    EviAtlas : a tool for visualising evidence synthesis databases

    Abstract: Systematic mapping assesses the nature of an evidence base, answering how much evidence exists on a particular topic. Perhaps the most useful outputs of a systematic map are an interactive database of studies and their meta-data, along with visualisations of this database. Despite the rapid increase in systematic mapping as an evidence synthesis method, there is currently a lack of Open Source software for producing interactive visualisations of systematic map databases. In April 2018, as attendees at and coordinators of the first ever Evidence Synthesis Hackathon in Stockholm, we decided to address this issue by developing an R-based tool called EviAtlas, an Open Access (i.e. free to use) and Open Source (i.e. software code is freely accessible and reproducible) tool for producing interactive, attractive tables and figures that summarise the evidence base. Here, we present our tool which includes the ability to generate vital visualisations for systematic maps and reviews as follows: a complete data table; a spatially explicit geographical information system (Evidence Atlas); Heat Maps that cross-tabulate two or more variables and display the number of studies belonging to multiple categories; and standard descriptive plots showing the nature of the evidence base, for example the number of studies published per year or number of studies per country. We believe that EviAtlas will provide a stimulus for the development of other exciting tools to facilitate evidence synthesis

    The Zeta Herculis binary system revisited. Calibration and seismology

    We have revisited the calibration of the visual binary system Zeta Herculis with the goal to give the seismological properties of the G0 IV sub-giant Zeta Her A. We have used the most recent physical and observational data. For the age we have obtained 3387 Myr, for the masses respectively 1.45 and 0.98 solar mass, for the initial helium mass fraction 0.243, for the initial mass ratio of heavy elements to hydrogen 0.0269 and for the mixing-length parameters respectively 0.92 and 0.90 using the Canuto & Mazitelli (1991, 1992) convection theory. Our results do not exclude that Zeta Her A is itself a binary sub-system; the mass of the hypothetical unseen companion would be smaller than 0.05 solar mass. The adiabatic oscillation spectrum of Zeta Her A is found to be a complicated superposition of acoustic and gravity modes; some of them have a dual character. This greatly complicates the classification of the non-radial modes. The echelle diagram used by the observers to extract the frequencies will work for ell=0, 2, 3. The large difference is found to be of the order of 42 mu Hz, in agreement with the Martic et al. (2001) seismic observations.Comment: 12 pages, A&A in pres

    Distribution of cytochrome P450 2C, 2E1, 3A4, and 3A5 in human colon mucosa

    BACKGROUND: Despite the fact that the alimentary tract is part of the body's first line of defense against orally ingested xenobiotica, little is known about the distribution and expression of cytochrome P450 (CYP) enzymes in human colon. Therefore, expression and protein levels of four representative CYPs (CYP2C(8), CYP2E1, CYP3A4, and CYP3A5) were determined in human colon mucosa biopsies obtained from ascending, descending and sigmoid colon. METHODS: Expression of CYP2C, CYP2E1, CYP3A4, and CYP3A5 mRNA in colon mucosa was determined by RT-PCR. Protein concentration of CYPs was determined using Western blot methods. RESULTS: Extensive interindividual variability was found for the expression of most of the genes. However, expression of CYP2C mRNA levels were significantly higher in the ascending colon than in the sigmoid colon. In contrast, mRNA levels of CYP2E1 and CYP3A5 were significantly lower in the ascending colon in comparison to the descending and sigmoid colon. In sigmoid colon protein levels of CYP2C8 were significantly higher by ~73% than in the descending colon. In contrast, protein concentration of CYP2E1 was significantly lower by ~81% in the sigmoid colon in comparison to the descending colon. CONCLUSION: The current data suggest that the expression of CYP2C, CYP2E1, and CYP3A5 varies in different parts of the colon

    Gender differences in beliefs about health:A comparative qualitative study with Ghanaian and Indian migrants living in the United Kingdom

    Background There is a well-established association between migration to high income countries and health status, with some groups reporting poorer health outcomes than the host population. However, processes that influence health behaviours and health outcomes across minority ethnic groups are complex and in addition, culture ascribes specific gender roles for men and women, which can further influence perspectives of health. The aim of this study was to undertake a comparative exploration of beliefs of health among male and female Ghanaian and Indian migrants and White British participants residing in an urban area within the UK. Methods Thirty-six participants (12 each Ghanaian, Indian and White British) were recruited through community settings and participated in a semi-structured interview focusing on participant’s daily life in the UK, perceptions of their own health and how they maintained their health. Interviews were analyzed using a Framework approach. Results Three super ordinate themes were identified and labelled (a) beliefs about health; (b) symptom interpretation and (c) self-management and help seeking. Gender differences in beliefs and health behaviour practices were apparent across participants. Conclusions This is the first study to undertake a comparative exploration of health beliefs among people who have migrated to the UK from Ghana and India and to compare with a local (White British) population. The results highlight a need to consider both cultural and gender-based diversity in guiding health behaviours, and such information will be useful in the development of interventions to support health outcomes among migrant populations

    Prevalence and impact of alcohol and other drug use disorders on sedation and mechanical ventilation: a retrospective study

    BACKGROUND: Experience suggests that patients with alcohol and other drug use disorders (AOD) are commonly cared for in our intensive care units (ICU's) and require more sedation. We sought to determine the impact of AOD on sedation requirement and mechanical ventilation (MV) duration. METHODS: Retrospective review of randomly selected records of adult patients undergoing MV in the medical ICU. Diagnoses of AOD were identified using strict criteria in Diagnostic and Statistical Manual of Mental Disorders, and through review of medical records and toxicology results. RESULTS: Of the 70 MV patients reviewed, 27 had AOD (39%). Implicated substances were alcohol in 22 patients, cocaine in 5, heroin in 2, opioids in 2, marijuana in 2. There was no difference between AOD and non-AOD patients in age, race, or reason for MV, but patients with AOD were more likely to be male (21 versus 15, p < 0.0001) and had a lower mean Acute Physiology and Chronic Health Evaluation II (22 versus 26, p = 0.048). While AOD patients received more lorazepam equivalents (0.5 versus 0.2 mg/kg.day, p = 0.004), morphine equivalents (0.5 versus 0.1 mg/kg.day, p = 0.03) and longer duration of infusions (16 versus 10 hours/day. medication, p = 0.002), they had similar sedation levels (Richmond Agitation-Sedation Scale (RASS) -2 versus -2, p = 0.83), incidence of agitation (RASS ≥ 3: 3.0% versus 2.4% of observations, p = 0.33), and duration of MV (3.6 versus 3.9 days, p = 0.89) as those without AOD. CONCLUSION: The prevalence of AOD among medical ICU patients undergoing MV is high. Patients with AOD receive higher doses of sedation than their non-AOD counterparts to achieve similar RASS scores but do not undergo longer duration of MV

    Acetate Causes Alcohol Hangover Headache in Rats

    Background: The mechanism of veisalgia cephalgia or hangover headache is unknown. Despite a lack of mechanistic studies, there are a number of theories positing congeners, dehydration, or the ethanol metabolite acetaldehyde as causes of hangover headache. Methods: We used a chronic headache model to examine how pure ethanol produces increased sensitivity for nociceptive behaviors in normally hydrated rats. Results: Ethanol initially decreased sensitivity to mechanical stimuli on the face (analgesia), followed 4 to 6 hours later by inflammatory pain. Inhibiting alcohol dehydrogenase extended the analgesia whereas inhibiting aldehyde dehydrogenase decreased analgesia. Neither treatment had nociceptive effects. Direct administration of acetate increased nociceptive behaviors suggesting that acetate, not acetaldehyde, accumulation results in hangover-like hypersensitivity in our model. Since adenosine accumulation is a result of acetate formation, we administered an adenosine antagonist that blocked hypersensitivity. Discussion: Our study shows that acetate contributes to hangover headache. These findings provide insight into the mechanism of hangover headache and the mechanism of headache induction
