    Computer aided monitoring of breast abnormalities in X-ray mammograms

    X­ray mammography is regarded as the most effective tool for the detection and diagnosis of breast cancer, but the interpretation of mammograms is a difficult and error­prone task. Computer­aided detection (CADe) systems address the problem that radiologists often miss signs of cancers that are retrospectively visible in mammograms. Furthermore, computer­aided diagnosis (CADx) systems assist the radiologist in the classification of mammographic lesions as benign or malignant[1]. This paper details a novel alternative system namely computer­aided monitoring (CAM) system. The designed CAM system can be used to objectively measure the properties of a suspected abnormal area in a mammogram. Thus it can be used to assist the clinician to objectively monitor the abnormality. For instance its response to treatment and consequently its prognosis. The designed CAM system is implemented using the Hierarchical Clustering based Segmentation (HCS) [2] [3] [4] process. Brief description of the implementation of this CAM system is as follows : Using the approximate location and size of the abnormality, obtained from the user, the HCS process automatically identifies the more appropriate boundaries of the different regions within a region of interest (ROI), centred at the approximate location. From the set of, HCS process segmented, regions the user identifies the regions which most likely represent the abnormality and the healthy areas. Subsequently the CAM system compares the characteristics of the user identified abnormal region with that of the healthy region; to differentiate malignant from benign abnormality. In processing sixteen mammograms from mini­MIAS [5], the designed CAM system demonstrated a success rate of 100% in differentiating malignant from benign abnormalities

    A study with the use of dance movement therapy as a counseling approach in a chemical dependency setting was presented. The objective of the study was to find out if the use of dance movement therapy in male chemical dependency groups reduces overall anxiety. The literature review describes dance movement therapy, aspects of chemical dependency and addiction, dance movement therapy used with specific populations, chemical addiction with creative art therapies, the process of recovery (stages of change), fundamentals of group work, anxiety, and movement therapy techniques used in chemical dependency groups. Methods of the study were presented with the use of four movement therapy interventions. The instrument and participants were also described. The results were evaluated qualitatively and quantitatively through pre and post test results and observations. The discussion presents areas for additional research and implications for future research

Concerns about "inappropriate content" on the Internet have arisen, especially when children are involved. This study uses content analysis of messages from LM_NET (a popular mailing list for practicing school library media specialists) to examine common practices and decisions made by school library media specialists with respect to Internet filtering software.     Human papillomavirus seroprevalence and seroconversion following baseline detection of nine human papillomavirus types in young women

    Background: Estimates of the humoral immune response to incident human papillomavirus (HPV) infections are limited. Methods: In this post hoc analysis of 3875 women aged 16-23 years from a 4-valent HPV vaccine trial (NCT00092482), HPV seroprevalence on day 1 was measured with a 9-valent HPV (HPV 6/11/16/18/31/33/45/ 52/58) competitive Luminex immunoassay and compared with cervical/external genital HPV detection by po-lymerase chain reaction. In the control group, among women who were HPV DNA-negative on day 1, sero-conversion following initial HPV detection was estimated using Kaplan-Meier methods. Results: Type-specific HPV seropositivity among women with no day 1 cervical/external genital HPV detection was 0.6%-3.6%. Women with any 9-valent HPV (9vHPV) cervical/external genital detection (796/3875; 20.5%) had concordant seropositivity ranging from 13.4% (HPV 45) to 38.5% (HPV 6). Among women in the control group who were negative for all HPV types on day 1, seroconversion by month 30 after initial detection ranged from 29% (HPV 45) to 75% (HPV 16). Conclusions: Humoral immune response to HPV is variable and dynamic, depending on type-specific exposure. This longitudinal analysis provides insight into the relationship between incident infection and seropositivity

    Efficacy of FFP3 respirators for prevention of SARS-CoV-2 infection in healthcare workers.

We previously demonstrated frequent pauci- and asymptomatic severe acute respiratory syndrome coronavirus 2 infection HCWs during the first wave of the COVID-19 pandemic in the UK, using a comprehensive PCR-based HCW screening programme (Rivett et al., 2020; Jones et al., 2020). Methods: Here, we use observational data and mathematical modelling to analyse infection rates amongst HCWs working on 'red' (coronavirus disease 2019, COVID-19) and 'green' (non-COVID-19) wards during the second wave of the pandemic, before and after the substitution of filtering face piece 3 (FFP3) respirators for FRSMs. Results: Whilst using FRSMs, HCWs working on red wards faced an approximately 31-fold (and at least fivefold) increased risk of direct, ward-based infection. Conversely, after changing to FFP3 respirators, this risk was significantly reduced (52-100% protection). Conclusions: FFP3 respirators may therefore provide more effective protection than FRSMs for HCWs caring for patients with COVID-19, whether or not AGPs are undertaken.     Pediatricians' perspectives on the impact of MRSA in primary care: a qualitative study

    <p>Abstract</p> <p>Background</p> <p>The incidence of skin and soft-tissue infections (SSTIs) has rapidly increased among children in primary care settings since the emergence of community-associated methicillin-resistant <it>Staphylococcus aureus </it>(CA-MRSA). Recent treatment recommendations emphasize CA-MRSA as the primary cause, performing incision and drainage (I&D) as the primary therapy, and not prescribing antibiotics for uncomplicated cases. It is unknown how this epidemic has impacted primary care pediatricians in terms of their practice patterns and barriers they face to providing recommended therapies.</p> <p>Methods</p> <p>3 Focus groups among 29 primary care pediatricians in the San Francisco Bay Area were conducted. Transcripts were reviewed and coded into major themes by two investigators using modified grounded theory.</p> <p>Results</p> <p>Substantial changes in clinical practice have occurred since the emergence of CA-MRSA. These include increased office visits for SSTIs, patients with multiple recurrences and transmission within households. Additionally, our participants reported increased visits for mild skin problems due to media reports contributing to fears about CA-MRSA. Participants routinely prescribed antibiotics for SSTIs, however, few performed I&D. Few were aware of recent SSTI treatment recommendations. Barriers to prescribing antibiotics with CA-MRSA activity included concerns about side-effects and lack of local epidemiologic data showing that it is the primary etiology. Barriers to performing I&D included lack of training, resources and skepticism about its necessity. Important clinical challenges included increased time demands for follow-up visits and patient education along with the lack of evidence-based strategies for preventing recurrent inections and household transmission.</p> <p>Conclusion</p> <p>CA-MRSA has influenced the presentation and treatment of SSTIs especially in terms of case numbers and recurrences. Barriers to providing recommended therapies can be addressed through improved dissemination of treatment guidelines and epidemiologic data. Studies are urgently needed toimprove theevidence-base for treatment and prevention strategies.</p

    Prevalence, incidence, and natural history of HPV infection in adult women ages 24 to 45 participating in a vaccine trial

    Objectives The natural history of human papillomavirus (HPV) infection has been studied extensively in young women; this study investigated HPV infection in adult women. Methods Data from 3817 women aged 24–45 years in a global trial of the 4-valent HPV (6/11/16/18) vaccine were used to calculate prevalence of anogenital infections containing 9-valent (9v) HPV vaccine types (6/11/16/18/31/33/45/52/58) and five non-vaccine types (35/39/51/56/59). Incidence of infections and persistent infections was estimated for 989 placebo recipients naive to all 14 HPV types at baseline. Age-adjusted hazard ratios were calculated for various sociodemographic factors. Results Prevalence of anogenital infection was highest in France at 29.2% (9vHPV types) and 21.7% (non-vaccine types) and lowest in the Philippines at 7.6% (9vHPV types) and 5.1% (non-vaccine types). Overall, HPV incidence (per 100 person-years) was 5.2 (9vHPV types) and 4.7 (non-vaccine types), and incidence of persistent infection was 2.7 (9vHPV types) and 2.1 (non-vaccine types). Factors associated with new HPV infections included younger age, younger age at first intercourse, being single, current use of tobacco, and higher number of past and recent sex partners. Conclusions Because mid-adult women acquire new HPV infections, administration of the 9vHPV vaccine could reduce HPV-related morbidity and mortality in this population
