IMPLEMENTAZIONE DI TECNICHE DI TERMOGRAFIA ATTIVA IRNDT SU COMPOSITI GRP MEDIANTE LA DEPOSIZIONE DI IMPULSI TERMICI DI LUNGA DURATA
Nel presente lavoro viene analizzata l\u2019implementazione di tecniche attive di termografia ad infrarossi per analisi NDT su pannelli in composito. In particolare si propone l\u2019uso di lampade alogene come sorgente di calore esterna, le quali richiedono tempi di accensione dell\u2019ordine di diversi secondi al fine di depositare efficaci quantit\ue0 di calore. Si discutono quindi le problematiche connesse all\u2019implementazione di tecniche di Transient Thermography e di Lock-In Thermography in presenza di tempi lunghi di deposizione di calore. Viene implementata una tecnica di modulazione della deposizione di calore mediante treni di onde quadre di lunga durata (PMLT - Pulse Modulated Lock-In Thermography). Viene in particolare analizzata l\u2019influenza del periodo dell\u2019onda quadra, e del rapporto di acceso-spento del treno di onde quadre, attraverso l\u2019analisi dell\u2019andamento del contrasto di fase al variare della frequenza di correlazione (lock-in frequency)
Ab initio based equation of state of dense water for planetary and exoplanetary modeling

The final version is available from EDP Sciences via the DOI in this record.As a first step toward a multi-phase equation of state for dense water, we develop a temperature-dependent equation of state for dense water covering the liquid and plasma regimes and extending to the super-ionic and gas regimes. This equation of state covers the complete range of conditions encountered in planetary modeling. We use first principles quantum molecular dynamics simulations and its Thomas-Fermi extension to reach the highest pressures encountered in giant planets several times the size of Jupiter. Using these results, as well as the data available at lower pressures, we obtain a parametrization of the Helmholtz free energy adjusted over this extended temperature and pressure domain. The parametrization ignores the entropy and density jumps at phase boundaries but we show that it is sufficiently accurate to model interior properties of most planets and exoplanets. We produce an equation of state given in analytical form that is readily usable in planetary modeling codes and dynamical simulations {\bf (a fortran implementation can be found at http://www.ioffe.ru/astro/H2O/)}. The EOS produced is valid for the entire density range relevant to planetary modeling, {\bf for densities where quantum effects for the ions can be neglected, and for temperatures below 50,000K. We use this equation of state to calculate the mass-radius relationship of exoplanets up to 5,000M_Earth, explore temperature effects in ocean and wet Earth-like planets, and quantify the influence of the water EOS for the core on the gravitational moments of Jupiter.s. Rhinosinusitis and asthma: a very long engagement
Upper and lower airways may be considered as a unique entity, interested by coexisting inflammatory processes that share common etiopathogenic mechanisms. Previous studies have strongly demonstrated a relationship between rhinosinusitis and asthma. This has led to the introduction of the concept of United Airways , which has also been included in the WHO document Allergic Rhinitis and its Impact on Asthma (ARIA); this concept has important consequences also on the treatment of these disorders. To better summarize the evident connection between upper and lower airway disease we decided to describe it as a multilayered construction, each level pointing out more deeply the relationship between these entities

Rectal bleeding and prolapse\u2026 not always benign diseases rather anal cancer. The importance of a correct decision making since primary care
Rectal bleeding is very common in general population with a prevalence of 10-20 %. Primary care physicians have to stratify patients basing on urgency and on the colo-rectal cancer risk and to conduct a decision making for the correct management. We report a case of a 61-years-old woman, complaining rectal bleeding and an anal mass attended to their family doctor who does a visit but without a digital rectal examination and diagnosed a hemorrhoidal prolapse suggesting medical therapy. For the persistence of symptoms she comes to our service from emergency attention. Inspection and digital rectal examination revealed an anal mass. CT scan was performed showing a large anal mass involving half anal circumference. Histologic samples showed an epithelial proliferation compatible with a squamous carcinoma. Oncological consult was requested and a chemo-radiotherapy treatment was proposed. This case report highlights the difficulty when physicians assess patients with anorectal complaints in differentiating anal cancer from benign disease, presumably because symptoms are similar. Primary care physicians must maintain a high index of suspicion of cancer in high-risk population. Sensitization of these colleagues is required since digital rectal examination is of inestimable value to verify the presence of a rectal or an anal mass.Abstract

Mannheim Peritonitis Index (MPI) and elderly population: Prognostic evaluation in acute secondary peritonitis
Introduction. Acute Secondary Peritonitis due to abdominal visceral perforation is characterized by high mortality and morbidity risk. Risk stratification allows prognosis prediction to adopt the best surgical treatment and clinical care support therapy. In Western countries elderly people represent a significant percentage of population Aim. Evaluation of Mannheim Peritonitis Index (MPI) and consideration upon old people. Patients and methods. Retrospective study on 104 patients admitted and operated for \ue2\u80\u9cAcute Secondary Peritonitis due to visceral perforation\ue2\u80\u9d. MPI was scored. In our study we want to demonstrate efficacy of MPI and the possibility to consider older age an independent prognostic factor. Results. Mortality was 25.96%. Greatest sensitivity and specificity for the MPI score as a predictor of mortality was at the score of 20. MPI score of 22. Patients with MPI score 17-21 had 0.46 times lower risk of mortality compared to patients with MPI score >21. In the group of patients with MPI score of >20 the mortality rate was 48.5% for patients older than 80 years old and 12.1% for younger patients (p < 0.005); in the group with MPI score of < 20 mortality rate was respectively 8.4% and 1.4% (p < 0.005). Discussion and conclusions. Data confirm the accuracy of the test. MPI score and age over 80 years old resulted independent predictors of mortality at multivariate analysis
Pharmacogenomics: A Step forward Precision Medicine in Childhood Asthma
Personalized medicine, an approach to care in which individual characteristics are used for targeting interventions and maximizing health outcomes, is rapidly becoming a reality for many diseases. Childhood asthma is a heterogeneous disease and many children have uncontrolled symptoms. Therefore, an individualized approach is needed for improving asthma outcomes in children. The rapidly evolving fields of genomics and pharmacogenomics may provide a way to achieve asthma control and reduce future risks in children with asthma. In particular, pharmacogenomics can provide tools for identifying novel molecular mechanisms and biomarkers to guide treatment. Emergent high-throughput technologies, along with patient pheno-endotypization, will increase our knowledge of several molecular mechanisms involved in asthma pathophysiology and contribute to selecting and stratifying appropriate treatment for each patient
Is it really useful the harmonic scalpel in axillary dissection for locally advanced breast cancer? A case series
Background. The seroma is one of the most common complications in the axillary lymph nodal dissection (different surgical approaches have been tried to reduce the seroma incidence). In our study we evaluate the outcome of patients using or not the ultrasonic scalpel (Harmonic scalpel) according to a standardized surgical technique. Patients and Methods. From January 2011 to December 2015 120 patients underwent axillary dissection for breast cancer. Patients were divided in two groups: patients belonging to the first group underwent Harmonic scalpel dissection and patients belonging to the second group underwent classical dissection. Each group consisted of 60 patients. Quadrantectomy (QUAD) was performed in 54 patients, 66 women underwent mastectomy. In all patients axillary dissection included the I, II and III level. We compared two groups in terms of: time of surgery, hematoma, drainage volume, days of sealing drainage, seroma formation, number of post-seroma aspirations, upper limb lymphedema, wound infections, post-operative pain. Results. Statistically significant results were obtained in terms of the total volume of the breast and axillary drainage in the two techniques. There were no significant differences in the two samples in terms of operative time incidence of seroma, post-operative hematoma, wound infection, and lymphedema of the upper limb. Conclusion. The small number of cases did not allow us to reach definitive conclusions. The use of Harmonic scalpel seems to show smaller incidence of seroma and reduction of the amount of both breast and axillary drainages. Further studies are needed to define the real advantage in terms of cost benefit of using these devices in the axillary surgery
Novel therapeutic targets for allergic airway disease in children
The aim of precision medicine is setting up targeted therapies for selected patients that would ideally have high effectiveness and few side effects. This is made possible by targeted therapy drugs that selectively act on a specific pathway. Precision medicine is spreading to many medical specialties, and there is increasing interest in the context of allergic airway diseases, such as allergic rhinitis, chronic rhinosinusitis, and asthma. This review is an update of new targets in the treatment of childhood allergic upper airway diseases and asthma, including the most recent biologic drugs that have already been licensed or are in the pipeline to be tested with children
Immunomodulation in pediatric asthma
Childhood asthma is actually defined as a heterogeneous disease, including different clinical variants and partially sharing similar immune mechanisms. Asthma management is mainly focused on maintaining the control of the disease and reducing the risk of adverse outcomes. Most children achieve good control with standard therapies, such as low doses of inhaled corticosteroids (ICS) and/or one or more controller. These medications are targeted to suppress bronchial inflammation and to restore airway responsiveness. However, they are not disease-modifying and do not specifically target inflammatory pathways of asthma; in addition, they are not significantly effective in patients with severe uncontrolled asthma. The aim of this review is to update knowledge on current and novel therapeutic options targeted to immunomodulate inflammatory pathways underlying pediatric asthma, with particular reference on biologic therapies
