in most cases.</p> <p>Conclusion</p> <p>This report details our experience with the practical implementation of a management protocol for massive postpartum hemorrhage at a tertiary care hospital in a developing country. With the exception of arterial embolization, relatively newer, simpler and potentially safer techniques are now being employed for the management of massive postpartum hemorrhage at our institution. Particular attention should be paid to the documentation of the management steps while ensuring a stricter adherence to the formulated protocols and guidelines in order to further ameliorate patient outcomes in emergency obstetrical practice. More audits like the one we performed are important to recognize and rectify any deficiencies in obstetrical practice in developing countries. Dissemination of the same is pivotal to enable an open discourse on the improvement of existing obstetrical strategies.</p

    First-in-Human Clinical Trial of Oral ONC201 in Patients with Refractory Solid Tumors

    Purpose: ONC201 is a small-molecule selective antagonist of the G protein–coupled receptor DRD2 that is the founding member of the imipridone class of compounds. A first-in-human phase I study of ONC201 was conducted to determine its recommended phase II dose (RP2D). Experimental Design: This open-label study treated 10 patients during dose escalation with histologically confirmed advanced solid tumors. Patients received ONC201 orally once every 3 weeks, defined as one cycle, at doses from 125 to 625 mg using an accelerated titration design. An additional 18 patients were treated at the RP2D in an expansion phase to collect additional safety, pharmacokinetic, and pharmacodynamic information. Results: No grade \u3e 1 drug-related adverse events occurred, and the RP2D was defined as 625 mg. Pharmacokinetic analysis revealed a Cmax of 1.5 to 7.5 μg/mL (∼3.9–19.4 μmol/L), mean half-life of 11.3 hours, and mean AUC of 37.7 h·μg/L. Pharmacodynamic assays demonstrated induction of caspase-cleaved keratin 18 and prolactin as serum biomarkers of apoptosis and DRD2 antagonism, respectively. No objective responses by RECIST were achieved; however, radiographic regression of several individual metastatic lesions was observed along with prolonged stable disease (\u3e 9 cycles) in prostate and endometrial cancer patients. Conclusions: ONC201 is a selective DRD2 antagonist that is well tolerated, achieves micromolar plasma concentrations, and is biologically active in advanced cancer patients when orally administered at 625 mg every 3 weeks
