    Perceived body distortion rather than actual body distortion is associated with chronic low back pain in adults with cerebral palsy: A preliminary investigation

    Objectives: The aim of the present study was to investigate whether distorted body perception is a feature of the low back pain experience in people with Cerebral Palsy (CP) and whether any distortions noted are confounded by the presence of motor and postural impairments commonly seen in CP. Methods: Forty‐five individuals participated in this study: fifteen adults with CP with LBP (CP_Pain group), fifteen adults with CP without LBP (CP_noPain group), and fifteen age‐matched adults with LBP but no CP (Pain group). Body perception was evaluated using the Fremantle Back Awareness Questionnaire (FreBAQ) and by assessing two‐point discrimination (TPD) thresholds over the low back. A comprehensive assessment of motor function was also undertaken in the CP population and postural function was assessed in all three groups. Results: Significant differences between the three groups were found for FreBAQ scores (p \u3c 0.0001). The TPD threshold in the low back of the CP_Pain group was significantly larger than that of the CP_noPain group (p = 0.01), though we found no difference between the CP_noPain group and the Pain group (p = 0.21). We found no difference in motor or postural function between the two CP groups. Discussion: The present results suggest that body image is disrupted in people with CP who experience low back pain. The disruptions in perception were similar to those seen in people with LBP and no CP suggesting the distortions maybe more related to the presence of pain than the presence of CP.     In-patient step count predicts re-hospitalization after cardiac surgery

    Background Clinical significance of in-patient step count after cardiac surgery remains unknown. The aim of this study was to determine whether the number of steps walked during the in-patient stay after cardiac surgery predicts the risk of cardiac re-hospitalization in the following year. Methods One hundred and thirty-three patients who underwent cardiac surgery were included in this study. The number of steps was assessed using a triaxial accelerometer. One year after surgery, patients completed a postal survey to determine their health condition and occurrence of cardiac re-hospitalization. Results The mean number of steps walked during the last three in-patient days was 2460 ± 1549 (mean ± standard deviation). Of the 133 patients, there were 16 cases (12.0%) of cardiac re-hospitalization during the 1-year follow-up period. The average step count before discharge was significantly lower in the 16 patients who were re-hospitalized for cardiac causes (1297 ± 1232 versus 2620 ± 1524, p < 0.01). The cut-off value that predicted the occurrence of cardiac re-hospitalization on the receiver operating curve was 1308 steps (area under the curve: 0.783, p < 0.001, sensitivity: 0.814, specificity: 0.733). Cox proportional hazards analysis revealed that the strongest predictor of cardiac re-hospitalization was a low step count prior to discharge (=1308 steps, hazard ratio: 7.58; 95% confidence interval: 2.04–28.22). Conclusions In-patient step count appears to be a risk factor for cardiac re-hospitalization within the first year following cardiac surgery. Further studies are needed to clarify the clinical significance of step count both preoperatively and following discharge

    Wnt/β-Catenin Signaling and Resistance to Immune Checkpoint Inhibitors: From Non-Small-Cell Lung Cancer to Other Cancers

    Lung cancer is the leading cause of cancer-related deaths worldwide. The standard of care for advanced non-small-cell lung cancer (NSCLC) without driver-gene mutations is a combination of an anti-PD-1/PD-L1 antibody and chemotherapy, or an anti-PD-1/PD-L1 antibody and an anti-CTLA-4 antibody with or without chemotherapy. Although there were fewer cases of disease progression in the early stages of combination treatment than with anti-PD-1/PD-L1 antibodies alone, only approximately half of the patients had a long-term response. Therefore, it is necessary to elucidate the mechanisms of resistance to immune checkpoint inhibitors. Recent reports of such mechanisms include reduced cancer-cell immunogenicity, loss of major histocompatibility complex, dysfunctional tumor-intrinsic interferon-γ signaling, and oncogenic signaling leading to immunoediting. Among these, the Wnt/β-catenin pathway is a notable potential mechanism of immune escape and resistance to immune checkpoint inhibitors. In this review, we will summarize findings on these resistance mechanisms in NSCLC and other cancers, focusing on Wnt/β-catenin signaling. First, we will review the molecular biology of Wnt/β-catenin signaling, then discuss how it can induce immunoediting and resistance to immune checkpoint inhibitors. We will also describe other various mechanisms of immune-checkpoint-inhibitor resistance. Finally, we will propose therapeutic approaches to overcome these mechanisms

    Highly porous nature of a primitive asteroid revealed by thermal imaging

    International audienceCarbonaceous (C-type) asteroids1 are relics of the early Solar System that have preserved primitive materials since their formation approximately 4.6 billion years ago. They are probably analogues of carbonaceous chondrites2,3 and are essential for understanding planetary formation processes. However, their physical properties remain poorly known because carbonaceous chondrite meteoroids tend not to survive entry to Earth’s atmosphere. Here we report on global one-rotation thermographic images of the C-type asteroid 162173 Ryugu, taken by the thermal infrared imager (TIR)4 onboard the spacecraft Hayabusa25, indicating that the asteroid’s boulders and their surroundings have similar temperatures, with a derived thermal inertia of about 300 J m−2 s−0.5 K−1 (300 tiu). Contrary to predictions that the surface consists of regolith and dense boulders, this low thermal inertia suggests that the boulders are more porous than typical carbonaceous chondrites6 and that their surroundings are covered with porous fragments more than 10 centimetres in diameter. Close-up thermal images confirm the presence of such porous fragments and the flat diurnal temperature profiles suggest a strong surface roughness effect7,8. We also observed in the close-up thermal images boulders that are colder during the day, with thermal inertia exceeding 600 tiu, corresponding to dense boulders similar to typical carbonaceous chondrites6. These results constrain the formation history of Ryugu: the asteroid must be a rubble pile formed from impact fragments of a parent body with microporosity9 of approximately 30 to 50 per cent that experienced a low degree of consolidation. The dense boulders might have originated from the consolidated innermost region or they may have an exogenic origin. This high-porosity asteroid may link cosmic fluffy dust to dense celestial bodies
