    Teknolojinin gelişmesi, enerjiye olan talep gün geçtikçe artmaktadır. Fosil yakıtların sınırlı olması alternatif enerji kaynakları aramaya teşvik etmektedir. Yenilenebilir enerji kaynakları kullanımı artan enerji talebini karşılamada önemli bir yere sahiptir. Güneş enerji teknolojileri günümüzde sıkça kullanılmaya başlanmıştır. Güneş enerjisinden yüksek verimlilikle güç elde etmede güneş takip sistemleri gündeme gelmektedir. Bu çalışmada Güneş takip sistemleri çift eksenli olarak tasarlanmıştır. Güneş panelleri aynı teknik özelliklere sahiptir. Güneş panellerine üzerine üç farklı tip algılayıcı yerleştirilmiştir. Algılayıcı türlerinin aynı tip enerji üretimine olan performansları karşılaştırılmıştır. Sistemin sabit ve hareketli konumda belirli aralıklarla gerilim ve akım değerleri ölçülmüştür. Elde edilen verilere göre güneş panellerinin sabit ve hareketli konumda toplam üretimleri karşılaştırıldığında hareketli konumda sabit konuma göre; LilyPad Işık Sensörlü sistemde %36,68 oranında, SMD Sensörlü sistemde %38,42 ve LDR’li sistemde %43,58 oranında artış görülmüştürThe scope of the present study is to investigate the change of ultrasonic velocity with the physical and mechanical properties of Ni-TiWC composites. Since the materials used in the aerospace and automotive industries are exposed to high temperatures, friction andhigh stresses, the need for metal matrix composite materials used in manufacturing applications such as cutting discs and inserts hasincreased rapidly. Ni-Ti metal matrix composites reinforced with WC were circularly shaped in uniaxial hydraulic press. A mixture of30% Ni, 2% Ti and 68% WC powders was sintered at 1000°C-1400°C with argon atmosphere in a tube furnace and were fabricatedby electroless plating technique. XRD (X-Ray diffraction), SEM (Scanning Electron Microscope), compressive testing, hardness andultrasonic velocity measurements were employed to investigated the mechanical and physical properties of specimens. Experimentalresults showed that ultrasound velocity values increased polynomial linearly with increasing sintering temperature. In addition, thefact that the highest sintering temperature resulted in 429.1HV hardness at 1400°C is an indication that the structure has become morerobust and compact. The change in ultrasonic velocities with the increase in sintering temperature is proof that the bonding in thestructure is better and the particle growth is well achieved

    A Randomized Case-Controlled Study of Recombinant Human Granulocyte Colony Stimulating Factor for the Treatment of Sepsis in Preterm Neutropenic Infants

    BackgroundTo investigate the efficacy and safety of recombinant human granulocyte colony-stimulating factor, recombinant human granulocyte-macrophage colony-stimulating factor (rhG-CSF) to treat sepsis in neutropenic preterm infants.MethodsFifty-six neutropenic preterm infants with suspected or culture-proven sepsis hospitalized in Zeynep Kamil Maternity and Children's Educational and Training Hospital, Kozyatağı/Istanbul, Turkey between January 2008 and January 2010 were enrolled. Patients were randomized either to receive rhG-CSF plus empirical antibiotics (Group I) or empirical antibiotics alone (Group II). Clinical features were recorded. Daily complete blood count was performed until neutropenia subsided. Data were analyzed using SPSS version 11.5.ResultsThirty-three infants received rhG-CSF plus antibiotic treatment and 23 infants received antibiotic treatment. No drug-related adverse event was recorded. Absolute neutrophil count values were significantly higher on the 2nd study day and 3rd study day in Group I. Short-term mortality did not differ between the groups.ConclusionTreatment with rhG-CSF resulted in a more rapid recovery of ANC in neutropenic preterm infants. However, no reduction in short-term mortality was documented

    Bu çalışma, farklı sektörlerde faaliyet gösteren rakip firmaların Twitter verilerini analiz ederek, firmaların Twitter verilerinin firmalara göre anlamlı bir uyum gösterip göstermediğinin tespit edilmesini, firmaların Twitter’da paylaştıkları içeriklerin kümelenmesini ve hangi içerik kümesinin en fazla etkileşime yol açtığının belirlenmesini amaçlamaktadır. Bu kapsamda, 2017 yılı boyunca kozmetik, elektronik ve pazaryeri sektörlerinde faaliyet gösteren rakip firmalar tarafından paylaşılan Twitter verileri, Sosyal Medya Madenciliği süreci izlenerek analiz edilmiştir. Firmaların Twitter verilerinin firmalara göre anlamlı bir uyum gösterip göstermediği Uygunluk Analizi ile tespit edilmiştir. Firmaların Twitter paylaşımları ise Metin Madenciliği ön işleme metotlarından faydalanılarak “Özel Teklif”, “Yarışma & Etkinlik”, “Ürün”, “Sosyal”, “Destek & Geri Bildirim” ve “Özel Etkileşim” kategori başlıklarıyla kümelenmiştir.     Bu çalışmanın amacı farklı sektörlerde faaliyet gösteren lider rakip firmaların sosyal medya etkinliklerinin ölçülmesidir. Bu kapsamda, 2018 Şubat ayı boyunca Twitter kullanıcılarının kozmetik, pazaryeri ve elektronik sektöründe faaliyet gösteren rakip firmalar hakkında yaptıkları paylaşımlar ve bu firmaların kurumsal Twitter hesaplarından yapmış oldukları paylaşımlar Sosyal Medya Madenciliği yöntemi ile analiz edilmiştir. Firmalar hakkındaki tweet sayısı, tweet değeri (olumlu, olumsuz, nötr), takipçi kazanımı, yanıt sayısı, retweet sayısı ve beğeni sayısı başlıklarından oluşan Twitter etkinliğinin ölçüm kriterleri ele alınarak bir başarı sıralaması yapılmış ve en başarılı firmanın kozmetik, en düşük sıralamaya sahip firmanın ise elektronik sektöründen olduğu tespit edilmiştir.     Determination of carbon footprint for local administrations: a case study for Ümraniye Municipality

    Bugün olumsuz etkileri ile karşı karşıya olduğumuz küresel ısınmanın azaltılması yönünde uluslararası, ulusal, bölgesel ve yerel düzeyde önlemler alınması yoluna gidilmektedir. Küresel ısınmanın büyük ölçüde kentleşmeden ve kentlerden kaynaklanması, yerel yönetimlere önemli sorumluluklar yüklemektedir. Bu çalışmada İstanbul’un ilçelerinden hızlı bir şekilde gelişmekte olan ve önemli bir nüfusu barındıran Ümraniye ilçesi pilot bölge olarak seçilmiştir. İlçe sınırları içerisinde yer alan konutlar, sanayiler, ticarethaneler ve resmi kurumlar (hastane, okul, vb.) ile ulaşım kaynaklı karbon ayak izinin belirlenmesinin yanı sıra Ümraniye Belediyesinin kendi faaliyetlerinden kaynaklanan karbon ayak izi de belirlenmiştir. Bu kapsamda ilgili kurumlardan toplanan 2017 yılı doğalgaz ve elektrik tüketim verileri ile toplu taşıma ve özel araçların güzergah, sayı, yakıt cinsi ve tüketimleri dikkate alınmıştır. Elde edilen sonuçlardan, karbon ayak izinin bu kaynaklar arasında konutlarda %43, ticarethane ve resmi kurumlarda %35, ulaşımda %16, sanayide %4 ve dış ortam aydınlatmalarında %1 olarak dağıldığı gözlenmiştir. Bu kaynaklardan özellikle ticarethaneler ve resmi kurumlar (%29) ile konutlarda (%22) elektrik kullanımının etkisinin büyük olduğu belirlenmiştir. Ulaşımda özel araç kullanımının etkisinin (%50), toplu taşımaya (%12) göre oldukça yüksek olduğu tespit edilmiştir. Belediyenin kendi faaliyetlerinden kaynaklanan karbon ayak izinin ise toplam içinde %3 oranla düşük bir paya sahip olduğu belirlenmiştir. İlçede kişi baına düşen emisyon 2,55 ton CO2/kişi olarak hesaplanmıştır.Since the World is facing with detrimental effects of the global warming, some precautions are needed to be taken at international, national, regional and local levels. The fact that global warming originates mostly from urbanization and cities imposes serious responsibilities for local governments. In this study, Ümraniye which is a rapidly developing and populous district was chosen as the pilot area. The carbon footprint of transportation, housing, industry, public institutions (hospitals, schools, etc.) within the district was investigated. Also the carbon footprint of Ümraniye Municipality's own activities was calculated. In this context, the data of natural gas and electricity consumption as well as the route, number, fuel type and consumption of public transport and private vehicles, collected from related institutions for 2017 were taken into consideration. According to the obtained results, the carbon footprint in the district came from housing, business and public institutions, transportation, industry at the rate of 43%, 35%, 16%, 4% respectively. Among all, it was determined that especially the effect of the electricity consumption in business and public institutions (29%) and housing (22%) were major for the carbon footprint. It was found that the impact of private vehicle use (50%) was quite high compared to public transport (12%). The carbon footprint resulting from the municipality's own activities has a low share of 3% in total. The carbon footprint per person in the district was calculated as 2.55 tonne CO2/capita

    Evaluation of Four Adult Visceral Leishmaniasis Cases

    Leishmania infantum is the species responsible for visceral leishmaniasis [(VL), kala-azar], which is observed sporadically mainly in pediatric age groups in the Aegean, Mediterranean and Central Anatolian regions of Türkiye. The aim of this study is to evaluate the diagnosis, clinic, laboratory results and treatments of four adult patients with VL who applied to our hospital. The patients were referred to our hospital to investigate hematological malignancy. In the study, the data of four patients (three men, one woman; age range: 30-40 years) who were diagnosed with VL and treated in the infectious diseases clinic of our hospital between January 2022 and April 2022 were evaluated retrospectively. The diagnosis of VL was made according to appropriate clinical and physical examination findings, biochemical and serological tests (indirect fluorescent antibody test and rK39 rapid antigen test) and polymerase chain reaction (PCR) results, as well as the presence of amastigote forms of the parasite in bone marrow samples. Serology positivity was found in all patients, and bone marrow positivity was found in two patients. According to the results of RT-PCR in all patients, it was determined that the species causing the disease was L. infantum/L. donovani. Initially, the most common symptoms were fever, fatigue, and abdominal distension. None of the patients had an immunosuppressive condition. It was understood that all the patients lived in the rural area of Syria’s Idlib province. Hepatosplenomegaly, increased erythrocyte sedimentation rate, anemia, leukopenia and thrombocytopenia were found in all patients. The patients were treated with liposomal amphotericin-B (L-AMB). One patient did not come for follow-ups, the other three patients were found to have completely recovered in their follow-up. No recurrence was observed in any of the patients. In conclusion, VL should be considered in patients who apply to health institutions with complaints of fever, hepatosplenomegaly, increased erythrocyte sedimentation rate, anemia, leukopenia and thrombocytopenia

    Anxiety Levels of Children with Primary Ciliary Dyskinesia and Their Mothers at the Beginning of the COVID-19 Pandemic and Change in the First Year

    We aimed to assess anxiety of children with primary ciliary dyskinesia (PCD) and their primary caregivers at the beginning of Coronavirus disease-2019 (COVID-19) pandemic and change in levels of anxiety in first year with prolongation of pandemic. This was a two-step study; first step was questionnaire-based, conducted via teleconference. In first step, 29 patients and 105 healthy children and their mothers were participated; 25 children with PCD and their mothers were in second step. Demographic characteristics, clinical informations were recorded. Children’s and mothers’ state and trait anxiety levels were assessed and compared. Anxiety levels of mothers of patients were assessed according to clinical characteristics of children. Mothers’ knowledge of COVID-19 and effect of teleconference on their anxiety was evaluated. State anxiety levels in the first year of pandemic of children with PCD and their mothers were also compared. Compared to control group, state anxiety of children in 13-18 age group and trait anxiety of their mothers were lower (p<0.05). In both groups, trait and state anxiety of 13-18 years old children and mothers positively correlated. Trait anxiety of mothers of patients negatively correlated with patients’ FEV1 and MEF25-75. Patients’ mothers reported feeling less anxiety at the end of teleconference. Anxiety of mothers of patients (especially under 9 years old) had increased as pandemic continued. At the beginning of pandemic, children with PCD were less anxious than healthy children, and their mothers had lower trait anxiety than mothers of healthy children. Being followed for chronic disease and obtaining information about COVID-19 may have reduced anxiety of children with PCD and their mothers. However, as pandemic continues, need to protect their children with PCD from infection, especially of mothers with younger children, may have raised their concerns

    Increased mean platelet volume in patients with infective endocarditis and embolic events

    Background: Platelet activation appears to play an important role in thromboembolic com­plications of infective endocarditis (IE). Mean platelet volume (MPV) is a potentially useful marker of platelet activity and a quick and easy determinant of thrombotic risk. Hence the aim of this study was to investigate the baseline platelet volume indices (MPV and platelet distribution width [PDW]) in IE patients who developed embolic events in the follow-up period and who did not. Methods: The study group consisted of 76 consecutive patients (female: 55, male: 21, mean age: 26 years old, ranged: 8–64 years) with definite IE according to Duke Criteria. Thirty four healthy subjects, who were age and gender adjusted, served as the control group. The mean duration of hospital stay was 44 days. Results: Among the IE patients, 13 (13/76, 17.1%) had major embolic events. Significantly larger vegetations were observed in patients with embolic events as compared to non-embolic group (1.4 vs. 1.0 cm, p = 0.03). MPV at hospital admission was higher in patients who had embolic events in the follow-up period compared to both those who did not and the control subjects (10.62 ± 1.13 vs. 9.25 ± 0.97 and 8.93 ± 0.82 fL, p &lt; 0.001, respectively). Similarly, the patients with embolic events had increased PDW compared to the non-embolic ones and the control group (16.31 ± 2.42 vs. 14.35 ± 1.97 and 14.04 ± 1.82%, p &lt; 0.001, respectively). Conclusions: The present study demonstrated that IE patients with embolic events had in­creased MPV and PDW values, compared to non-embolics. Future prospective studies with standardized measurements may clarify the clinical role of platelet volume indices in thrombo­embolic complications of IE

    The frequency of Duchenne muscular dystrophy/Becker muscular dystrophy and Pompe disease in children with isolated transaminase elevation: results from the observational VICTORIA study

    IntroductionElevated transaminases and/or creatine phosphokinase can indicate underlying muscle disease. Therefore, this study aims to determine the frequency of Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) in male children and Pompe disease (PD) in male and female children with isolated hypertransaminasemia.MethodsThis multi-center, prospective study enrolled patients aged 3–216 months with serum alanine transaminase (ALT) and/or aspartate transaminase (AST) levels &gt;2× the upper limit of normal (ULN) for ≥3 months. Patients with a known history of liver or muscle disease or physical examination findings suggestive of liver disease were excluded. Patients were screened for creatinine phosphokinase (CPK) levels, and molecular genetic tests for DMD/BMD in male patients and enzyme analysis for PD in male and female patients with elevated CPK levels were performed. Genetic analyses confirmed PD. Demographic, clinical, and laboratory characteristics of the patients were analyzed.ResultsOverall, 589 patients [66.8% male, mean age of 63.4 months (standard deviation: 60.5)] were included. In total, 251 patients (188 male and 63 female) had CPK levels above the ULN. Of the patients assessed, 47% (85/182) of male patients were diagnosed with DMD/BMD and 1% (3/228) of male and female patients were diagnosed with PD. The median ALT, AST, and CPK levels were statistically significantly higher, and the questioned neurological symptoms and previously unnoticed examination findings were more common in DMD/BMD patients than those without DMD/BMD or PD (p &lt; 0.001).DiscussionQuestioning neurological symptoms, conducting a complete physical examination, and testing for CPK levels in patients with isolated hypertransaminasemia will prevent costly and time-consuming investigations for liver diseases and will lead to the diagnosis of occult neuromuscular diseases.
