Summary of the Meeting on 11 December 2015 on Adaptation of Structural Design to Climate Change
The objectives of the meeting were as follows:
1. Discuss the feasibility and needs for creating snow map for structural design which accounts for the climate change:
• availability of methodology and data;
• scope of a snow map project – geographic, time span;
• support / resources needed.
2. Set-up a group to create a document on the rational and needs for a snow map for structural design which accounts for the climate change.
3. Discuss the interaction with CEN/TC 250 Project Team (PT) on SC1.T5 (the Project Team on the CEN report on adaptation of the Eurocodes to the climate change, Task 5 of SC1, under Mandate M/515).

Sustainable Concrete with Recycled Aggregates: experiences and perspective
The recycling of concrete construction and demolition wastes to obtain coarse recycled aggregates for structural concrete production represents an interesting strategy fostering circular economy in the construction sector. In this work, the effects of parent concretes on coarse recycled aggregates and on new structural concretes produced with different replacement percentages of recycled aggregates have been investigated. The quality of parent concrete seems not directly related to the mechanical properties of the concrete prepared with recycled aggregates, while the mix design plays a key role. In addition, tests on concrete specimens (i.e. plinths) have been carried out to demonstrate the feasibility of structural elements with recycled aggregates concrete. In the manuscript we present an overview of these results, highlighting pros and cons of using concrete with recycled aggregates for future developments of the concrete construction market, also stressing the influence of climate change
Bayesian methodology for probabilistic description of mechanical parameters of masonry walls
In consideration of the high vulnerability of the built environment, the assessment of seismic behavior of existing masonry buildings is a key topic in view of their retrofitting and reuse. Because masonry's behavior depends on complex nonhomogeneous, anisotropic, asymmetric, and nonlinear properties, the definition of suitable mechanical models is still a critical issue, especially for stone masonry. Structural analyses of existing masonry buildings in seismic-prone areas are thus significantly influenced by the adopted mechanical models and assumptions about their relevant masonry properties, which are characterized by large uncertainty. In this study, a procedure for the definition of masonry classes and probability density functions of relevant mechanical parameters, such as elastic modulus and shear modulus, is proposed. The general procedure is illustrated referring to a significant number of in situ double-flat-jack test results on stone masonry obtained by the authors during an ad hoc experimental campaign. Finally, combining information on masonry quality obtained by visual inspection with results of in situ tests, a Bayesian methodology is proposed for the updating of masonry mechanical parameters, thereby providing the basis for a more refined probabilistic assessment of the seismic risk index. Real-world use of oral versus subcutaneous semaglutide in a cohort of type 2 diabetic patients: which option to which patient?
Purpose To evaluate the variables influencing the therapeutic choice toward oral versus subcutaneous semaglutide in a cohort of diabetic subjects.Methods We retrospectively collected data of 292 patients followed at the Diabetes Unit of the University Hospital of Siena and the Hospital of Grosseto, who were prescribed oral (n = 115) or subcutaneous (n = 177) semaglutide between October 2021 and October 2022.Results Oral semaglutide was preferentially prescribed in older subjects with longer disease duration in replacement of other antidiabetic drugs, while subcutaneous semaglutide was preferentially prescribed in add-on to metformin in subjects with higher body weight and BMI. After 6 months, both formulations significantly improved glycemic control and body weight, however injectable semaglutide showed a greater efficacy on A1c levels, weight loss, BMI and waist circumference reduction. No differences were found in terms of adverse events.Conclusion In our experience, injectable semaglutide is preferred in patients with excess weight and shorter disease duration, while the oral formulation was used later and especially after therapeutic failure of previous therapies. Follow-up data indicate similar tolerability and efficacy of both formulations, despite subcutaneous semaglutide demonstrated greater efficacy
‘NOT A RELIGIOUS STATE’ A study of three Indonesian religious leaders on the relation of state and religion
This article explores the concept of a ‘secular state’ offered by three
Indonesian religious leaders: a Catholic priest, Nicolaus Driyarkara
(1913–1967), and two Muslim intellectuals who were also state
officials, Mukti Ali (1923–2004) and Munawir Sjadzali (1925–2004).
All three, who represented the immediate generation after the
revolution for Indonesian independence from the Dutch (1945),
defended the legitimacy of a secular state for Indonesia based on
the state ideology Pancasila (Five Principles of Indonesia). In doing
so, they argued that a religious state, for example an Islamic state,
is incompatible with a plural nation that has diverse cultures,
faiths, and ethnicities. The three also argued that the state should
remain neutral about its citizens’ faith and should not be
dominated by a single religion, i.e. Islam. Instead, the state is
obliged to protect all religions embraced by Indonesians. This
argument becomes a vital foundation in the establishment of
Indonesia’s trajectory of unique ‘secularisation’. Whilst these three
intellectuals opposed the idea of establishing a religious or Islamic
state in Indonesia, it was not because they envisioned the decline
of the role of religion in politics and the public domain but rather
that they regarded religiosity in Indonesia as vital in nation
building within a multi-religious society. In particular, the two
Muslim leaders used religious legitimacy to sustain the New
Order’s political stability, and harnessed state authority to
modernise the Indonesian Islamic community
Regulatory T-cells from pancreatic lymphnodes of patients with type-1 diabetes express increased levels of microRNA miR-125a-5p that limits CCR2 expression
Autoimmune type 1 diabetes (T1D) is thought to be caused by a defective immune regulation with regulatory T (Treg) cells playing a fundamental role in this process. Tolerance mechanisms depend on tunable responses that are sensitive to minor perturbations in the expression of molecules that can be carried out by multiple epigenetic mechanisms, including regulation by microRNAs. In this study, microRNA expression profile was investigated in Treg cells isolated from peripheral blood (PB) and from pancreatic draining lymph nodes (PLN) of T1D patients and non-diabetic subjects. Among 72 microRNAs analyzed, miR-125a-5p resulted specifically hyper-expressed in Treg cells purified from PLN of T1D patients. TNFR2 and CCR2 were identified as miR-125a-5p target genes. Elevated miR-125a-5p was detected in Treg cells isolated from PLN but not from PB of donors with T1D and was associated with reduced CCR2 expression. A specific beta-cell expression of the CCR2-ligand (CCL2) was observed in the pancreata of cadaveric donors, suggesting that beta-cells are prone to attract CCR2(+) Treg cells. These novel data propose a mechanism, occurring in PLNs of T1D patients, involving increased expression of miR-125a-5p on Treg cells which results into reduced expression of CCR2, thus limiting their migration and eventual function in the pancreas
A proposed index of myocardial staining for vein of Marshall ethanol infusion: an Italian single-center experience.
Background: Mitral isthmus (MI) conduction block is a fundamental step in anatomical approach treatment for persistent atrial fibrillation (PeAF). However, MI block is hardly achievable with endocardial ablation only. Retrograde ethanol infusion (EI) into the vein of Marshall (VOM) facilitates MI block. Fluorographic myocardial staining (MS) during VOM-EI could be helpful in predicting procedural alcoholization outcome even if its role is qualitatively assessed in the routine. The aim was to quantitatively assess MS during VOM-EI and to evaluate its association with MI block achievement.
Methods: Consecutive patients undergoing catheter ablation for PeAF at Fondazione Toscana Gabriele Monasterio (Pisa, Italy) from February 2022 to May 2023 were considered. Patients with identifiable VOM were included. A proposed index of MS (MSI) was retrospectively calculated in each included patient. Correlation of MSI with low-voltage zones (LVZ) extension after VOM-EI and its association with MI block achievement were assessed.
Results: In total, 42 patients out of 49 (85.8%) had an identifiable VOM. MI block was successfully achieved in 35 patients out of 42 (83.3%). MSI was significantly associated with the occurrence of MI block (OR 1.24 (1.03-1.48); p = 0.022). A higher MSI resulted in reduced ablation time (p = 0.014) and reduced radiofrequency applications (p = 0.002) to obtain MI block. MSI was also associated with MI block obtained by endocardial ablation only (OR 1.07 (1.02-1.13); p = 0.002). MSI was highly correlated with newly formed LVZ extension (r = 0.776; p = 0.001).
Conclusions: In our study cohort, optimal MSI predicts MI block and facilitates its achievement with endocardial ablation only
New Advances in Metabolic Syndrome, from Prevention to Treatment: The Role of Diet and Food
The definition of metabolic syndrome (MetS) has undergone several changes over the years due to the difficulty in establishing universal criteria for it. Underlying the disorders related to MetS is almost invariably a pro-inflammatory state related to altered glucose metabolism, which could lead to elevated cardiovascular risk. Indeed, the complications closely related to MetS are cardiovascular diseases (CVDs) and type 2 diabetes (T2D). It has been observed that the predisposition to metabolic syndrome is modulated by complex interactions between human microbiota, genetic factors, and diet. This review provides a summary of the last decade of literature related to three principal aspects of MetS: (i) the syndrome’s definition and classification, pathophysiology, and treatment approaches; (ii) prediction and diagnosis underlying the biomarkers identified by means of advanced methodologies (NMR, LC/GC-MS, and LC, LC-MS); and (iii) the role of foods and food components in prevention and/or treatment of MetS, demonstrating a possible role of specific foods intake in the development of MetS
