Women's Participation in the NREGA: Some Observations from Fieldwork in Himachal Pradesh, Kerala and Rajasthan

    Get PDF
The broad objective of this study was to understand the reasons behind the wide variations in participation by women in the NREGA and the policy implications that follow. Based on fieldwork in two states where women form a high proportion of the workers, Kerala and Rajasthan, and one where the proportion was low, Himachal Pradesh, emerging policy implications include the need to develop a wider range of activities that acknowledge life cycle issues and bodily ability; in sparsely populated and remote areas a different design or even a cash transfer programme may be better able to meet the objective of assuring minimum income; creation of spaces that allow non?governmental local groups to be participants in implementation and outreach work and not just watchdogs; immediate revision of the schedule of rates and implementing the revised rates to allow better earning, or else payment on a daily wage basis

    Get PDF
Setting up fully functional national eHealth systems is a challenge. Developing eHealth capacities in Asia can be achieved through distance learning such as webinars. The Asia eHealth Information Network (AeHIN) is a community of about 900 professionals in South and Southeast Asia, convened as a peer group dedicated to strengthening local health systems through the use of information and communications technology. It organised the AeHIN Hour held twice monthly as a venue for knowledge sharing. This paper will describe and analyse the context, inputs, processes and products of the AeHIN Hour webinars implemented over 47 months. Data is analysed through review of literature and AeHIN Hour technical reports. Analysis revealed that the AeHIN Hour provides individuals with a platform to learn and establish connections with other experts and like-minded individuals who could assist them in their current health system strengthening efforts. AeHIN paves the way for countries to recognise challenges and issues they face which are important for substantiating existing technical assistance. Shared interests among stakeholders sustain the activity. Management of the AeHIN Hour should be strengthened in order to maximise its value in building capacities among country-level actors who would shape the course of health systems strengthening

Implementation of Telemedicine Services in Lower-Middle Income Countries: Lessons for the Philippines

    Get PDF
Regardless of the promising potential of telemedicine to address healthcare problems, especially in lower-middle income countries, its success rate has been unsatisfactory and many telemedicine services fail to sustain their implementation shortly after initial funding or after a pilot phase. Therefore, it is important to document existing models of telemedicine implementation in these countries, to identify commonalities and extract experiences that would be useful for implementers, policy makers and future researchers. This review seeks to review and describe the experience of Low and Middle Income Countries (LMICs) in implementing telemedicine services. Evidence extracted from the included studies were analysed through a narrative synthesis which suggests a multi-sectoral approach for implementing telemedicine. It highlights the importance of education, financing options, policy, technology, governance, and partnership, in the wider picture of a sustainable telemedicine implementation among developing countries such as the Philippines. Moreover, the literature reveals both top-down and bottom-up approach for successful telemedicine implementation. These approaches include strengthening the local health workers and integrating telemedicine into the health system. Studies included in this review have been helpful, but there is an obvious lack of studies with high level of evidence that can yield generalisable, thus findings must be inferred with prudence. Even so, this review described and summarised the data which allowed description of factors and lessons in the implementation of telemedicine in LMICs

Exploring the Influence of Trauma-Informed Care on Pelvic Examinations for Women

    Get PDF
Trauma-informed care (TIC) has become increasingly common in discussions regarding women’s health. TIC refers to a clinical approach that implements compassion, respect, and acknowledges the impact of trauma on patients. In the United States, pelvic exams are routinely performed on women over 21 for gynecological abnormalities. Many of these women present with histories of post-traumatic stress disorder, sexual violence, intimate partner violence, and other psychological comorbidities. These histories greatly influence women’s behaviors, including their likelihood to seek gynecological care in fear of being retraumatized. A literature search was conducted to determine the impact of TIC on the rates of women with histories of trauma who are receiving pelvic examinations. The literature search occurred on CINAHL, PubMed, and Nursing Reference Center databases using the following search terms: trauma-informed care, women’s health, and pelvic exams. A total of 11 articles met the inclusion criteria. While the results regarding rates of pelvic exams were inconclusive, TIC leads to better long-term health outcomes and may lead to an increased sense of control for patients. The literature indicates a common theme of clinicians feeling there is a lack of training or information regarding TIC. Additional recommendations include further research on interventions that should be implemented, such as explanation of exams; allowing patients to be involved in their care, and collecting pertinent trauma history on patients. Trauma can have a major impact on all aspects of women’s health and further studies are needed to determine best approaches to implementation

The Role of Telehealth in Disaster Management: Lessons for the Philippines

    Get PDF
According to the WorldRiskIndex 2016, the Philippines is the third country most at risk of disasters in the world. Typhoon Haiyan, the strongest on local record, caused widespread destruction to life and property. Current disaster management strategies in the country do not include telehealth as a formal tool in disaster mitigation, response, or recovery. This study reviewed research incorporating telehealth in disaster management from multiple low and lower middle income countries like the Philippines to address this gap by identifying lessons the country might be able to adopt. Studies show that most initiatives centre on evaluating telehealth’s effectiveness during the response phase. Unsurprisingly, mobile technology and satellite communications predominated, and most projects were launched using donor funding. Use of telehealth in disaster management in the Philippines could begin by recognising and including telehealth in formal government protocols. The government could leverage the National Telehealth Service Program of the University of the Philippines National Telehealth Center. Documentation and systematic research on telehealth’s expected positive contributions to disaster preparedness and response should also be initiated

Increasing trust in new data sources: crowdsourcing image classification for ecology

Crowdsourcing methods facilitate the production of scientific information by non-experts. This form of citizen science (CS) is becoming a key source of complementary data in many fields to inform data-driven decisions and study challenging problems. However, concerns about the validity of these data often constrain their utility. In this paper, we focus on the use of citizen science data in addressing complex challenges in environmental conservation. We consider this issue from three perspectives. First, we present a literature scan of papers that have employed Bayesian models with citizen science in ecology. Second, we compare several popular majority vote algorithms and introduce a Bayesian item response model that estimates and accounts for participants' abilities after adjusting for the difficulty of the images they have classified. The model also enables participants to be clustered into groups based on ability. Third, we apply the model in a case study involving the classification of corals from underwater images from the Great Barrier Reef, Australia. We show that the model achieved superior results in general and, for difficult tasks, a weighted consensus method that uses only groups of experts and experienced participants produced better performance measures. Moreover, we found that participants learn as they have more classification opportunities, which substantially increases their abilities over time. Overall, the paper demonstrates the feasibility of CS for answering complex and challenging ecological questions when these data are appropriately analysed. This serves as motivation for future work to increase the efficacy and trustworthiness of this emerging source of data.Comment: 25 pages, 10 figure
