    Safety profile of oxcarbazepine: results from a prescription-event monitoring study

    Purpose: To monitor safety of oxcarbazepine, prescribed in primary care in England, using prescription-event monitoring (PEM). Methods: Postmarketing surveillance using observational cohort technique of PEM. Exposure data were obtained from dispensed British National Health Service prescriptions issued by general practitioners (GPs) March 2000–July 2003. Demographic, drug utilization, and clinical event data were collected from questionnaires posted to GPs at least 6 months after first prescription date for each patient. Incidence densities (IDs) (number of first reports per 1,000 patient-months of treatment) were calculated and differences for events reported in month 1 (ID1) and months 2–6 (ID2–6) (99% confidence intervals) were examined for changes in event rates. Follow-up and causality assessment of medically significant events were undertaken. Results: The cohort comprised 2,243 patients [mean age 40.4 years; range 2–99 years; standard deviation (SD) 18.8; 46.3% (n = 1,038) male]. Most frequently reported primary indications were epilepsy, convulsion (n = 1,111; 49.5%, n = 209; 9.3%, respectively). GPs recorded 932 reasons for stopping medication in 698 (31.1%) patients; most frequent clinical reason “drowsiness/sedation” (n = 57; 2.5% of cohort). Clinical events (excluding indication) associated with starting treatment (lower 99% CI > 0) included: “drowsiness/sedation” (ID1-ID2–6 = 14.2), “nausea/vomiting” (ID1-ID2–6 = 13.0), and dizziness (ID1-ID2–6 = 11.6). Events followed up and assessed as probably related to oxcarbazepine use included rash (7 of 11) and hyponatremia (15 of 38). Discussion:  There were no serious adverse drug reactions reported during this study. Results of the study should be taken in context with other epidemiologic studies

    Advanced composite aileron for L-1011 transport aircraft: Design and analysis

    Detail design of the composite aileron has been completed. The aileron design is a multi-rib configuration with single piece upper and lower covers mechanically fastened to the substructure. Covers, front, spar and ribs are fabricated with graphite/epoxy tape or fabric composite material. The design has a weight savings of 23 percent compared to the aluminum aileron. The composite aileron has 50 percent fewer fasteners and parts than the metal aileron and is predicted to be cost competitive. Structural integrity of the composite aileron was verified by structural analysis and an extensive test program. Static, failsafe, and vibration analyses have been conducted on the composite aileron using finite element models and specialized computer programs for composite material laminates. The fundamental behavior of the composite materials used in the aileron was determined by coupon tests for a variety of environmental conditions. Critical details of the design were interrogated by static and fatigue tests on full-scale subcomponents and subassemblies of the aileron

    Structural design and maintainability criteria were established and used as a guideline for evaluating a variety of configurations and materials for each of the major subcomponents. From this array of subcomponent designs, several aileron assemblies were formulated and analyzed. The selected design is a multirib configuration with sheet skin covers mechanically fastened to channel section ribs and spars. Qualitative analysis of currently available composite material systems led to the selection of three candidate materials on which comparative structural tests were conducted to measure the effects of environment and impact damage on mechanical property retention. In addition, each system was evaluated for producibility characteristics. From these tests, Thornel 300/5208 unidirectional tape was selected for the front spar and covers, and Thornel 300 fabric/5208 was chosen for the ribs

    Study of advanced fuel system concepts for commercial aircraft and engines

    The impact on a commercial transport aircraft of using fuels which have relaxed property limits relative to current commercial jet fuel was assessed. The methodology of the study is outlined, fuel properties are discussed, and the effect of the relaxation of fuel properties analyzed. Advanced fuel system component designs that permit the satisfactory use of fuel with the candidate relaxed properties in the subject aircraft are described. The two fuel properties considered in detail are freezing point and thermal stability. Three candidate fuel system concepts were selected and evaluated in terms of performance, cost, weight, safety, and maintainability. A fuel system that incorporates insulation and electrical heating elements on fuel tank lower surfaces was found to be most cost effective for the long term

    Evidence of traffic-related pollutant control in soil-based Sustainable Urban Drainage Systems (SUDS)

    SUDS are being increasingly employed to control highway runoff and have the potential to protect groundwater and surface water quality by minimising the risks of both point and diffuse sources of pollution. While these systems are effective at retaining polluted solids by filtration and sedimentation processes, less is known of the detail of pollutant behaviour within SUDS structures. This paper reports on investigations carried out as part of a co-ordinated programme of controlled studies and field measurements at soft-engineered SUDS undertaken in the UK, observing the accumulation and behaviour of traffic-related heavy metals, oil and PAHs. The field data presented were collected from two extended detention basins serving the M74 motorway in the south-west of Scotland. Additional data were supplied from an experimental lysimeter soil core leaching study. Results show that basin design influences pollutant accumulation and behaviour in the basins. Management and/or control strategies are discussed for reducing the impact of traffic-related pollutants on the aqueous environment

    Heterocyst placement strategies to maximize growth of cyanobacterial filaments

    Under conditions of limited fixed-nitrogen, some filamentous cyanobacteria develop a regular pattern of heterocyst cells that fix nitrogen for the remaining vegetative cells. We examine three different heterocyst placement strategies by quantitatively modelling filament growth while varying both external fixed-nitrogen and leakage from the filament. We find that there is an optimum heterocyst frequency which maximizes the growth rate of the filament; the optimum frequency decreases as the external fixed-nitrogen concentration increases but increases as the leakage increases. In the presence of leakage, filaments implementing a local heterocyst placement strategy grow significantly faster than filaments implementing random heterocyst placement strategies. With no extracellular fixed-nitrogen, consistent with recent experimental studies of Anabaena sp. PCC 7120, the modelled heterocyst spacing distribution using our local heterocyst placement strategy is qualitatively similar to experimentally observed patterns. As external fixed-nitrogen is increased, the spacing distribution for our local placement strategy retains the same shape while the average spacing between heterocysts continuously increases.Comment: This is an author-created, un-copyedited version of an article accepted for publication in Physical Biology.     Generalizing power transitions as a cause of war

First, I ask whether power transition theory can be generalized beyond identification of great powers or regional hierarchies. Lemke and Werner introduce the concept of a multiple hierarchical order, in which mutually relevant regional powers can go to war over dissatisfaction with a regional status quo. I submit that this concept can be generalized into a continuous concept to include all states within the umbrella of the theory. Second, I ask how often status quo states initiate war in power transition cases. Jack Levy explains that status quo states have a motive to launch a preemptive war against a revisionist state, before it becomes too powerful to defeat. I submit that these motivations lead to a high incidence of status quo actor-initiated war in power transitions. Finally, I ask whether the rate of change of relative power matters during a transition period. I hypothesize that quick changes in the relative difference in power between two states would create a fast-closing window of opportunity. This closing window creates a crisis and motivates leaders to move quickly, leading to a higher probability of avoidable war. Incorporation of rate of power transition could explain war in power transition cases yet to achieve true parity, or even explain peace in a period of parity and revisionism. To test these questions, I create a large, inclusive (571,000+ N) dataset of nearly all dyads between 1821 and 2001, using the Correlates of War Composite Index of National Capabilities as the basis of power independent variables, and a composite of distance and power measurements to determine the relevance independent variable. I run a number of regressions of the power and relevance independent variables against the onset of war. I reach decisive conclusions about the nature of power dynamics in the international system, and propose their incorporation into the power transition literature. Generalized, continuous measurements of relevance, parity, and rate of change of power transition increase the explanatory power of the model; the revisionist state does not always or even usually provoke power transition war; finally, higher rates of power transition lead to a higher probability of war. The thesis ends with a number of shortfalls with the model I propose, and a number of further revisions and expansions of power transition theory.by Erik Fogg.S.M.and S.B
