Une complication musculaire rarissime de l’hémophilie: la myosite ossifiante

La myosite ossifiante circonscrite est une affection bénigne caractérisée par une prolifération hétérotopique d'os dans les tissus mous. Elle survient généralement chez des adolescents ou de jeunes adultes. De très rares cas ont été décrits chez les enfants. Son étiopathogénie n'est pas claire, il semble toutefois qu'un foyer de nécrose musculaire ou un hématome puissent être à l'origine de la lésion. Les sièges les plus fréquents sont le quadriceps et le biceps brachial. Nous présentons le cas d'un patient âgé de vingt ans, aux antécédents d'hémophilie A, admis dans notre service de Médecine Physique et de Réadaptation Fonctionnelle pour prise en charge d'une limitation de la mobilité des deux hanches. L'histoire remonte à un an où le patient a présenté suite à une chute de sa propre hauteur avec réception sur les hanches une douleur et une tuméfaction à leur niveau. L'évolution était marquée par l'installation d'une limitation de la mobilité des deux hanches responsable d'une gêne fonctionnelle et d'une réduction du périmètre de marche. L'examen clinique objectivait une limitation douloureuse de la mobilité des hanches sans autre signe accompagnateur. Les radiographies standards (A, B,C) et le scanner du bassin (D, E) ont montré une ossification musculaire autour des deux hanches, aspect compatible avec une myosite ossifiante progressive des muscles carrés fémoraux des deux côtés et du jumeau inférieur droit. Une intervention chirurgicale a été proposée mais refusée par le patient. Il a alors bénéficié d'un traitement médical et de rééducation à visée antalgique. L'évolution était marquée par une légère amélioration fonctionnelle.Pan African Medical Journal 2015; 2

Cerebral tuberculoma: an entity not to ignore

Cerebral tuberculomas are a rare and serious form of tuberculosis due to the haematogenous spread of Mycobacterium Tuberculosis. Symptoms and radiologic features are nonspecific, leading sometimes to misdiagnosis. We report the case of a 60-year-old male, with a history of diffuse bilateral infiltrative pulmonary disease at the stage of fibrosis, he made two generalized seizures associated with occipital headaches. CT scan showed a left frontal tumor, calcified lesion with edema around it. The patient was put under Depakine and corticosteroids. The evolution is marked by the occurrence of new seizures associated with heaviness of the right arm. Brain MRI showed a left posterior peripheral frontal meningioma with intralesional bleeding and significant edema around it with mass effect on the ipsilateral lateral ventricle. The patient was operated and the tumor was removed. In postoperative there was a Broca's aphasia with right hemiplegia. Pathological anatomical examination of the surgical specimen found a cerebromeningeal Granuloma with caseous necrosis in its pseudo tumor presentation (tuberculoma). The thoraco abdominal scan did not show any other tuberculosis lesions. The patient started antituberculosis treatment with 4 drugs (HRZE) for 2 months, followed by maintenance therapy (HR).The evolution was marked by the persistence of a right hemiplegia with Broca's aphasia. Paralysie de la branche externe du nerf spinal sur cicatrice cheloide

The paralysis of the external branch of spinal nerve is very rare. It manifests clinically by a weakness and abnormal morphology of the shoulder. We must think about it in front of any simple surgery of the cervical region. We report the case of a 20 year old patient, who consulted several doctors for pain and progressive weakness of the left shoulder appeared a few days after resumption of a keloid scar complicating surgical excision of a cervical lipoma operated some months earlier. Physical examination revealed a strength of the left shoulder listed on 3 without articular limitation, atrophy of the trapezius muscle with ipsilateral asymmetry and fall of the left shoulder. A lesion of spinal nerve was suspected and an EMG was executed. The EMG objectified a partial lesion of the left spinal Nerve. The patient was sent in Plastic and Reconstructive surgery for nerve repair. The achievement of the external branch of spinal nerve is manifested by pain and weakness in the shoulder triggered by the anteflexion movements of the upper limb. The most usual cause is cervical lymph node biopsy. In our case, the spinal nerve lesion occurred while the resumption in keloid skin scar. Poland’s syndrome

Poland’s syndrome is characterized by agenesis of the pectoralis major muscle and mammary agenesis associated to the presence or not of bone or renal alterations. In rare cases, severely affected individuals have abnormalities of internal organs such as a lung or a kidney, or the heart is abnormally located in the right side of the chest (dextrocardia). It has been estimated to occur in 1 to 3 per 100,000 newborns. In Poland’s syndrome, males predominate by at least 3:1 and usually the right side is affected in 75% of cases. The cause of Poland’s syndrome is unknown. Researchers have suggested that it may result from a disruption of blood flow during development before birth. We report a case of an adolescent patient who consulted for back pain. Physical examination showed left anterior chest-wall depression, lateral to the nipple. There were no other anomalies seen on further systemic evaluation. Neurological examination and skeletal survey were normal. Poland’s syndrome was diagnosed with absence of left pectoralis major muscle and mammary agenesis. A global evaluation of the patient was carried out with no findings of other markers of the disease. Mild cases of Poland syndrome without hand involvement may not be evident until puberty, when the differences between the two sides of the chest become more apparent. During adolescence, the disease assumes a risk factor due to alteration of the patient’s body image and consequent reduction in self-esteem.Pan African Medical Journal 2015; 2

Does the School Textbook still have its Place in the Teaching/Learning of French as a Foreign Language? A State of Affairs The textbook is a didactic tool of great importance. Its role is decisive in the learning of French as a foreign language and allows, if it is well used, to effectively take charge of verbal memory and semantic memory, two important parameters for harmonious learning. Verbal and semantic memory: it is this type of memory that the teaching of language acquisition makes use of. In fact, the reader or the interlocutor reminds a given number of words. This verbal capacity permits the learners to utter a discourse the way it takes place in class. According to psycholinguists who are unanimous about the subject, memorizing takes place on a semantic and non-linguistics basis. In classroom environment, memorizing is based on linguistic grounds “the text can only have meaning if it integrates itself after selection, filtering and in the pre-existing structure of the subject. This research showed that the methodological situation attributed to the French language in schools is somewhat paradoxical, as textbooks revealed it. The second generation textbooks promise to present an originality that would make them seem like the product of a new and national pedagogy.If the development of the program and the writing of the textbook do not take into account everything that motivates the pupil, if the content is not varied, diversified according to the needs of the pupils and their cognitive abilities, the teacher must remedy this situation and overcome all these obstacles which constitute the failure of the teaching / learning process. L’application de l’Intelligence Artificielle aux activités des sites marchands en Algérie durant la pandémie de la COVID 19

E-commerce experienced a major leap whereby digital platforms became the sole outlet for buying and selling transactions. Customers have demanded a faster and smoother shopping experience, and since then, artificial intelligence has become crucial in e-commerce with its strengths and benefits for individuals and businesses. This research aims to address artificial intelligence and its effects on e-commerce in Algeria during the COVID 19 period; using the qualitative approach through semi-directive interviews with managers of 49 merchant sites in Algeria. The evaluation and analysis of the results obtained from this study led to the conclusion that despite the great advantages offered by AI to e-commerce in the world, merchant sites in Algeria did not respond to this technology in an optimal manner.Le commerce électronique a connu un bond important par lequel les plateformes numériques sont devenues le seul débouché pour les opérations d’achat et de vente. Les clients ont exigé une expérience d’achat plus rapide et plus fluide, et depuis lors, l’intelligence artificielle est devenue cruciale dans le e-commerce avec ses forces et ses avantages pour les clients et l’entreprise elle-même. L’objectif de cette recherche est d’aborder l’intelligence artificielle et ses effets sur le commerce électronique en Algérie en période de la COVID 19 ; en utilisant l’approche qualitative à travers des entretiens semi-directifs avec les responsables de 49 sites marchands en Algérie. La pubalgie du sportif: mise au point à propos d’une étude rétrospective de 128 joueurs

La pubalgie du sportif de haut niveau représente une entité nosologique à part entière, tant en raison du mécanisme à l'origine de la pathologie que des lésions objectivables au niveau de la paroi abdominale. C'est un syndrome douloureux de la région inguino-pubienne qui touche particulièrement les footballeurs. Son étiologie est attribuée à la répétition de mouvements des membres inférieurs et du tronc associant rotation et adduction forcées. Son incidence est nettement plus élevée chez les hommes. Après avoir écarté une pathologie d'organe, le patient devrait bénéficier tout au début de son traitement conservateur d'une IRM afin d'effectuer un bilan lésionnel complet qui, selon les situations, permet d'écourter le traitement conservateur et de proposer un traitement chirurgical optimal. Le traitement doit être  résolument conservateur pendant 3 mois. La rééducation est le plus souvent le traitement de première intention. Les patients présentant une persistance des symptômes sont candidats à la chirurgie.  L'intervention de Nesovic est le traitement de choix chez les sportifs de haut niveau et qui permet, dans la très grande majorité des cas, une reprise sans aucune limitation de l'activité sportive antérieure. La technique de Bassini semble être moins lourde que celle de Nesovic, puisqu'elle est moins invasive. Une prise en charge multidisciplinaire centrée sur l'athlète avant et après l'intervention permet un retour aux activités physiques après quelques mois. Nous rapportons l'expérience de la prise en charge de 128 joueurs opérés selon la technique de Bassini et nous comparons nos résultats avec celles de la littérature.Key words: Pubalgie, sportif, chirurgie, prévention, rééducation, intervention de Bassin

En effet, l intégration toujours croissante de fonctions au sein d une même puce a permis le développement de ces nouvelles technologies basses consommations et à un moindre coût. Les travaux de recherche présentés dans ce manuscrit s inscrivent dans le cadre de ces deux caractéristiques (faible coût et faible consommation). Nous avons donc développé deux solutions innovantes d architectures radiofréquences : half-complex et full-complex en technologie CMOS.
