Smoking and Radiolucent Periapical Lesions in Root Filled Teeth: Systematic Review and Meta-Analysis

    Get PDF
Aim: This systematic review and meta-analysis aimed to investigate the association between smoking habits and the prevalence of radiolucent periapical lesions (RPLs) in root-filled teeth (RFT). Methods: The Population, Intervention, Comparison, and Outcome (PICO) question was: in adult patients who have RFT, does the absence or presence of a smoking habit affect the prevalence of RPLs associated with RFT? Systematic MEDLINE/PubMed, Wiley Online Database, Web of Science, Scopus, and PRISMA protocol were used to evaluate and present the results. Studies comparing smokers with control non-smoker subjects, including RFT, and providing data on the prevalence of RFT with RPLs, were included. The Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system was used for certainty in the evidence. The risk of bias was assessed according to Cochrane Collaboration common scheme for bias and ROBINS-I tool. Cumulative meta-analysis was performed with a random effects model. PROSPERO registration code: CRD42020165279. Results: Four studies reported data on inclusion criteria, representing data from 9257 root-filled teeth—4465 from non-smokers and 4792 from smoker patients. The meta-analysis provided an odds ratio indicating a significant association between smoking and higher prevalence of root filled teeth with radiolucent periapical lesions (OR = 1.16; 95% CI = 1.07–1.26; p = 0.0004). The certainty of the literature assessment was moderate per GRADE. The ROBINS-I tool classified three studies as low risk of bias, and the fourth as moderate risk of bias. Conclusions: Moderate, quality scientific evidence indicates a weak but significant relationship between smoking and the prevalence of RPLs in RFT. Smoking can be considered a negative prognostic factor for the outcome of root canal treatment. Endodontic providers should be aware of the relationship between smoking and persistent apical periodontitis, assessed as RPLs, in RFT

Sellar Ependymoma: Case Report and Literature Review

    Get PDF
Ependymoma is a neuroectodermal neoplasm whose main location is infratentorial in children and spinal in adults. Only a few cases of atypical sellar location have been reported and there is no standardized diagnosis or management protocol. A rare case of sellar ependymoma is reported in an adult with cognitive and visual impairment. The clinical suspicion of this entity, as well as the maximum safe surgical resection, may allow an accurate diagnosis and could improve the therapeutic approach. More studies on this topic are needed to assess long-term treatment and over-all survival

Antibiotic-loaded implants for the management of osteomyelitis

    Get PDF
The management of osteomyelitis (OM) is quite challenging and diverse. The optimal therapeutic choice varies according to the duration of symptoms. Typically, acute OM involves antibiotic treatment, and chronic OM requires surgical procedures. Despite current advances in the understanding of OM and novel therapeutic options, this condition remains a significant health problem, non-resolution and recurrence being significantly frequent. To address this problem, new therapeutic approaches have been developed. For instance, antibiotic-loaded implants (ABLI) have been proposed as a novel approach in surgically-managed OM cases to decrease recurrence. A hefty amount of evidence supports the effectiveness and safety of ABLI. Current investigations are focused on establishing the best vehicles for local antibiotic therapy and antibiotic choice. This review aims to analyze available evidence regarding ABLI in the treatment of OM

Multi-Omic Epigenetic-Based Model Reveals Key Molecular Mechanisms Associated with Palmitic Acid Lipotoxicity in Human Astrocyte

Astrocytes are critical for the metabolic, structural and functional modulatory support of the brain. Lipotoxicity or high levels of saturated fatty acid as Palmitate (PA) has been associated with neurotoxicity, the loss or change of astrocytic functionality, and the etiology and progression of neurodegenerative diseases such as Parkinson or Alzheimer. Several molecular mechanisms of PA’s effect in astrocytes have been described, yet the role of epigenetic regulation and chromatin architecture have not been fully explored. In this study, we developed a multi-omic epigenetic-based model to identify the molecular mechanisms of lipotoxic PA activity in astrocytes. We used data from nine histone modifications, location of Topological Associated Domains (TADs) and transcriptional CTCF regions, where we identified the basal astrocyte epigenetic landscape. Moreover, we integrated transcriptomic data of astrocytic cellular response to PA with the epigenetic multi-omic model to identify lipotoxic-induced molecular mechanisms. The multi-omic model showed that chromatin conformation in astrocytes treated with PA have response genes located within shared topological domains, in which most of them also showed either repressive or enhancing marks in the Chip-Seq enrichment, reinforcing the idea that epigenetic regulation has a huge impact on the lipotoxic mechanisms of PA in the brain

Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, individuals with clonal mosaic events (clonal mosaicism for chromosome alterations and/or loss of chromosome Y) showed an increased risk of COVID-19 lethality

Partial Upgrading of Extra-Heavy Crude Oils via Fixed Bed Aquaprocessing Using Two Catalysts with Different Pore Size Distributions

Partial upgrading of heavy oil represents a great alternative to cope with current crude oil low prices, in particular if the chosen path is catalytic, considerably reducing energy usage, i.e. emissions, and replacing hydrogen with an inexpensive and abundant source of it such as steam. The convenient yield to desirable stable products makes Aquaprocessing an attractive process to reduce upgrading costs. This thesis is focused on the comparison of the results obtained with an in-house formulated catalyst called CAT-J20 with results obtained using a previous less macroporous version of this catalyst (CAT-J15). CAT-J15 support presents a smaller average pore width size of 8.2 nm when compared with the improved solid having 18.1 nm. The higher pore size catalyst allows better results when upgrading a SAGD produced Canadian oil (JACOS) and steam produced Mexican oil (Samaria), the former being more naphthenic and the latter more aromatic. Results obtained with both feedstocks demonstrated that CAT-J20 presents a great activity running JACOS but presents moderate results with Samaria due to its molecular complexity
