    BACKGROUND In recent years, genotypic characterization of congenital vascular malformations (CVM) has gained attention; however, the spectrum of clinical phenotype remains difficult to attribute to a genetic cause and is rarely described in the adult population. AIM The aim of this study is to describe a consecutive series of adolescent and adult patients in a tertiary center, where a multimodal phenotypic approach was used for diagnosis. METHODS We analyzed clinical findings, imaging, and laboratory results at initial presentation, and set a diagnosis according to the International Society for the Study of Vascular Anomalies (ISSVA) classification for all consecutively registered patients older than 14 years of age who were referred to the Center for Vascular Malformations at the University Hospital of Bern between 2008 and 2021. RESULTS 457 patients were included for analysis (mean age 35 years; females 56%). Simple CVMs were the most common (n=361, 79 %), followed by CVM associated with other anomalies (n=70, 15%), and combined CVM (n=26, 6%). Venous malformations (n=238) were the most common CVM overall (52%), and the most common simple CVM (66%). Pain was the most frequently reported symptom in all patients (simple, combined and vascular malformation with other anomalies). Pain intensity was more pronounced in simple venous and arteriovenous malformation. Clinical problems were related to the type of CVM diagnosed, with bleeding and skin ulceration in arteriovenous malformations, localized intravascular coagulopathy in venous malformations and infectious complications in lymphatic malformations. Limb length difference occurred more often in patients with CVM associated with other anomalies as compared to simple or combined CVM (22.9 vs 2.3%, p< 0.001). Soft tissue overgrowth was seen in one quarter of all patients independent of the ISSVA group. CONCLUSIONS In our adult and adolescent population with peripheral vascular malformations, simple venous malformations predominated, with pain as the most common clinical symptom. In a quarter of cases, patients with vascular malformations presented with associated anomalies on tissue growth. The differentiation of clinical presentation with or without accompanying growth abnormalities need to be added to the ISSVA classification. Phenotypic characterization considering vascular and non-vascular features remains the cornerstone of diagnosis in adult-as well as pediatric patients

    A chronometric exploration of high-resolution ‘sensitive TMS masking’ effects on subjective and objective measures of vision

    Transcranial magnetic stimulation (TMS) can induce masking by interfering with ongoing neural activity in early visual cortex. Previous work has explored the chronometry of occipital involvement in vision by using single pulses of TMS with high temporal resolution. However, conventionally TMS intensities have been high and the only measure used to evaluate masking was objective in nature. Recent studies have begun to incorporate subjective measures of vision, alongside objective ones. The current study goes beyond previous work in two regards. First, we explored both objective vision (an orientation discrimination task) and subjective vision (a stimulus visibility rating on a four-point scale), across a wide range of time windows with high temporal resolution. Second, we used a very sensitive TMS-masking paradigm: stimulation was at relatively low TMS intensities, with a figure-8 coil, and the small stimulus was difficult to discriminate already at baseline level. We hypothesized that this should increase the effective temporal resolution of our paradigm. Perhaps for this reason, we are able to report a rather interesting masking curve. Within the classical-masking time window, previously reported to encompass broad SOAs anywhere between 60 and 120 ms, we report not one, but at least two dips in objective performance, with no masking in-between. The subjective measure of vision did not mirror this pattern. These preliminary data from our exploratory design suggest that, with sensitive TMS masking, we might be able to reveal visual processes in early visual cortex previously unreported

    (Against a) Theory of Audience Engagement with News

    Audience engagement has become a key concept in contemporary discussions on how news companies relate to the public and create sustainable business models. These discussions are irrevocably tied to practices of monitoring, harvesting and analyzing audience behaviours with metrics, which is increasingly becoming the new currency of the media economy. This article argues this growing tendency to equate engagement to behavioural analytics, and study it primarily through quantifiable data, is limiting. In response, we develop a heuristic theory of audience engagement with news comprising four dimensions—the technical-behavioural, emotional, normative and spatiotemporal—and explicate these in terms of different relations of engagement between human-to-self, human-to-human, human-to-content, human-to-machine, and machine-to-machine. Paradoxically, this model comprises a specific theory of audience engagement while simultaneously making visible that constructing a theory of audience engagement is an impossible task. The article concludes by articulating methodological premises, which future empirical research on audience engagement should consider

    Enhanced C2_2H2_2 absorption within Jupiter's southern auroral oval from Juno UVS observations

    Reflected sunlight observations from the Ultraviolet Spectrograph (UVS) on the Juno spacecraft were used to study the distribution of acetylene (C2_2H2_2) at Jupiter's south pole. We find that the shape of the C2_2H2_2 absorption feature varies significantly across the polar region, and this can be used to infer spatial variability in the C2_2H2_2 abundance. There is a localized region of enhanced C2_2H2_2 absorption which coincides with the location of Jupiter's southern polar aurora; the C2_2H2_2 abundance poleward of the auroral oval is a factor of 3 higher than adjacent quiescent, non-auroral longitudes. This builds on previous infrared studies which found enhanced C2_2H2_2 abundances within the northern auroral oval. This suggests that Jupiter's upper-atmosphere chemistry is being strongly influenced by the influx of charged auroral particles and demonstrates the necessity of developing ion-neutral photochemical models of Jupiter's polar regions.Comment: Accepted in JGR: Planet
