Assessment of flatness and symmetry of megavoltage x-ray beam with an electronic portal imaging device (EPID)

The document attached has been archived with permission from the publisher.The input/output characteristics of the Wellhofer BIS 710 electronic portal imaging device (EPID) have been investigated to establish its efficacy for periodic quality assurance (QA) applications. Calibration curves have been determined for the energy fluence incident on the detector versus the pixel values. The effect of the charge coupled device (CCD) camera sampling time and beam parameters (such as beam field size, dose rate, photon energy) on the calibration have been investigated for a region of interest (ROI) around the central beam axis. The results demonstrate that the pixel output is a linear function of the incident exposure, as expected for a video-based electronic portal imaging system. The field size effects of the BIS 710 are similar to that of an ion chamber for smaller field sizes up to 10 x 10 cm2. However, for larger field sizes the pixel value increases more rapidly. Furthermore, the system is slightly sensitive to dose rate and is also energy dependent. The BIS 710 has been used in the current study to develop a QA procedure for measurements of flatness and symmetry of a linac x-ray beam. As a two-dimensional image of the radiation field is obtained from a single exposure of the BIS 710, a technique has been developed to calculate flatness and symmetry from a defined radiation area. The flatness and symmetry values obtained are different from those calculated conventionally from major axes only (inplane, crossplane). This demonstrates that the technique can pick up the "cold" and "hot" spots in the analysed area, providing thus more information about the radiation beam. Factors Impacting Work Engagement of Gen Z Employees: A Regression Analysis
This study examined the factors that impacted work engagement for Gen Z employees. Six factors were identified for analysis and hypothesized to be positively related to employee engagement, including: corporate social responsibility, leadership – transformational and transactional, work-life balance, autonomy, and technology. A survey was sent out through Amazon Mechanical Turk in April 2020. From the responses, Gen Z respondents accounted for 69. This study used a correlation analysis and a multiple regression analysis. Results revealed positive correlations between the variables and employee engagement, with transformational leadership showing the highest correlation and transactional leadership the lowest. Technology, autonomy, and work-life balance fell in between these two extremes. The results of the multiple regression analysis indicated that transformational leadership had the strongest influence on employee engagement, while transactional leadership had no impact. Autonomy and work-life balance were shown to have positive influences on employee engagement; technology and corporate social responsibility were not significant

Saturated Zone Plumes in Volcanic Rock: Implications for Yucca Mountain
This paper presents a literature survey of the occurrences of radionuclide plumes in saturated, fractured rocks. Three sites, Idaho National laboratory, Hanford, and Oak Ridge are discussed in detail. Results of a modeling study are also presented showing that the length to width ratio of a plume starting within the repository footprint at the Yucca Mountain Project site, decreases from about 20:1 for the base case to about 4:1 for a higher value of transverse dispersivity, indicating enhanced lateral spreading of the plume. Due to the definition of regulatory requirements, this lateral spreading does not directly impact breakthrough curves at the 18 km compliance boundary, however it increases the potential that a plume will encounter reducing conditions, thus significantly retarding the transport of sorbing radionuclides
An international multidisciplinary consensus statement on fasting before procedural sedation in adults and children
The multidisciplinary International Committee for the Advancement of Procedural Sedation presents the first fasting and aspiration prevention recommendations specific to procedural sedation, based on an extensive review of the literature. These were developed using Delphi methodology and assessment of the robustness of the available evidence. The literature evidence is clear that fasting, as currently practiced, often substantially exceeds recommended time thresholds and has known adverse consequences, for example, irritability, dehydration and hypoglycaemia. Fasting does not guarantee an empty stomach, and there is no observed association between aspiration and compliance with common fasting guidelines. The probability of clinically important aspiration during procedural sedation is negligible. In the post-1984 literature there are no published reports of aspiration-associated mortality in children, no reports of death in healthy adults (ASA physical status 1 or 2) and just nine reported deaths in adults of ASA physical status 3 or above. Current concerns about aspiration are out of proportion to the actual risk. Given the lower observed frequency of aspiration and mortality than during general anaesthesia, and the theoretical basis for assuming a lesser risk, fasting strategies in procedural sedation can reasonably be less restrictive. We present a consensus-derived algorithm in which each patient is first risk-stratified during their pre-sedation assessment, using evidence-based factors relating to patient characteristics, comorbidities, the nature of the procedure and the nature of the anticipated sedation technique. Graded fasting precautions for liquids and solids are then recommended for elective procedures based upon this categorisation of negligible, mild or moderate aspiration risk. This consensus statement can serve as a resource to practitioners and policymakers who perform and oversee procedural sedation in patients of all ages, worldwide
A spatial analysis of employment multipliers in the US
The actual effectiveness of employment promotion policies depends on the ability of the intervention at creating new jobs in the targeted sector, but also, to a large extent, on the impact they have on other parts of the local economy. Estimating the latter effect is therefore quite important for regional economic development policies. Along the lines of Moretti (Am Econ Rev Pap Proc 100: 373-377, 2010), we present an empirical analysis of local employment multipliers using data on 123 US Metropolitan Statistical Areas over the period 1980-2010. From the methodological point of view, in this work not only endogeneity (via instrumental variables estimates), but also spatial spillovers are taken into account. According to the results, the magnitude of the multiplier could be rather limited. On the other hand, there is clear indication that the impact of these interventions is not fully contained within the local economy and they have a positive effect on closely surrounding ones
